Abstract:The advance of molecular diagnosis has evolved the field of detection of pathogens in pediatric disease towards the high-throughput ultra-sensitive molecular testing. This review provides an overview and comparison of current molecular tools on the market, including qPCR, multiplex PCR, digital PCR, isothermal amplification, and microfluidic chip, highlighting the future trends of integrated high-throughput molecular testing and automation. These evolutions will maximize the utility of limit patient samples from children and contribute significantly on the diagnosis of infectious disease.
Application of molecular biology techniques in the identification of pathogenic fungi and the diagnosis of fungal infection

Abstract:With the increasing incidence and mortality of fungal infection, the requirements for strict diagnostic approaches became a very urgent issue. Because of the traditional detective techniques, such as culture, gave poor diagnostic outcomes, the molecular biological techniques are expected to develop the potential diagnostic approaches. During the past decades, we have carried out serial studies on the molecular properties of pathogenic fungi, and we would like to review as following. Firstly, we applied several molecular tools in classification and identification of pathogenic fungi. We performed random amplification of polymorphic DNA (RAPD), restriction fragment length polymorphism (RFLP) and other techniques in studying the typing, to classify and identify the properties of Dermatophytes, Candida spp., Cryptococcus neoformans, Dematiaceous fungi, and Aspergillus spp. Interestingly, we found the same T. rubrum strain might infect different sites of the host, while a site-specificity displayed in T. mentagrophytes. This finding indicated the genetic discrepancies among the fungi. Beside, we also found that the E. dermatitis strains with different virulences possessed some discrepancies at gene level. We then developed a PCR-based molecular procedure to identify the novel species in Exophiala spp. As the applicable strategy, we also investigated the rDNA sequence properties in several fungi. And as a result, we submitted for the first time to GenBank the complete sequence of Aspergillus fumigatus rDNA/ITSI/ITSII, which provided the basis for designing the species-specific probes and for its further clinical applications. Secondly, we have tried to develop the molecular diagnostic approaches based on our DNA sequence data which were used for identification studies previously. By analyzing the DNA sequence of Aspergillus fumigatus rDNA/ITSI/ITSII, we developed a nested PCR method to detect Aspergillus fumigatus genes. Our preliminary results indicated that this PCR-based molecular approach has great importance in the diagnosis of invasive aspergillosis. We also designed the species-specific probes and then established several in situ hybridization procedures. We found these hybridization methods could get the positive rate up to 81% (13/16), which suggests that these methods have potential diagnostic value for invasive candidiasis and aspergillosis. Based on our experiences, we would conclude that the molecular biological techniques possess great value to investigate the biological properties of pathogenic fungi, and we are looking forward to see more and more molecular tools will be used in the pathogenic mechanisms of fungal infections and antifungal activity studies.

Abstract:Next-generation sequencing (NGS) has become a widely used technology in biological research. NGS applications for clinical pathogen detection have become vital technologies. It is increasingly common to perform fast, accurate, and specific detection of clinical specimens using NGS. Pathogenic fungi with high virulence and drug resistance cause life-threatening clinical infections. NGS has had a significant biotechnological impact on detecting bacteria and viruses but is not equally applicable to fungi. There is a particularly urgent clinical need to use NGS to help identify fungi causing infections and prevent negative impacts. This review summarizes current research on NGS applications for fungi and offers a visual method of fungal detection. With the development of NGS and solutions for overcoming sequencing limitations, we suggest clinicians test specimens as soon as possible when encountering infections of unknown cause, suspected infections in vital organs, or rapidly progressive disease.

Abstract:Improvement of early and specific diagnosis of fungal infection is the key point remains to be resolved in the clinical mycology field.Besides the basic mycological methods,there is an urgent needs to develop the new diagnosis assays such as the non-culture methods for detection of the fungal antigen and nuclear acid.The prospective study performed by multiple-center is quite necessary to better standardize these methods.The role of clinical mycology laboratory should be fully emphasized.Through the setting up of standardized mycological diagnosis system and the application of modernized methods,and the diagnosis of funsal infection could be improved.

