Abstract:Objective To investigate whether ATB FUNGUS 3 method has the agreement in determining the susceptibility of pathogenic yeasts to common antifungal drugs with clinical and laboratory standards in-stitute(CLSI) microdilution method M27-A2. Methods The in vitro susceptibilities of 172 isolates of Candida albicans and Cryptococcus neoformans to the common antifungal agents, including amphtericin B (AMB), flucytosine (5-FC), fluconazole (FLC), voriconazole ( VRC ), and itraconazole (ITC), were assayed by using both ATB FUNGUS 3 method and CLSI M27-A2 broth microdilution method. Results The two methods show 93.0% agreements for ITC, 95.9% for AMB, 98.8% for 5-FC, 92.4% for FLC, and 93.6% for VRC. For Candida species, it was 81.7% agreement between the two methods for FLC, 59.2%, 90.9% and 96.5% agreement for ITC,VRC and 5-FC,respectively. Conclusion ATB FUNGUS 3 method is a rapid and simple procedure, and has good agreement with CLSI M27-A2 method in determining the antifungal susceptibility of pathogenic yeast to common antifungal drugs. Abstract:The keys for improving the diagnosis and treatment for deep fungal infection are to establish standardized and reliable laboratory methods as well as reasonable,practical,feasible and relatively uniform diagnostic criteria,and to select the appropriate antifungal agents.This review introduces the current detection methods of deep fungal infection applied extensively in our country,including traditional direct microscopy and fungal culture as well as a variety of fungal antigen-based immunoassay methods,such as Aspergillus antigen determination of galactomannan(GM),1,3-β-D-glucan test(G test) and cryptococcal latex agglutination tests,and analyzes the characteristics of these methods.

Abstract:Objective:To analyze the antifungal susceptibility and fungi distribution in clinical.Methods: Sabouraud agar and CHROMagar were used to culture fungi.Identifications of fungi strains were detected by API Candida and API20C AUX.Antifungal susceptibility was determined by Rosco disc agar diffusion method.Results: Candida was 53.5%,of which C.albicans was 25.6% in total of 700 clinical isolates;C.albicans were mainly isolated from ICU and Dept.of nephrology with 59.3% and 45.2% respectively;C.parapsilosis was main isolated from Dept.of Dermatology.Antifungal suscepitibility rates of Yeast-like fungi to itraconazole,fluconazole,amphotericin B,nystatin,ketoconazole were 89.4%,83.9%,98.2%,95.9% and 96.0% respectively.Conclusion: Candida spp.is the main fungi isolated from clinical.Species of fungi distribution depends on disease category of patients and the part of the body infected.To identify fungi accurately is important to avoid use fluconale for fluconale-resistant strain.Antifungal susceptibility test is helpful to know trends of antifungal resistance.

Abstract:Background Fusarium species are emerging causative agents of superficial and disseminated human infections. Early diagnosis and treatment contribute to better prognosis of severe infection. Objectives To detect the effectiveness of matrix-assisted laser desorption ionisation time of flight mass spectrometry (MALDI-ToF MS) for Fusarium identification, and evaluate the susceptibility profiles to clinical available antifungals. Methods All 203 clinical Fusarium isolates and 25 environmental isolates were identified by using translation elongation factor 1-alpha (TEF1) and RNA polymerase subunit II (RPB2) sequencing and MALDI-ToF MS. Antifungal susceptibility testing was determined by a microdilution method following the CLSI approved standard M38-A3 document. Results Correct identification rates at the species and genus levels were 89.04% (203/228) and 95.18% (217/228), respectively, using Bruker Filamentous Fungi Library 1.0 combined with the novel database. Seven species complexes with 19 Fusarium species were identified, including F. solani (59.21%, n = 135), F. verticillioides (17.54%, n = 40), F. proliferatum (6.58%, n = 15) and F. oxysporum (4.39%, n = 10). Four uncommon species complexes (F. incarnatum-equiseti SC, F. dimerum SC, F. redolens SC and F. sporotrichioides SC) were also identified. A high degree of antifungal resistance was observed. Fusarium isolates exhibited lower MICs to luliconazole and terbinafine compared with amphotericin B and voriconazole, which in turn were significantly more active than amorolfine, fluconazole and itraconazole. Conclusions MALDI-ToF MS showed good performance in Fusarium species with an adapted Bruker library and expanded database. Fusarium isolates exhibited lower MICs to luliconazole and terbinafine compared to amphotericin B and voriconazole.

