Abstract:Calcifying odontogenic cyst (COC) is a rare lesion, accounting for less than 1% of jaw cysts, and it is classified among developmental cysts in the WHO classification of tumors of the head and neck in 2017. A 26-year-old female patient presented with an asymptomatic swelling located on the right mentual region that had been growing over the past 8 years. Intraoral examination revealed a palpable mass with a solid consistency extending from tooth 42 to 45, measuring 2.5 cm, with no surface alterations.

Abstract:Calcifying odontogenic cyst (COC) is an uncommon intraosseous lesion with almost equal frequency in the maxilla and mandible. Therefore, we report a case of a 17-year-old male presenting with a painless swelling on the left hemiface. A computed tomography exam revealed a well-defined unilocular radiolucency with focal areas of radiopacity associated with an impacted upper left lateral incisor, extending from the apical region of tooth 62 posteriorly to the left maxillary sinus. An incisional biopsy was performed, and the histopathologic analysis was characterized by a cystic lesion lined by ameloblastoma-like epithelium with the presence of ghost cells and calcifying material consistent with the diagnosis of COC.

Abstract:The calcifying odontogenic cyst (COC) is an uncommon developmental odontogenic cyst, the oral counterpart of Malherbe's cutaneous calcifying epithelioma (pilomatricoma). This article presents two unique cases of calcifying odontogenic cysts each exhibiting distinctive histopathological features and its literature review. One case with an unexpected finding of cholesterol granuloma (CG), a rare occurrence in non-inflammatory cysts within an unusual location between two maxillary central incisors. One more instance involves the presence of a compound odontome in conjunction with COC. The cases underscore the clinical and histopathological diversity of COC and highlight the importance of radiological and histopathological assessments for accurate diagnosis. The unexpected association of COC with cholesterol granuloma challenges traditional diagnostic expectations. Additionally, the second case suggests that COCs may warrant sub-categorization to better understand their varied presentations and biological behavior. This article contributes to the expanding knowledge of COC, emphasizing the significance of documenting rare cases to enhance comprehension of its nature, pathogenesis, and oral cavity origin.

Characteristics of Calcifying Odontogenic Cyst in Taiwanese.

Abstract:BACKGROUND AND PURPOSE:Calcifying odontogenic cyst (COC) is a rare type of odontogenic cyst. This retrospective study analyzed the clinical, radiographic, and histopathologic features of COC in Taiwanese.METHODS:Ten cases of COC in 2 male and 8 female patients with a mean age of 29 years (range, 11 to 48 years) treated from January 1985 to December 2002 were included. Microscopic slides, clinical histories, and radiographic features of these 10 COC cases were reviewed and analyzed.RESULTS:COCs occurred in the maxilla in 3 cases and in the mandible in 7 cases. COCs were associated with impacted teeth in 6 cases and with odontomas in 3 cases. All COCs appeared as either unilocular (9 cases) or multilocular (1 case) radiolucencies. In 7 cases, spotty radiopaque materials were scattered throughout the radiolucency. Histologically, all of the lesions were at least partially lined by epithelium with cuboidal to columnar basal cells and stellate reticulum-like suprabasal cells. Variable numbers of ghost cells, some of which were calcified, were observed in the lining epithelium or in the fibrous connective tissue wall of all 10 cases. Juxta-epithelial dentinoid was also found in all cases. However, proliferation of ameloblastoma-like tumor nests was observed in only 1 case. Based on the above histologic findings, 6 COC lesions were classified as simple unicystic type, 3 as unicystic odontoma-producing type, and 1 as unicystic ameloblastomatous proliferating type.CONCLUSIONS:COC occurs frequently in the second and third decades and is commonly associated with an impacted tooth or an odontoma. It usually appears as a mixed radiolucent and radiopaque lesion radiographically. Simple unicystic type is the most common type of COC. No recurrences were found after conservative surgical removal in this series.

Abstract:OBJECTIVE:To clarify the clinicopathological and behavioral spectrum of the so-called calcifying odontogenic cysts (COC).METHODS:Retrospective analysis of 21 cases previously diagnosed as COC was undertaken to evaluate their clinical, radiographic, pathological and behavioral features.RESULTS:The lesions of this series were divided into three groups, including simple cysts, benign and malignant tumors. The cyst group was composed of 16 cases (9 men, 7 women). The age of the patients peaked at the second decade, with a predilection for the premolar region of the jaws. The clinicopathological features of this group were consistent with that of developmental odontogenic cysts. Follow-up of the 13 patients treated by enucleation revealed no recurrence. The benign tumor group consisted of 4 cases with variable clinicopathologic features. Two cases were solid tumors containing ameloblastomatous epithelium, ghost cells and calcification foci. The other two cases had lesions that contained typical areas of COC and other types of odontogenic tumors (1 ameloblastoma and 1 odontogenic fibromyxoma). All the 4 cases occurred in the mandible with a relative large size. Two of which had a history of multiple recurrences. Only one case was identified as malignant tumor based on its infiltrative growth pattern and histological features.CONCLUSIONS:The so-called COC, previously recognized as a pathologic entity, can show extreme diversity in its clinical and histopathological features as well as in its biological behavior. Thus, the subgroups as simple cyst, benign and malignant tumors should be clarified and treated accordingly. Their terminology and classification should be reconsidered.
Peripheral calcifying odontogenic cyst in maxillary anterior gingiva: A case report

