Abstract:Objective: To assess surgical outcomes of implanted porcine small intestinal submucosa (SIS) mesh in the rabbit vesicovaginal space (VVS) and explore its application value in pelvic floor reconstruction surgery. Methods: Sixteen male rabbits were randomly divided into four groups, and each group had four rabbits. All groups of rabbits were implanted with SIS mesh in the vesicovaginal space. They were humanely killed after a postoperative period of 7, 30, 90 and 180 days by group. The grafted area was removed with the surrounding bladder and vaginal tissues. The specimens were embedded in paraffin and then stained with HE and Masson's trichrome stains for visual observations, cells counts, and assessment of tissues and collagen fibers. Results: (1) After HE staining, a large number of inflammatory response cells mainly eosinophils and lymphocytes infiltrated around the SIS mesh in 7 days group, and neovascularization was observed, the infiltration area of inflammatory response cells further increased in 30 days group, the infiltration area of inflammatory response cells significantly reduced in 90 days group, while the inflammatory response basically subsided in 180 days group. (2) After Masson's trichromestaining, the collagen structure of SIS mesh in 7 days group was clear and intact. While, the collagen structure of SIS mesh was partially degraded in 30 days group, the SIS meshes of 4 rabbits were completely degraded, but the collagen fragments of SIS remained in 90 days group. In 180 days group, the SIS mesh of all rabbits was degraded, and one of them had the formation of new collagen fibers. Conclusions: SIS mesh implanted into the VVS of rabbits can lead to a transient non infective inflammatory reaction, which could be completely degraded and a small amount of new collagen fibers could be produced after 180 days of implantation. Which shown that SIS mesh should be used cautiously in pelvic floor reconstruction surgery.
Tissue Reactions to Heterogenic and Allogeneic Acellular Dermal Matrix Mesh Placed in the Vesicovaginal Space in a Rabbit Model

Abstract:Background and Aim: This study is aimed at investigating the tissue reactions of heterogenic and allogeneic acellular dermal matrix (ADM) mesh implanted in the vesicovaginal space in a rabbit model. Methods: Twenty eight rabbits were implanted with heterogenic or allogeneic mesh, respectively, in the vesicovaginal space, and 4 served as the no-implantation control group. Rabbits were sacrificed at 7, 30, and 90 days after implantation. Histopathological study and immunohistochemical examination for collagen were performed. Results: Heterogenic but not allogeneic mesh was identifiable at 90 days. In the allogeneic group, neovascularization was observed from day 7 after implantation. A chronic inflammatory reaction was noted in the heterogenic group at 30 days that decreased at 90 days. Inflammation was less in the allogeneic group, but giant cells and fibroblasts were present. With respect to collagen, the heterogenic mesh remained structurally unchanged at 90 days, while new collagen fibers were observed in the allogeneic group from day 7. Conclusion: The immunological outcomes of heterogenic and allogeneic ADM mesh are different. Heterogenic mesh induces a chronic inflammatory reaction at day 30 after implantation, and maintains its original form longer. Allogeneic mesh is associated with new collagen generation, but degrades earlier.
Outcome of a Novel Porcine-Derived UBM/SIS Composite Biological Mesh in a Rabbit Vaginal Defect Model

