Abstract:Objectives: To investigate the performance of radiomic-based quantitative analysis on CT images in predicting invasiveness of lung adenocarcinoma manifesting as pure ground-glass nodules (pGGNs). Methods: A total of 275 lung adenocarcinoma cases, with 322 pGGNs resected surgically and confirmed pathologically, from January 2015 to October 2017 were enrolled in this retrospective study. All nodules were split into training and test cohorts randomly with a ratio of 4:1 to establish models to predict between pGGN-like adenocarcinoma in situ (AIS)/minimally invasive adenocarcinoma (MIA) and invasive adenocarcinoma (IVA). Radiomic feature extraction was performed using Pyradiomics with semi-automatically segmented tumor regions on CT scans that were contoured with an in-house plugin for 3D-Slicer. Random forest (RF) and support vector machine (SVM) were used for feature selection and predictive model building in the training cohort. Three different predictive models containing conventional, radiomic, and combined models were built on the basis of the selected clinical, radiological, and radiomic features. The predictive performance of each model was evaluated through the receiver operating characteristic curve (ROC) and the area under the curve (AUC). The predictive performance of two radiologists (A and B) and our radiomic predictive model were further investigated in the test cohort to see if radiomic predictive model could improve radiologists' performance in prediction between pGGN-like AIS/MIA and IVA. Results: Among 322 nodules, 48 (14.9%) were AIS and 102 (31.7%) were MIA with 172 (53.4%) for IVA. Age, diameter, density, and nine meaningful radiomic features were selected for model building in the training cohort. Three predictive models showed good performance in prediction between pGGN-like AIS/MIA and IVA (AUC > 0.8, P < 0.05) in both training and test cohorts. The AUC values in the test cohort were 0.824 (95% CI, 0.723-0.924), 0.833 (95% CI, 0.733-0.934), and 0.848 (95% CI, 0.750-0.946) for conventional, radiomic, and combined models, respectively. The predictive accuracy was 73.44 and 59.38% for radiologist A and radiologist B in the test cohort and was improved dramatically to 79.69 and 75.00% with the aid of our radiomic predictive model. Conclusion: The predictive models built in our study showed good predictive power with good accuracy and sensitivity, which provided a non-invasive, convenient, economic, and repeatable way for the prediction between IVA and AIS/MIA representing as pGGNs. The radiomic predictive model outperformed two radiologists in predicting pGGN-like AIS/MIA and IVA, and could significantly improve the predictive performance of the two radiologists, especially radiologist B with less experience in medical imaging diagnosis. The selected radiomic features in our research did not provide more useful information to improve the combined predictive model's performance.

Abstract:Abstract Background Clinically differentiating preinvasive lesions (atypical adenomatous hyperplasia, AAH and adenocarcinoma in situ, AIS) from invasive lesions (minimally invasive adenocarcinomas, MIA and invasive adenocarcinoma, IA) manifesting as ground-glass opacity nodules (GGOs) is difficult due to overlap of morphological features. Hence, the current study was performed to explore the diagnostic efficiency of radiomics in assessing the invasiveness of lung adenocarcinoma manifesting as GGOs. Methods A total of 1018 GGOs pathologically confirmed as lung adenocarcinoma were enrolled in this retrospective study and were randomly divided into a training set (n = 712) and validation set (n = 306). The nodules were delineated manually and 2446 intra-nodular and peri-nodular radiomic features were extracted. Univariate analysis and least absolute shrinkage and selection operator (LASSO) were used for feature selection. Clinical and semantic computerized tomography (CT) feature model, radiomic model and a combined nomogram were constructed and compared. Decision curve analysis (DCA) was used to evaluate the clinical value of the established nomogram. Results 16 radiomic features were selected and used for model construction. The radiomic model exhibited significantly better performance (AUC = 0.828) comparing to the clinical-semantic model (AUC = 0.746). Further analysis revealed that peri-nodular radiomic features were useful in differentiating between preinvasive and invasive lung adenocarcinomas appearing as GGOs with an AUC of 0.808. A nomogram based on lobulation sign and radiomic features showed the best performance (AUC = 0.835), and was found to have potential clinical value in assessing nodule invasiveness. Conclusions Radiomic model based on both intra-nodular and peri-nodular features showed good performance in differentiating between preinvasive lung adenocarcinoma lesions and invasive ones appearing as GGOs, and a nomogram based on clinical, semantic and radiomic features could provide clinicians with added information in nodule management and preoperative evaluation.