Abstract:Background: Calcifying odontogenic cysts (Gorlin cysts) most commonly present centrally and have only rarely been reported in peripheral locations. The purpose of this report is to describe a new case of peripheral calcifying odontogenic cyst (PCOC) occurring in the anterior maxillary gingiva and to review the management and differential diagnosis of such a lesion. Methods: A 37-year-old female presented with a long-standing submucosal nodule on the gingiva between the maxillary central incisors, with asymptomatic growth over the last three years. Following an initial incisional biopsy, a diagnosis of PCOC was established. To exclude the possibility of a central process, a corresponding small field of view cone beam CT scan was obtained and the patient returned for a 6 mm excisional biopsy to the depth of the periosteum. Results: Results of these additional assessments supported the original diagnosis of PCOC. Following uneventful healing of the second biopsy, no recurrence or other clinical findings were noted at 1-year follow-up. Conclusion: While rare, the peripheral variant of calcifying odontogenic cyst, and other peripheral counterparts to recognized central cysts and tumors, should be considered in a differential diagnosis for a benign gingival nodule. Gingival tissue should be submitted for histologic evaluation to ensure a neoplastic process is not present. Key points: Various lesions may present on the gingiva as a "bump"; these can represent common clinical entities, such as pyogenic granuloma, peripheral ossifying fibroma, peripheral giant cell granuloma, and fibroma, or more rare conditions that may not be adequately considered in the differential diagnosis. A rarely documented case of peripheral calcifying odontogenic cyst (PCOC; Gorlin cyst) on the maxillary anterior gingiva of an adult female is reported here and compared with the few other similar PCOC cases in the literature. A biopsy of gingival lesions is always necessary to establish the correct diagnosis and provide the appropriate treatment. Plain language summary: Several different lesions can appear on the gingiva (gums). Some are quite common, and some are rare. This report documents the occurrence of a new case of calcifying odontogenic cyst (Gorlin cyst), a type of cyst that has been rarely found outside the jawbone, presenting as a "bump" on the gingiva between the maxillary central incisor teeth of an adult female. Because of the patient history, a peripheral calcifying odontogenic cyst (PCOC) was not initially suspected. Following a biopsy, a PCOC diagnosis was given. The possibility of a lesion within the bone was then excluded by an X-ray (cone beam CT) scan examination. A second, more extensive biopsy confirmed the diagnosis and the removal of the lesion. The patient had no complications or recurrence for the following 12 months. This case highlights the need to always biopsy lesions presenting on the gums to obtain a proper diagnosis and provide the correct treatment.
Clinicopathologic Spectrum of the So-Called Calcifying Odontogenic Cysts: a Study of 21 Intraosseous Cases with Reconsideration of the Terminology and Classification.

Abstract:The so-called calcifying odontogenic cyst (COC) represents a heterogeneous group of lesions that exhibit a variety of clinicopathologic and behavioral features. Because of this diversity, there has been confusion and disagreement on the terminology and classification of these lesions. We reviewed the clinicopathologic features of 21 intraosseous cases that were previously diagnosed as COC or under related diagnostic terms. Based on the biologic behavior, the lesions of the present series were divided into three subgroups: cyst, benign tumor, and malignant tumor. Sixteen cases (nine men and seven women) proved to be unicystic lesions with (five cases) or without associated odontoma. The lining epithelium of the cystic lesions fulfilled the histologic criteria for COC proposed by the World Health Organization, and their overall clinicopathologic features were consistent with that of developmental odontogenic cysts. The age of patients from the cyst group peaked at the second decade. The maxilla was affected more often (69%) than the mandible, with a predilection for the canine-premolar region (62.5%). Thirteen patients with follow-up information revealed no recurrence following enucleation. The four cases in the benign tumor group had variable clinicopathologic features. Two cases were solid tumors consisting of ameloblastoma-like sheets of odontogenic epithelium that contained ghost cells/calcification foci and juxtaepithelial dentinoid. Both patients experienced multiple recurrences following conservative surgeries. The other two lesions contained typical areas of COC and other types of odontogenic tumors (one ameloblastoma and one odontogenic myxofibroma). All four lesions occurred in the mandible and were relatively large. In the present series one case identified as malignant tumor arose from a previously benign COC. The tumor shared some features of COC (ghost cell foci and dystrophic calcification) but also had prominent mitotic activity, nuclear and cytoplasmic pleomorphism, areas of tumor necrosis, and infiltrative/destructive growth. Recognizing the extreme diversity in clinicopathologic features and biologic behavior among the so-called COCs, we suggest that the term COC should be used to specifically designate the unicystic lesions with or without an associated odontoma, i.e., lesions of the cyst group, and other related lesions identified as benign tumor and malignant tumor should be termed and classified separately. A tentative scheme with respect to the terminology and classification for this group of disparately behaving lesions was herein proposed to reflect the likely difference of their nature.

