Abstract:We report a 38-year-old man who presented with bilateral conjunctival congestion, hoarseness, and progressively growing pruritic, infiltrated skin lesions that had first begun over the face and neck, and later spread to the trunk and the limbs in 4 months. The clinical appearance of the lesions mimics granulomatous rosacea, acne vulgaris, or pityrosporum folliculitis. Histopathologic examination of the lesions from the face and chest both revealed dense dermal nodular lymphohistiocytic infiltrates which were positive for CD68 and S-100, but negative for CD1a. A systemic work-up for him detected no lymphadenopathy or other systemic involvement. A diagnosis of extranodal Rosai-Dorfman disease was made, and the patient received systemic glucocorticoids, with considerable improvement after 4 months of therapy.

Abstract:Aims Persistent pruritic eruptions (PPEs), presenting with dyskeratotic keratinocytes histologically, are characteristic skin rash in patients with adult-onset Still's disease (AOSD). The lesions may be histologically similar to other entities that present with dyskeratosis. In the present study, we compared the histopathological features between PPEs and other entities presenting with dyskeratosis. Methods and results To investigate whether histopathological findings can be used to discriminate among PPEs and other entities presenting with dyskeratotic keratinocytes, cutaneous histopathological changes of PPEs associated with AOSD (n = 26) were compared with those of systemic lupus erythematosus (SLE) (n = 16), dermatomyositis (n = 19), and drug eruption (n = 16). Dyskeratosis was observed in the upper one-third of the epidermal layer in all 26 PPEs. The rate of dyskeratosis for PPEs was higher than that for SLE (18.8%) and dermatomyositis (15.8%). In drug eruptions, the dyskeratotic cells were distributed in all levels of the epidermis. Variable densities of neutrophils were found in the dermis in all PPEs. Conclusions Although this was a retrospective study conducted at a single centre, presentation of dyskeratotic keratinocytes in the upper one-third of the epidermal layer is a distinctive histopathological reactive pattern of PPEs. This pattern may be a useful histopathological marker for early diagnosis of AOSD.

Abstract:The "Tian Di Chang Chun" horizontal plaque was inscribed by General Feng Yü-hsiang in R.O.C. 25 (1936). Due to the aging of the materials and some factors in the preservation environment, the red painted layer on the surface of the plaque has shed. In this study, in order to analyse the materials and techniques used for the production of the inscribed plaques, the digital microscope system, scanning electron microscopy–energy dispersive spectroscopy (SEM-EDS), X-ray diffraction (XRD), and micro-Fourier transform infrared spectroscopy (μ-FTIR) were used to analyse the materials and techniques used on this plaque. It has been shown that the decorative layer of the plaque consists of a red Chinese lacquer film layer on the surface and a ground layer. The red lacquer film layer comprises Chinese lacquer, tung oil, and cinnabar. The materials used for the ground layer are blood putty made of a blood product, youman (flour–oil–lime mixture), and brick powder. The technique used is the SDH (San Dao Hui) layer technique in ancient buildings. This study provides physical evidence regarding the materials and techniques used in inscribed plaque relics, and also provides technical support to further protect and restore the plaque.

Abstract:A 57-year-old woman presented with erythematous papules arranged in an annular on both arms,neck,legs and abdomen.Histopathologic analysis of a skin specimen revealed the granulomatous infiltrate and altered collagen in the upper dermis.Mucin accumulation in collagen fibers was obvious by an alcian blue stain.Mild elastic fiber fragmentation was observed by elastic stain.Diagnosis:generalized granuloma annulare.

Abstract:Senile plaques are one of the major neuropathological features of Alzheimer's disease (AD). Aggregation of Amyloid beta-protein(A beta) in senile plaques is related to the pathogenesis of AD, so it is very important for the studies on the prevention of A beta deposition and on trying to disaggregate the senile plaques, aimed at treatment and prevention of the AD. In this report, the three-dimension structures of senile plaques are reconstructed by AVS software by using immunohistochemical method and MATLAB software. The diffuse plaques and classical plaques in hippocampus all have complicated porous structures; no core structures were present in diffuse plaques, and the deposition density of A beta in this kind of plaque diminishes from interior region to the microenvironment. The deposition density of A beta was highest in the central region of classical plaques, while was lowest in the region around central, and was intermediate in the extreme region of this kind of plaques. The method of reconstruction of three-dimension structure of senile plaques helps studying on dynamics of senile plaques, and it is also important for the studies on the pathogenesis of AD.

Twenty years ago, he had a scratch on the right side of his face caused by glass, which healed well after sutures were applied, leaving a mature flat scar with a length of 5.5 cm. However, an asymptomatic reddish lesion had gradually developed in the old scar over the last six months, with no obvious triggers. Physical examination revealed an irregular firm erythematous indurated nodular plaque located in and surrounding the prior scar (figure 1A). Skin ultrasound demonstrated that the lesion (size: 4.9×1.0×2.6 cm) was ill-defined, hypoechoic, and had an abundant blood supply. The patient was otherwise in good health with no history of infectious disease or tumour.

Abstract:Pemphigus foliaceus is a superficial blistering disorder characterized by erosions and scaling in a seborrheic distribution. The condition typically occurs in healthy individuals but issues arise from delayed diagnosis. Many cases remain undiagnosed or misdiagnosed due to the lack of awareness of the condition. With use of common diagnostic tools, pemphigus foliaceus can be easily identified and monitored. Histological analysis exhibits "chicken wire" patterning along keratinocytes in the upper epidermis, whereas immunofluorescence study displays subcorneal acantholysis. Pemphigus foliaceus is confirmed via ELISA studies revealing the presence of autoantibodies against desmoglein 1. Once correctly diagnosed, typically the condition is responsive to corticosteroid therapy. However in recalcitrant cases such as in ours, adjunctive immunosuppressive therapy with dapsone or rituximab may be indicated.

Abstract:Objective:To evaluate the relationship of the shape,location of superficial calcification with surface ulcer,intraplaque hemorrhage in the atherosclerotic carotid plaque (ACP).Methods:Fifty-one patients received pre-and post-contrast high-resolution 3T MRI for ACP and a total 183 layers with superficial calcification were enrolled in our study.The shape of the superficial calcification was categorized as irregular type(dotted/arcuated) and patch type; the location of the calcification was categorized as marginal type and central type.Intraplaque hemorrhage and surface ulcer at the same or adjacent slice to the superficial calcification were identified.Chi-Square test was used to analyze the relationship of the shape,location of superficial calcification with surface ulcer and intraplaque hemorrhage.Results::We found that 122(66.7%) of the 183 slices had intraplaque hemorrhage and 86(47%)slices had surface ulcer.We also found that 158 slices had irregular type(dotted/arcuated)calcification and 142 slices had marginal calcification.Irregular superficial calcification was found more frequently with intraplaque hemorrhage than patch type (72.8% vs 28%,P<0.01).Marginal type had higher possibility of intraplaque hemorrhage than central type did (71.1% vs 51.2%,P<0.05).However,the incidence of surface ulcer was not significantly different between different types and locations of superficial calcification.Conclusion:It can be concluded that superficial calcification is an important factor for instability of atherosclerotic carotid plaque.The shape and location of superficial calcification are important factors of intraplaque hemorrhage.
