Abstract:Calcifying epithelioma is one kind of benign tumour,which rarely happens in the neck. The clinical diagnosis of calcifying epithelioma is difficult and misdiagnosis is common. This paper reported one case of calcifying epithelioma in the neck, and discussed its clinical features and therapy based on literature review.

Abstract:A 55-year-old female patient underwent surgery for her thyroid lumps 10 years ago and pathological examination confirmed the diagnosis of primary papillary carcinoma of the thyroid. The metastasis of neck lymph nodes was conformed 8 years after operation, and the patient received metastatic lymph nodes resection. During the 3-year follow-up the patient had recurrent tumor. The pathological results showed that the lesion changed from papillary carcinoma to papillary carcinoma accompanied with squamous differentiation and transformed to squamous cell carcinoma of the thyroid at last. Finally the patient died from deterioration of the disease rapidly. From the literature review, it has been shown that the squamous cell carcinoma of the thyroid might originate from adenocarcinoma or squamous metaplasia of papillary carcinoma. Once the transformation occurs, the progress of the disease may be accelerated. This transformation is rare and usually confers a poor prognosis.

Abstract:Objective: To investigate the clinical characteristics and auxiliary examination of calcifying epithelioma,to improve the preoperative diagnosis rate.Methods: Clinical analysis of 13 patients with calcifying epithelioma at Dept.of Oral and Maxillofacial Surgery,1st Hospital of Lanzhou University during 2008-2011.Results: Based on literatures we found the calcifying epithelioma is a tumor,which is single local subcutaneous induration,size in 5cm the following ranges,rhombus or round,may bulge at the skin surface,and surface skin color showed normal or local redness.Sometimes the tumor tightly adhered to the skin.B ultrasound and CT can provide the internal structure characteristics of tumor.Conclusion: According to the clinical characteristics combined with B ultrasound and CT examination results we can obviously improve the calcifying epithelioma preoperative correct diagnosis rate.

Abstract:BACKGROUND:Malignant proliferating trichilemmal tumor (MPTT) is an infrequent malignant neoplasm originating from cutaneous appendages, with only a handful of documented cases. This report delineates a unique instance of MPTT situated in the neck, accompanied by lymph node metastasis. A comprehensive exposition of its clinical trajectory and imaging manifestation is presented, aiming to enhance comprehension and management of this atypical ailment.CASE SUMMARY:Patient concerns: A 79-year-old male presented with a longstanding right neck mass persisting for over six decades, exhibiting recent enlargement over the past year. Diagnoses: Enhanced magnetic resonance imaging of the neck unveiled an elliptical mass on the right neck side, characterized by an ill-defined border and a heterogeneous signal pattern. The mass exhibited subdued signal intensity on T1-weighted imaging (T1WI) and a heterogeneous high signal on T2-weighted imaging (T2WI), interspersed with a lengthy T1 and T2 cystic signal motif. Close anatomical association with the submandibular gland joint was noted, and intravenous gadolinium diethylene triamine pentaacetic acid administration facilitated conspicuous enhancement. Substantial enhancement of the solid segment prompted an initial preoperative diagnosis of malignant nerve sheath tumor. However, post-surgery histopathological and immunohistochemical analysis conclusively confirmed the diagnosis as malignant hyperplastic external hair root sheath tumor. Intervention: Complete excision of the tumor was successfully executed. Outcomes: The patient experienced a favorable postoperative recovery.CONCLUSION:Malignant proliferative trichilemmal tumor external hair root sheath tumor is a cystic-solid lesion, appearing as low signal on T1WI images or high signal on T2WI with enhancement of the solid component. Suspicions of malignancy are heightened when the tumor border is indistinct, tissue planes are breached, or when linear or patchy high signals are observed in the subcutaneous tissue on T1 liver acquisition with volume acceleration enhanced images along with intermediate signal on T2WI and restricted diffusion on diffusion-weighted imaging images. Strong consideration for malignancy should arise if there are signs of compromised adjacent tissue relationships or direct invasion evident on imaging. We have incorporated the above-mentioned content into the entire manuscript.
Non-Calcifying Variant of Calcifying Epithelial Odontogenic Tumor with Langerhans Cells