Abstract:Objective: To evaluate whether radiomic features extracted from intra and peri-nodular lesions can enhance the ability to differentiate between invasive adenocarcinoma (IA), minimally invasive adenocarcinoma (MIA), and adenocarcinoma in situ (AIS) manifesting as ground-glass nodule (GGN).Materials and Methods: This retrospective study enrolled 120 patients with a total of 121 pathologically confirmed lung adenocarcinomas (85 IA and 36 AIS/MIA) from January 2015 to May 2019. The recruited patients were randomly divided into training (84 nodules) and validation sets (37 nodules), with a ratio of 7:3. The minority group in the training set was balanced by the synthetic minority over-sampling (SMOTE) method. The intra-, peri-nodular, and gross region of interests (ROI) were delineated with manual annotation. Image features were quantitatively extracted from each ROI on CT images. The minimum redundancy maximum relevance (mRMR) feature ranking method and the least absolute shrinkage and selection operator (LASSO) classifier were used to eliminate unnecessary features. The intra- and peri-nodular radiomic features were combined to produce the gross radiomic signature. A combined clinical-radiomic model was constructed by multivariable logistic regression analysis. The predicted performances of different models were evaluated using receiver operating curve (ROC) and calibration curve.Results: The gross radiomic signature (AUC: training set = 0.896; validation set = 0.876) showed a good ability to discriminate the invasiveness of adenocarcinoma, comparing to intra-nodular (AUC: training set = 0.862; validation set = 0.852) or peri-nodular radiomic signature (AUC: training set = 0.825; validation set = 0.820). The AUC of the combined clinical-radiomic model was 0.917 for the training and 0.876 for the validation cohort, respectively.Conclusions: The gross radiomic signature of intra- and peri-nodular regions improved the prediction ability and aided predicting the invasiveness of lung adenocarcinoma appearing as GGN.

Abstract:OBJECTIVE:To develop a radiomic model based on low-dose CT (LDCT) to distinguish invasive adenocarcinomas (IAs) from adenocarcinoma in situ/minimally invasive adenocarcinomas (AIS/MIAs) manifesting as pure ground-glass nodules (pGGNs) and compare its performance with conventional quantitative and semantic features of LDCT, radiomic model of standard-dose CT, and intraoperative frozen section (FS).METHODS:A total of 147 consecutive pathologically confirmed pGGNs were divided into primary cohort (43 IAs and 60 AIS/MIAs) and validation cohort (19 IAs and 25 AIS/MIAs). Logistic regression models were built using conventional quantitative and semantic features, selected radiomic features of LDCT and standard-dose CT, and intraoperative FS diagnosis, respectively. The diagnostic performance was assessed by area under curve (AUC) of receiver operating characteristic curve, sensitivity, and specificity.RESULTS:The AUCs of quantitative-semantic model, radiomic model of LDCT, radiomic model of standard-dose CT, and FS model were 0.879 (95% CI, 0.801-0.935), 0.929 (95% CI, 0.862-0.971), 0.941 (95% CI, 0.876-0.978), and 0.884 (95% CI, 0.805-0.938) in the primary cohort and 0.897 (95% CI, 0.768-0.968), 0.933 (95% CI, 0.815-0.986), 0.901 (95% CI, 0.773-0.970), and 0.828 (95% CI, 0.685-0.925) in the validation cohort. No significant difference of the AUCs was found among these models in both the primary and validation cohorts (all p > 0.05).CONCLUSION:The LDCT-based quantitative-semantic score and radiomic signature, with good predictive performance, can be pre-operative and non-invasive biomarkers for assessing the invasive risk of pGGNs in lung cancer screening.ADVANCES IN KNOWLEDGE:The LDCT-based quantitative-semantic score and radiomic signature, with the equivalent performance to the radiomic model of standard-dose CT, can be pre-operative predictors for assessing the invasiveness of pGGNs in lung cancer screening and reducing excess examination and treatment.

