Abstract:We report a 38-year-old man who presented with bilateral conjunctival congestion, hoarseness, and progressively growing pruritic, infiltrated skin lesions that had first begun over the face and neck, and later spread to the trunk and the limbs in 4 months. The clinical appearance of the lesions mimics granulomatous rosacea, acne vulgaris, or pityrosporum folliculitis. Histopathologic examination of the lesions from the face and chest both revealed dense dermal nodular lymphohistiocytic infiltrates which were positive for CD68 and S-100, but negative for CD1a. A systemic work-up for him detected no lymphadenopathy or other systemic involvement. A diagnosis of extranodal Rosai-Dorfman disease was made, and the patient received systemic glucocorticoids, with considerable improvement after 4 months of therapy.

Abstract:Aims Persistent pruritic eruptions (PPEs), presenting with dyskeratotic keratinocytes histologically, are characteristic skin rash in patients with adult-onset Still's disease (AOSD). The lesions may be histologically similar to other entities that present with dyskeratosis. In the present study, we compared the histopathological features between PPEs and other entities presenting with dyskeratosis. Methods and results To investigate whether histopathological findings can be used to discriminate among PPEs and other entities presenting with dyskeratotic keratinocytes, cutaneous histopathological changes of PPEs associated with AOSD (n = 26) were compared with those of systemic lupus erythematosus (SLE) (n = 16), dermatomyositis (n = 19), and drug eruption (n = 16). Dyskeratosis was observed in the upper one-third of the epidermal layer in all 26 PPEs. The rate of dyskeratosis for PPEs was higher than that for SLE (18.8%) and dermatomyositis (15.8%). In drug eruptions, the dyskeratotic cells were distributed in all levels of the epidermis. Variable densities of neutrophils were found in the dermis in all PPEs. Conclusions Although this was a retrospective study conducted at a single centre, presentation of dyskeratotic keratinocytes in the upper one-third of the epidermal layer is a distinctive histopathological reactive pattern of PPEs. This pattern may be a useful histopathological marker for early diagnosis of AOSD.

Abstract:A 57-year-old woman presented with erythematous papules arranged in an annular on both arms,neck,legs and abdomen.Histopathologic analysis of a skin specimen revealed the granulomatous infiltrate and altered collagen in the upper dermis.Mucin accumulation in collagen fibers was obvious by an alcian blue stain.Mild elastic fiber fragmentation was observed by elastic stain.Diagnosis:generalized granuloma annulare.

Abstract:Adult-onset Still disease (AOSD) is a rare autoinflammatory condition. The presence of an evanescent, salmon-pink, nonpruritic rash is one of the major diagnostic criteria for the disease. The rash occurs with fever and subsides with defervescence. The presence of dyskeratotic keratinocytes in the upper one-third layer of the epidermis is a distinctive histopathological feature of persistent pruritic lesions associated with AOSD. Here, we report 2 cases of AOSD characterized by persistent pruritic lesions resembling those observed in dermatomyositis. Identifying the clinical and histopathological manifestation of the cutaneous lesions is essential for the early diagnosis of AOSD and for differentiating this condition from those presenting with dyskeratotic cells in the epidermis.

Abstract:AN 85-YEAR-OLD MAN IS ADMITTED FOR EVALUATION OF MULTIPLE PAPULES, plaques, and subcutaneous nodules on his face, trunk, and extremities. The patient has itching. There is no history of trauma. He has an 8-year history of diabetes and Sjogren syndrome with eye and mouth involvement and has been treated with varied doses of oral methylprednisolone for 5 years. During the 3 years prior to admission, he took 12 mg/d of methylprednisolone. Over the last 5 months, he received topical steroids and antifungal agents. Skin examination reveals multiple erythematous papules on his face and eyelids, clustered or solitary reddish or purple papules, indurated shining plaques, and subcutaneous nodules on his wrists (FIGURE 1A), arms, and lower limbs, mainly around the knees (Figure 1B) and thighs. Risk of Macrophage Activation Syndrome in Patients with Adult-Onset Still's Disease with Skin Involvement: A Retrospective Cohort Study