Abstract:Calcifying epithelial odontogenic tumor (CEOT) is a rare type of odontogenic tumor. The most characteristic feature of the classical CEOT is the presence of amyloid globules and Liesegang ring calcification in the tumor tissue. Here, we present a non-calcifying variant of intraosseous CEOT with the presence of Langerhans cells within tumor epithelial nests in a 52-year-old Taiwanese woman. The patient was referred from a local dentist to our hospital for treatment of a unilocular radiolucent lesion at the right anterior region of the maxilla. The lesion was excised. Microscopically, the tumor was composed of small nests or strands of odontogenic epithelial cells and amorphous eosinophilic globules of amyloid-like materials in a loose fibrous connective tissue stroma. The tumor epithelial cells were positive for pan-cytokeratins (AE1 and AE3). Langerhans cells demonstrated by anti-CD1a staining were found in nests or strands of tumor epithelial cells. The eosinophilic globules were positive for Congo red and showed green birefringence when subjected to polarized light. Review of the English literature revealed two cases of non-calcifying variant of intraosseous CEOT with Langerhans cells in the anterior and premolar regions of the maxilla. Taken together, we suggest that the non-calcifying, Langerhan cell-rich variant of CEOT may have a distinct predilection for occurrence in the anterior and premolar region of the maxilla in contrast to the classical CEOTs that usually occur in the molar and ascending ramus area of the mandible.

Abstract:Epithelioid sarcoma (ES) is an uncommon soft tissue sarcoma. It is usually located in the extremities and exceptionally in the neck. Its diagnosis constitutes a real challenge which is based on histology and immunohistochemistry staining that must be interpreted with caution given the anatomopathological similarities to other tumors. In this article, we report a case of a 37-year-old man admitted for a locally advanced ES of the neck. There were suspicions of lymph node metastases of nasopharyngeal carcinoma at the first pathological examination. The patient received palliative chemotherapy and was referred to the supportive care department. Through this case, we will discuss the clinical and anatomopathological characteristics and therapeutic options of this extremely rare tumor which poses a diagnostic challenge.

Abstract:OBJECTIVE:To analyze the clinical presentation, pathological feature, therapy and outcome of epithelioid hemangioma.METHOD:We reviewed the clinical findings, pathologic changes and outcomes of six cases with epithelioid hemangioma from Aug. 2007 to Sept. 2011.RESULT:There are four males and two females ranged from 18 to 44 years(median year was 29.5). Primary lesions sites included the parotid gland, neck and ear. Four lesions presented as a solitary mass, and no patient had history of trauma. All patients were treated by surgeries, and not complemented by medical. There were four patients relapsed during follow-up, and three of them accepted reoperations (two patients received postoperative radiotherapies), and no recurrence was detected.CONCLUSION:Epithelioid hemangioma represents a benign vascular tumor, accurate diagnosis relies on clinical characteristic, histopathologic appearance and immunohistochemical findings, and it must be differentiated with Kimura's disease. The main treatment is surgery and the prognosis is well, but easy to recurrence.