Abstract:Introduction and hypothesis To investigate the tissue reactions of a novel porcine-derived urinary bladder matrix/small intestinal submucosa (UBM/SIS) biological mesh and SIS mesh implanted in a rabbit vaginal defect model. Methods Thirty-two rabbits were implanted with UBM/SIS mesh (Group A) and SIS mesh (Group B), respectively. Rabbits were sacrificed at 7, 14, 60, and 180 days after implantation. The tensile strength, elongation at break, and elastic modulus of the tissue were measured using biomechanical methods. The inflammatory response, cell infiltration, vascularization, and collagen fibers were observed. Results Compared with Group B, the tensile strength and elongation at break of group A was higher at 14, 60, and 180 days. The elastic modulus of group A was lower at 180 days. Inflammatory response of group A was milder at 14, 60, and 180 days. There was more cell infiltration in group A at 7 and 14 days. Vascularization was higher in group A at 7 days and 14 days. The order of collagen in group A was better at 14, 60, and 180 days. The proportion of thick red fibers in both groups showed an increasing trend. At 14 days, group A had more thick red fibers. Conclusions The novel UBM/SIS composite mesh had a milder inflammatory response; earlier induction of cell infiltration, angiogenesis, and collagen regeneration. Collagen fibers had a better order. It has a higher tensile strength and greater elongation at break, and can be used as a potential material for the treatment of pelvic organ prolapse.
[Histocompatibility of an Original Chinese-made Mid-Urethral Sling in a Rabbit Model].

Abstract:Objective: To evaluate the histocompatibility of an original Chinese-made mid-urethral sling (Repelvica mid-urethral sling). Methods: In total 10 female New Zealand white rabbits were implanted with Repelvica mid-urethral sling or tension-free vaginal tape-obturator tape (TVT-O; Gynecare). Both brands of sling were implanted under deep fascia of the abdominal wall and in the space between vagina and bladder. All animal groups were sacrificed at set time intervals (4 weeks and 12 weeks), and the abdominal and vaginal slings were harvested for histological evaluation. Results: All slings appeared to be well incorporated into the abdominal wall and anterior vaginal wall. All specimens showed a thin, loose, fibrous interface between the synthetic graft and abdominal wall or vaginal wall, along with mild inflammatory reaction from 4 weeks to 12 weeks. Abdominal grafts of Repelvica mid-urethral sling and TVT-O induced comparable tissue reaction (histological score 10.5 versus 10.5 at 4 weeks, 10.0 versus 9.5 at 12 weeks; both P>0.05). Vaginal grafts of Repelvica mid-urethral sling had lower histological score than TVT-O (histological score 6.0 versus 12.0 at 4 weeks, 8.5 versus 12.5 at 12 weeks), however the differences were not statistically significant (both P>0.05). Conclusions: Chinese-made Repelvica mid-urethral sling exhibits good histocompatibility. Vaginal graft of Repelvica mid-urethral sling evoks minor tissue reaction, which could be attributed to its lightweighted property and favored its clinical application.

Abstract:Objective To evaluate the host tissue reaction and safety in a rabbit model after in situ implantation of three different synthetic anti-incontinence slings.MethodS A total of 45 clean female New Zealand rabbits were randomized into three groups with 15 in each group, group A used three -dimensional electrospinning Polyvinylidene fluoride ( PVDF ) sling, ( group B used polypropylene ( PP) sling, and group C used traditional technique PVDF sling.In each group, a piece of implant was placed into the space between urethra and vagina of the rabbits.Animals were sacrificed at week 4, 12, 26 after implantation for gross observation and histopathologic study.Results Postoperative abnormal urination, sling erosion or retraction were not observed in 3 groups.No obvious difference was observed in 3 groups under gross observation before and after implantation.Minimal fibrosis as well as mild inflammatory reaction and rich angiogenesis were observed in group A.In goup B and C, thicker fibers encompassed the implants and penetrated into the surrounding tissue under a long term observation.However, the histopathologic grading showed no significant difference in week 4, week 12 or week 26 among three groups (P>0.05).Conclusion The 3 different slings were all safe in rabbits’ body tissue with good biocompatibility.Three-dimensional electrospinning PVDF sling may be a promising synthetic sling for the treatment of female SUI.
Long-term histologic response to synthetic and biologic graft materials implanted in the vagina and abdomen of a rabbit model