Abstract:Background: Small case series and case reports indicated that atypical persistent pruritic eruptions (PPEs), another type of skin lesions seen in adult-onset Still's disease (AOSD), imply a worse prognosis than typical evanescent rashes. Objective: To investigate clinical characteristics and macrophage activation syndrome (MAS) occurrence in AOSD with PPEs. Methods: A retrospective cohort study analyzed 150 patients with AOSD with rashes at the First Affiliated Hospital of Zhejiang University from January 2013 to December 2019. Results: Patients with AOSD with PPEs had higher lactate dehydrogenase (492.00 U/L vs 382.00 U/L; P < .001) and ferritin (6944.10 ng/ml vs 4286.60 ng/ml; P = .033), and lower fibrinogen (5.05 g/L vs 5.77 g/ L; P = .014) than those with evanescent rashes. Patients with AOSD with PPEs had a higher incidence (17.4% vs 3.1%, P = .006) and cumulative event rate for MAS (P = .008) and tended to receive high-dose glucocorticoid (36% vs 20.3%; P = .036). Multivariate analysis indicated that PPEs (hazard ratio [HR], 5.519; 95% confidence interval [CI], 1.138-26.767; P = .034), aspartate aminotransferase of greater than 120 U/L (HR, 8.084; 95% CI, 1.728-37.826; P = .008), and splenomegaly (HR, 21.152; 95% CI, 2.263-197.711; P = .007) were independent risk factors for MAS. Limitations: Single-center, retrospective nature, small sample size. Conclusion: PPEs indicated increased severity and MAS occurrence versus evanescent rashes. PPEs, aspartate aminotransferase of greater than 120 U/L, and splenomegaly were risk factors for MAS in AOSD with skin involvement.

Generalized familial benign pemphigus: a case report

Abstract:A case of generalized familial benign pemphigus is reported. A 56-year-old male presented with recurrent pruritic eruptions all over the body for more than 40 years. Generalized erythematous macules, maculopapules and the vesicles on the macules or normal skin were noted, predominantly on the trunk. Histological examination revealed extensive acantholysis and intraepidermal blistering. Direct immunoflouresence test with IgG, IgM, IgA and C3 showed a negative result. Twelve patients out of total 66 members in the family were revcaled by the family survey. The case was finally diagnosed as generalized familial benign pemphigus, and the aggravation by mechanical friction, infection and scratching was noticed.

Erythroderma with islands of sparing

Abstract:This man in his 70s presented with a two month history of pruritic plaques on his upper body (fig 1). Although psoriasis was initially diagnosed, symptoms did not improve with phototherapy or methotrexate and acitretin. His condition progressed to erythrodermic salmon coloured scaly patches with sharply demarcated islands of sparing skin, follicular hyperkeratotic papules, and waxy yellowish palmoplantar keratoderma. A skin biopsy sample showed alternating parakeratosis and orthokeratosis in a checkerboard pattern. Test results for HIV, autoimmune markers, and malignancy were negative.bmj;385/jun20_7/e078122/F1F1f1Fig 1Findings were indicative of type 1 (classic adult) pityriasis rubra pilaris. The cause of this condition is usually unknown, although HIV, malignancy, and autoimmune diseases should be excluded. Differential diagnoses include erythrodermic psoriasis, drug reactions, eczema, Sézary syndrome, and dermatomyositis. Approach to red, scaly eruptions in the older patient

Abstract:Background: Older patients with a red scaly eruption often present first to a primary care practitioner. A thorough clinical assessment can help delineate between common causes and assist the clinician with the next steps in management. Objective: This article discusses the assessment of acute- to subacute-onset erythematous and scaly plaques that are present on multiple body sites in a patient aged >65 years. Discussion: The differential diagnosis of a red, scaly rash in an older patient includes atopic dermatitis, psoriasis, generalised drug eruption, tinea, scabies and non-bullous pemphigoid. Less common causes include subacute cutaneous lupus and mycosis fungoides. If the diagnosis is unclear after clinical assessment, a skin biopsy sent for histopathology, and/or direct immunofluorescence can be very useful. Management requires consideration of physical impairments, carer availability and cost.

