Abstract:We have created a shaped tube that connects the superior and inferior vena caval orifices to drain the blood from superior vena cava to the inferior vena cava and eventually to the extracorporeal circulation through the femoral vein. We specifically use it in cardiac surgeries in cases of right atriotomy operations requiring femoral vein cannulation, such as tricuspid valvuloplasty or replacement, atrioventricular septal defect repair, and so on. Abstract:External jugular vein (EJV) cutdown for totally implantable venous access device (TIVAD) placement has been accepted as an alternative to the percutaneous subclavian vein approach. The aim of this work was to report a retrospective and monocentric study concerning the feasibility and complications of this surgical technique. Patients receiving a TIVAD from January 2010 to December 2015 were included in this study. Age, sex, surgical technique, disease, device used, length of the procedure, and morbidity were considered. There were 426 TIVADs placed in 418 patients: 149 males (35.6%) and 269 females (64.4%) aged 1 to 73 years. Of 425 patients, 172 underwent an EJV cut down on the first attempt, and 161 (93.6%) procedures were successful. Among the remaining 11 patients, seven (63.6%) underwent a TIVAD placement through the ipsilateral internal jugular vein and through the ipsilateral subclavian vein in the other four. Complications were identified in 11 ports in EJV group (6.25%) and in 35 ports in percutaneous puncture group (14.1%; P < .05). TIVAD placement by the EJV cutdown approach is safe and fast, and its success rate is very high. By avoiding the immediate complications associated with the percutaneous approach, the EJV cutdown has to be considered a valid, safe.

Abstract:Introduction: Totally Implantable Venous Access Devices (TIVADs) contribute significantly to the treatment progress and comfort of patients requiring long-term therapy. However, the procedure for implanting TIVADs, as well as its very presence, may be associated with complications. Aim: This study evaluates the indications, safety, and complication rates of venous port implantations in pediatric patients. It also explores factors influencing the occurrence of early and late complications post-implantation. Materials and methods: The study included 383 pediatric patients treated at the Department of Pediatric Surgery, Traumatology, and Urology in Poznan between 2013 and 2020 who underwent 474 implantations of intravenous ports. Venous access was achieved using the Seldinger technique. Statistical analysis was performed using Statistica 13 with TIBCO and PQStat 1.8.2.156 with PQStat. Results: Venous ports were used in 345 oncology patients requiring chemotherapy (90% of the total group) and in 38 children (10%) with non-oncology indications. There were 36 early complications (7.6%) and 18 late complications (3.8%), excluding infectious complications. The most common early, non-infectious complications included pneumothorax (15 patients; 3%) and port pocket hematoma (12 patients; 2.5%). The most common late, non-infectious complications observed were venous catheter obstruction (8 children; 1.7%) and port system leakage (5 children; 1%). Infectious complications occurred in 129 cases (27.2%). Children with a diagnosis of non-Hodgkin's lymphoma, acute myeloid leukemia, and acute lymphoblastic leukemia had a significantly higher incidence of port infections. Venous ports equipped with a polyurethane catheter, compared to systems with a silicone catheter, functioned significantly shorter. Conclusions: The Seldinger method of port implantation is quick, minimally invasive, and safe. The type of port, including the material of the port's venous catheter, and the underlying disease have an impact on the durability of implantable intravenous systems. The experience of the surgeon is related to the frequency of complications associated with the procedure.

