Abstract:Objective To investigate the characteristic CT features in differentiation of pulmonary neoplastic diseases which appear as focal ground-glass opacity.Methods Forty-five cases of neoplastic diseases(including 17 cases of adenocarcinomas,19 cases of bronchioloalveolar carcinoma,5 cases of lymphoma,and 4 cases of atypical adenomatous hyperplasia) which appeared as focal ground-glass opacity were collected,and their CT manifestations including type of GGO,location,size,internal structure,margin and surrounding change,were analyzed.Results Pure GGO was mostly seen in atypical adenomatous hyperplasia(n=4) and bronchioloalveolar carcinoma(n=6);mixed GGO was mostly seen in adenocarcinoma(n=13) and bronchioloalveolar carcinoma(n=13).All the lesions were classified into two groups including ≤1 cm and 1 cm in size.There was significant difference in constituent ratio of different nature of lesion between two groups(χ2=9.12,P0.05).In ≤1 cm group,the common nature of disease was atypical adenomatous hyperplasia and bronchioloalveolar carcinoma,while in 1 cm group,the common diseases were adenocarcinoma and bronchioloalveolar carcinoma.All the lesions were classified into two groups including 50% group and ≤50% group according to the percentage of GGO in the entire lesion.There was significant difference in constituent ratio of different nature of lesion between two groups(χ2=8.24,P0.05).In 50% group,the common diseases were atypical adenomatous hyperplasia and bronchioloalveolar carcinoma,while in ≤50% group,the common disease was adenocarcinoma.Conclusion Comprehensively analyzing the type,internal structure,margin and size is very helpful for differentiation of focal GGO.In the pulmonary tumor which appears as GGO,small pure GGO is common in atypical adenomatous hyperplasia,air bronchogram is common in bronchioloalveolar carcinoma,and mixed GGO which had large area of solid lesion is common in adenocarcinoma.

Abstract:The incidence of pulmonary ground-glass opacity (GGO) lesions is increasing as a result of the widespread use of multislice spiral computed tomography (CT) and the low-dose CT screening for lung cancer detection.Besides benign lesions, GGOs can be a specific type of lung adenocarcinomas or their preinvasive lesions.Evaluation of pulmonary GGO and investigation of the correlation between CT imaging features and lung adenocarcinoma subtypes or driver genes can be helpful in confirming the diagnosis and in guiding the clinical management.Our review focuses on the pathologic characteristics of GGO detected at CT, involving histopathology and molecular pathology.
Impact of a Ground-glass Opacity Component on C-Stage IA Lung Adenocarcinoma

Abstract:Studies have shown that tumors with ground-glass opacity (GGO) components are associated with favorable outcomes. However, this view should be confirmed in an international cohort. We aimed to verify the impact of a GGO component on clinical (c)-stage IA lung adenocarcinoma and to describe the biological discrepancies between the part-solid and pure-solid groups. We evaluated 1333 cases of surgically resected c-stage IA lung adenocarcinomas, including 484 part-solid and 849 pure-solid tumors. Furthermore, we matched the solid size between the 2 groups and examined 470 patients. We compared the prognoses between the 2 groups before and after matching. The prognostic and biological differences were described before and after matching. Compared with the pure-solid group, the part-solid group was associated with favorable outcomes [5-year overall survival (OS) 99.4% vs 87.6%, P < 0.001; 5-year recurrence-free survival (RFS) 96.9% vs 82.2%, P < 0.001]. Similar results were obtained after matching (5-year OS 98.9% vs 92.2%, P = 0.012; 5-year RFS 95.0% vs 88.5%, P = 0.007). Multivariable analyses revealed that GGO component appearance was a factor of better OS and RFS. The part-solid tumor, regardless of the size of the solid component, had a similar outcome to the pure-solid tumor of c-stage T1a classification. Also, more epidermal growth factor receptor, human epidermal growth factor receptor-2 mutations, and receptor tyrosine kinase ROS-1-positive were observed in the part-solid group. In comparison, more wild types and Kirsten-Ras were observed in the pure-solid group. Adenocarcinomas with a GGO component were associated with superior outcomes. The GGO component should be considereda new clinical T descriptor. Early-stage lung adenocarcinomas with and without a GGO component may be 2 distinct tumor types.