Abstract:T the editor: A 28-year-old Chinese woman was referred for the evaluation of whitish macules. The macules firstly appeared in left upper back three years ago and then spread to left limb slowly but steadily in size. After the first gravidity at age of 26 years, the patient observed a marked acceleration of this process. She was previously diagnosed with pityriasis versicolor and was treated with ciclopirox cream and zinc pyrithione shampoo without improvement. There was no history of itching or dysaesthesia. The patient denied recreational sun exposure in past five years. When exposed to sunlight, the whitish macules did not change in the color whereas the non-affected skin became more tanned, thus the contrast against the whitish macules skin was remarkably intensified. There is no similar individual in her family. Skin findings of this patient consisted of scattered, partial confluent, non-scaly, well-defined, round or oval hypopigmented macules which asymmetrically distributed on left upper back and left limb (Figure 1A and 1C), and no similar lesions on the other side of the body (Figure 1B). No signs of inflammation were present in these macules.

Abstract:Background: Atopic dermatitis (AD) is an inflammatory skin disease characterized by chronic recurrent dermatitis with profound itching. Most patients have personal and/or family history of atopic diseases. Several criteria have been proposed for the diagnosis of AD. Although the clinical features of childhood AD have been widely studied, there has been less large-scale study on adult/adolescent AD. The aim of this study was to investigate the clinical features of adult/adolescent patients with chronic symmetrical eczema/AD and to propose Chinese diagnostic criteria for adult/adolescent AD.Methods: A hospital-based study was performed. Forty-two dermatological centers participated in this study. Adult and adolescent patients (12 years and over) with chronic symmetrical eczema or AD were included in this study. Questionnaires were completed by both patients and investigators. The valid questionnaires were analyzed using EpiData 3.1 and SPSS 17.0 software.Results: A total of 2662 valid questionnaires were collected (1369 male and 1293 female). Of all 2662 patients, 2062 (77.5%) patients had the disease after 12 years old, while only 600 (22.5%) patients had the disease before 12 years old, suggesting late-onset eczema/AD is common. Two thousand one hundred and thirty-nine (80.4%) patients had the disease for more than 6 months. One thousand one hundred and forty-four (43.0%) patients had a personal and/or family history of atopic diseases. One thousand five hundred and forty-eight (58.2%) patients had an elevated total serum IgE and/or eosinophilia and/or positive allergen-specific IgE. Based on these clinical and laboratory features, we proposed Chinese criteria for adult/adolescent AD. Of all 2662 patients, 60.3% were satisfied with our criteria, while only 48.2% satisfied with Hanifin Rajka criteria and 32.7% satisfied with Williams criteria, suggesting a good sensitivity of our criteria in adult/adolescent AD patients.Conclusion: Late-onset of eczema or AD is common. The clinical manifestations of AD are heterogeneous. We have proposed Chinese diagnostic criteria for adolescent and adult AD, which are simple and sensitive for diagnosis of adult/adolescent AD.

Abstract:A case of porokeratosis presented as eruptive pruritic papular lesions is reported. The patient was a 50-year-old man. He complained of pruritic and hyperkeratotic papules distributed over his buttocks for 7 years. The lesions aggravated 1 year ago, and the lower limbs were involved,with severe pruritus especially when sweating. Histopathological examination revealed the characteristic comoid lamellae. He was treated with topical keratolytics and oral acitretin, but the lesions showed no evident improvement.