Abstract:Objectives To investigate the value of radiomics based on CT imaging in predicting invasive adenocarcinoma manifesting as pure ground-glass nodules (pGGNs). Methods This study enrolled 395 pGGNs with histopathology-confirmed benign nodules or adenocarcinoma. A total of 396 radiomic features were extracted from each labeled nodule. A Rad-score was constructed with the least absolute shrinkage and selection operator (LASSO) in the training set. Multivariate logistic regression analysis was conducted to establish the radiographic model and the combined radiographic–radiomics model. The predictive performance was validated by receiver operating characteristic (ROC) curve. Based on the multivariate logistic regression analysis, an individual prediction nomogram was developed and the clinical utility was assessed. Results Five radiomic features and four radiographic features were selected for predicting the invasive lesions. The combined radiographic–radiomics model (AUC 0.77; 95% CI, 0.69–0.86) performed better than the radiographic model (AUC 0.71; 95% CI, 0.62–0.81) and Rad-score (AUC 0.72; 95% CI, 0.63–0.81) in the validation set. The clinical utility of the individualized prediction nomogram developed using the Rad-score, margin, spiculation, and size was confirmed in the validation set. The decision curve analysis (DCA) indicated that using a model with Rad-score to predict the invasive lesion would be more beneficial than that without Rad-score and the clinical model. Conclusions The proposed radiomics-based nomogram that incorporated the Rad-score, margin, spiculation, and size may be utilized as a noninvasive biomarker for the assessment of invasive prediction in patients with pGGNs. Key Points • CT-based radiomics analysis helps invasive prediction manifested as pGGNs. • The combined radiographic–radiomics model may be utilized as a noninvasive biomarker for predicting invasive lesion for pGGNs. • Radiomics-based individual nomogram may serve as a vital decision support tool to identify invasive pGGNs, obviating further workup and blind follow-up.

Abstract:The aim of the present study was to develop and validate a radiomics-based nomogram for differentiation of pre-invasive lesions from invasive lesions that appearing as ground-glass opacity nodules (GGNs) ≤10 mm (sub-centimeter) in diameter at CT. A total of 542 consecutive patients with 626 pathologically confirmed pulmonary subcentimeter GGNs were retrospectively studied from October 2011 to September 2017. All the GGNs were divided into a training set (n = 334) and a validation set (n = 292). Researchers extracted 475 radiomics features from the plain CT images; a radiomics signature was constructed with the least absolute shrinkage and selection operator (LASSO) based on multivariable regression in the training set. Based on the multivariable logistic regression model, a radiomics nomogram was developed in the training set. The performance of the nomogram was evaluated with respect to its calibration, discrimination, and clinical-utility and this was assessed in the validation set. The constructed radiomics signature, which consisted of 15 radiomics features, was significantly associated with the invasiveness of subcentimeter GGNs (P < 0.0001 for both training set and validation set). To build the nomogram model, radiomics signature and mean CT value were used. The nomogram model demonstrated good discrimination and calibration in both training set (C-index, 0.716 [95% CI, 0.632 to 0.801]) and validation set (C-index, 0.707 [95% CI, 0.625 to 0.788]). Decision curve analysis (DCA) indicated that radiomics-based nomogram was clinically useful. A radiomics-based nomogram that incorporates both radiomics signature and mean CT value is constructed in the study, which can be conveniently used to facilitate the preoperative individualized prediction of the invasiveness in patients with subcentimeter GGNs.
Hybrid Clinical-Radiomics Model for Precisely Predicting the Invasiveness of Lung Adenocarcinoma Manifesting as Pure Ground-Glass Nodule