Abstract:With the widespread use of multi-slice spiral computed tomography(CT) and the increasing level of diagnosis and treatment,the incidence of pulmonary ground glass opacity(GGO) is increasing.The etiology of GGO is complex and the pathological types are diverse.In addition to benign lesions,GGO can be a specific type of lung adenocarcinoma or its invasive lesions.Some scholars think it is closely related to early lung adenocarcinoma.Histopathology of lung cancer is crucial for the prognosis and treatment of lung cancer.However,under normal circumstances,the pathology of the patient can be known by surgery,bronchoscopy or puncture.However,many patients may not have the pathological conditions for obtaining the pathological specimen.CT as the primary method of diagnosis of lung adenocarcinoma,lung adenocarcinoma in the new classification,based on the lung GGO CT findings to determine its ability to provide a characteristic indicator of tumor histopathology in order to help GGO diagnosis and guidance of clinical treatment.

Abstract:OBJECTIVE:To explore the CT features of ground-glass nodules (GGN) including preinvasive lesions [atypical adenomatous hyperplasia (AAH) and adenocarcinoma in situ (AIS)], minimally invasive adenocarcinoma (MIA) and invasive adenocarcinoma (IAC).METHODS:Ninety-seven GGN lesions confirmed by operation pathology were included in this study. The lesions were divided into three groups: preinvasive lesion group (24 cases), MIA group (39 cases), IAC group (34 cases). The lesion size, 3-dimensional ratio, 2-dimensional ratio in axial images, lesion density, shape, speculation, lobulation, air-containing space and pleural indentation on the preoperative CT images in the three groups were analyzed and compared with pathological results. The data were statistically analyzed using SPSS 17.0.RESULTS:All preinvasive lesions presented as pure GGN on CT image, most showed round-like shape, clear and smooth border. MIA presented as pure GGN or mixed GGN on CT image, most showed round-like shape, with a clear and smooth border. IAC most presented as mixed GGN on CT image, often showed irregular shape. Speculation, lobulation, air-containing space and pleural indentation displayed gradually increasing from preinvasive lesions to MIA and IAC. There were statistically significant differences in lesion size, CT density, shape, air-containing space, speculation, pleural indentation and long diameter of solid component between the MIA and IAC groups (P < 0.05 for all). There were statistically significant differences in CT density values and long diameters of solid component of the lesions between the preinvasive lesion group and MIA group (P < 0.05). The AUC of solid component of the preinvasive lesion group and MIA group was 0.705, and that of the MIA and IAC groups was 0.814.CONCLUSION:Comprehensive analysis of the CT image features of GGNs, especially the solid component in the lesions, may help to the preoperative and differential diagnosis of preinvasive lesions, MIA and IAC.
Pathological invasiveness of lung adnenocarcinomas presenting as ground-glass opacity with different proportion of consolidation

Abstract:Objective To compare the difference of pathological invasiveness between lung adnenocarcinomas presenting as ground-glass opacity(GGO) with different proportion of consolidation.Methods CT and pathological data of 59 patients with adenocarcinoma confirmed by pathology presenting as GGO on CT were retrospectively studied.The thin-slice images of the focal lesions were analyzed and according to the maximal diameter ratio of the consolidation and the tumor,the patients were divided into main GGO group(G_G) and main consolidation group(G_C).The CT features and pathological invasiveness of the tumor were observed and compared.Results The diameter of the lesions was(2.71±1.08)cm.There were 8 of pure GGOs and 51 of mixed GGOs.Bubble-like low attenuation appeared in 43(43/59,72.88%) cases and most of the lesions(53/59,89.83%) had clear margin.There were 19 cases in G_G group and 40 cases in G_C group.CT features:There was not obvious difference of the tumor diameter,location,shape and clear margin between the two groups(all P0.05).The ratios of lobulated and(or) burr margin,sign of bubble-like attenuation and pleural indentation in G_C group were higher than those in G_G group(all P0.05).Pathology results:No differences of lymph node metastasis and cancer embolus were found(both P0.05) between the two groups.The rate of pleural invasion and the pathological staging were higher in G_C group than those in G_G group(both P0.05).Conclusion With increasing of the solid component ratio in lung adenocarcinas presenting as GGO,the lesions are tended to demonstrate malignant features in CT and high invasiveness in pathology.

