Abstract:Calcifying epithelial odontogenic tumor (CEOT) is a rare type of odontogenic tumor. The most characteristic feature of the classical CEOT is the presence of amyloid globules and Liesegang ring calcification in the tumor tissue. Here, we present a non-calcifying variant of intraosseous CEOT with the presence of Langerhans cells within tumor epithelial nests in a 52-year-old Taiwanese woman. The patient was referred from a local dentist to our hospital for treatment of a unilocular radiolucent lesion at the right anterior region of the maxilla. The lesion was excised. Microscopically, the tumor was composed of small nests or strands of odontogenic epithelial cells and amorphous eosinophilic globules of amyloid-like materials in a loose fibrous connective tissue stroma. The tumor epithelial cells were positive for pan-cytokeratins (AE1 and AE3). Langerhans cells demonstrated by anti-CD1a staining were found in nests or strands of tumor epithelial cells. The eosinophilic globules were positive for Congo red and showed green birefringence when subjected to polarized light. Review of the English literature revealed two cases of non-calcifying variant of intraosseous CEOT with Langerhans cells in the anterior and premolar regions of the maxilla. Taken together, we suggest that the non-calcifying, Langerhan cell-rich variant of CEOT may have a distinct predilection for occurrence in the anterior and premolar region of the maxilla in contrast to the classical CEOTs that usually occur in the molar and ascending ramus area of the mandible.

Abstract:The calcifying epithelial odontogenic tumor (CEOT) is a rare benign odontogenic neoplasm which was first described by Pindborg in 1955 and accounts for less than 1% of all odontogenic lesions. Recently, a clear cell variant of CEOT has been identified with only eight well-documented cases in the literature. We present an additional case of clear cell CEOT of the mandible and review the salient clinical, radiologic, and histopathologic features of this entity and CEOTs in general. The differential diagnosis of clear cell tumors in the mandible includes: clear cell odontogenic tumor, clear cell ameloblastoma (odontogenic carcinoma), metastatic clear cell adenocarcinoma, primary intraosseous mucoepidermoid carcinoma, acinic cell carcinoma, epithelial-myoepithelial carcinoma, clear cell salivary gland tumors, and clear cell variant of squamous cell carcinoma. Because of the belief that clear cell odontogenic tumors are locally aggressive neoplasms, definitive resection of the entire mass with tumor-free surgical margins and long-term follow-up are recommended.

Abstract:Calcifying Epithelial Odontogenic Tumor (CEOT) is a rare benign lesion known for its locally aggressive behavior, with no gender predilection, and it is most commonly found in the mandible. This case involves a 16-year-old female patient who presented with edema on the left maxilla, facial asymmetry, and slow growth. The lesion affected the anterior region of the maxilla and involved teeth 21 to 23. Imaging examinations revealed a hypodense cystic image measuring 22mm with bone cortical expansion.

Abstract:We describe the case of a 41-year-old woman who presented with a malignant myoepithelioma (MME) in her left maxillary sinus. Exploratory biopsy of the left maxillary sinus was performed and pathological examination demonstrated that the tumour was positive for calponin and cytokeratin 14, which are indicative of MME. Lateral rhinotomy and left total maxillectomy were undertaken and the patient received radiotherapy and chemotherapy post-surgery. Primary recurrence and metastasis to the left angle of the mandible occurred 9 months after the surgery. The patient died of cachexia 13 months after the surgery.

Abstract:This study reported the clinicopathological features, treatment and prognosis of nine cases of noncalcifying and Langerhans cell (LC)-rich calcifying epithelial odontogenic tumor (CEOT) collected from the English literature. Of the nine cases, seven were intraosseous and two were extraosseous. All nine tumors were found in Asian patients. The age of the nine patients ranged from 20 years to 58 years with a mean age of 41 years. There were five female and four male patients. The seven intraosseous cases included six in the anterior and premolar region of the maxilla and one in the posterior region and ascending ramus of the mandible. The two extraosseous cases were located at the upper lateral incisor and premolar gingivae, respectively. Of the seven intraosseous cases, five showed unilocular and two multilocular radiolucency without foci of calcification. Six of the seven intraosseous cases showed resorption of the tooth roots in the tumor-involved region. Histologically, noncalcifying and LC-rich CEOTs were composed of small nests and thin strands of tumor epithelial cells with a relatively high number of LCs among them. This was the reason why we classed these nine cases as noncalcifying and LC-rich CEOTs. Two extraosseous cases received total excision of the gingival mass. For the seven intraosseous cases, four accepted partial maxillectomy or mandibulectomy, two received total excision or enucleation, and one underwent curettage. The six cases with the follow-up information available showed no tumor recurrence after a follow-up period of 6 months to 10 years.