Abstract:To describe the anatomic variations of total anomalous pulmonary venous connection (TAPVC) and its corresponding surgical techniques.A total of 143 TAPVC subjects were hospitalized from April 1981 to July 2010. Those patients with other complex congenital heart diseases, such as transposition of great artery and single ventricle, were excluded. A pathological diagnosis was made by echocardiography, magnetic resonance imaging, computed tomography, catheterization and intra-operative findings. The specific types of TAPVC were as follows:supra-cardiac (49.7%, 71/143), cardiac (40.6%, 58/143), infra-cardiac (4.2%, 6/143) and mixed (5.6%, 8/143). The subtypes were classified by the pathway of common confluence, distribution of pulmonary vein and their orifice site. The techniques of surgical repairs included modified Warden procedure and pulmonary vein transplantation.The patients with supra-cardiac type were further divided into 4 subtypes according to the course of vertical veins and their orifice site: right and left veins forming a common confluence, then draining into vertical and innominate veins (n = 65); common confluence of pulmonary vein drainage into superior vena cava through a short vertical vein at the right pulmonary hilus (n = 3); right and left pulmonary veins separately draining into superior vena cava (n = 2); common confluence draining into innominate vein through a right path beside trachea (n = 1). Cardiac types were further divided into 3 subtypes: coronary sinus (n = 20), right atrium (n = 37) and right atrium & sinus (n = 1). Infra-cardiac type had no subtype. Mixed type was more complex and it was further divided into 3 subtypes: bilateral & symmetrical connection (right 2 + left 2, n = 5); bilateral & asymmetrical connection (3 + 1, n = 3). Surgical repairs were performed on 135 patients. The surgical mortality of TAPVC was 5.9% (8/135). And there was no late death. The major causes of death were pulmonary infection and low cardiac output syndrome.A detailed classification of TAPVC is of great importance for surgical approaches and methodological designs. And an individualized surgical plan yields excellent patient outcomes.
The utilization of guidewires for adjusting the intraoperative catheter malposition during the venous access port implantation: A retrospective study

Abstract:The totally implantable venous access port has been widely used since first reported by Niederhuber et al in 1982, [ 1 ] especially for patients with malignant tumors. [ 2 , 3 ] Nowadays most medical centers can carry out the port implantation maturely. While the chest wall ports via the internal jugular vein (IJV) approach are the most widely used, [ 4 , 5 ] the port implantation via the AxV/SCV approach has been on the rise in recent years. [ 6 ] Due to the ultrasound-guided technique, the chest wall port via the AxV is becoming more and more mature, [ 7–9 ] which greatly reduces the risk of pinch-off syndrome caused by the SCV approach and significantly improves the patient's comfort and appearance because it avoids the subcutaneous tunnel crossing the collarbone. [ 10 ] However, there is still a risk that the guidewire or catheter may easily enter the IJV using the AxV/SCV approach for totally implantable venous access ports (TIVAP) implantation, especially when the angle between the SCV and the ipsilateral IJV is >90°. [ 11 ] Typically, most physicians will ask the patient to tilt the head to the operative side or press on the supraclavicular fossa to allow the guidewire or catheter to enter the superior vena cava (SVC). [ 12 ] However, in many cases, this method may not obtain good efficiency due to the deep location of the vein. In the field of endovascular therapy, the guidewire can easily help the catheter to arrive into the targeted vessel. [ 13–15 ] If the technique of endovascular guidewire application can be utilized in TIVAP implantation, the above problem will be well solved. Under fluoroscopic guidance, the guidewire can enter into the SVC explicitly, which means the success of the TIVAP implantation. In the present study, we will share some experiences of how to dexterously use guidewire in TIVAP implantation in different situations.

Abstract:Objective: To summarize the disposal methods and the reasons of complications in operation of totally implantable central venous port (TICVP). Methods: A total of 2 007 patients were enrolled in this observational, single-center study between December 2008 and March 2013. TICVP implantation was performed with one small skin incision and subcutaneous puncture of subclavian or jugular vein. Patient's profiles, indications of port system, early and delayed complications, and disposal methods were evaluated. There were 38 male and 1 969 female patients, aged from 21 to 85 years, with a mean of 47.6 years. Results: The mean duration of the TICVP system was (242 ± 12) days, ranging from 9 to 1 243 days. The achievement rate of puncture in the right jugular vein (99.76%) was the highest. Sonographic approach using the internal jugular vein were better than the external landmark-guided technique (99.80% vs. 96.34%, χ² = 29.905, P = 0.000). The rate of immediate complication was 0.80%, which included pneumothorax, hemothorax, lymphatic fistula and thrombosis. Early complications rate was 0.10%, which included pocket hematoma, catheter migration, venous thrombosis, port pocket infection, fibrin sheath formation. Late complications rate was 7.87%, which included catheter fracture, pinch-off syndrome, catheter-related bloodstream infection, fibrin sheath formation, catheter migration, extravasation, port inversion and port reveal. The rate of removal due to complications was 1.34% (27/2 007), and the early complication was higher (χ² = 8.053, P = 0.011). Conclusions: The low incidence of complications suggests that TICVP is safe and reliable for long term intermittent venous access. The results support the use of TICVP in the oncology patients and patients requiring long-term intravenous therapy.
Effect and Safety of an Innovative Improved Totally implanted venous access port (TIVAP) Implantation (Noninvasive Chest Wall Implantation) via the Axillary Approach on Patients with Breast Cancer