Abstract:Objective To review the clinicopathologic features and differential diagnosis of epithelioid sarcoma(ES).Methods Ten cases of ES,6 males and 4 females aged 21～50 years(mean 35 years) were included in the study.Tissue specimens were investigated by light microscope and immunohistochemistry(En Vision method).The clinical data were analyzed.Results Clinically,all patients presented a slowly growing mass,4 tumors were located in the hands,2 in forearm,and others in arm,foot,lower leg and back.Grossly,ES presented as indurated ill-defined nodules,measuring 2.0 to 8.4 cm(mean 5.2 cm).The cut surface showed a gray-white lesion with focal necrosis.Histologically,ES showed a nodular pattern,which underwent central necrosis resulting in a pseudogranulomatous appearance.The tumors consisted of large polygonal cells and plump spindle-shaped cells with deeply eosinphilic cytoplasm.Immunohistochemically,all cases were immunoreactive with CK,Vim.Nine cases were positive for epithelial membrane antigen,6 for CD34.S-100 and smooth muscle actin were focally expressed only in one case.Myoglobin,FⅧRAg,CD31 and HMB45 were negative.Conclusions ES is an uncommon soft tissue tumor and mostly occurs in the distal extremities with undetermined differentiation and complex immune phenotype.

Abstract:One case report of a middle aged female complaining “chronic bone pain for 20 years, renal failure for two years, detectable neck mass for a month” admitted by Xiangya hospital. The patient had once re-ceived neck mass resections in the same position twice due to “parathyroid adenoma”, the state of the disease was poorly controlled. After the resection of the neck mass and all the parathyroid glands, the pathological diag-nosis of the neck mass and the resected parathyroid glands are all turned out to be malignant. Through analyzing the clinical features of the case and systematically reviewing relevant literatures, we try to figure out the current situation and the diagnosis and treatment scheme for parathyroid carcinoma.
Metastatic Squamous Cell Carcinoma of the Gingiva Appearing As a Solitary Branchial Cyst Carcinoma: Diagnostic Role of PET/CT.

Abstract:We herein present a case of a left cervical cystic mass, for which the initial pathological diagnosis was branchial cleft cyst carcinoma (following complete mass excision). Thorough postoperative examinations, including with FDG positron emission tomography/computed tomography (PET/CT), revealed a primary tumor in the retromolar region of the left mandible. A 52-year-old female presented with a 2-month history of a painless, progressively enlarged left-sided neck mass. Fine-needle aspiration biopsy suggested a branchial cleft cyst. Physical examination revealed a 3 × 3-cm smooth, tender mass in the upper-left neck and anterior border of the sternocleidomastoid muscle. Examination using nasendoscopy and a strobolaryngoscope revealed no abnormalities of the nasal cavity, nasopharynx, oropharynx, hypopharynx or larynx. MRI of the neck revealed a solitary, round, cystic mass under the left parotid gland. The mass was excised completely. Pathologic results indicated a branchial cleft cyst carcinoma. According to the diagnostic criteria for a branchial cleft cystic carcinoma, PET/CT was performed to detect the occult primary site. PET/CT revealed high FDG uptake in the tooth root of the left mandible. Frozen sections of the mass were indicative of moderate, differentiated squamous cell carcinoma. The carcinoma in the retromolar region of the left mandible was locally excised under general anesthesia. A partial left maxillectomy, partial mandibulectomy, and left radical neck dissection were performed. The patient received postoperative concurrent chemoradiotherapy, and was disease-free at the 8-month follow-up. True branchial cleft cyst carcinoma is rare: once diagnosed, it should be distinguished from metastatic cystic cervical lymph and occult primary carcinoma. FDG PET/CT is useful in the identification of occult primary tumor.
Agressive small cell neuroendocrine carcinoma of the neck: A case report