Abstract:Despite the scientific advances observed in the recent decades and the emergence of new methodologies, the diagnosis of systemic fungal infections persists as a problematic issue. Fungal cultivation, the standard method that allows a proven diagnosis, has numerous disadvantages, as low sensitivity (only 50% of the patients present positive fungal cultures), and long growth time. These are factors that delay the patient's treatment and, consequently, lead to higher hospital costs. To improve the accuracy and quickness of fungal infections diagnosis, several new methodologies attempt to be implemented in clinical microbiology laboratories. Most of these innovative methods are independent of pathogen isolation, which means that the diagnosis goes from being considered proven to probable. In spite of the advantage of being culture-independent, the majority of the methods lack standardization. PCR-based methods are becoming more and more commonly used, which has earned them an important place in hospital laboratories. This can be perceived now, as PCR-based methodologies have proved to be an essential tool fighting against the COVID-19 pandemic. This review aims to go through the main steps of the diagnosis for systemic fungal infection, from diagnostic classifications, through methodologies considered as "gold standard", to the molecular methods currently used, and finally mentioning some of the more futuristic approaches.

Abstract:Infectious diseases are a major global cause of morbidity and mortality, seriously affecting public health and socioeconomic stability. Since infectious diseases can be caused by a wide variety of pathogens with similar clinical manifestations and symptoms that are difficult to accurately distinguish, selecting the appropriate diagnostic techniques for the rapid identification of pathogens is crucial for clinical disease diagnosis and public health management. However, traditional diagnostic techniques have low detection rates, long detection times and limited automation, which means that they do not meet the requirements for rapid diagnosis. Recent years have seen continuous developments in molecular detection technology, which has a higher sensitivity and specificity, shorter detection time and increased automation, and performs an important role in the early and rapid detection of infectious disease pathogens. The present study summarizes recent progress in molecular diagnostic technologies such as PCR, isothermal amplification, gene chips and high‐throughput sequencing for the detection of infectious disease pathogens, and compares the technical principles, advantages and disadvantages, applicability and costs of these diagnostic techniques.

Abstract:The global burden of invasive fungal infections (IFIs) has shown an upsurge in recent years due to the higher load of immunocompromised patients suffering from various diseases. The role of early and accurate diagnosis in the aggressive containment of the fungal infection at the initial stages becomes crucial thus, preventing the development of a life-threatening situation. With the changing demands of clinical mycology, the field of fungal diagnostics has evolved and come a long way from traditional methods of microscopy and culturing to more advanced non-culture-based tools. With the advent of more powerful approaches such as novel PCR assays, T2 Candida, microfluidic chip technology, next generation sequencing, new generation biosensors, nanotechnology-based tools, artificial intelligence-based models, the face of fungal diagnostics is constantly changing for the better. All these advances have been reviewed here giving the latest update to our readers in the most orderly flow.

Abstract:Candida albicans is a pathogenic yeast fungus that causes oral,upper respiratory,vaginal and systemic infections in huaman and animals.With the large useage of the antifungal agents,drug resistance of fungi has becoming more and more serious.Clinical investigation showed C.albicans infection has already gone up to the second in fungous infection.Correct identifications and classifications have an advantage for prompt diagnosis,prevention and treatment of these infection.The paper summarized the methods of typing C.albicans by molecular biological technique.

