The Clinical Investigation of the 125i Radioactive Seeds Brachytherapy Combined Chemotherapy in the Treatment of Pediatric Rhabdomyosarcoma in Head and Neck

Abstract:To analyze the preliminary effects of the 125I radioactive seeds brachytherapy combined chemotherapy in the treatment of pediatric rhabdomyosarcoma which occurs in the head and neck. 17 pediatric patients with rhabdomyosarcoma in head and neck were treated in the Peking University School of stomatology from June 2013 to June 2015. Of the patients, the median age was 4 years old (range 1-16), the ratio of male:female was 12:5. The multimodality treatment of the 125I radioactive seeds brachytherapy combined chemotherapy was done after the pathological diagnosis was clear. The curative effect was observed by means of clinical indications, imaging examination, PET - CT, etc. This group of patients was followed up for 6 to 30 months. The clinical effect was evaluated until the 3rd half-life, without any additional local treatment. One patient developed intracranial metastases (12 months). Another patient of multiple rhabdomyosarcoma realized local control of implanted area, with intracranial and pulmonary metastasis. The local control was 100%, all patients survived during the follow-up period. The recent side effects were slight, mainly included pigment changes and minor pains of the mucosa, which could get spontaneous remission in half a year, and the long-term side effects will be under observation. 1. Compared with the conventional radiotherapy, 125I radioactive seeds brachytherapy realized the precise location of the target, and the dose reduction of surrounding nomal tissues and organs. It can also reduce the incidence of functional injury for the peripheral tissues and organs. The method is more suitable for the children with tumor in head and neck during the period of growth and development on the basis of safety and effectiveness. 2.For the pediatric rhabdomyosarcoma which occurs in head and neck, because of its complex anatomical structures, which is adjacent to the important blood vessels and nerves, complete resection surgery isn’t the first choice. Chemotherapy can decrease the stage of sarcoma and the rate of systemic metastasis in some degree, but radiotherapy plays a very important role as a necessary means to improve the local control rates of tumor. 3. In this paper, we had a discussion of the relationship among the curative effect of rhabdomyosarcoma children in the head and neck and the pathological type, stage, target and prescription dose. 4.The short-term outcome was good, the local control rate was high and there were no severe side effects. But there are lots of questions to be solved, including the prescription dose, the PTV, the long-term outcome (the influence of tooth jaw system, growth and physiological function, etc), which still need further research.
