Abstract:AIM: To retrospectively investigate the relationship and therapeutic strategy of knee osteoarthritis (KOA) combined with degenerative lumbar disease through the reviews of therapeutic effect after total knee arthroplasty (TKA) and the measurements of preoperative and postoperative lumbar lordotic angle (LLA) in KOA patients combined with degenerative lumbar disease or not. METHODS: The experiment was carried out among 213 knee joints of 129 KOA patients, who were performed TKA in Department of Orthopaedics, Second Affiliated Hospital of Inner Mongolian Medical College from January 2002 to December 2003. All the patients received neurologic examination before TKA and diagnosed by X rays on P-A and lateral position of knee joints and MR films on lumbar in standing position. According to the preoperative data, the patients were divided into three groups: A group: 35 simple KOA patients; B group: 54 KOA patients combined with asymptotic lumbar stenosis (LS); C group: 40 KOA patients combined with symptomatic LS. All patients were performed HSS score and functional score of knee joints preoperatively and postoperatively, and 62 patients were followed up and allocated into unilateral knee replacement group (n =25) and bilateral knee replacement (n =37). Knee flexion deformity correction degree was compared between two groups, and LLA was used to compare change preoperatively and postoperatively. RESULTS: The average follow-up period was 16.3 months after operation (6-30 months).①There was not significantly statistical difference of knee joint HSS score and functional score between pre-operation and post-operation, and among A, B, C groups (P > 0.05).②There was not significant difference of knee flexion deformity correction degree between unilateral knee replacement group and bilateral knee replacement group (P > 0.05). In bilateral knee replacement group, LLA was significantly increased post-operation, compared with pre-operation [(49.25±2.42)°, (41.19±2.15)°, P < 0.05]. And there was no statistical difference between pre-operation and post-operation in unilateral knee replacement group (P > 0.05).③The lumbar spine symptoms were presented in 8 patients of B group after TKA and aggravated in 5 patients of C group. There were 1 patient in B group and 2 patients in C group were performed lumbar spinal canal decompression operation after mean 11 months of TKA. CONCLUSION: For the middle and aged KOA patients combined with lumbar spinal canal stenosis, there is possibility that the original lumbar spine symptoms would appear or aggravate after TKA, which should be informed before operation. So the possibility of lumbar spinal canal decompression operation after TKA is not excluded to treat the lumbar spine symptoms.