Abstract:Abstract Purpose The totally implanted venous access port (TIVAP) is widely applied to chemotherapy. The traditional approach is to implant the port by directly cutting skin at the chest wall, but surgical scarring on the chest wall may bring permanent psychological trauma to patients and affect the port once the incision is infected. In this study, the effect and safety of an improved port implantation (noninvasive chest wall implantation) via the axillary approach on breast cancer patients were evaluated. Methods This study discusses the surgical steps of the improved port implantation. The incidences of complication, comfort, convenience, aesthetics, and privacy from the improved operation were analyzed and compared with the traditional operation. Results All patients successfully presented improved infusion port implantation through the axillary access (noninvasive chest wall implantation). Two cases had a hemorrhage during the operation. One case had a postoperative subcutaneous hemorrhage, and one case had a folded catheter. Nevertheless, the patients did not need a secondary operation for adjustment. The average operation time of the improved infusion port implantation was 51.85 min (range: 37–69 min). The improved operation was significantly better than the traditional operation in terms of aesthetics and privacy. In terms of comfort and convenience, the difference between the two operations was not significant. Conclusions This study described the specific steps and particular aspects of the improved operation. The effectiveness and safety of the improved operation were reported for the first time. The improved operation has been proven safe and reliable, and it entails only a few intraoperative and postoperative complications.

Abstract:Introduction and importance: The use of a Totally Implantable Venous Access Port (TIVAP) has been a popular access option in chemotherapy for cancer patients, but complications, both long-term and short-term, may arise in the fixation process. This paper discusses the importance of detection and management of complications that arise as a result of TIVAP insertion. Case presentation: A 51-year-old female patient came to the hospital to undergo a TIVAP implantation for her chemotherapy through the right subclavian vein; however, a false route occurred to the right internal jugular vein instead of the right atrium. No direct revision was conducted for this case. Follow-up was conducted for 3 months. The first month post-chemotherapy, the access flow remained smooth. No complications were found. Clinical discussion: The right subclavian vein is preferred for its low complication and high success rates. Typically, a C-Arm is used to guide TIVAP insertion, allowing immediate corrections to prevent complications. In this patient, the C-Arm was unavailable due to logistical constraints. The TIVAP catheter tip was evaluated and sustained. High-velocity flow at the catheter tip can cause a jet effect, potentially shifting the catheter tip cranially or to the proper position in the right atrium. Conclusion: We conclude that TIVAP attachment does not have to be redone when a false route happens, and routine observation on possible complications as well as gradual drug administration should be done instead.
Implantation of Totally Implantable Venous Access Ports via External Jugular Vein in Chemotherapy for Cancer

Abstract:Objective To explore the feasibility and complications of totally implantable venous access ports ( TIVAP ) implanted via external jugular vein in chemotherapy for cancers. Methods A retrospective review was conducted on 96 patients undergoing a TIVAP implantation for chemotherapy at this hospital from January 2010 to December 2013.Perioperational and long-term complications of the procedure were observed. Results The TIVAP was inserted in all the cases.The external jugular vein approach was successfully utilized in 92 cases (95.8%).In the remaining 4 cases (4.2%), the TIVAP placement was performed through ipsilateral internal jugular vein because of contractured vein due to preoperatively radiotherapy in the neck area in 2 cases and fine veins unable to implant the catheter in 2 cases.No immediate postoperative complications were detected.The patients were followed up for a median of 26 months (8-46 months).The overall complication rate was 6.2% (6/96).There were 2 cases of catheter occlusion because of tumor recurrence, 1 case of migration of the catheter, 2 cases of thrombogenesis, and 1 case of clavicular skin pain.TIVAP worked well without leakage, pneumothorax, infection, or pinch-off syndrome. Conclusions External jugular vein approach appears to be a highly successful and safe alternative route and may be considered a potentially useful primary route for TIVAP placement in cancer patients.Main complications include catheter migration, occlusion, and thrombosis.
Durable Vascular Access in Neonates in the Cardiac ICU: A Novel Technique for Tunneled Femoral Central Venous Catheters