Abstract:Background: Lung cancer can be detected in its early stages with computed tomography (CT). Early lung adenocarcinoma often is displayed as ground glass opacity (GGO), an entity that has been well studied over the past decade. However, few studies have focused on the correlation between CT characteristics and pathologic subtype of GGO. We aimed to explore the correlation between CT characteristics, pathologic subtype, and gene mutation associated with GGO in an effort to aid in the treatment of lung adenocarcinoma. Methods: In this retrospective study, patients with GGO who underwent surgery in our institution between 2013 and 2016 were included. Patients were divided into 2 groups on the basis of CT characteristics: group 1, diameter <20 mm and solid component <50%; and group 2, diameter >= 20 mm or solid component >= 50%. Differences in pathologic subtype and gene mutation pattern between groups were compared using the chi(2) test. The correlation between pathologic subtype and epidermal growth factor receptor (EGFR) mutation was also tested using the chi(2) test. Results: A total of 1018 cases (408 in group 1, 610 in group 2) were included; of these, 544 were tested for the EGFR gene mutation. There was a significant difference in predominant subtype (P < .001) and all included subtypes (P = .044) between the groups. Of 59 cases with the pathologic subtype of micropapillary or solid, 57 were in group 2. The EGFR gene mutation rate was significantly higher in group 2 than group 1 (P < .001) and significantly correlated with pathologic subtype (P < .001); adenocarcinoma in situ was the lowest (31.4%) and papillary was the highest (85.7%). EGFR mutation subtype did not significantly differ between groups (P = .499). Conclusions: CT characteristics of GGO significantly correlated with pathologic subtype and gene mutation rate. The EGFR mutation rate differed significantly among pathologic subtypes. GGOs with a diameter of < 20 mm and with a solid component < 50% seldom contain subtypes with poor prognosis (micropapillary and solid) and the EGFR mutation rate was significantly lower.

Abstract:Background. Along with the popularity of low-dose computed tomography lung cancer screening, an increasing number of lung ground-glass opacity (GGO) lesions are detected. This review focuses on lung adenocarcinoma manifesting as GGO. Methods. We performed a literature search of the PubMed/MEDLINE database to identify articles reporting GGO. The following terms were used: GGO, ground-glass opacity, GGN, ground-glass nodule, part-solid nodule, and subsolid nodule. Results. GGO is a nonspecific radiologic finding showing a hazy opacity without blocking underlying pulmonary vessels or bronchial structures. The pathology of GGO can be benign, preinvasive, or invasive adenocarcinoma. Although radiographic features may indicate malignancy, a short period of follow-up is the optimal method to distinguish between benign and malignant GGO lesions. Pathologically, not only lepidic, but also nonlepidic growth patterns can present as GGO. Lung adenocarcinoma with a GGO component is associated with excellent survival compared with solid lesions. Moreover, there are distinct prognostic factors in patients with lung adenocarcinoma manifesting as GGO or solid lesions. For selected GGO-featured lung adenocarcinoma, sublobar resection with selective or no mediastinal lymph node dissection may be sufficient. Intraoperative frozen section is an effective method to guide resection strategy. A less intensive postoperative surveillance strategy may be more appropriate given the excellent survival. Management of multiple GGO lesions requires comprehensive considerations of GGO characteristics and patient conditions. Conclusions. Lung adenocarcinoma manifesting as GGO defines a special clinical subtype with excellent prognosis. The management of GGO-featured lung adenocarcinoma should be distinct from that of solid lesions. Abstract:Background:Increasing evidence suggests that ground-glass opacity featured lung adenocarcinoma (GGO-LUAD) and pure solid-LUAD have significantly different tumor biological behaviors; the former is usually indolent. Genetic variations fundamentally contribute to this distinct tumor behaviors. This study aims to investigate and compare the gene mutations using next-generation sequencing (NGS) technology in these two subtypes of LUAD.Methods:The clinical data and gene testing results of 46 patients suffering from LUAD with a histologically invasive subtype ≤3 cm and operated in the Thoracic Surgery Department of Beijing Tsinghua Changgung Hospital from May 2019 to December 2022 were retrospectively analyzed; a case-control study was performed to compare the pathological and genetic differences between LUAD with a GGO component and pure solid-LUAD.Results:Notable differences existed in vascular invasion, tumor spread through air spaces (STAS) and high-risk histological subtypes (micropapillary or solid subtypes) between the two types of LUAD with similar histologically invasive size. No significant difference was found in the mutation frequency of EGFR and KRAS. However, gene mutations were more prevalent in the cell cycle and TP53 signaling pathway for solid-LUAD. A significant difference was found in the mutation frequency of the tumor suppressor genes TP53 and CDKN2A between the two types.Conclusions:The wild-type TP53 and CDKN2A genes could potentially be used as molecular indicators for indolent LUAD characterized by GGO-featured.