Abstract:Objective: To analyze MICs of four kinds of antifungal drugs against the common pathogenic Fusarium spp.in order to provide reference for clinical therapy.Methods: The strains tested were cultured on the PDA at 25℃ for 7-10 days.Results: The MICs of amphotericin B and natamycin against F.solani and F.moniliforme were relatively lower,the MICs of itraconazole agaist F.solani and F.moniliforme were higher,and the MICs of terbinafine against F.solani were higher than those against F.moniliforme.According to the NCCLS M38-P,the MICs of four antifungal drugs including amphotericin B,itraconazole,natamycin and terbinafine were tested against the 91 strains of Fusarium spp.(Conclusion): Amphotericin B and natamycin have better inhibitory effects against F.solani and F.moniliforme in vitro,while itraconazole and terbinafine have lower inhibition.

Abstract:Over the past decade,due to broad-spectrum antibiotics,corticosteroid hormones,immunosuppressive and chemotherapy drugs were widely used,and organ transplantation,a variety of invasive examinations,treatment technology evolution,autoimmune diseases,cancer and other factors,the increasing immunocompromised population,causing deep fungal infection incidence was rising rapidly.Some new pathogenic fungus was emerging.But the rate of diagnosis in deep fungal infection was far lower than the actual rate of infection,mainly due to the uneven local hospital development,and varying levels of fungi detection technology.The status of current domestic deep fungal infection and its detection were analyzed.

Abstract:Objective:To study the diagnosis of feet infections with Fusarium solani and bacteria.Methods: The successive collection of pus and necrotic tissue were inoculated to media to cultivate bacteria and medical fungi.At the same time,the direct mycological examination was conducted.The bacteria was identified and tested for susceptibility with VITEK-2 full-automatic microbiological analyzer.Results: The Fusarium solani were isolated from the samples.According to the results of the drug sensitive tests,the Fusarium solani was sensitive to the voriconazole and the itraconazole.The Fusarium solani is a rare pathogen in the clinical infections.In cases of hypoimmunity,the multiple infections may occur.This case is mixed infection with Pseudomonas fluorescens.Conclusion: This case is mixed infection of Fusarium solani and Pseudomonas fluorescens on feet.

Abstract:In the last few years,the morbidity of pulmonary fungal infection is keeping increasing.However,misdiagnosis is very common in clinical practice since it has no specific clinical characteristics.Subsequently,delayed treatment happened.Laboratory test is the key method to diagnose pulmonary fungal infection.In fact,only pathology and positive cultural results obtained from germfree specimens could make the final diagnosis.Recently,some new methods put into effect such as G-test and GM-test.These methods greatly improve the early diagnosis of invasive fungal infection.Among them,PCR is a very prospective way.Nevertheless,the most basic and common used way is to detect and analysis sputum,blood and the specimen obtained through bronchoscopy.The specimen collecting process need to be standardized and the results need to be evaluated along with the clinical data. Abstract:Introduction. The increasing prevalence and growing resistance of fungi present a significant peril to public health. There are only four classes of antifungal medicines available today, and few candidates are in clinical trials. Hypothesis/Gap Statement. Rapid and sensitive diagnostic techniques are lacking for most fungal pathogens, and those that do exist are expensive or hard to obtain. Aim. This study aimed to evaluate the feasibility of a novel automated antifungal susceptibility testing system, Fungus AST, in comparison to the broth microdilution method (BMD) recommended by the Clinical and Laboratory Standards Institute (CLSI). Methodology. A total of 101 clinical Candida spp. isolates were collected from the Zengcheng Branch of Nanfang Hospital and subjected to antifungal susceptibility testing. Antifungal susceptibility was assessed using the Fungus AST method and the BMD. Results. In this study, we introduce a novel automated antifungal susceptibility testing system, Fungus AST, which detects the turbidity and/or colour intensity of microdilution wells using a four-wavelength detection technology in real time and is designed to match the growth characteristics of strains over time. Based on our analysis, all reportable ranges of Fungus AST were suitable for clinical fungal isolates in PR China. Within ±twofold dilutions, reproducibility was 100 %. Considering the BMD as a referenced method, ten antifungal agents (anidulafungin, caspofungin, micafungin, fluconazole, voriconazole, posaconazole, itraconazole, amphotericin B, 5-flucytosine and nystatin) showed an essential agreement of >95 %. The category agreement of five antifungal agents (anidulafungin, caspofungin, micafungin, fluconazole and voriconazole) was excellent at >90 %. One Candida albicans isolate and voriconazole showed a major error (ME) (1.7 %), and no other ME or very ME agents were found. Conclusion. Given the above, it can be argued that the utilization of Fungus AST is a discretionary automated approach. More improvements are needed in Fungus AST compared to the BMD system for a wider range of clinical isolates, including different types of fungi.