Abstract:Objective: We sought to evaluate the effects of graft type, site of implantation, and ovariectomy on the long-term histologic response to graft materials used in pelvic reconstructive surgery. Study design: In all, 45 rabbits underwent ovariectomy or sham laparotomy and were implanted with polypropylene (PP) mesh (n = 23) or cross-linked porcine dermis (PS) (n = 22) in the vagina and abdomen. Grafts were harvested 9 months later and were processed for histology to evaluate the host inflammatory response and degree of tissue incorporation within the grafts. Results: Polypropylene induced a milder (P < .007), more uniform response than PS, whereas PS elicited a more variable response and degraded by 9 months. Vaginal grafts had higher scores for inflammation (P = .005) and neovascularization (P < .001), and had lower scores for fibroblastic proliferation (P < .001) than abdominal grafts. Ovariectomy had no effect (P > .05). Conclusion: Synthetic and biologic grafts evoke different foreign body responses, which may have implications for surgical outcomes in women.

Abstract:Complications following surgical repair of pelvic organ prolapse (POP) with polypropylene mesh (PPM) are common. Recent data attributes complications, in part, to stiffness mismatches between the vagina and PPM. We developed a 3D printed elastomeric membrane (EM) from a softer polymer, polycarbonate urethane (PCU). EMs were manufactured with more material given the low inherent material strength of PCU. We hypothesized that the EMs would be associated with an improved host response as compared to PPM. A secondary goal was to optimize the material distribution (fiber width and device thickness) within EMs, in regards to the host response. EM constructs (2×1cm 2 ) with varied polymer stiffness, fiber width, and device thickness were implanted onto the vagina of New Zealand white rabbits for 12 weeks and compared to similarly sized PPMs. Sham implanted animals served as controls. Mixed effects generalized linear models were used to compare the effect of construct type accounting for differences in independent variables. EMs had an overall superior host response compared to PPM as evidenced by preservation of vaginal smooth muscle morphology (p-values<0.01), decreased total cellular response to construct fibers (p-values<0.001), and a reduced percent of macrophages (p-values<0.02) independent of how the material was distributed. Both PP and EMs negatively impacted vaginal contractility and glycosaminoglycan (GAG) content relative to Sham (all p-values<0.001) with EMs having less of an impact on GAGs (p-values<0.003). The results suggest that softer PCU EMs made with more material are well tolerated by the vagina and comprises a future material for POP repair devices. Statement of Significance Prolapse is a debilitating condition in which loss of support to the vagina causes it and the organs supported by it to descend from their normal position in the pelvis. Surgical solutions to rebuild support involves the use of polypropylene mesh which is orders of magnitude stiffer than the vagina. This mismatch results in complications including exposure of the mesh into the vagina and pain. To provide an innovative solution for women, we have developed an elastomeric membrane from a soft polymer that matches the stiffness of the vagina. Here, we show in a rabbit animal model that this device incorporates better into the vagina and is associated with an overall improved host response as compared to polypropylene mesh.

Abstract:In studies to date, meshes based on extracellular matrix (ECM) have been extensively used in clinical applications. Unfortunately, little is known about the function of the immunogenic residual, absorbable profile during the tissue repair process. Moreover, there needs to be a recognized preclinical animal model to investigate the safety and efficacy of extracellular matrix meshes. Herein, we designed and fabricated a kind of SIS mesh followed by a scanned electron micrograph characterization and tested α-Gal antigen clearance rate and DNA residual. In order to prove the biocompatibility of the SIS mesh, cell viability, chemotaxis assay and local tissue reaction were assessed by MTT and RTCA cytotoxicity test in vitro as well as implantation and degradation experiments in vivo. Furthermore, we developed a stable preclinical animal model in the porcine ventral hernia repair investigation, which using laparoscopic plus open hybridization method to evaluate tissue adhesion, explant mechanical performance, and histologic analysis after mesh implantation. More importantly, we established a semi-quantitative scoring system to examine the ECM degradation, tissue remodeling and regeneration in the modified porcine surgical hernia model for the first time. Our results highlight the application prospect of the improved porcine ventral hernia model for the safety and efficacy investigation of hernia repair meshes.