Abstract:Introduction: The location in the neck of small cell neuroendocrine carcinoma (SCNEC) is rare and aggressive with a poor prognosis. We herein describe an extremely uncommon example of cervical SCNEC probably arising in the parotid gland, diagnosed at an advanced metastatic stage. Presentation of case: A 76 year old man, non-smoking, non-alcoholic presented a left cervical and rapidly progressive mass. It was suspicious of malignancy and located in the parotid space and II, III, IV and V lymph node groups. In two weeks, the patient had presented a rapid increase in size, with extensive skin permeation. CT scan revealed a large mass infiltrating the left parotid and submandibular glands compressing the jugulo-carotid vascular axis and liver metastases. Diagnosis was made on the morphological pathological examination supplemented by the immunohistochemistry examination, indeed, cells were immunoreactive for chromogranin A as well as synaptophysin. Chemotherapy based on cisplatin and Etoposide VP 16 was started but the evolution was unfavorable: death occurred after 2 cycles. Clinical discussion: Neuroendocrine neoplasm arises from neural crest cells. Its location in the head and neck is extremely rare with a tendency for aggressive local invasion and a strong propensity for both regional and distant metastasis. Treatment may include surgical resection, radiotherapy, chemotherapy, or combination of these modalities. The prognosis is poor. Conclusion: The clinician must discuss the diagnosis of cervical SCNEC when faced with a man in the seventh decade who presents an agressive rapidly expanding cervical mass. The diagnosis confirmation is pathological combining morphological and immunohistochemical evaluations.
Report of a Case of Penile Epithelioid Sarcoma

Abstract:To enhance the knowledge and the effect of the diagnosis and treatment of primary epithelioid sarcoma of the penis.One rare case of primary epithelioid sarcoma of the penis was studied with regard to its primary clinical process and characteristics, differential diagnosis and method of treatment.An operation was performed on the penis to treat the epithelioid sarcoma. The diagnosis was confirmed by immunohistological and pathological techniques. There was no evidence of relapse during the three-year follow-up after operation.The possibility of primary epithelioid sarcoma of the penis should be considered if a mass or induration of the proximal penis and the symptoms of urethremphraxis are found. Total phallectomy could be chosen as an appropriate method of treatment. Unless adenopathy is palpable, node dissections are not recommended.

Cutaneous myoepithelioma on the head: a case report

Abstract:A case of cutaneous myoepithelioma on the head is reported. The 42-year-old male patient presented with a tumor on the scalp for two years without any symptom. Skin examination showed that a red, hemispherical, well-circumscribed tumor on the scalp, in a size of 1 cm x 1 cm, with telangectasis on the smooth surface, no ulceration and hemorrage were noted. The mass was hard without tenderness, it was completely excised. Pathologically, the epidermis was normal, a well-circumscribed tubercle was seen in the dermis. The tumor cells in the tubercle consisted of epithelioid and plasmacytoid cells, and the intercellular matrix showed hyaloid degeneration. These cells were separated as cord-like, trabecular shapes and masses by the denaturedmatrix. Immunohistochemically, the tumor cells were strongly positive for CK, focally for S-100, and diffusely strongly positive for P63. Combined with clinical manifestations, histopathologic features and immunohistologic findings, the patient was diagnosed as cutaneous myoepithelioma. No recurrence had been found for one year follow-up after operation. The patient is still being followed-up.

Abstract:Branchial cleft carcinoma is a rare malignancy, there still exist some controversies regarding to the differentiation of branchial cleft carcinomas. This article is aimed to familiarize clinicians with the presentation and treatment of the tumour and explores its origin. A 41-year old man who was seen with a large lesion of branchial cleft carcinoma with a slowly growing mass in the area of the left parotid gland was reported. The histological finding of dysplastic epithelium was next to direct invasive carcinoma. The diagnosis of a branchial cleft carcinoma requires the fulfillment of strict criteria. This case supports the origin of the carcinoma as being from an epithelial-lined cyst.
Non-calcifying and Langerhans Cell-Rich Variant of Calcifying Epithelial Odontogenic Tumor