Abstract:The incidence of opportunistic fungal infection has been gradually increasing over recent years,especially the high morbidity and mortality of invasive Candida infections in immunocompromised individuals.With the emergence and increasing of rare species and drug-resistant strains, the accurate detection and identifica‐tion of pathogenic Candida species have been paid more and more attention in clinical practice. In addition to thetraditional direct microscopic examination and culture, the commercial identification methods developed depend‐ing on physiological characteristics also play an important role. Moreover, based on the development of molecularbiology theory and technology of Candida species, some methods for detecting and identifying pathogenic Candidaspecies have been generated, such as gene amplification and sequencing, real-time quantitative PCR, mNGS, andMALDI-TOF-MS. It should be noted that T2 Candida system has been approved for clinical diagnosis of candidemia,showing the intersection and integration of pathogenic mycology, medical imaging, and biomedical engineering.The progress in the detection and identification of pathogenic Candida species has important implications and putsforward new efforts for the research of pathogenic mycology and related transformation in China.

Abstract:Purpose of Review Molecular assays for fungal pathogen detection directly in patient specimens represent valuable diagnostic tools, with the potential to increase sensitivity and reduce turnaround time. This review highlights existing and emerging molecular methods for the diagnosis of fungal infections, their clinical performance, limitations, and knowledge gaps. Recent Findings Broad-range PCR and metagenomic sequencing detect diverse pathogens but have not replaced traditional methods. Limitations are cost, availability, and limited real-world performance data. Multi-target panels and organism-specific assays can provide increased sensitivity and, frequently, speed. Some single- or multi-target assays are commercially available to many clinical laboratories. Summary Molecular diagnostic methods for fungal pathogens are diverse; complement culture, stains, and biomarkers; and increasingly fulfill case definitions. To maximize yield, assay selection should be guided by suspected organisms and patient-specific risk factors. Collaboration between patient-facing providers and laboratorians/pathologists promotes optimal testing strategies and the deployment of more rapid and specific assays.
The future of novel diagnostics in medical mycology

Abstract:Several fungal diseases have become serious threats to human health and life, especially upon the advent of human immunodeficiency virus/AIDS epidemics and of other typical immunosuppressive conditions of modern life. Accordingly, the burden posed by these diseases and, concurrently, by intensive therapeutic regimens against these diseases has increased worldwide. Existing and available rapid tests for point-of-care diagnosis of important fungal diseases could enable the limitations of current laboratory methods for detection and identification of medically important fungi to be surpassed, both in low-income countries and for first-line diagnosis (screening) in richer countries. As with conventional diagnostic methods and devices, former immunodiagnostics have been challenged by molecular biology-based platforms, as a way to enhance the sensitivity and shorten the assay time, thus enabling early and more accurate diagnosis. Most of these tests have been developed in-house, without adequate validation and standardization. Another challenge has been the DNA extraction step, which is especially critical when dealing with fungi. In this paper, we have identified three major research trends in this field: (1) the application of newer biorecognition techniques, often applied in analytical chemistry; (2) the development of new materials with improved physico-chemical properties; and (3) novel bioanalytical platforms, allowing fully automated testing. Keeping up to date with the fast technological advances registered in this field, primarily at the proof-of-concept level, is essential for wise assessment of those that are likely to be more cost effective and, as already observed for bacterial and viral pathogens, may provide leverage to the current tepid developmental status of novel and improved diagnostics for medical mycology.

Abstract:Invasive fungal diseases (IFDs) comprise a growing healthcare burden, especially given the expanding population of immunocompromised hosts. Early diagnosis of IFDs is required to optimise therapy with antifungals, especially in the setting of rising rates of antifungal resistance. Molecular techniques including nucleic acid amplification tests and whole genome sequencing have potential to offer utility in overcoming limitations with traditional phenotypic testing. However, standardisation of methodology and interpretations of these assays is an ongoing undertaking. The utility of targeted Aspergillus detection has been well-defined, with progress in investigations into the role of targeted assays for Candida, Pneumocystis, Cryptococcus, the Mucorales and endemic mycoses. Likewise, whilst broad-range polymerase chain reaction assays have been in use for some time, pathology stewardship and optimising diagnostic yield is a continuing exercise. As costs decrease, there is also now increased access and experience with whole genome sequencing, including metagenomic sequencing, which offers unparalleled resolution especially in the investigations of potential outbreaks. However, their role in routine diagnostic use remains uncommon and standardisation of techniques and workflow are required for wider implementation.