Abstract:Objectives: There is an ongoing need for a method of obtaining long-term venous access in critically ill pediatric patients that can be completed at the bedside and results in a durable, highly functional device. We designed a novel technique for tunneled femoral access to address this need. Herein, we describe the procedure and review the outcomes at our institution. Design: A single-center retrospective chart review identifying patients who underwent tunneled femoral central venous catheter (tfCVC) placement between 2017 and 2021 using a two-puncture technique developed by our team. Setting: Academic, Quaternary Children's Hospital with a dedicated pediatric cardiac ICU (CICU). Patients: Patients in our pediatric CICU who underwent this procedure. Interventions: Tunneled femoral central line placement. Measurements and main results: One hundred eighty-two encounters were identified in 161 patients. The median age and weight at the time of catheter placement was 22 days and 3.2 kg. The median duration of the line was 22 days. The central line-associated bloodstream infection (CLABSI) rate was 0.75 per 1,000-line days. The prevalence rate of thrombi necessitating pharmacologic treatment was 2.0 thrombi per 1,000-line days. There was no significant difference in CLABSI rate per 1,000-line days between the tfCVC and nontunneled peripherally inserted central catheters placed over the same period in a similar population (-0.40 [95% CI, -1.61 to 0.82; p = 0.52]) and no difference in thrombus rates per 1,000-line days (1.37 [95% CI, -0.15 to 2.89; p = 0.081]). Conclusions: tfCVCs can be placed by the intensivist team using a two-puncture technique at the bedside with a high-rate of procedural success and low rate of complications. Advantages of this novel technique of obtaining vascular access include a low rate of CLABSIs, the ability to place it at the bedside, and preservation of the upper extremity vasculature.
Perioperative and Postoperative Complications of Ultrasound-Guided Totally Implantable Venous Access Ports via the Brachiocephalic Vein in Patients with Cancer: A Prospective Study

Abstract:<p><b>Objectives:</b> To evaluate the safety and efficacy of ultrasound (US)-guided totally implantable venous access ports (TIVAPs) via the right brachiocephalic vein (BCV) or the left BCV approach.</p><p><b>Methods</b>: Patients requiring TIVAP for chemotherapy were included in the study. US-guided TIVAPs via BCV were used for patients from July 2018 to December 2018. General information about the patients (sex, age, and diagnosis), side (right or left), surgical procedures and complications were recorded.</p><p><b>Results:</b> A total of 107 TIVAPs in 107 patients (ages 38-73 years) were included, 75 via the right BCV and 32 via the left BCV. All of the patients underwent successful surgery. The BCV was successfully punctured on the first attempt in 99 patients (92.52%). Two attempts were needed in 6 patients (5.61%), and three attempts were necessary in 2 patients (1.87%). The mean operation time was 29 ± 5 min (range: 24 to 38 min). No serious complications occurred during the surgery, except the formation of a local haematoma in 1 case after artery puncture. During the follow-up period of 12 months, the incidence of long-term complications was 3.74% (4/107), including 2 cases of catheter-related infection and 2 cases of fibrin sheath formation. No serious complications such as catheter malposition or rupture were found.</p><p><b>Conclusion:</b> US-guided TIVAP via the BCV offers an alternative for adults with good needle guidance and a low rate of perioperative and postoperative complications.</p>

