Abstract:In studies to date, meshes based on extracellular matrix (ECM) have been extensively used in clinical applications. Unfortunately, little is known about the function of the immunogenic residual, absorbable profile during the tissue repair process. Moreover, there needs to be a recognized preclinical animal model to investigate the safety and efficacy of extracellular matrix meshes. Herein, we designed and fabricated a kind of SIS mesh followed by a scanned electron micrograph characterization and tested α-Gal antigen clearance rate and DNA residual. In order to prove the biocompatibility of the SIS mesh, cell viability, chemotaxis assay and local tissue reaction were assessed by MTT and RTCA cytotoxicity test in vitro as well as implantation and degradation experiments in vivo. Furthermore, we developed a stable preclinical animal model in the porcine ventral hernia repair investigation, which using laparoscopic plus open hybridization method to evaluate tissue adhesion, explant mechanical performance, and histologic analysis after mesh implantation. More importantly, we established a semi-quantitative scoring system to examine the ECM degradation, tissue remodeling and regeneration in the modified porcine surgical hernia model for the first time. Our results highlight the application prospect of the improved porcine ventral hernia model for the safety and efficacy investigation of hernia repair meshes.

Abstract:A ventral hernia involves tissue protrusion through the abdominal wall (AW). It is a common surgical issue with high recurrence rates. Primary stability of hernia meshes is essential to guarantee mesh integration, yet existing meshes often fail to match the AW's complex biomechanics. This study proposes a novel method aiming at understanding post-operative mesh-AW interactions. Three fresh frozen human specimens underwent an open Rives-Stoppa implantation of a synthetic hernia mesh coated with metallic micro-beads. Additional beads were placed into the AW muscle tissue. CT scans were conducted at increasing levels of intra-abdominal pressure to reproduce forced breathing. Beads 3D coordinates were exported from the CT-scans and motion and strain of both the hernia mesh and the AW were calculated. At 30 mmHg, the mesh-muscle motion (or sliding) was 2.3 ± 1.3 mm. Muscle exhibited significantly higher strains (12.9 ± 4.7 %) than the hernia mesh (4.7 ± 1.1 %), most likely due to difference in material properties between the mesh and the AW. A repeatability study was carried out to build confidence in the proposed method. This protocol can bring insights of the hernia mesh use-conditions to improve hernia mesh design requirements and develop safer implants to reduce hernia recurrence.

Abstract:Injectable surgical sealants and adhesives, such as biologically derived fibrin gels and synthetic hydrogels, are widely used in medical products. While such products adequately adhere to blood proteins and tissue amines, they have poor adhesion with polymer biomaterials used in medical implants. To address these shortcomings, we developed a novel bio-adhesive mesh system utilizing the combined application of two patented technologies: a bifunctional poloxamine hydrogel adhesive and a surface modification technique that provides a poly-glycidyl methacrylate (PGMA) layer grafted with human serum albumin (HSA) to form a highly adhesive protein surface on polymer biomaterials. Our initial in vitro tests confirmed significantly improved adhesive strength for PGMA/HSA grafted polypropylene mesh fixed with the hydrogel adhesive compared to unmodified mesh. Toward the development of our bio-adhesive mesh system for abdominal hernia repair, we evaluated its surgical utility and in vivo performance in a rabbit model with retromuscular repair mimicking the totally extra-peritoneal surgical technique used in humans. We assessed mesh slippage/contraction using gross assessment and imaging, mesh fixation using tensile mechanical testing, and biocompatibility using histology. Compared to polypropylene mesh fixed with fibrin sealant, our bio-adhesive mesh system exhibited superior fixation without the gross bunching or distortion that was observed in the majority (80%) of the fibrin-fixed polypropylene mesh. This was evidenced by tissue integration within the bio-adhesive mesh pores after 42 days of implantation and adhesive strength sufficient to withstand the physiological forces expected in hernia repair applications. These results support the combined use of PGMA/HSA grafted polypropylene and bifunctional poloxamine hydrogel adhesive for medical implant applications.
Metabolic profile of lipid extracts obtained by astrocytic brain tumors

Abstract:Introduction Astrocytic brain tumors – Gliomas in particular – are the most common brain tumors in adults. These malignant neoplasms are characterized by heterogeneous tissue with no distinct differentiation to healthy tissue because of their infiltrative growth. Therefore, the ratio of tumor cells versus healthy tissue in the tumor margin needs to be determined very precisely when the decision is made to which extent this tissue has to be resected surgically. As high resolution magnetic resonance spectra of intact tissue or tissue extracts reflect numerous metabolic information, the metabolic composition of tissue samples from the tumor margin and the tumor core were recorded. In addition, analysis of H-NMR spectra with pattern recognition algorithms permits a tumor classification, since different neoplastic tissue types show different metabolic profiles. Therefore, 22 astrocytic brain tumors were analyzed with H-NMR spectroscopy, subdivided in 22 biopsates of tumor core, 14 biopsates of tumor margin and 4 reference extracts of normal tissue.
Tension free open inguinal hernia repair using an innovative self gripping semi-resorbable mesh