Abstract:OBJECTIVE:To explore the clinicopathological characteristics and imaging features of lung adenocarcinoma with a micropapillary pattern (MPP).METHODS:Eighty cases of pulmonary adenocarcinoma with a micropapillary pattern treated in our hospital from July 2011 to December 2012 were selected to retrospectively analyze their clinicopathological characteristics and imaging features.RESULTS:Among the 80 cases of lung adenocarcinoma with MPP, there were 38 cases of stage I (47.5%), 12 cases of stage II (15.0%), 25 cases of stage III (31.3%) and 5 cases of stage IV (6.2%). There were 14 cases of moderately differentiated (17.5%) and moderately/poorly differentiated (82.5%) tumors. Sixty-three cases had pleural involvement, vascular invasion, involving the bronchial wall, invasion of large vessels, nerve invasion, and lymph node metastasis (at least one of them) (78.8%). Immunohistochemical staining revealed that both positive rates of TTF-1 and CK7 were 100%, and that of pulmonary surfactant apolipoprotein-A (SPA) was 84.0%. Imaging examination revealed hilar or mediastinal lymph node enlargement in 15 cases (18.8%). but the pathology confirmed hilar or mediastinal lymph node metastasis in 36 cases (45.0%). Lung CT imaging showed that the majority of the cases were peripheral type, and only a few of central type, and most cases were solid lesions, with lobulation, spiculation, pleural indentation, and vascular convergence sign, while there were few ground-glass opacity sign and vacuole sign.CONCLUSIONS:Lung adenocarcinoma with MPP component often presents with early invasions of pleura, blood vessels, lymphatic vessels, and lymph nodes. Imaging manifestation of this cancer mainly shows as peripheral and solid lesions, often with lobulation, spiculation, pleural indentation, vascular convergence sign, but GGO and vacuole signs are unusual. Overexpression of TTF-1, CK7 and SPA, and elevated CEA level are associated with clinical staging of the disease.

Abstract:Abstract Objective Pure ground-glass nodules (pGGNs) are considered to be radiologically noninvasive in lung adenocarcinoma. However, some pGGNs are found to be invasive adenocarcinoma (IAC) pathologically. This study aims to identify the CT parameters distinguishing IAC from adenocarcinoma in situ (AIS) and minimally invasive adenocarcinoma (MIA). Methods From May 2011 to December 2015, patients with completely resected adenocarcinoma appearing as pGGNs were reviewed. To evaluate the association between CT features and the invasiveness of pGGNs, logistic regression analyses were conducted. Results Among 432 enrolled patients, 118 (27.3%) were classified as AIS, 213 (49.3%) as MIA, 101 (23.4%) as IAC. There was no postoperative recurrence for patients with pGGNs. Logistic regression analyses demonstrated that CT size was the only independent radiographic factor associated with IAC (odds ratio [OR]: 47.165, 95% confidence interval [CI]: 19.279-115.390, P Conclusions In patients with pGGNs, CT size was the only radiographic parameter associated with tumor invasion. Measuring CT density provided no advantage in differentiating IAC from AIS and MIA.
CT Characterization of Different Pathological Types of Subcentimeter Pulmonary Ground-Glass Nodular Lesions.