Abstract:Background Following the severe complications widely reported in some women following polypropylene suburethral mesh implantation for the treatment of stress urinary incontinence, it has become clear that careful preclinical study in an appropriate animal model is essential. The preclinical model described here allows the postoperative clinical and histochemical assessment of material implanted in the sheep vagina and represents a model that allows prediction of the potential vaginal tissue responses to suburethral mesh implantation before implantation into humans. Objectives To develop and evaluate a relevant preclinical animal model to mimic suburethral implantation of support materials for stress urinary incontinence. Material Fourteen parous ewes of 30–40 kg were used. Under general anesthetic, animals were placed in the lithotomy position. A 12‐French Foley catheter was inserted. The ventral vaginal wall was longitudinally incised. The space between the vaginal wall and the urethra was developed, with lateral retropubic dissection. Either a standard transvaginal polypropylene tape (seven animals) or a fascia mimetic microfibre spun polyurethane tape (seven animals) were inserted using an introducer either via a suprapubic (seven animals) or a transvaginal approach (seven animals) into the suburethral and retropubic space. The incisions were closed, and the catheter was removed. A betadine vaginal pack was placed and removed after 24 h. Postoperatively, the animals were observed carefully for pain and complications and allowed access to water and food under continuous veterinary supervision adhering to USA‐approved protocols. Results On the second postoperative day, all the animals passed urine freely and had regular bowel motions. No procedure related complications were observed. Conclusion This is the first large animal model which has been designed for suburethral implantation. and which can be effectively used for the preoperative evaluation of novel materials designed for suburethral implantation. We believe that this technique in this large animal model accurately reproduces the surgical technique used to treat stress urinary incontinence in women. It provides a new and effective in vivo model for the objective evaluation of new materials to treat female stress incontinence.

Abstract:Purpose Develop biodegradable meshes as a novel solution to address issues associated with using synthetic meshes for POP repair. Design/methodology/approach Computational models were created with variations in the pore geometry, pore size, filament thickness, and inclusion of filaments around specific mesh regions. Subsequently, one of the meshes was 3D printed to validate the results obtained from the simulations. Following this, a uniaxial tensile test was carried out on the vaginal tissue of a sow to compare with the simulations, to identify meshes that displayed behaviour akin to vaginal tissue. Finally, the most promising outcomes were compared with those of the uterosacral ligament and a commercially available mesh. Findings Following a comprehensive analysis of the results, the mesh that most accurately replicates the behaviour of the vaginal tissue showcases a smaller pore diameter (1.50 mm), filaments in specific areas of the mesh, and variable filament thickness across the mesh. Nevertheless, upon comparing the outcomes with those of the uterosacral, the meshes do not exhibit similar behaviour to the ligament. Finally, the commercially available mesh does not represent the behaviour of both the vaginal tissue and the uterosacral ligament and in this sense may not be the best treatment option for POP repair. Originality/value Their biocompatibility and biomechanical properties make them a potential solution to the disadvantages of synthetic meshes. Personalized/customized meshes could be part of the future of surgical POP repair.

Abstract:The use of vaginal surgical mesh to treat pelvic organ prolapse (POP) has been associated with high rates of mesh-related complications. In the present study, we prepared new kinds of meshes based on bacterial cellulose (BC) and collagen-coated BC (BCCOL) using a laser cutting method and perforation technique. The mechanical properties of pre-implanted BC meshes, including breaking strength, suture strength and rigidity, were equal to or exceeded those of available clinically used polypropylene meshes. An <i>in vitro</i> cellular assay revealed that BCCOL meshes exhibited enhanced biocompatibility by increasing collagen secretion and cell adhesion. Both BC and BCCOL meshes only caused weak inflammation and were surrounded by newly formed connective tissue composed of type I collagen after implantation in a rabbit subcutaneous model for one week, demonstrating that the novel mesh is fully biocompatible and can integrate into surrounding tissues. Furthermore, a long-term (ninety days) ewe vaginal implantation model was used to evaluate foreign body reactions and suitability of BC and BCCOL meshes as vaginal meshes. The results showed that the tissue surrounding the BC meshes returned to its original physiology as muscle tissue, indicating the excellent integration of BC meshes into the surrounding tissues without triggering severe local inflammatory response post-implantation. The collagen coating appeared to induce a chronic inflammatory response due to glutaraldehyde remnants. The present exploratory research demonstrated that the developed BC mesh might be a suitable candidate for treating POP.

