Abstract:A 39 year old woman presented with an asymptomatic ulcer on her left nipple. The ulcer had been present for five weeks. She also reported malaise. She denied any travel history in the past three months, but recalled a history of breast bites and breast-genital contact during sexual intercourse, which had occurred two weeks before the skin lesion appeared. Physical examination revealed demarcated ulceration with a border that was slightly elevated, crusted, and indurated (fig 1). Left axillary lymphadenopathy was noted. Abstract:An 8-year-old Chinese girl had a 7-year history of blisters and erythema affecting the right side of her trunk and extremities (Fig. 1).These itchy lesions first appeared on the dorsum of her right hand at the age of 1 year, and they easily ruptured, leading to macerated or crusted erosions/plaques.Hyper-or hypopigmentation was noted after the lesions healed.The lesions progressed to involve her right body generally, with a linear distribution on her right extremities.The lesions exacerbated in summer.No extracutaneous abnormalities were found.Her father and grandfather had mild and recurrent blisters and erosions distributed symmetrically at intertriginous sites, with an onset age of around 30 years.A biopsy specimen was obtained from the lesion on the abdomen.

Abstract:Background: This study aimed to examine the knowledge, attitude, and practice (KAP) of women in Beijing regarding cervical precancerous lesions. Methods: This web-based, cross-sectional study included women at Dongzhimen Hospital of Beijing University of Chinese Medicine between March 13, 2024 and April 9, 2024. A self-administered questionnaire was developed to collect participants' demographic information and KAP scores toward cervical precancerous lesions. Results: The study included 951 valid questionnaires, with a mean age of 40.0 years. The mean knowledge, attitude, and practice scores were 12.55 ± 6.23 (possible range: 0-26), 50.66 ± 7.48 (possible range: 13-65), and 26.13 ± 4.98 (possible range: 7-35), respectively. The knowledge score (OR = 1.071, 95%CI: 1.040-1.103, p < 0.001), currently married (OR = 0.548, 95%CI: 0.304-0.985, p = 0.045), and with a history of HPV infection (OR = 2.302, 95%CI: 1.062-4.990, p = 0.035) were independently associated with the attitude score. The knowledge score (OR = 1.155, 95%CI: 1.119-1.192, p < 0.001), monthly income >20,000 (OR = 2.793, 95%CI: 1.249-6.248, p = 0.012), a history of HPV infection (OR = 0.380, 95%CI: 0.222-0.650, p < 0.001), unknown HPV infection status (OR = 0.289, OR = 0.177-0.473, p < 0.001), vaccinated against HPV (OR = 1.752, 95%CI: 1.221-2.514, p = 0.002), giving birth to one child (OR = 1.991, 95%CI: 1.186-3.341, p = 0.009), and giving birth to two or more children (OR = 2.160, 95%CI: 1.240-3.763, p = 0.007) were independently associated with the practice score. The structured equation model showed that knowledge directly influenced attitude (β = 0.237, p = 0.004) and practice (β = 0.490, p = 0.010). Attitude directly influenced practice (β = 0.193, p = 0.009). Conclusion: This study revealed inadequate knowledge, moderate attitude, and practice toward cervical precancerous lesions among women in Beijing. Educational interventions should be developed to enhance knowledge in this regard.

A 32-year-old woman presented with a three-month history of clustered erythematous papules and nodules on her forehead without any symptoms (figure 1A). She was otherwise healthy. No lymphadenopathy was found. Skin biopsy on a nodule revealed a dense dermal infiltrate of large polygonal histiocytes admixed with lymphocytes and plasma cells (figure 1B). The histiocytes had abundant, eosinophilic cytoplasm and showed variable emperipolesis (figure 1C). They were S100+ (figure 1D), CD68+, and negative for CD1a.

Abstract:Background: Osteoarthritis (OA) is a complex and heterogeneous condition, and one of the most common degenerative joint diseases. A standardized OA classification based on molecular criteria has yet to be defined. We performed transcriptomic profiling to determine the different molecular signatures associated with knee OA phenotypes. Methods: Five centres clinical research with next-generation sequencing was carried out using 201 samples from patients with knee osteoarthritis. Transcriptomic analysis and unsupervised clustering methods were performed on 131 cartilage samples. To investigate tissue crosstalk of the whole knee microenvironment, transcriptomic analysis was performed in 60 synovia and 65 subchondral bone samples. Findings: We identified four OA subgroups with distinct biological signatures: 1) the glycosaminoglycan metabolic disorder subgroup highly expressed ACAN, lowly expressed VCAN, and was enriched with glycosaminoglycan metabolic functions; 2) the collagen metabolic disorder subgroup presented with osteophyte formation, highly expressed COL1A1 and COL6A1, and was enriched with collagen catabolic functions; 3) the sensory neurons activated subgroup highly expressed GRIK2 and GRM7, and was enriched with synapse assembly functions; and 4) the immunogenic subgroup presented with high inflammation and narrow joint space, highly expressed IL1ÃŸ and CD34, and was enriched with immune responses. We also explored immune cell composition and tissue crosstalk for three identified metabolic disorder subgroups. Interpretation: These findings challenge the classical diagnosis model based on medical imaging, revealing distinct molecular subgroups in knee OA patients, and opening the door to precision diagnosis and treatment of OA. Abstract:Society for Publication of Acta Dermato-Venereologica A 13-year-old girl presented to the hospital with lesions on her scalp, face, and neck that had developed over the course of 2.5 years, together with fever and cough present for 2 weeks. Physical examination was notable for multiple round granulomatous lesions involving the scalp, face, neck, and upper back area (Fig. 1). The patient had no further history of skin or systemic disease. Initially, she was misdiagnosed with eczema and systemic lupus ery thematosus at a local hospital. She then received systemic therapy with glucocorticoids for 2 months, but the lesions were aggravated over time. The patient was transferred to Huashan Hospital in Shanghai for further investigation.