Abstract:<h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Rationale and Objectives</h3><p>To identify whether the radiomics features of computed tomography (CT) allowed for the preoperative discrimination of the invasiveness of lung adenocarcinomas manifesting as pure ground-glass nodules (pGGNs) and further to develop and compare different predictive models.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Materials and Methods</h3><p>We retrospectively included 187 lung adenocarcinomas presenting as pGGNs (66 preinvasive lesions and 121 invasive lesions), which were randomly divided into the training and test sets (8:2). Radiomics features were extracted from non-enhanced CT images. Clinical features, including patient's demographic characteristics, smoking status, and conventional CT features that reflect tumor's morphology and surrounding information were also collected. Intraclass correlation coefficient and ℓ<sub>2.1</sub>-norm minimization were used to identify influential feature subset which was then used to build three predictive models (clinical, radiomics, and clinical-radiomics models) with the gradient boosting regression tree classifier. The performances of the predictive models were evaluated using the area under the curve (AUC).</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Results</h3><p>Of the 1409 radiomics features and 27 clinical feature subtypes, 102 features were selected to construct the hybrid clinical-radiomics model, which achieved the best discriminative power (AUC = 0.934 and 0.929 in training and test set). The radiomics model showed comparable predictive performance (AUC = 0.911 and 0.901 in training and test set) compared to the clinical model (AUC = 0.911 and 0.894 in training and test set).</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Conclusion</h3><p>The radiomics model showed good predictive performance in discriminating invasive lesions from preinvasive lesions for lung adenocarcinomas presenting as pGGNs. Its performance can be further improved by adding clinical features.</p>

Use of a Radiomics Model to Predict Tumor Invasiveness of Pulmonary Adenocarcinomas Appearing As Pulmonary Ground-Glass Nodules

Abstract:Background. It is important to distinguish the classification of lung adenocarcinoma. A radiomics model was developed to predict tumor invasiveness using quantitative and qualitative features of pulmonary ground-glass nodules (GGNs) on chest CT. Materials and Methods. A total of 599 GGNs [including 202 preinvasive lesions and 397 minimally invasive and invasive pulmonary adenocarcinomas (IPAs)] were evaluated using univariate, multivariate, and logistic regression analyses to construct a radiomics model that predicted invasiveness of GGNs. In primary cohort (comprised of patients scanned from August 2012 to July 2016), preinvasive lesions were distinguished from IPAs based on pure or mixed density (PM), lesion shape, lobulated border, and pleural retraction and 35 other quantitative parameters (P < 0.05) using univariate analysis. Multivariate analysis showed that PM, lobulated border, pleural retraction, age, and fractal dimension (FD) were significantly different between preinvasive lesions and IPAs. After logistic regression analysis, PM and FD were used to develop a prediction nomogram. The validation cohort was comprised of patients scanned after Jan 2016. Results. The model showed good discrimination between preinvasive lesions and IPAs with an area under curve (AUC) of 0.76 [95% CI: 0.71 to 0.80] in ROC curve for the primary cohort. The nomogram also demonstrated good discrimination in the validation cohort with an AUC of 0.79 [95% CI: 0.71 to 0.88]. Conclusions. For GGNs, PM, lobulated border, pleural retraction, age, and FD were features discriminating preinvasive lesions from IPAs. The radiomics model based upon PM and FD may predict the invasiveness of pulmonary adenocarcinomas appearing as GGNs.