Abstract:AIMS: Inguinal hernia repair according to Lichtenstein technique has become the most common procedure performed by general surgeons. Heavy weight polypropylene meshes have been reported to stimulate inflammatory reaction responsible for mesh shrinkage when scar tissue evolved. Additionally, some concerns remain regarding the relationship between chronic pain and mesh fixation technique. In order to reduce those drawbacks, we have developed a new mesh for anterior tension free inguinal hernia repair which exhibits self-gripping absorbable properties.MATERIALS AND METHODS: 52 patients (69 hernias) were prospectivly operated with this mesh (SOFRADIM-France) made of low-weight isoelastic large pores knitted fabric which incorporated resorbable micro hooks that provides self gripping properties to the mesh during the first months post-implantation. The fixation of the mesh onto the tissues is significantly facilitated. The mesh is secured around the cord with a self gripping flap. After complete tissular ingrowth and resorption of the PLA hooks, the low-weight (40 g/m(2)) polypropylene mesh insures the long term wall reinforcement.RESULTS: Peroperativly, no complication was reported, the mesh was easy to handle and to fix. Discharge was obtained at Day 1. No perioperative complication occurred, return to daily activities was obtained at Day 5.5. At one month, no neurological pain or other complications were described.CONCLUSIONS: Based on the first results of this clinical study, this unique concept of low density self gripping mesh should allows an efficient treatment of inguinal hernia. It should reduce postoperative complications and the extent of required suture fixation, making the procedure more reproducible.

Nanofibrous Biomimetic Mesh Can Be Used for Pelvic Reconstructive Surgery: A Randomized Study.

Abstract:Background: Implantation of nonabsorbable polypropylene (PP) mesh in the vagina is the main surgical treatment for pelvic organ prolapse (POP); however, clinical outcomes remain controversial and far from satisfactory. In particular, reducing the exposure or erosion of vaginal implants to obtain improved functional reconstruction is challenging. There is an urgent need for the development of new materials and/or products for POP treatment. A nanofibrous biomimetic mesh was recently developed to address this issue. Objective: In this study, the basic properties of the newly developed mesh, including structural characteristics, mechanical properties, biological response of human umbilical cord mesenchymal stem cells in vitro, and tissue regeneration and biocompatibility in vivo, were evaluated and compared with those of Gynemesh (TM) PS.Methods: Scanning electron microscopy and uniaxial tensile methods were used to evaluate microstructure and mechanical properties, respectively. Mesenchymal stem cell growth on the meshes was observed by fluorescence microscopy to visualize the expression of enhanced red fluorescent protein. Twenty-four mature female Sprague Dawley rats were randomly assigned to two groups: group 1 (nanofibrous biomimetic mesh, Medprin, Germany, n=12) and group 2 (Gynemesh (TM) PS, Ethicon, USA; n=12). The posterior vaginal wall was incised from the introitus, and the mesh was then implanted. Three implants of each type were tested for 1, 4, 8 and 12 weeks. Connective tissue organization, inflammation, vascularization, and regenerated tissue were histologically assessed.Results: The nanofibrous biomimetic mesh is a relatively heavy material and exhibited lower porosity than Gynemesh (TM) PS. The new mesh was stiffer than Gynemesh (TM) PS (p < 0.001) but supported human umbilical cord mesenchymal stem cell attachment. Erosion of the grafts did not occur in any animal. The nanofibrous biomimetic mesh was encapsulated by a thicker layer of connective tissue and was associated with significantly greater inflammatory scores compared with Gynemesh (TM) PS. At 12 weeks, the vascularization of the new mesh was greater than that of Gynemesh (TM) PS (p < 0.05). No significant difference in the thickness of the smooth muscle layer following implantation was observed between the two groups (p > 0.05).Conclusions: The nanofibrous biomimetic mesh is a candidate for reinforcing pelvic reconstruction. The mesh could be improved by decreasing its weight and stiffness and increasing its porosity. This mesh could serve as a carrier for stem cells in future regenerative medicine and tissue engineering research. (C) 2016 Elsevier Ltd. All rights reserved.
Evaluating Tissue-Engineered Repair Material for Pelvic Floor Dysfunction: a Comparison of in Vivo Response to Meshes Implanted in Rats