Abstract:Objective To establish the diagnostic assays of quantitative real-time polymerase chain reaction (PCR) for the diagnosis of invasive fungal disease and evaluate their accuracy in clinical samples.Methods Three assays have been developed for the diagnosis of clinically prevalent fungi,Aspergillus spp.and Candida spp..Their analytical sensitivity and specificity were evaluated.Twenty-one serum samples from invasive fungal disease patients and non-invasive fungal disease patients were analyzed by the diagnostic assays and the accuracy was evaluated.Results Three assays were managed to run at the same condition.The universal fungi assay was able to detect,but not differentiate between most of the pathogenic fungi,including A.fumigatus,A.flavus,A. niger,C albicans,C. parapsilosis,C. tropicalis,C kruseii,C.glabrata,Cryptococcus neoformans,Rhizomucor variabilis,Rhizopus arrhizus and Penicillosis marneffei.The assays of Aspergillus spp. and Candida spp. were able to detect,but not differentiate between A.fumigatus,A.flavus,A.niger and C.albicans,C.parapsilosis,C.tropicalis.The detection limits of assays were 4 pg of A.fumigatus gDNA,2 copies of A.fumigatus and 2 copies of C albicans respectively.The results of quantitative real-time PCR matched well with each other and were identical to the classical procedures in all patients.Among the 11 patients with invasive fungal disease,the pathogens were identified down to a genus level in 2 invasive aspergillosis patients and 2 invasive candidiasis patients.And the other 7 patients could be diagnosed with invasive fungal disease by the universal fungi assay. All the 10 serum samples from non-invasive fungal disease patients were revealed as negative.Conclusion The universal fungi assay is helpful in screening while the assays of Aspergillus spp.and the Candida spp.may identify the pathogens down to a genus level.
The separation and identification of clinical deep fungus and the analysis of drug susceptibility results

Abstract:Objective To identify 818 strains of yeastlike fungi and to detect their drug susceptibility for clinical reference. Methods Fungi were isolated by CHRM agar medium. Identification of fungi was performed by ID 32C. The resistance to antifungal agents, such as 5 fluorocystine, Amphotricin B, Nystatin, Miconazol, Econazol, Ketoconazol, was detected by ATB Fungus automatic analysis system. The susceptibility to flucona zole was tested by K B methods. Results The main strain isolated from 818 strains of yeastlike fungi was Candida , the isolation rate of C.albicans was the hightest. From the result of drug susceptibility of 157 strains of yeast, the susceptibilities to Amphotricin B, Nystatin, 5 fluorocystine were the highest, and Econazol was the lowest. From the drug susceptibility of 95 strains of yeast, the resistance of C.krusei to fluconazole was the strongest (100%). Conclusions By abusing broad antibiotic, immunity inhibitor and antitumour drugs, the change of fungus infection was increased. The isolation of fungi and the antifungal sensitivity test should be strengthened in order to guide clinical selection of the reasonable antibiotic.
High-Throughput Monitoring of Pathogenic Fungal Growth Using Whole Slide Imaging for Rapid Antifungal Susceptibility Assessment