Abstract:This study aims to develop a CT-based radiomic features analysis approach for diagnosis of groundg-lass opacity (GGO) pulmonary nodules, and also assess whether computer-aided diagnosis (CADx) performance changes in classifying between benign and malignant nodules associated with histopathological subtypes namely, adenocarcinoma in situ (AIS), minimally invasive adenocarcinoma (MIA), and invasive adenocarcinoma (IAC), respectively. The study involves 182 histopathology-confirmed GGO nodules collected from two cancer centers. Among them, 59 are benign, 50 are AIS, 32 are MIA, and 41 are IAC nodules. Four training/ testing data sets-(1) all nodules, (2) benign and AIS nodules, (3) benign and MIA nodules, (4) benign and IAC nodules-are assembled based on their histopathological subtypes. We first segment pulmonary nodules depicted in CT images by using a 3D region growing and geodesic active contour level set algorithm. Then, we computed and extracted 1117 quantitative imaging features based on the 3D segmented nodules. After conducting radiomic features normalization process, we apply a leave-one-out cross-validation (LOOCV) method to build models by embedding with a Relief feature selection, synthetic minority oversampling technique (SMOTE) and three machine-learning classifiers namely, support vector machine classifier, logistic regression classifier and Gaussian Nave Bayes classifier. When separately using four data sets to train and test three classifiers, the average areas under receiver operating characteristic curves (AUC) are 0.75, 0.55, 0.77 and 0.93, respectively. When testing on an independent data set, our scheme yields higher accuracy than two radiologists (61.3% versus radiologist 1: 53.1% and radiologist 2: 56.3%). This study demonstrates that: (1) the feasibility of using CT-based radiomic features analysis approach to distinguish between benign and malignant GGO nodules, (2) higher performance of CADx scheme in diagnosing GGO nodules comparing with radiologist, and (3) a consistently positive trend between classification performance and invasive grade of GGO nodules. Thus, to improve the CADx performance in diagnosing of GGO nodules, one should assemble an optimal training data set dominated with more nodules associated with non-invasive lung adenocarcinoma (i.e. AIS and MIA).
A comparative study for the evaluation of CT-based conventional, radiomic, combined conventional and radiomic, and delta-radiomic features, and the prediction of the invasiveness of lung adenocarcinoma manifesting as ground-glass nodules

Abstract:Aim: To investigate and compare the performance of conventional, radiomic, combined, and delta-radiomic features to predict the invasiveness of lung adenocarcinoma manifesting as ground-glass nodules (GGNs). Materials and methods: The present retrospective study included 216 GGNs confirmed surgically as pulmonary adenocarcinomas. All the thin-section computed tomography (CT) images were imported into the software of the United Imaging Intelligence research portal, and radiomic features were extracted with three-dimensional (3D) regions of interest. Least Absolute Shrinkage and Selection Operator was used to select the optimal radiomic features. Four models were constructed, including conventional, radiomic, combined conventional and radiomic, and delta-radiomic models. The receiver operating characteristic curves were built to evaluate the validity of these. Results: The type, long diameter, shape, margin, vacuole, air bronchus, vascular convergence, and pleural traction exhibited significant differences between pre-invasive lesions (PILs)/minimally invasive adenocarcinoma (MIA), and invasive adenocarcinoma (IA) groups were selected for conventional model building. Nine radiomic features were selected to build the radiomic model. The four models indicated optimal performance (AUC > 0.7). The radiomic and combined models exhibited the highest diagnostic efficiency, and their AUC were 0.89 and 0.88 in the training set, and 0.87 and 0.88 in the validation set, respectively. The delta-radiomic model indicated that the AUC was 0.83 in the training set, and 0.76 in the validation set. Finally, the conventional model exhibited an AUC in the training and validation sets of 0.78 and 0.76. Conclusions: The radiomic model and combined model, in particular, and the delta-radiomic model all demonstrated improved diagnostic efficiency in differentiating IA from PIL/MIA than that of the conventional model.