Biomechanical properties of synthetic and biologic graft materials following long-term implantation in the rabbit abdomen and vagina

Abstract:Objective: We sought to evaluate the effects of anatomic location and ovariectomy on biomechanical properties of synthetic and biologic graft materials after long-term implantation. Study design: A total of 35 rabbits underwent ovariectomy or sham laparotomy and were implanted with polypropylene (PP) mesh (n = 17) or cross-linked porcine dermis (PS) (n = 18) in the vagina and abdomen. Grafts were harvested 9 months later and underwent mechanical properties testing. Results: After implantation, PS was similar in strength (P = .52) but was twice as stiff as PP (P = .04) and had a maximal elongation only half that of PP (P < .001). Degradation of PS was associated with decreased ultimate tensile strength (P = .03) and elastic modulus (P = .046). Vaginal PP grafts shrunk more (P < .001) and were less stiff than abdominal PP grafts (P = .049) but were not different in strength (P = .19). Ovariectomy had no effect (P > .05). Conclusion: Cross-linked PS undergoes long-term degradation resulting in compromised biomechanical properties and thus is likely inferior to lightweight PP meshes for pelvic organ prolapse and incontinence procedures.
Research on the Explant of Composite Polypropylene Fibers Substrate /Silicone Ventral Hernia Mesh

Abstract:In this study, an explanted ventral hernia mesh was set to analyze the interaction between mesh and tissue in human body and the reasons of hernia recurrence via comparing the changes of physicochemical properties, as well as histopathology analyzing before and after implantation. Our research suggested that most of the human ventral tissue grew on the surface of polypropylene (PP) fibers side. As for silicone side, tissue only grew into the holes of mesh and formed partial adhesions. The mesh -tissue interaction made the mesh shrank rate up to 49. 89％. After the removal of human tissue, further SEM characterization showed that there were many irregular and horizontalcracks on the surface of explanted PP fibers, but the silicone side maintained pristine structure. Moreover, the results of DSC and TGA showed that the thermal stability of PP fibers reduced. The carbonyl peaks representing the oxidation of the polypropylene fibers were not shown in the ATR -FTIR results, which meant that the chemical stability of silicone was good. Histopathological analysis showed that some inflammatory cells appeared around the polypropylene fibers, but the overall inflammation was not serious. The results indicated the main reasons of the hernia recurrence, including the degradation of PP fibers, the adhesions of silicone side holes, and the severe mesh shrink. Meanwhile, the results suggest that as for the meshes, there should be less holes and smaller hole aperture, as well as bigger fibers spacing.

Abstract:Surgeries for treating pelvic organ prolapse involving the utilization of synthetic mesh have been associated with complications such as mesh erosion, postoperative pain, and dyspareunia. This work aimed to reduce the surgical implantation-associated complications by nanofibrous membranes on the surface of the polypropylene mesh. The nanofiber of the nanofibrous membrane, which was fabricated by co-axial electrospinning, was composed of polyurethane as fiber core and gelatin as the fiber out layer. The biocompatibility of the modified mesh was evaluated in vitro by cell proliferation assay, immunofluorescence stain, hematoxylin-eosin (HE) staining, and mRNA sequencing. Polypropylene mesh and modified mesh were implanted in a rat pelvic organ prolapse model. Mesh-associated complications were documented. HE and Picro-Sirius red staining, immunohistochemistry, and western blotting were conducted to assess the interactions between the modified mesh and vaginal tissues.