Abstract:Calcifying epithelial odontogenic tumor (CEOT) and ameloblastoma are types of odontogenic tumors accounting for 1%, and 10% of all the odontogenic tumors. While sharing same odontogenic origin, these tumors are found to exhibit distinct clinicopathological features. In the present study, we present the third hybrid CEOT/Ameloblastoma tumor ever reported. The current CEOT/Amelobastoma is occurred after a previously operated CEOT in the same area. The patient was refered with distict clinical features of swelling and paresthesia. In the radiographic examination, a unilocular lesion with mixed internal structure and ambiguous periphery was seen which exhibited buccal and lingual cortical expansion, thining, and perforation as well as inferior alveolar canal perforation. The histopathology results suggested a CEOT/Ameloblastoma lesion. After the tumor removal, the patient was set up for further follow-ups and maxillofacial prosthesis.

Abstract:The Calcifying Cystic Odontogenic Tumor develops from remnants of the odontogenic epithelium inside the mandible or maxilla, typically in the anterior region, affecting patients between the 2nd and 4th decades of life. This case report describes a 39-year-old female patient who experienced discomfort in the right mandibular region near teeth 43, 44, and 45, as reported by a dentist. Intraoral examination revealed a well-defined, smooth-surfaced, hard nodule with a color similar to the mucosa, measuring approximately 12 mm.

Abstract:Squamous odontogenic tumor (SOT) is an exceedingly rare, benign epithelial odontogenic tumor showing squamous differentiation. It is composed of variably sized and shaped islands of cytologically bland, mature squamous epithelium within a fibrous stroma. In this report, we present a rare transformation of a squamous odontogenic tumor (SOT) of the maxilla into a well-differentiated squamous cell carcinoma (SCC) with involvement of the pterygoid plates. To the best of our knowledge, only two cases of malignant transformation of SOT has been reported in the literature. Herein, we seek to report this extremely rare occurrence to raise awareness of oral and maxillofacial surgeons and pathologists of this unusual, but serious event and perform a literature review of squamous odontogenic tumors.

Abstract:Malignant transformation of epidermoid cysts is a rare complication. Most of the previously reported cases have involved postoperative malignant transformations. We present a case of malignant transformation of a nonpostoperative epidermoid tumor into squamous cell carcinoma (SCC) that occurred in a 61-year-old Chinese woman. The patient's initial cranial MRI scan showed an epidermoid cyst with marginal enhancement in the pre-pontine cistern, and the lesion gradually enlarged after 10 months. A craniotomy was performed using to remove part of the tumor via the right retrosigmoid approach, and postoperative pathology confirmed that the transformation of the epidermoid cyst was malignant. Our case study suggests that the possibility of malignant transformation of epidermoid cyst should not be ignored on the basis of enhanced imaging features, regardless of whether they are nodular, annular, or patchy, as is the case for inflammation. Strict follow-up is required for early detection of malignant transformation to prompt correspondingly early clinical treatment.

[The Nature of Calcifying Odontogenic Cysts of the Jaws].

Abstract:OBJECTIVE:To clarify the clinicopathological and behavioral spectrum of the so-called calcifying odontogenic cysts (COC).METHODS:Retrospective analysis of 21 cases previously diagnosed as COC was undertaken to evaluate their clinical, radiographic, pathological and behavioral features.RESULTS:The lesions of this series were divided into three groups, including simple cysts, benign and malignant tumors. The cyst group was composed of 16 cases (9 men, 7 women). The age of the patients peaked at the second decade, with a predilection for the premolar region of the jaws. The clinicopathological features of this group were consistent with that of developmental odontogenic cysts. Follow-up of the 13 patients treated by enucleation revealed no recurrence. The benign tumor group consisted of 4 cases with variable clinicopathologic features. Two cases were solid tumors containing ameloblastomatous epithelium, ghost cells and calcification foci. The other two cases had lesions that contained typical areas of COC and other types of odontogenic tumors (1 ameloblastoma and 1 odontogenic fibromyxoma). All the 4 cases occurred in the mandible with a relative large size. Two of which had a history of multiple recurrences. Only one case was identified as malignant tumor based on its infiltrative growth pattern and histological features.CONCLUSIONS:The so-called COC, previously recognized as a pathologic entity, can show extreme diversity in its clinical and histopathological features as well as in its biological behavior. Thus, the subgroups as simple cyst, benign and malignant tumors should be clarified and treated accordingly. Their terminology and classification should be reconsidered.

Abstract:To analyze a case series of calcifying epithelial odontogenic tumors (CEOTs)-Pindborg tumor. Twenty cases of CEOT were collected. Their histopathological, histochemical-immunohistochemical, and clinical-radiographic features were reevaluated. CEOT accounted for 0.02% of oral pathologies. Seventeen cases (85%) were intraosseous: 14 (82%) solid and three (18%) cystic CEOTs. Three cases (15%) were peripheral-extraosseous: two (67%) solid and one (33%) cystic CEOT. One case (intraosseous) was malignant.

Intraoral examination revealed a palpable mass with a solid consistency extending from tooth 42 to 45, measuring 2.5 cm, with no surface alterations.