Abstract:Abstract Poster session 3, September 23, 2022, 12:30 PM - 1:30 PM Objective The main objective of this study was to design and develop a detection system based on qRT-PCR that can quickly and accurately identify the pathogenic fungi of tinea capitis, in order to improve the diagnostic ability of tinea capitis. Methods Results Conclusions The seven single-tube qRT-PCR assays validated in this study can rapidly detect a variety of pathogenic fungi causing tinea capitis, with a high level of sensitivity and specificity. The combination of qRT-PCR and traditional mycological identification methods can further improve the diagnostic efficacy of tinea capitis.

Abstract:BACKGROUND: Bacterial and fungal infections that caused by various kinds of pathogens are frequently-occurring disease all over the world. To overcome the shortcomings of traditional culture method, we have adapted next-generation sequencing (NGS) technology to identify pathogens.METHODS: In this study, clinical samples from 20 patients pre-diagnosed with bacterial and fungal infections in the Shanghai Pulmonary Hospital of Tongji University, China, were investigated retrospectively to compare the NGS with conventional "gold standard" culture methods.RESULTS: The detection rates of bacterial or fungal infections were 95.0% (19/20) by NGS and 60.0% (12/20) by culture method. There was a significant difference between the results of NGS and traditional culture method by using McNemar's χ2 test (P=0.008).CONCLUSIONS: NGS, as an emerging diagnostic technology, shows outstanding advantages in the diagnosis of bacterial and fungal infections, and optimizes the treatment of infectious diseases. The clinical application and future development of NGS technology are worthy of expectation.

Abstract:With the development of molecular biology over the decades, molecular technologies have become available for clinical diagnosis of infectious disease. The principle of the technique is to detect specific genes of microorganisms. Compared with the traditional culturing techniques, molecular technologies have several advantages: 1) direct detection and rapid identification of organisms that are slow growing or those currently lacking a system for in vitro cultivation; 2) nucleic acid-based methods for epidemiologic typing of microorganisms; and 3) identification of genotypic markers of microbial resistance to specific antibiotics. When sufficient amounts of nucleic acid can be obtained, direct probe tests (simple molecular hybridization-based techniques) may be performed. In many instances with small amounts of sample nucleic acids, amplification techniques(e.g., polymerase chain reaction[PCR]) are extremely useful. However, the application of PCR to clinical specimens has potential pitfalls due to the susceptibility of PCR to inhibitors, contamination, and experimental conditions. In some cases, DNA fingerprinting or restriction fragment-length polymorphism analysis is used and it has become the method of choice for epidemiologic analysis. The major disadvantage of the molecular method is that we cannot differentiate whether the microorganisms identified are alive or dead and chemosensitivity testing cannot be performed.

Abstract:Real-time Quantitative PCR(RQ-PCR) has become an important tool for rapidly detecting and studying fungal plant disease depending on its high specificity,exact quantitation and fast speed.The article reviewed this technology application in detection of fungal plant disease,including pathogenic bacterium qualitative and quantitative detection of fungal plant disease,fungal DNA quantification in host tissue and soil-inhabiting fungi.These studies showed that rapid growth RQ-PCR was opening a new research opportunity for the study of diagnosis,epidemiology and host-pathogen interactions.

Abstract:Development of polymerase chain reaction (PCR)-based molecular techniques has initiated a revolution in the field of diagnostic microbiology. These techniques have not only provided rapid, noninvasive detection of microorganisms that cause central nervous system (CNS) infections, but have also demonstrated that several neurologic disorders are linked to infectious agents. While PCR-based techniques are predicted to be widely used in diagnosing and monitoring CNS infections, the limitations, as well as strengths, of these techniques must be clearly understood by both clinicians and laboratory personnel to ensure proper utilization.