Abstract:Background: The purpose of our study is to compare the reliability and consistency of three commonly used techniques, which are surface measurement, intracardiac electrocardiogram (IECG) and tracheal bifurcation methods in measuring the tip location of totally implantable venous access port (TIVAP) in the same patient. Methods: The thirty-five patients scheduled for implantation of TIVAP were included and right subclavian vein was selected for venous access. We used surface measurement method (Group L) to estimate the length and position of catheter before catheterization, and then we used IECG method (Group E) to confirm the position of catheter during catheterization and after catheterization used tracheal bifurcation method by CT (Group T) for measurement. The differences of catheter length measured by three methods were compared by non-parametric Kruskal-Wallis test. Intraclass correlation coefficient (ICC) was used to evaluate the reliability of three methods and Bland-Altman chart was used to evaluate consistency. Results: There are no difference in the length of TIVAP catheter between L and T groups (P > 0.05), but they have significant differences comparing with E group (P < 0.05). The three positioning methods have a good consistency (ICC = 0.886, P < 0.05).Through linear regression analysis, the regression equation are: catheter length (mm) = 77.32 + 0.66 x height (cm), 36.25 + 0.81 x height (cm) and 68.82 + 0.70 x height (cm). The distances from catheter tip to the junction of superior vena cava and right atrium measured by IECG (Group E) was closer to the target value. Conclusions: All three methods can be used to measure the length of TIVAP catheter, and IECG method is the most accurate. And similar to most studies, returning the catheter 20 mm after positioning can avoid catheter entering the right atrium, but we found that 10 mm is a sufficient length to achieve this. Methods: The 110 children who received TIVAD implantation for short bowel syndrome and solid tumors in our department from 2021.12 to 2022.12 were selected as the study subjects. Blinded method was used and divided into experimental group and control group according to random number table The experimental group underwent painless surround puncture method to place the needles and compound lidocaine ointment for topical anesthesia, while the control group underwent traditional puncture method to complete this operation. The effects of the two puncture methods on pain, catheter seal fluid volume, and catheter occlusion rate were evaluated using the Facial Pain Scale Revised, Behavioral Assessment Scale, and in vitro digital subtraction angiography test. Results: In the control group, the degree of puncture pain was mild in 5 patients, moderate in 19 patients, and severe in 28 patients; the amount of catheter sealing solution was 9.32 ± 1.32 mL, and the catheter occlusion rate was 25.00%. In the experimental group, the degree of puncture pain was mild in 16 patients, moderate in 22 patients, and severe in 16 patients; the amount of sealing solution was 7.66 ± 1.08 mL, and the blocking rate was 9.26%. The total pain score in the experimental group was lower than that in the control group (5.23± 6.17 VS 7.89± 2.38). The difference between the two groups had statistical significance (P < 0.05). Conclusion: The use of the painless surround puncture method can effectively reduce the pain experienced by children during puncture, decrease the volume of catheter sealing fluid, reduce the rate of catheter blockage, provide a valuable basis for enhancing the maintenance effect of TIVAD in clinical practice for children. The 2016 edition of the Chinese Consensus on the Diagnosis and Treatment of Short Bowel Syndrome proposes that children with short bowel syndrome need long-term parenteral nutritional support interventions due to the fact that most of the small bowel has been resected and absent, which makes the absorption capacity of the small bowel limited. And the treatment method for children with solid tumors is mostly chemotherapy. During the chemotherapy process, multiple punctures of the patient's veins are required, and the extravasation of chemotherapy drugs can cause complications such as skin necrosis. To ensure the smooth completion of parenteral nutrition and chemotherapy treatment, effective venous channels need to be established. 1–3 Totally implantable venous access device (TIVAD) is a kind of closed venous infusion device that can be placed into the subcutaneous and left in the body for a long period of time, which consists of a pump body and a catheter that can be visualized, and it can prevent the damage of irritating drugs to the vein and reduce the risk of drug extravasation and chemotherapy by the local high flow rate and high flow rate of the blood that can rapidly dilute and disperse the drugs. Implantable Venous Access Devices in Pediatric Malignancies - Institutional Experience in a Developing Nation