Methods: A retrospective analysis was conducted on the data of T1-stage lung adenocarcinoma patients who underwent surgical resection and whose preoperative CT scans revealed pGGNs from June 2020 to June 2023 in our hospital. 3D Slicer was used to extract radiomic features of the intratumoral (VOI entire) and peritumoral regions (VOI +2 mm), and Rad-scores were calculated from the coefficients of features obtained after dimensionality reduction with LASSO regression. Results: A total of 131 patients with T1-stage lung adenocarcinoma presenting as pGGNs were included in this study; of these, 84 were pathologically diagnosed with the lepidic-predominant (LPA) subtype, and 47 were diagnosed with non-LPA. The diagnostic performance of the VOI entire and VOI +2 mm features for the pathological subtype of pGGN was superior to that of conventional features, with the VOI +2 mm features showing the best performance: the area under the curve, sensitivity, specificity, and accuracy in the training set were 0.883, 0.964, 0.667, and 0.761, respectively. Conclusion: Intra- and especially peritumoral-based radiomic features have high diagnostic performance for the pathological subtype of T1-stage lung adenocarcinoma presenting as pGGNs.

Abstract:It is vital to distinguish indolent pulmonary adenocarcinomas from invasive pulmonary adenocarcinomas before surgery.Radiomics is a cutting-edge technology that mines quantitative features from CT images.We designed a nomogram, which incorporated clinical and CT morphological characteristics with the radiomics signature.We applied the radiomics nomogram to preoperatively predict the invasiveness of GGNs.In this study, we aimed to establish a radiomics nomogram that noninvasively evaluates the invasiveness of pulmonary adenocarcinomas manifesting as ground-glass nodules (GGNs). Computed tomography (CT) images of 509 patients manifesting as GGNs were collected: 70% of cases were included in the training cohort and 30% in the validation cohort. The Max-Relevance and Min-Redundancy (mRMR) and the least absolute shrinkage and selection operator (LASSO) algorithm were used to select the radiomics features and construct a radiomics signature. Univariate and multivariate logistic regression were used to select the invasiveness-related clinical and CT morphological predictors. Age, smoking history, long diameter, and average CT value were retained as independent predictors of GGN invasiveness. A radiomics nomogram was established by integrating clinical and CT morphological features with the radiomics signature. The radiomics nomogram showed good predictive ability in the training set (area under the curve [AUC], 0.940; 95% confidence interval [CI], 0.916–0.964) and validation set (AUC, 0.946; 95% CI, 0.907–0.986). This radiomics nomogram may serve as a noninvasive and accurate predictive tool to determine the invasiveness of GGNs prior to surgery and assist clinicians in creating personalized treatment strategies.

Abstract:To evaluate the efficacy of radiomics features extracted from preoperative high-resolution computed tomography (HRCT) scans in distinguishing benign and malignant pulmonary pure ground-glass nodules (pGGNs), a retrospective study of 395 patients from 2016 to 2020 was conducted. All nodules were randomly divided into the training and validation sets in the ratio of 7:3. Radiomics features were extracted using MaZda software (version 4.6), and the least absolute shrinkage and selection operator (LASSO) was employed for feature selection. Significant differences were observed in the training set between benign and malignant pGGNs in sex, mean CT value, margin, pleural retraction, tumor-lung interface, and internal vascular change, and then the mean CT value and the morphological features model were constructed. Fourteen radiomics features were selected by LASSO for the radiomics model. The combined model was developed by integrating all selected radiographic and radiomics features using logistic regression. The AUCs in the training set were 0.606 for the mean CT value, 0.718 for morphological features, 0.756 for radiomics features, and 0.808 for the combined model. In the validation set, AUCs were 0.601, 0.692, 0.696, and 0.738, respectively. The decision curves showed that the combined model demonstrated the highest net benefit.