Abstract:This study reported the clinicopathological features, treatment and prognosis of nine cases of noncalcifying and Langerhans cell (LC)-rich calcifying epithelial odontogenic tumor (CEOT) collected from the English literature. Of the nine cases, seven were intraosseous and two were extraosseous. All nine tumors were found in Asian patients. The age of the nine patients ranged from 20 years to 58 years with a mean age of 41 years. There were five female and four male patients. The seven intraosseous cases included six in the anterior and premolar region of the maxilla and one in the posterior region and ascending ramus of the mandible. The two extraosseous cases were located at the upper lateral incisor and premolar gingivae, respectively. Of the seven intraosseous cases, five showed unilocular and two multilocular radiolucency without foci of calcification. Six of the seven intraosseous cases showed resorption of the tooth roots in the tumor-involved region. Histologically, noncalcifying and LC-rich CEOTs were composed of small nests and thin strands of tumor epithelial cells with a relatively high number of LCs among them. This was the reason why we classed these nine cases as noncalcifying and LC-rich CEOTs. Two extraosseous cases received total excision of the gingival mass. For the seven intraosseous cases, four accepted partial maxillectomy or mandibulectomy, two received total excision or enucleation, and one underwent curettage. The six cases with the follow-up information available showed no tumor recurrence after a follow-up period of 6 months to 10 years.

Due to the limited number of recorded cases, the biological behaviour and histogenesis of the Langerhans cell variant of CEOT are not yet fully understood. Thus, the correlation between conventional CEOT and the Langerhans cell variant remains to be clarified.

Abstract:OBJECTIVE:To evaluate clinical feature, diagnosis, treatment and prognosis of myoepithelial carcinoma (MC) in the head and neck.METHODS:Clinical data of 11 patients which were confirmed by pathology and immunohistochemistry in Cancer Center, Sun Yat-sen University from Jan. 1992 to Dec. 2006, were reviewed. There were 5 cases in parotid gland, 1 in hard palate, 1 in maxillary sinus, 1 in pharyngeal recess, 1 in bucca cavioris, 1 in scalp, and 1 in gingiva. Male female ratio is 4 to 7. The median age at diagnosis was 37 years (range: 14 - 60 years).RESULTS:All cases were operated, 4 underwent surgery alone, 2 underwent surgery plus adjuvant radiotherapy, 2 received surgery plus adjuvant chemotherapy, 3 underwent surgery plus adjuvant chemoradiation. After operation, 8 cases occurred local recurrence and 4 cases occurred distance and lymph nodes metastasis. There was spindle cell type in 5 cases, clear cell type, plasmacytoid cell type in 2 cases, epithelioid cell type, mixed type in 1 case. The median follow-up time was 40 months. AS to the last follow-up time, 8 patients died.CONCLUSIONS:The characteristics of the tumor were rapidly enlarging, invading the surrounding regions, high rates of lymph node metastasis, high rates of distance metastasis. It was easily recurrence if the surgical excision was not radically for MC, and the prognosis was poorly. MC was a sort of malignant tumor. Radical operation must be advocated initially. Chemotherapy and radiotherapy may be effective after operation.

Abstract:Objectives To determine the clinicopathological features of epithelioid sarcoma presenting in head and neck region (HNES) and elucidate diagnostic key points and treatment options for HNES. Materials and Methods A total of 12 HNES cases were collected in our department from 2010 to 2020. Their clinical information and pathological features were documented, and relevant follow-up was performed. Immunohistochemistry was carried to analyze the protein markers of HNES. Results Of the 12 HNES cases, 10 were primary tumors and 2 were metastasized from foot and shoulder, respectively. The patients with primary tumors were significantly younger than those with metastasized ones (22.7 vs 41.5, p = .0157), and male patients outnumbered female patients (3:1). Of all HNES cases, 9 were classic subtype, and 3 were proximal subtype. HNES patients had a poor prognosis, with 5-year overall survival of 41.5% and 5-year relapse-free survival of 22.5%. A loss of INI1 was identified as the hallmark of HNES with 83.3% (10/12) of HNES cases presenting as EZH2 positive. Conclusions HNES is more prevalent at younger ages and in males, has a poor prognosis, and exhibits a greater proportion of classic subtype than proximal subtype. EZH2 inhibitor has therapeutic potential in HNES.