Abstract:A 20-year-old male presented with an asymptomatic swelling on the left side of his midface. Upon intraoral examination, a noticeable volume was observed in the left labial mandibular area, accompanied by dental mobility of vital elements 32 to 36. Radiographic findings revealed a well-defined unilocular radiolucency measuring 4 cm in size, along with evidence of root resorption in the dental elements. The radiological examination, coupled with a positive aspiration of cystic fluid, raised provisional diagnoses of odontogenic keratocyst, unicystic ameloblastoma, and calcifying odontogenic cyst (COC).

Abstract:Glandular Odontogenic Cyst (GOC) is a rare developmental cyst, accounting for only 0.2% of jaw lesions. In this report, we present a case of a 68-year-old female who had a painless swelling in the right maxilla. Cone beam computed tomography revealed a large hypodense lesion that extended into the right maxillary sinus and nasal fossa, resulting in buccal cortical perforation. An incisional biopsy was conducted, and it revealed a pathological cavity lined by odontogenic epithelium, featuring duct-like structures, goblet cells, and apocrine metaplasia.

Abstract:The Calcifying Cystic Odontogenic Tumor develops from remnants of the odontogenic epithelium inside the mandible or maxilla, typically in the anterior region, affecting patients between the 2nd and 4th decades of life. This case report describes a 39-year-old female patient who experienced discomfort in the right mandibular region near teeth 43, 44, and 45, as reported by a dentist. Intraoral examination revealed a well-defined, smooth-surfaced, hard nodule with a color similar to the mucosa, measuring approximately 12 mm.

Abstract:Ghost cell odontogenic carcinoma (GCOC) is a rare malignant tumor of odontogenic origin, with only about 50 cases reported in the English literature so far. Histologically, it is characterized by ghost cells, dentinoid deposits, high grade malignant cellular features, and areas of necrosis and invasion. Having common histological features with other odontogenic ghost cell lesions (OGCL) like calcifying odontogenic cyst (COC) and dentinogenic ghost cell tumors, it is crucial to recognize GCOC malignant features, as it can be destructive and invasive, sometimes showing distant metastases and high recurrence rate. For this reason, it may entail more aggressive surgical approach and multimodal therapeutic regimen. Here we present a case report of GCOC arising in a previous COC, treated with surgical excision that showed persistence and recurrence after two years. The clinical and histological features of this rare occurrence are presented, in addition to the surgical approach, and a summary of literature review of OGCL.

Abstract:Dentigerous cysts are the second most common developmental odontogenic cysts that develop around the crown of unerrupted teeth with the maxillary canine region being one of the common sites of occurrence. The cystic lining of this lesion has been shown to develop into ameloblastoma, Muco epidermoid carcinoma, and squamous cell carcinomas. However, the development of cholesterol granuloma (CG) in the cystic lining of a dentigerous cyst is extremely rare. CG is a histological observation distinguished by the presence of a conglomeration of connective tissue and granulation tissue. The condition is predominantly seen in the field of otolaryngology, with very few cases reported in the maxillofacial region, most of which are associated with the maxillary sinus. This article presents the findings of a CG in a 39-year-old male patient that developed within the dentigerous cyst and discusses the possible etiopathogenesis, surgical management, and histological presentation.
Non-Calcifying Variant of Calcifying Epithelial Odontogenic Tumor with Langerhans Cells