Abstract:AIM: To investigate the value of computed tomography (CT) quantitative parameters in predicting the invasiveness of lung pure ground-glass nodules (pGGNs). MATERIALS AND METHODS: Chest CT images and pathological findings of 163 pGGNs in 154 consecutive patients were reviewed. According to the clinical management strategies, cases were divided into pre-invasive and MIA groups (atypical adenomatous hyperplasia [AAH], adenocarcinoma in situ [AIS], and minimally invasive adenocarcinoma [MIA]) and invasive group (invasive adenocarcinoma [IAC]). CT quantitative parameters including maximum diameter, largest diameter perpendicular to the maximum diameter, maximum cross-sectional area, volume, mass, and mean attenuation value were measured and compared between two groups. Their diagnostic performances were evaluated using receiver operating characteristic (ROC) and logistic regression analysis. RESULTS: Significant differences existed for all the CT quantitative parameters in both groups (p<0.01). The values of area under the curve (AUC) were 0.783 of maximum diameter (95% CI: 0.711-0.843), 0.779 of longest diameter perpendicular to maximum diameter (95% CI: 0.707-0.840), 0.796 of largest cross-sectional area (95% CI: 0.726-0.855), 0.781 of volume (95% CI: 0.710-0.842), 0.794 of mass (95% CI: 0.722-0.865) and 0.625 of mean attenuation value (95% CI: 0.546-0.700), respectively. A pairwise-manner comparison showed the AUC of mean attenuation value was the smallest (p<0.01). Logistic regression analysis showed the largest cross-sectional area (OR = 2.307, 95% CI: 1.689-3.150) was the independent predictor for IAC with a cut-off value of 2.22 cm(2). CONCLUSIONS: CT quantitative parameters could predict the invasiveness of lung pGGNs. Abstract:Objectives: To explore the value of radiomics based on Dual-energy CT (DECT) for discriminating preinvasive or MIA from IA appearing as GGNs before surgery. Methods: The retrospective study included 92 patients with lung adenocarcinoma comprising 30 IA and 62 preinvasive-MIA, which were further divided into a training (n=64) and a test set (n=28). Clinical and radiographic features along with quantitative parameters were recorded. Radiomics features were derived from virtual monoenergetic images (VMI), including 50kev and 150kev images. Intraclass correlation coefficients (ICCs), Pearson's correlation analysis and least absolute shrinkage and selection operator (LASSO) penalized logistic regression were conducted to eliminate unstable and redundant features. The performance of the models was evaluated by area under the curve (AUC) and the clinical utility was assessed using decision curve analysis (DCA). Results: The DECT-based radiomics model performed well with an AUC of 0.957 and 0.865 in the training and test set. The clinical-DECT model, comprising sex, age, tumor size, density, smoking, alcohol, effective atomic number, and normalized iodine concentration, had an AUC of 0.929 in the training and 0.719 in the test set. In addition, the radiomics model revealed a higher AUC value and a greater net benefit to patients than the clinical-DECT model. Conclusion: DECT-based radiomics features were valuable in predicting the invasiveness of GGNs, yielding a better predictive performance than the clinical-DECT model.