Abstract:Calcifying odontogenic cyst (COC) is an uncommon intraosseous lesion with almost equal frequency in the maxilla and mandible. Therefore, we report a case of a 17-year-old male presenting with a painless swelling on the left hemiface. A computed tomography exam revealed a well-defined unilocular radiolucency with focal areas of radiopacity associated with an impacted upper left lateral incisor, extending from the apical region of tooth 62 posteriorly to the left maxillary sinus. An incisional biopsy was performed, and the histopathologic analysis was characterized by a cystic lesion lined by ameloblastoma-like epithelium with the presence of ghost cells and calcifying material consistent with the diagnosis of COC.

Abstract:This article is a discussion of two cases of young adults with lesions in similar locations in the anterior maxilla, i.e., the canine-to-canine region, similar history, and comparable radiology. Both cases were histologically diagnosed as calcifying odontogenic cysts. Case 1 was a male aged 28 years with diffuse, firm left malar area facial swelling with pain in associated teeth for a month. Intraorally, he had a gingivo-vestibular swelling also extending palatally in the anterior left maxillary region extending from the distal surface of the left maxillary central incisor to the mesial surface of the left maxillary canine. The overlying mucosa was normal in appearance. The radiograph showed a large unilocular radiolucency in the affected region. The lesion was excised followed by curettage and primary closure. Case 2 was a female aged 25 years with a lumpy mass and pain in associated teeth since one year in the left canine-premolar region with an external swelling in the left ala of the nose region that extended superiorly to the zygomatic arch. The color of the skin as well as the intraoral mucosa was normal, and an orthopantomogram (OPG)revealed a unilocular radiolucency in the left maxillary canine-premolar region with resorption of premolar roots. Treatment included surgical enucleation and bone curettage. Both cases have been in follow-up for about a year and have shown non-incidental healing.

Abstract:Objective To make a summary of clinical and pathological features of benign cementoblastoma.Methods We reported 12 cases of benign cementoblastoma,and analyzed their clinical and pathological data,including patient ages at diagnosis,sex,occurrence,clinical presentation,radiographic and pathological features and their treatment.Results Patient ages at diagnosis ranged from 13 to 58 years,with a mean age of 33.8 years.The tumors affected 6 males and 6 females.The main site of tumors involvement was mandibular(66.7%),with retromolar area most commonly involved(91.7%).Pain(25%) and swelling(75%) were the most common symptoms.Radiographically,most cemetoblastomas exhibited a central opacity surrounded by a radiolucent halo.Microscopically,cementoblastomas were formed by proliferating cementum-like tissue attached to tooth root and exhibiting prominent reversal lines.A regime of tumor resection and affected tooth extraction can get good therapeutic result.Conclusion Benign cementoblastoma occurs more commonly in youth and middle aged patients,with premolar-molar region of jaw frequently involved;usually it presents with pain or expansion of jaw;X ray and histopathological features combined can distinguish it from osteoblastoma,cemento-ossifying fibroma and atypical osteosarcoma.

Abstract:Odontogenic lesions are a category of lesions, which are found to be arising from the remnants of the tooth-bearing tissues of the body, that can be cystic in nature as a result of degeneration or as a result of excessive proliferation of these cells, can result in the formation of odontogenic tumours which are found in gnathic bones in the body. Since their discovery in literature and the explanation provided for their pathogenesis, these lesions have been the subject of debate and controversy amongst researchers as well as practitioners. Thereby, this review has taken into consideration one such odontogenic tumour, Calcifying Cystic Odontogenic Tumour (CCOT), which first were included under the namesake (Calcifying odontogenic cyst) as a sperate subheading under this cyst, but now has been designated under the category of tumours along with various histologic subtypes classified and described henceforth. Although the lesion has been removed in the recent classification, a wide variety of lesions in biphasic form has been reported in the past. Therefore, this present review takes a sneak-peek into this lesion with insight into its presentation, incidence, aetiology, pathogenesis, histopathology and all the controversies surrounding this category of lesion and the current literature about this lesion with proving the fact that this needs to be considered again in the category of odontogenic tumours.

Abstract:Calcifying odontogenic cyst, also known as Gorlin cyst is a rare benign cystic lesion primarily found in the jawbones, accounting less than 1% of odontogenic cysts. It can be associated with odontogenic tumors such as odontomas. We report a rare case of COC associated with complex odontoma in a young patient and discuss its clinical features, diagnosis, and treatment options. An 18-year-old female patient presented with a painless radiopaque lesion of the right mandibular bone at Oral Medicine and Oral Surgery department. Radiographs revealed irregular tooth-like structures in the canine-premolar area. The lesion was surgically removed, and histopathology confirmed COC with a complex odontoma. As of the World Health Organization's 2022 definition, COC is a developmental odontogenic cyst characterized by calcified ghost cells. It typically affects individuals during their second and third decades of life, with no gender preference, almost equally in the maxilla and the mandible. The main treatment is total enucleation, with a generally favorable prognosis. Histopathology is essential for diagnosis due to its mimicry of other jaw conditions. Long-term follow-up is needed to prevent recurrences.