Nanofibrous Biomimetic Mesh Can Be Used for Pelvic Reconstructive Surgery: A Randomized Study.

Abstract:Background: Implantation of nonabsorbable polypropylene (PP) mesh in the vagina is the main surgical treatment for pelvic organ prolapse (POP); however, clinical outcomes remain controversial and far from satisfactory. In particular, reducing the exposure or erosion of vaginal implants to obtain improved functional reconstruction is challenging. There is an urgent need for the development of new materials and/or products for POP treatment. A nanofibrous biomimetic mesh was recently developed to address this issue. Objective: In this study, the basic properties of the newly developed mesh, including structural characteristics, mechanical properties, biological response of human umbilical cord mesenchymal stem cells in vitro, and tissue regeneration and biocompatibility in vivo, were evaluated and compared with those of Gynemesh (TM) PS.Methods: Scanning electron microscopy and uniaxial tensile methods were used to evaluate microstructure and mechanical properties, respectively. Mesenchymal stem cell growth on the meshes was observed by fluorescence microscopy to visualize the expression of enhanced red fluorescent protein. Twenty-four mature female Sprague Dawley rats were randomly assigned to two groups: group 1 (nanofibrous biomimetic mesh, Medprin, Germany, n=12) and group 2 (Gynemesh (TM) PS, Ethicon, USA; n=12). The posterior vaginal wall was incised from the introitus, and the mesh was then implanted. Three implants of each type were tested for 1, 4, 8 and 12 weeks. Connective tissue organization, inflammation, vascularization, and regenerated tissue were histologically assessed.Results: The nanofibrous biomimetic mesh is a relatively heavy material and exhibited lower porosity than Gynemesh (TM) PS. The new mesh was stiffer than Gynemesh (TM) PS (p < 0.001) but supported human umbilical cord mesenchymal stem cell attachment. Erosion of the grafts did not occur in any animal. The nanofibrous biomimetic mesh was encapsulated by a thicker layer of connective tissue and was associated with significantly greater inflammatory scores compared with Gynemesh (TM) PS. At 12 weeks, the vascularization of the new mesh was greater than that of Gynemesh (TM) PS (p < 0.05). No significant difference in the thickness of the smooth muscle layer following implantation was observed between the two groups (p > 0.05).Conclusions: The nanofibrous biomimetic mesh is a candidate for reinforcing pelvic reconstruction. The mesh could be improved by decreasing its weight and stiffness and increasing its porosity. This mesh could serve as a carrier for stem cells in future regenerative medicine and tissue engineering research. (C) 2016 Elsevier Ltd. All rights reserved.
Irritation test and endometrial transformation experiment of progesterone gel on rabbits after vaginal delivery

Abstract:[ABSTRACT]Objective: To study vaginal irritation and effect on the lining of the uterus after vaginal delivered progesterone gel, and investigate the practicability of progesterone gel preparation.Methods: A total of 20 female rabbits were randomly divided into ifve groups: blank control group, blank matrix group, low dose group, middle dose group, and high dose group. Then the irritation of progesterone vaginal gel was studied after a single dose and multiple doses. The results were scored according to vaginal irritation grading standard tables and classiifcation criteria for vaginal irritation after observation. Furthermore, initial pharmacodynamics of progesterone gel was studied. The vagina and uterus of every rabbit was detected as well as the results were measured by transforming activity of each rabbit.Results: Compared with the blank control group, after a single dose and multiple doses, the rabbits in each group showed no obvious erythema and exudate. Biopsy results showed that there was no significant difference between rabbits in each group. In the initial pharmacodynamical study, the Mcphail indexes in blank matrix group, low, middle, high dose group and positive control group were (0.25 ± 0.46), (1.00 ± 0.61), (1.75 ± 0.50), (2.25 ± 0.50), (3.20 ± 0.27) respectively after 23 d treatments. Uterine pathological results showed the uterus changes from the proliferative phase to the secretory phase were all observed in the low, middle and high dose group.Conclusion:No significant vaginal and uterus irritation was observed in rabbits after vaginal delivery of progesterone gel with single or multiple doses. Furthermore, progesterone gel has certain progesterone activity, which shows that this formulation is feasible.
Observation on mechanical properties and collagen content of porcine intestinal sub-mucosa biological mesh after implantation into rat abdominal wall