Abstract:Background Solid components of part-solid nodules (PSNs) at CT are reflective of invasive adenocarcinoma, but studies describing radiomic features of PSNs and the perinodular region are lacking. Purpose To develop and to validate radiomic signatures diagnosing invasive lung adenocarcinoma in PSNs compared with the Brock, clinical-semantic features, and volumetric models. Materials and Methods This retrospective multicenter study (, NCT03872362) included 291 patients (median age, 60 years; interquartile range, 55-65 years; 191 women) from January 2013 to October 2017 with 297 PSN lung adenocarcinomas split into training ( = 229) and test ( = 68) data sets. Radiomic features were extracted from the different regions (gross tumor volume [GTV], solid, ground-glass, and perinodular). Random-forest models were trained using clinical-semantic, volumetric, and radiomic features, and an online nodule calculator was used to compute the Brock model. Performances of models were evaluated using standard metrics such as area under the curve (AUC), accuracy, and calibration. The integrated discrimination improvement was applied to assess model performance changes after the addition of perinodular features. Results The radiomics model based on ground-glass and solid features yielded an AUC of 0.98 (95% confidence interval [CI]: 0.96, 1.00) on the test data set, which was significantly higher than the Brock (AUC, 0.83 [95% CI: 0.72, 0.94]; = .007), clinical-semantic (AUC, 0.90 [95% CI: 0.83, 0.98]; = .03), volumetric GTV (AUC, 0.87 [95% CI: 0.78, -0.96]; = .008), and radiomics GTV (AUC, 0.88 [95% CI: 0.80, 0.96]; = .01) models. It also achieved the best accuracy (93% [95% CI: 4%, 98%]). Both this model and the model with added perinodular features showed good calibration, whereas adding perinodular features did not improve the performance (integrated discrimination improvement, -0.02; = .56). Conclusion Separating ground-glass and solid CT radiomic features of part-solid nodules was useful in diagnosing the invasiveness of lung adenocarcinoma, yielding a better predictive performance than the Brock, clinical-semantic, volumetric, and radiomics gross tumor volume models. Abstract:Rationale and objectives: Extraction of intratumoral and peritumoral radiomics features combined with clinical factors to establish nomograms to predict high-grade patterns (micropapillary and solid) of invasive adenocarcinoma of the lung (IAC). Materials and methods: A retrospective study was conducted on 463 patients with pathologically confirmed IAC. Patients were randomized in a 7:3 ratio into a training cohort (n = 324) and a testing cohort (n = 139). A total of 2154 CT-based radiomic features were extracted from each of the four regions: gross tumor volume (GTV) and gross peritumoral tumor volume (GPTV3, GPTV6, GPTV9) containing peri-tumor regions of 3 mm, 6 mm, and 9 mm. A radiomics nomogram was constructed based on the optimal radiomics model and clinically independent predictors. Results: The GPTV3 radiomics model showed better predictive performance in the testing group compared to the GTV (0.840), GPTV6 (0.843), and GPTV9 (0.734) models, with an AUC value of 0.889 in the testing group. In the clinical model, tumor density and the presence of a spiculation sign were identified as independent predictors. The nomogram, which combined these independent predictors with the GPTV3-Radscore, proved to be clinically useful. Conclusion: The GPTV3 radiomics model was superior to the GTV, GPTV6, and GPTV9 radiomics models in predicting high-grade patterns (HGP) of IAC. In addition, nomograms based on GPTV3 radiomics features and clinically independent predictors can further improve the prediction efficiency.

Abstract:Objective: The invasiveness of lung adenocarcinoma significantly impacts clinical decision-making. However, assessing this invasiveness preoperatively, especially when it manifests as pure ground-glass nodules (pGGN) on CT scans, poses challenges. This study aims to quantify intratumor heterogeneity (ITH) and determine whether the ITH score can enhance the accuracy of invasiveness predictions. Methods: A total of 524 patients with lung adenocarcinomas presenting as pGGN were enrolled in the study, with 177 (33.78%) receiving a pathologic diagnosis of invasiveness. Four diagnostic approaches were developed to predict the invasiveness of lung adenocarcinoma presenting as pGGN: (1) conventional lesion size, (2) ITH score, (3) clinical-radiological features (ClinRad), and (4) integration of the ITH score with ClinRad. ClinRad alone or in combination with the ITH score served as the input for 11 machine learning approaches. The trained models were evaluated in an independent validation cohort, and the area under the curve (AUC) was calculated to assess classification performance. Results: The conventional lesion size showed the lowest performance, with an AUC of 0.826 (95% confidence interval [CI]: 0.758-0.894), while the ITH score outperformed it with an AUC of 0.846 (95% CI: 0.787-0.905). The CatBoost model performed best when the ITH score and ClinRad were both used as input features, leading to the development of an ITH-ClinRad-guided CatBoost classifier. CatBoost also excelled with ClinRad alone, resulting in a ClinRad-guided CatBoost classifier with an AUC of 0.830 (95% CI: 0.764-0.896), surpassed by the ITH-ClinRad-guided CatBoost classifier with an AUC of 0.871 (95% CI: 0.818-0.924). Conclusion: The ITH-ClinRad-guided CatBoost classifier emerges as a promising tool with significant potential to revolutionize the management of lung adenocarcinomas presenting as pGGNs.