Abstract:Objective To explore the CT characteristics of small lung nodules and improve the diagnosis of pulmonary ground-glass nodules less than 10 mm in size. Methods We retrospectively analyzed CT images of 161 pulmonary nodules (less than 10 mm in size) with spiculation, lobulation, vacuoles, and pleural indentation and compared these images with pathological results or follow-up CT images. The relationships between the ground-glass nodules (GGNs) and blood vessels were observed. The GGN-vessel relationship was divided into four types, Type I (pass-by), Type II (pass-through), Type III (distorted/dilated), Type IV (complicated). The vessels traveling through a GGN were divided into three categories, category A (arteries), category B (veins), category C (arteries and veins). Results 161 GGNs were divided into three groups (benign group, pre-invasive group, and adenocarcinoma group) according to their pathological diagnosis. Significant differences in density of nodules were observed among the three different groups (p < 0.05). Significant differences in the shape (round/round-like or not) of the nodules were observed between the benign group and the pre-invasive group and between the pre-invasive group and the adenocarcinoma group (p < 0.05). No significant differences in the presence of vacuoles were observed between the benign group and the pre-invasive group or between the pre-invasive group and the adenocarcinoma group (p(sic)05), but a significant difference was observed between the benign group and the adenocarcinoma group (p < 0.05). The differences in the vascularization of the lesions among the three groups were statistically significant (p < 0.05). No significant differences or correlations were observed between vascular categories and GGN groups (p > 0.05). Conclusion For subcentimeter nodules, mixed GGNs with vacuoles, well-defined border, combined with Type III or Type IV GGN-vessel relationship may strongly suggest malignant. Abstract:Aim: To investigate the prognostic impact of computed tomography (CT)-defined ground glass opacity (GGO) in patients with clinical stage I-IIA grade 3 invasive non-mucinous pulmonary adenocarcinoma (INPA). Materials and methods: The present study retrospectively enrolled 187 patients diagnosed with stage I-IIA grade 3 INPA. Their clinicopathological, radiological, and genetic information was evaluated systematically, and a 5-year follow-up was conducted to monitor disease recurrence and mortality. Patients were stratified based on the presence of a GGO component, and the Cox proportional hazard model was employed to assess the influence of clinicopathological factors and genetic variables on tumour outcomes. Recurrence-free survival (RFS) and overall survival (OS) were estimated using the Kaplan-Meier method and compared using the log-rank test. Results: Significant differences were observed in both OS and RFS based on the presence of a GGO component. The group with GGO exhibited superior OS (p=0.002) and RFS (p=0.029). Multivariate analysis revealed that the presence of a GGO component (hazard ratio [HR] = 0.412, 95% confidence interval [CI]: 0.177-0.959, p=0.040), clinical T2 stage (HR=2.473, 95% CI: 1.498-4.083, p<0.001), pathological N2 stage (HR=3.049, 95% CI: 1.800-5.167, p<0.001), and mixed high-grade patterns (HR=2.392, 95% CI: 1.418-4.036, p=0.001) were predictors of RFS. Conclusion: The presence of a GGO component is strongly associated with a favourable prognosis in grade 3 INPA.

Abstract:Risk factors for stage I lung adenocarcinoma were analyzed using low-dose high-resolution computed tomography (CT). The patients were divided into case group (stage I lung adenocarcinoma patients) and control group (benign pulmonary nodules patients). All patients were subjected to low-dose high-resolution CT. Multiple linear regression was performed to analyze the CT imaging features of the two groups. Stage I lung adenocarcinoma patients were significantly associated with nodular site (X-3, upper left lobe) [95% CI (1.796, 54.695), p=0.008], nodule type (X-4) (p<0.001), nodule size (X-5) [95% CI (0.614, 0.803), p<0.001], spicule sign (X-7) [95% CI (0.029, 0.580), p=0.008], lobulation sign (X-8) [95% CI (0.048, 0.673), p=0.011]. The stepwise regression equation is: Logistic (p) =-12.009 + 2.294X(3) - 0.327X(4) - 0.354X(5) - 2.042X(7) - 1.713X(8). Risk factors of low-dose and high-resolution CT imaging for patients with stage I lung adenocarcinoma are nodular site (upper left lobe), nodule type, nodule size, spicule sign, and lobulation sign.