Abstract:Invasive fungal infections are a major health threat with high morbidity and mortality, highlighting the urgent need for rapid diagnostic tools to detect antifungal resistance. Traditional culture-based antifungal susceptibility testing (AFST) methods often fall short due to their lengthy process. In our previous research, we developed a whole-slide imaging (WSI) technique for the high-throughput assessment of bacterial antibiotic resistance. Building on this foundation, this study expands the application of WSI by adapting it for rapid AFST through high-throughput monitoring of the growth of hundreds of individual fungi. Due to the distinct "budding" growth patterns of fungi, we developed a unique approach that utilizes specific cell number change to determine fungi replication, instead of cell area change used for bacteria in our previous study, to accurately determine the growth rates of individual fungal cells. This method not only accelerates the determination of antifungal resistance by directly observing individual fungal cell growth, but also yields accurate results. Employing Candida albicans as a representative model organism, reliable minimum inhibitory concentration (MIC) of fluconazole inhibiting 100% cells of Candida albicans (denoted as MIC100) was obtained within 3h using the developed method, while the modified broth dilution method required 72h for the similar reliable result. In addition, our approach was effectively utilized to test blood culture samples directly, eliminating the need to separate the fungi from whole blood samples spiked with Candida albicans. These features indicate the developed method holds great potential serving as a general tool in rapid antifungal susceptibility testing and MIC determination.

Abstract:Eleven reference and 25 clinical isolates of Fusarium were subject to multilocus DNA sequence analysis to determine the species and haplotypes of the fusarial isolates from Beijing and Shandong, China. Seven loci were analyzed: the translation elongation factor 1 alpha gene (EF-1α); the nuclear rRNA internal transcribed spacer (ITS), large subunit (LSU), and intergenic spacer (IGS) regions; the second largest subunit of the RNA polymerase gene (RPB2); the calmodulin gene (CAM); and the mitochondrial small subunit (mtSSU) rRNA gene. We also evaluated an IGS-targeted PCR/reverse line blot (RLB) assay for species/haplotype identification of Fusarium. Twenty Fusarium species and seven species complexes were identified. Of 25 clinical isolates (10 species), the Gibberella (Fusarium) fujikuroi species complex was the commonest (40%) and was followed by the Fusarium solani species complex (FSSC) (36%) and the F. incarnatum-F. equiseti species complex (12%). Six FSSC isolates were identified to the species level as FSSC-3+4, and three as FSSC-5. Twenty-nine IGS, 27 EF-1α, 26 RPB2, 24 CAM, 18 ITS, 19 LSU, and 18 mtSSU haplotypes were identified; 29 were unique, and haplotypes for 24 clinical strains were novel. By parsimony informative character analysis, the IGS locus was the most phylogenetically informative, and the rRNA gene regions were the least. Results by RLB were concordant with multilocus sequence analysis for all isolates. Amphotericin B was the most active drug against all species. Voriconazole MICs were high (>8 μg/ml) for 15 (42%) isolates, including FSSC. Analysis of larger numbers of isolates is required to determine the clinical utility of the seven-locus sequence analysis and RLB assay in species classification of fusaria.
Analytical and clinical validation of multiplex droplet digital PCR assay for detecting pathogenic fungal infection in lungs

Abstract:Pulmonary invasive fungal infection in immunocompromised hosts is difficult to diagnose, and current tools for diagnosis or monitoring of response to antifungal treatments have inherent limitations. Droplet digital PCR (ddPCR) has emerged as a promising tool for pulmonary pathogen detection with high sensitivity. This study presents a novel ddPCR panel for rapid and sensitive identification of pulmonary fungal pathogens. First, a ddPCR method for detecting three fungal genera, including Pneumocystis, Aspergillus, and Cryptococcus, was established and evaluated. Then, the clinical validation performance of ddPCR was compared with that of qPCR using 170 specimens, and the 6 specimens with inconsistent results were further verified by metagenomics next-generation sequencing, which yielded results consistent with the ddPCR findings. Finally, the area under the ROC curve (AUC) was used to evaluate the efficiency of ddPCR. While the qPCR identified 16 (9.41%) cases of Aspergillus and 6 (3.53%) cases of Pneumocystis, ddPCR detected 20 (11.76%) Aspergillus cases and 8 (4.71%) Pneumocystis cases. The AUC for Aspergillus, Cryptococcus, and Pneumocystis was 0.974, 0.998, and 0.975, respectively. These findings demonstrated that the ddPCR assay is a highly sensitive method for identifying pathogens responsible for invasive fungal pulmonary infections, and is a promising tool for early diagnosis. .