Abstract:Background: Implantable venous access devices (IVAD) are preferred over long term external catheters in children due to less infection rates and better patient compliance in pediatric malignancies. Use of IVAD is a routine practice in developed part of the world. However it needs more emphasis for its widespread use in developing nation in order to improve the quality of care in children with malignancy. Aims and objectives: We aimed at analyzing the outcome of IVAD in pediatric malignancies in a tertiary care set up of developing nation. Our objective is to enlighten the importance and feasibility of IVAD in childhood malignancies with review of literature. Materials and methods: There were 152 children who underwent IVAD insertion in the study period. The parameters analyzed were indications, patient demography, size of the port, laterality of insertion, method of access to internal jugular vein (IJV), duration of surgery, time for access, complications, indication for removal and the parental satisfaction. Results: Mean age was 48 months. 112/152 patients had hematological malignancies. Right sided IJV was used by default in 97.4% patients, while remaining 2.6% had their left IJV cannulated. Open venotomy was used in 14 cases and 138 underwent ultrasound guided IJV access. The position of the catheter was reconfirmed in the X-ray, 6-8 hours after surgery. 149/152 ports were accessed 12 hours after surgery, whereas remaining 3 had a delay in access for 24 hours. Post operative complications were divided into early and late. 141 ports were removed after completion of chemotherapy, 4 were removed due to complications. 93 of parents gave the response as "satisfied". Conclusion: With proper training and expertise, insertion and care of IVAD is safe in pediatric malignancies without significant complications.

Abstract:<h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Background</h3><p>Optimal catheter tip position of totally implantable venous access port (TIVAP) is important to maintain its function and to avoid severe complications. In the present study, we aimed to assess the reliability of a modified surface measurement method to determine optimal tip position of a TIVAP catheter inserted through the right subclavian vein.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Patients and methods</h3><p>Clinical and radiological information of 105 patients who underwent TIVAP implantation through right subclavian vein were collected retrospectively. The length of implanted catheter was determined by a modified surface measurement method described as following. Distance from puncture point (point A) to middle point of sternal notch (point B), then middle point of sternal notch (point B) to middle point of Loui's angle (point C) was added up. The equation for the catheter length is given by: catheter length (cm) = AB + BC + 3. Post-procedure plain chest radiography (CXR) and enhanced chest computer tomography (CT) were used to check the catheter tip position and calculate optimal position rate. Distance from carina to catheter tip and length of vertebral body unit (VBU) was measured on both CXR and CT. Distance from carina to caval-atrial junction (CAJ) and catheter tip to CAJ was measured on CT.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Results</h3><p>Mean length of implanted catheter of all patients was 17.0±0.7cm (male vs. female, 17.3±0.5cm vs. 16.7±0.7cm, <em>P</em><0.001). On CXR test, catheter tip located within 2.4 VBUs below carina was identified as optimal position, and the optimal position rate was 97.1% (102/105 cases). On CT test, two definitions of optimal position were used (First, within 2cm above or below CAJ; Second, within 2cm above or at CAJ), and the optimal position rate was 92.4% (97/105 cases) and 78.1% (82/105 cases), respectively. Median follow-up time was 9.4 months. During the follow-up, no severe cardiac related complication was recorded.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Conclusion</h3><p>The modified surface measurement had a high reliability when determining the optimal catheter length to accurately place the tip in the SVC near the cava-atrial junction.</p>

Abstract:In children, there are two main techniques for placing a tunneled central venous catheter: single-incision (single puncture) and conventional (two punctures). Both have unique advantages and disadvantages. The modified single-stick technique combines the two aforementioned techniques to access the central venous system in an optimized way. This technique is feasible to perform particularly in young children and has a short learning curve for adult interventional radiologists.

Abstract:We reviewed the different approaches used for central vein access during insertion of cardiac implantable electronic devices. The benefits and hazards of each approach (cephalic vein cutdown, axillary vein cannulation using venography and ultrasound) are discussed. Each approach has its advantages and hazards that need to be considered for the individual patient and balanced against the skills of the operator. The benefits of ultrasound guided venous access in reducing radiation exposure to the patient and implanter, avoiding the need for angiographic contrast and in minimizing the risk of pneumothorax and inadvertent arterial puncture are highlighted. Trainees should be taught each approach to deal with patient variability. Ultrasound guidance should be considered as a mainstream option for most patients.