Abstract:Calcifying epithelial odontogenic tumor (CEOT) is a rare type of odontogenic tumor. The most characteristic feature of the classical CEOT is the presence of amyloid globules and Liesegang ring calcification in the tumor tissue. Here, we present a non-calcifying variant of intraosseous CEOT with the presence of Langerhans cells within tumor epithelial nests in a 52-year-old Taiwanese woman. The patient was referred from a local dentist to our hospital for treatment of a unilocular radiolucent lesion at the right anterior region of the maxilla. The lesion was excised. Microscopically, the tumor was composed of small nests or strands of odontogenic epithelial cells and amorphous eosinophilic globules of amyloid-like materials in a loose fibrous connective tissue stroma. The tumor epithelial cells were positive for pan-cytokeratins (AE1 and AE3). Langerhans cells demonstrated by anti-CD1a staining were found in nests or strands of tumor epithelial cells. The eosinophilic globules were positive for Congo red and showed green birefringence when subjected to polarized light. Review of the English literature revealed two cases of non-calcifying variant of intraosseous CEOT with Langerhans cells in the anterior and premolar regions of the maxilla. Taken together, we suggest that the non-calcifying, Langerhan cell-rich variant of CEOT may have a distinct predilection for occurrence in the anterior and premolar region of the maxilla in contrast to the classical CEOTs that usually occur in the molar and ascending ramus area of the mandible.

Abstract:Odontogenic lesions are a category of lesions, which are found to be arising from the remnants of the tooth-bearing tissues of the body, that can be cystic in nature as a result of degeneration or as a result of excessive proliferation of these cells, can result in the formation of odontogenic tumours which are found in gnathic bones in the body. Since their discovery in literature and the explanation provided for their pathogenesis, these lesions have been the subject of debate and controversy amongst researchers as well as practitioners. Thereby, this review has taken into consideration one such odontogenic tumour, Calcifying Cystic Odontogenic Tumour (CCOT), which first were included under the namesake (Calcifying odontogenic cyst) as a sperate subheading under this cyst, but now has been designated under the category of tumours along with various histologic subtypes classified and described henceforth. Although the lesion has been removed in the recent classification, a wide variety of lesions in biphasic form has been reported in the past. Therefore, this present review takes a sneak-peek into this lesion with insight into its presentation, incidence, aetiology, pathogenesis, histopathology and all the controversies surrounding this category of lesion and the current literature about this lesion with proving the fact that this needs to be considered again in the category of odontogenic tumours.

Abstract:This simulation study investigates the performance of three test statistics, T1, T2, and T3, used to evaluate structural equation model fit under non normal data conditions. T1 is the well-known mean-adjusted statistic of Satorra and Bentler. T2 is the mean-and-variance adjusted statistic of Sattertwaithe type where the degrees of freedom is manipulated. T3 is a recently proposed version of T2 that does not manipulate degrees of freedom. Discrepancies between these statistics and their nominal chi-square distribution in terms of errors of Type I and Type II are investigated. All statistics are shown to be sensitive to increasing kurtosis in the data, with Type I error rates often far off the nominal level. Under excess kurtosis true models are generally over-rejected by T1 and under-rejected by T2 and T3, which have similar performance in all conditions. Under misspecification there is a loss of power with increasing kurtosis, especially for T2 and T3. The coefficient of variation of the nonzero eigenvalues of a certain matrix is shown to be a reliable indicator for the adequacy of these statistics.

Abstract:An 11-year-old female patient visited the Stage VI clinic in 2020 due to an enlargement in the left mandibular region. The patient had been experiencing pain upon palpation for the past 2 months. Radiographically, the lesion presented as a large radiolucent area with well-defined circular borders, located in the mandibular region between teeth 31 and 35. Additionally, an impacted tooth and dental mobility were observed. Preoperative examinations and cone beam tomography were requested to further evaluate the condition.

Abstract:Botryoid odontogenic cyst (BOC) is a rare lesion, with only one hundred reported cases in the 43 years since its initial description. BOC is a developmental odontogenic cyst, considered a variant of the lateral periodontal cyst. We present a case of a 73-year-old female patient who presented with an asymptomatic intraosseous lesion discovered during a routine radiographic examination. The lesion was radiolucent, multilocular, and well-circumscribed, located in the distal region of the lower right canine tooth.

Abstract:A 19-year-old female patient presented at our clinic service with ocular proptosis, hypertelorism, palmoplantar pitting, melanocytic nevi on her face, a tongue with dark spots, and multiple skin lesions consistent with basal cell carcinoma. The patient reported social difficulties, intellectual deficit, and a history of numerous odontogenic cysts and ovarian fibromas/cysts in the past six years. Abstract:We present a rare case of a dentinogenic ghost cell tumor (DGCT) associated with a compound odontoma (CO) in a 13-year-old male patient, affecting the anterior region of the maxilla. A computed tomography scan revealed a well-defined, unilocular hypodense lesion that involved tooth-like structures of varying shapes and sizes, leading to divergence of the roots of teeth 11 and 12. An excisional biopsy was performed, and the histopathological analysis revealed the presence of dentin, enamel matrix, and loose connective tissue resembling dental pulp.