Abstract:Objective To compare local mechanical properties and the changes of the contents of type Ⅰ collagen fibers of the degradable porcine intestinal submucosa acellular matrix (SIS) biological mesh and non-absorbable artificial synthetic polypropylene mesh after implantation into the abdominal wall of rats.Methods A total of 60 adult Sprague-Dawley (SD) rats were randomly divided into three groups,20 rats in each group,and 2 cm×2 cm artificial defects were formed in abdominal wall of these rats.The polypropylene synthetic mesh (group A) and SIS biological mesh (group B) were implanted into abdominal wall of the rats,and the outcomes were compared with mesh-free implantation group (group C).At the third,ninth,fifteenth,and twenty-first months after operation,the maximal tensile resistance of the implanted tissue and the contractility of the muscles around the rectus abdominis muscle fibers were compared,and the content of collagen fibers in the implanted site was detected by immunohistochemistry.Results There were 10 rats of death after operation,including 4 rats in group A,3 rats in group B,and 3 rats in group C,and there was no statistically significant difference among these three groups (P＞ 0.05).There were 1 rat of abdominal wall bulge in group A,6 rats in group B anl no bulge in group C,and there was statistically significant difference among the three groups (P＜0.05).There were statistically significant differences between group B and group A,group C,respectively (all P＜0.05),and there was no statistically significant difference between group A and group C (P＞0.05).With the extension of postoperative time,the local tensions of defect repair in group A and group C were the strongest,which were statistically significantly better than that in group B (P＜0.05).In group B,the early tension was acceptable,then it decreased with the degradation of SIS and gradually increased with the proliferation of collagen fibers,the tension gradually increased.The contractility of the muscles around the rectus abdominis muscle fibers increased in each group,but there was no statistically significant difference between the two groups.The proliferation of local collagen fibers in group A and group C was more significant than that in group B (P＜0.05).There was no mesh exposure or erosion in the two groups.Conclusion The local tension of SIS mesh used to repair abdominal wall defect in SD rats is lower than that of polypropylene mesh.The utilization of SIS mesh in the repair of human pelvic organ prolapse requires more research evidence.

Abstract:For patients with medium to severe incontinence, sub-urethral support surgery has a high cure rate, but using synthetic meshes leads to some complications such as mesh erosion/exposure and thigh pain. Autologous or acellular extracellular matrix grafts present few complications but have a high recurrence rate. Regensling™ is a new sling product made of a synthetic material with a biomimetic structure, aiming to provide long-term mechanical support with a lower complication rate. To assess the safety and effectiveness of Regensling™, both in vitro and in vivo experiments were performed. The mesh was implanted in the subcutaneous, intramuscular and sub-urethral regions of rabbits. At 4, 12, and 26 weeks post-implantation, the animals were executed and the implants were studied for their mechanical and biocompatible properties. Compared to the control material, the Regensling™ was covered by a thin layer of fibrous tissue with good compliance, and had a milder inflammatory response. During the period of the experiment, Regensling™ showed stable strength with an increasing trend over time.
Characterization of the T-cell response to polypropylene mesh in women with complications

