Abstract:Society for Publication of Acta Dermato-Venereologica A 13-year-old girl presented to the hospital with lesions on her scalp, face, and neck that had developed over the course of 2.5 years, together with fever and cough present for 2 weeks. Physical examination was notable for multiple round granulomatous lesions involving the scalp, face, neck, and upper back area (Fig. 1). The patient had no further history of skin or systemic disease. Initially, she was misdiagnosed with eczema and systemic lupus ery thematosus at a local hospital. She then received systemic therapy with glucocorticoids for 2 months, but the lesions were aggravated over time. The patient was transferred to Huashan Hospital in Shanghai for further investigation.

Abstract:An 8-year-old Chinese girl had a 7-year history of blisters and erythema affecting the right side of her trunk and extremities (Fig. 1).These itchy lesions first appeared on the dorsum of her right hand at the age of 1 year, and they easily ruptured, leading to macerated or crusted erosions/plaques.Hyper-or hypopigmentation was noted after the lesions healed.The lesions progressed to involve her right body generally, with a linear distribution on her right extremities.The lesions exacerbated in summer.No extracutaneous abnormalities were found.Her father and grandfather had mild and recurrent blisters and erosions distributed symmetrically at intertriginous sites, with an onset age of around 30 years.A biopsy specimen was obtained from the lesion on the abdomen.

Abstract:The immune system plays an essential role in maintaining health, protecting humans from pathogens and preventing the majority of tumors. Immunocompromised individuals are more likely to encounter infectious and neoplastic diseases. Excessive immune responses, which are also involved in the pathogenesis of many diseases, have drawn increasing attention in recent years.[1] Blood and nervous system manifestations have been reported to be related to some specific viruses, and potential associations of these viruses with autoimmune diseases have come into the spotlight. Certain infections might arouse immune overreaction in susceptible individuals or even cytokine storms that are harmful to organs and tissues.
Clinical Problem-Solving: A 19-Year-Old Woman With Progressive Neurological Decline and Multiple Intracranial Lesions

Abstract:The differential diagnosis for multiple intracranial lesions in a young adult is broad and includes demyelinating, neoplastic, and infectious etiologies. In this report, we describe the case of a 19-year-old immunocompetent woman presenting with progressive headaches and aphasia. MRI of the brain revealed multiple, large supratentorial lesions with concentric bands of alternating T2 signal intensities and peripheral contrast enhancement. Cerebrospinal fluid (CSF) analysis was overall bland with negative oligoclonal bands. Serum antibody testing for neuromyelitis optica (NMO) and myelin-oligodendrocyte associated disease (MOGAD) were negative. A broad infectious work-up was also unrevealing. A definitive diagnosis was ultimately obtained after brain biopsy and the patient was started on appropriate therapy. This case highlights a diagnostic framework in evaluating immunocompetent patients presenting with multiple intracranial lesions and progressive neurological decline. The main differential diagnoses for this constellation of radiological and clinical findings are discussed and a literature review is performed on the revealed diagnosis. Lastly, both acute and long-term therapeutic approaches are reviewed.

Abstract:Dear Editor, A 20-year-old man presented with reticulated hyper- and hypopigmentation with erythema since early childhood, overlying his body (a, b). His nails showed pachyonychia (c). Additionally, he reported a history of photosensitivity, interventricular septal defect and recurrent infections. Laboratory examinations showed neutropenia (0·63 × 109 cells L−1, normal range 2·00–8·00 × 109). Mutation analysis revealed compound heterozygous mutations in USB1: a nonsense mutation (c.229G>T, p.E77*) and a frameshift mutation (c.748delinsGG, p.G250Efs*67). Taking the evidence together, the diagnosis of poikiloderma with neutropenia (PN) was confirmed. PN is characterized by poikiloderma, pachyonychia and persistent neutropenia; it is caused by recessive mutations in USB1.1 This patient was recommended to avoid sun exposure.

Abstract:To study the microstructural change in different sections of the necrotic femoral head.According to the inner bleeding,the femoral head was divided into two parts,which were defined as bleeding section and referenced section apart,and half of the material from the two sections is decalcified.Four sorts of samples were prepared and were named as bleeding-mineralized,reference-mineralized,bleeding-demineralized,and reference-demineralized respectively.Both SEM and TEM were used to analyze the micro-and nanostructure of the four samples.Results showed that a great number of collagen fibrils could be seen arrayed orderly in the samples from the bleeding section,whereas no microstructure was observed in the referenced samples.The different sections with different appearances in a necrotic femoral head show different change in the micro and nano scale.

Contrast Study between Syndrome Type of Traditional Chinese Medicine(TCM) and Gensini Score,Number of Coronary Artery Lesion in Patients with Coronary Heart Disease(CHD)

Abstract:Objective: To define the relationship between the TCM syndrome type and Gensini score,number of coronary artery lesion and find the macroscopic index in different syndrome type.Methods: 102 patients with CHD,were divided into different syndrome types,received coronary arteriography(CAG),analysed on the relationship between the TCM syndrome type and Gensini score,number of coronary artery lesion.Results: One artery lesion constituted the majority in blood stasis syndrome group,and Qi stagnation and blood stasis syndrome group,while three-artery and four-artery lesion were the most in phlegm turbid and blood stasis syndrome group and cold condensation and blood stasis syndrome group;one artery lesion and two-artery lesion constituted the majority in Yin deficiency group,two-artery lesion and three artery lesion constituted the majority in Yang deficiency group.There were differences in Gensini score between secondary excess syndrome groups and primary deficiency syndrome groups(P0.01).Conclusion: There is a correlation between the TCM syndrome type and Gensini score,number of coronary artery lesion.