Abstract:The increasing resistance among fungi to antimicrobials are posing global threats to health. Early treatment with appropriate antifungal drugs guided by the antifungal susceptibility testing (AFST) can dramatically reduce the mortality of severe fungal infections. However, the long test time (24–48 h) of the standard AFSTs cannot provide timely results due to the slow growth of the pathogen. Herein, we report a new AFST that is independent of growth rate analysis using a luminogen with aggregation-induced emission characteristics (AIEgen) named DMASP. DMASP is a water-soluble small-molecule probe that can readily penetrate the dense fungal cell wall. Based on its mitochondria-targeting ability and AIE characteristics, fungal activity can be dynamically indicated via real-time fluorescence monitoring. This allows fungal susceptibility to various antimicrobials to be assessed within 12 h in a wash-free, one-step manner. This method may serve as a promising tool to rapidly detect possible drug-resistant fungal strain and guide the precise use of antimicrobial against fungal diseases.

Abstract:<h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Background/purpose</h3><p>As the incidence of fungal infections in China increases, the demand for rapid and accurate diagnosis of mycoses is growing. Yet, information on current diagnostic capacity is scarce.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Methods</h3><p>An online survey was conducted in February 2018 to collect information on mycology testing from tertiary care hospitals across China. Responses from 348 hospitals were analyzed, and a scoring system was designed and employed to assess the overall diagnostic capacity.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Results</h3><p>Most of the surveyed hospitals did not have separate laboratory space, manpower, or equipment dedicated for fungal testing. Conventional staining methods were widely available (>70%), whereas GMS and fluorescent staining were less common. Fungal identification services were offered mostly with chromogenic medium, morphological characterization or automated identification systems, other than more advanced methods such as MALDI-TOF MS and DNA sequencing. Fungal serology testing was available in 81.1%, with G test being the most often used. Though 91.8% of the respondents had the ability to perform antifungal susceptibility testing for yeasts, less than 13% conducted such testing for molds. The percentage of laboratories participating in External Quality Assessment programs and research was 57.5% and 32.5%, respectively. The average score for the 348 surveyed hospitals was 37.2 (out of a maximum of 89 points), with only 15 hospitals scoring >60, suggesting a general lack of high-quality mycology laboratories.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Conclusions</h3><p>The overall clinical testing capacity for fungal infection in China is insufficient. More investment and training efforts are warranted to establish centers of excellence and promote access to high-quality diagnostic services.</p>

Abstract:Aim: To investigate the species distribution and antifungal susceptibility profiles of yeast isolates causing invasive infections across Beijing. Materials & methods: A total of 1201 yeast isolates recovered from blood and other sterile body fluids were correctly identified by matrix-assisted laser desorption/ionization TOF MS supplemented by DNA sequencing. Antifungal susceptibility testing was performed according to the Clinical and Laboratory Standards Institute broth microdilution method. Results: Candida (95.5%) remained the most common yeast species isolated; Candida albicans (38.8%) and Candida parapsilosis (22.6%) were the leading species of candidemia. Azole resistances were mainly observed in Candida glabrata and Candida tropicalis isolates. Conclusion: This study outlined the epidemiologic data of invasive yeast infections and highlighted the need for continuous monitoring of azole resistances among C. glabrata and C. tropicalis isolates in Beijing.

Abstract:Abstract Aspergillus species is a widespread environmental mould that can cause aspergillosis. The purpose of this study was to investigate the antifungal susceptibility profile and genotypic characterization of clinical Aspergillus isolates from different provinces in Eastern China. The data included the antifungal susceptibility distributions with eight common antifungal drugs, cyp51A gene mutations of triazole-resistant A. fumigatus sensu stricto, and the genotypic relationships among the A. fumigatus sensu stricto isolates based on microsatellite typing. A. fumigatus sensu lato was the most common clinical Aspergillus species (n = 252), followed by A. flavus (n = 169), A. terreus (n = 37), A. niger (n = 29) and A. nidulans (n = 4). The modal minimum effective concentration values of micafungin and anidulafungin were lower than those of caspofungin for all Aspergillus species. The in vitro efficacy of isavuconazole was similar to that of voriconazole against most Aspergillus species. Sequencing revealed cyp51A gene mutations TR34/L98H, TR34/L98H/S297T/F495I, and TR46/Y121F/T289A in four triazole-resistant A. fumigatus sensu stricto. Phylogenetic analyses using microsatellite markers of A. fumigatus sensu stricto revealed that 211 unique genotypes clustered into two clades. The data demonstrate the diversity of clinically relevant Aspergillus species in Eastern China. Routine antifungal susceptibility testing should be performed to monitor the antifungal resistance and guide clinical therapy.