Abstract:Background: Polypropylene mesh is used widely for surgical treatment of pelvic organ prolapse and stress urinary incontinence. Although these surgeries demonstrate favorable functional and anatomic outcomes, their use has been limited by complications, the 2 most common being exposure and pain. Growing evidence suggests that T lymphocytes play a critical role in the regulation of the host response to biomaterials. Objective: The purpose of this study was to define and characterize the T-cell response and to correlate the response to collagen deposition in fibrotic capsules in mesh tissue complexes that are removed for the complications of pain vs exposure. Study design: Patients who were scheduled to undergo a surgical excision of mesh for pain or exposure at Magee-Women's Hospital were offered enrollment. Forty-two mesh-vagina tissue complexes were removed for the primary complaint of exposure (n=24) vs pain (n=18). Twenty-one patients agreed to have an additional vaginal biopsy away from the site of mesh that served as control tissue. T cells were examined via immunofluorescent labeling for cell surface markers CD4+ (Th), CD8+ (cytotoxic) and foxp3 (T-regulatory cell). Frozen sections were stained with hematoxylin-eosin for gross morphologic condition and picrosirius red for collagen fiber analysis. Interrupted sodium-dodecyl sulfate gel electrophoresis was used to quantify the content of collagens type I and III, and the collagen III/I ratio. Transforming growth factor-β and connective tissue growth factor, which are implicated in the development of fibrosis, were measured via enzyme-linked immunosorbent assays. Data were analyzed with the Student's t tests, mixed effects linear regression, and Spearman's correlation coefficients. Results: Demographic data were not different between groups, except for body mass index, which was 31.7 kg/m2 for the exposure group and 28.2 kg/m2 for pain (P=.04). Tissue complexes demonstrated a marked, but highly localized, foreign body response. We consistently observed a teardrop-shaped fibroma that encapsulated mesh fibers in both pain and exposure groups, with the T cells localized within the tip of this configuration away from the mesh-tissue interface. All 3 T-cell populations were significantly increased relative to control: CD4+ T helper (P<.001), foxp3+ T regulatory (P<.001), and CD8+ cytotoxic T cell (P=.034) in the exposure group. In the pain group, only T-helper (P<.001) and T-regulatory cells (P<.001) were increased, with cytotoxic T cells (P=.520) not different from control. Picrosirius red staining showed a greater area of green (thin) fibers in the exposure group (P=.025) and red (thick) fibers in the pain group (P<.001). The ratio of area green/(yellow + orange + red) that represented thin vs thick fibers was significantly greater in the exposure group (P=.005). Analysis of collagen showed that collagen type I was increased by 35% in samples with mesh complications (exposure and pain) when compared with control samples (P=.043). Strong correlations between the profibrosis cytokine transforming growth factor-β and collagen type I and III were found in patients with pain (r≥0.833; P=.01) but not exposure (P>.7). Conclusion: T cells appear to play a critical role in the long-term host response to mesh and may be a central pathway that leads to complications. The complexity of this response warrants further investigation and has the potential to broaden our understanding of mesh biology and clinical outcomes.

Abstract:Purpose: Conventional prostheses used for inguinal hernia repair are static and passive. This feasibility-study shows the features of a new 3D tension-free prosthesis in an experimental model. Methods: This study was divided into two-phases: 1) aimed to test the physics intrinsic features and the anatomical adaptability of a new 3D designed mesh, and 2) aimed to evaluate the inflammatory reaction associated with different materials used. On phase-1 implantations were performed in pigs. During the first trial phase, the prostheses were also implanted on human cadavers. On phase-2, implantation was carried out on large swine. Follow-up was of 60-days, after which the animals were anaesthetized for laparoscopic assessment, and for sample collection of mesh implantation site for histological analysis. Results: All animals showed good 3D mesh tolerance, and the follow-up period was uneventful. The laparoscopy showed no inflammatory lesions on the internal surface of the peritoneum. Macroscopic observation of implantation site revealed some local fibrosis and reorganization of tissue, no signs of infection, and no changes on original implant positioning. Histological analysis on phase-1 showed in most sample segments the deferent duct maintaining its central position and surrounded by vascular and nervous structures. On phase-2 differences in inflammatory lesion score could be found between subjects. Conclusions: This new 3D mesh can be placed appropriately and from this preliminary animal study no untoward complications were noted over a 60 day period.