Abstract:Purpose To evaluate the correlations between high resolution CT (HRCT) findings and IASLC/ATS/ERS pathological classification of ground glass nodule (GGN). Materials and Methods 121 patients with confirmed GGN were selected, and divided into benign group (22 cases), PIL group (21 cases), microinvasive carcinoma group (26 cases) and invasive carcinoma group (52 cases), then the imaging, pathology and prognosis data of patients with pulmonary GGN were reviewed, and the differences among GGN of different pathological types were analyzed.Results Maximum diameter, margin, vacuole sign, solid component, shape and blood vessels through of GGN were significantly different among the four groups (χ2=9.945-31.068,P<0.05). Maximum diameter and margin were significantly different between invasive adenocarcinoma and other groups (P<0.008); vacuole sign of the benign group was significantly different with other groups (P<0.008); the existence of solid component and shape were significantly different between invasive adenocarcinoma and minimally invasive adenocarcinoma (P<0.008); there was significant difference of blood vessels through between invasive adenocarcinoma and benign lesions (P<0.008). Among the 121 lesions, no metastasis except one invasive adenocarcinoma case complicated with distant metastasis.Conclusion Maximum diameter of GGN greater than 16.35 mm, with spiculation or lobulation represent invasive adenocarcinoma; vacuole sign within the GGN represent malignancy; with solid component and irregular shape can be used to identify invasive adenocarcinoma from minimally invasive adenocarcinoma; while blood vessels through can be used to identify invasive adenocarcinoma from benign lesions; the prognosis of GGNs is well with only 0.83% probability of distant metastasis.

Abstract:Pulmonary ground-glass opacity (GGO) has raised increasing attention of clinical oncologists and thoracic surgeons in recent years. GGO appears as hazy increased opacity of lung, with preservation of bronchial and vascular margins. GGO is less opaque than consolidation, in which bronchovascular margins are obscured. In most cases, GGOs are indolent, while in other cases, they may grow malignant. This pattern makes it difficult to find a standard way to treat patients with GGO. Moreover, with the improvement of technology, more and more GGOs are found in patients' computed tomgraphy (CT) scan, making it a global problem. In the literature, many clinicians have reported their findings on this topic from histologic, radiologic and therapeutic perspectives. In this study, we reviewed the development on this topic in recent 10 years. We hope our study can be helpful for clinicians to better understand this problem, collect more data in their clinical work and most importantly, let evidence guide our future therapeutic choices.
Morphological Classification of Pre-Invasive Lesions and Early-Stage Lung Adenocarcinoma Based on CT Images.

Abstract:To retrospectively analyze the computed tomography (CT) features in patients with pre-invasive lesions and early-stage lung adenocarcinoma and to explore the correlation between tumor morphological changes and pathological diagnoses. CT morphological characteristics in 2106 patients with pre-invasive (stage 0) and early-stage (stage I) lung adenocarcinoma were analyzed; lesions were confirmed by surgical pathology. Based on the morphological characteristics, the lesions were divided into eight types: I (cotton ball, ground-glass nodules), II (solid fill), III (granular), IV (dendriform), V (bubble-like lucencies), VI (alveolate or honeycomb), VII (scar-like), and VIII (notched or umbilication). The different distributions of eight morphological types in pathological types of the lesions and subtypes of invasive adenocarcinoma were analyzed by chi-squared or Fisher’s exact test. Correlation between the percentage of ground-glass opacity in the lesions and pathology types were analyzed by two-tailed Pearson’s test. A negative correlation was observed between the pathological types and proportion of ground-glass component in the lesions (p < 0.001 and r = − 0.583). Significant differences in morphological characteristics among various pathological types of pre-invasive lesions and early lung adenocarcinomas were observed (p < 0.05). Furthermore, among the different pathological subtypes of stage I invasive adenocarcinoma, the differences in their manifestation as morphological types I, II, III, and VI were statistically significant (p < 0.05). The eight types of morphological classification of pre-invasive lesions and early-stage (stage 0 or stage I) lung adenocarcinoma has different pathological bases, and morphological classification may be useful for the diagnosis and differential diagnosis of lung adenocarcinoma. • CT morphological classification of pre-invasive lesions and lung adenocarcinoma is intuitive. • CT morphological classification characterizes morphological changes of the entire lesion. • Different pathological types of lung adenocarcinoma have different morphological features.