Abstract:Introduction and hypothesis Achieving better anatomic restoration and decreasing the associated complications are necessary for material repair of pelvic floor dysfunction (PFD). This study was aimed to investigate host response to tissue-engineered repair material (TERM) in rat models by comparing different materials and study the changes in biomechanical properties over time. Methods TERM was constructed by seeding adipose-derived stem cells (ADSCs) on electrospun poly(L-lactide)-trimethylene carbonate-glycolide (PLTG) terpolymers. The TERM, PLTG, porcine small intestine submucosa mesh (SIS), and polypropylene (PP) ( n = 6 / group per time point) were implanted in rats for 7, 30, 60, and 90 days. Hematoxylin-eosin and Masson’s trichrome staining were used to assess the host response, and mechanical testing was used to evaluate the changes in biomechanical properties. Results In vivo imaging showed that the ADSCs were confined to the abdominal wall and did not migrate to other organs or tissues. The TERM was encapsulated by a thicker layer of connective tissue and was associated with less reduced inflammatory scores compared with PLTG and PP over time. The vascularization of the TERM was greater than that with PP and PLTG over time ( p < 0.05) and was greater than that with SIS on day 90. The ultimate tensile strain and Young’s modulus of the PP group showed the greatest increases, and the TERM group followed on day 90. Conclusions This TERM achieved better host integration in rat models and better biomechanical properties, and it may be an alternative material for PFD.
Construction and properties of the silk fibroin and polypropylene composite biological mesh for abdominal incisional hernia repair

Abstract:Background: In this study, a new composite biological mesh named SFP was prepared by combining silk fibroin with polypropylene mesh. The mechanism and clinical application value of the SFP composite mesh were explored. Methods: The fibrous membrane was prepared by electrospinning of silk fibroin. The silk fibrous membrane was adhered to the polypropylene mesh by fibrin hydrogel to make a new composite mesh. The characterizations were verified by structural analysis and in vitro cell experiments. A total of 40 Sprague-Dawley rats were randomly divided into two groups, and 20 rats in each group were implanted with the SFP mesh and pure polypropylene mesh, respectively. The rats were sacrificed in batches on the 3rd, 7th, 14th, and 90th days after surgery. The adhesion degree and adhesion area on the mesh surface were compared, and a histopathological examination was carried out. Results: In vitro cell function experiments confirmed that the SFP mesh had good cell viability. The control group had different degrees of adhesion on the 3rd, 7th, 14th, and 90th days after surgery. However, there was almost no intraperitoneal adhesions on the 3rd and 7th days after surgery, and some rats only had mild adhesions on the 14th and 90th days after surgery in the SFP group. There were statistically significant differences in the postoperative intraperitoneal adhesion area and adhesion degree between the two groups (p < 0.05). Histopathological examination confirmed that the mesenchymal cells were well arranged and continuous, and there were more new capillaries and adipocyte proliferation under the mesenchymal cells in the SFP group. Conclusion: The SFP mesh shows good biocompatibility and biofunction in vitro and in vivo. It can promote the growth of peritoneal mesenchymal cells. The formation of a new mesenchymal cell layer can effectively reduce the extent and scope of adhesion between the mesh and abdominal organs. The SFP mesh will have a good application prospect in the field of abdominal wall hernia repair.

Abstract:Purpose Develop biodegradable meshes as a novel solution to address issues associated with using synthetic meshes for POP repair. Design/methodology/approach Computational models were created with variations in the pore geometry, pore size, filament thickness, and inclusion of filaments around specific mesh regions. Subsequently, one of the meshes was 3D printed to validate the results obtained from the simulations. Following this, a uniaxial tensile test was carried out on the vaginal tissue of a sow to compare with the simulations, to identify meshes that displayed behaviour akin to vaginal tissue. Finally, the most promising outcomes were compared with those of the uterosacral ligament and a commercially available mesh. Findings Following a comprehensive analysis of the results, the mesh that most accurately replicates the behaviour of the vaginal tissue showcases a smaller pore diameter (1.50 mm), filaments in specific areas of the mesh, and variable filament thickness across the mesh. Nevertheless, upon comparing the outcomes with those of the uterosacral, the meshes do not exhibit similar behaviour to the ligament. Finally, the commercially available mesh does not represent the behaviour of both the vaginal tissue and the uterosacral ligament and in this sense may not be the best treatment option for POP repair. Originality/value Their biocompatibility and biomechanical properties make them a potential solution to the disadvantages of synthetic meshes. Personalized/customized meshes could be part of the future of surgical POP repair.

Abstract:Iron and sulfur reducing conditions generally develop in permeable reactive barrier systems (PRB) constructed to treat contaminated groundwater. These conditions allow formation of FeS mineral phases. FeS readily degrades TCE, but a transformation of FeS to FeS2 could dramatically slow the rate of TCE degradation in the PRB. This study uses acid volatile sulfide (AVS) and chromium reducible sulfur (CRS) as probes for FeS and FeS2 to investigate iron sulfide formation and transformation in a column study and PRB field study dealing with TCE degradation. Solid phase iron speciation shows that most of the iron is reduced and sulfur partitioning measurements show that AVS and CRS coexist in all samples, with the conversion of AVS to CRS being most significant in locations with potential oxidants available. In the column study, 54% of FeS was transformed to FeS2 after 2.4 years. In the field scale PRB, 43% was transformed after 5.2 years. Microscopy reveals FeS, Fe3S4 and FeS2 formation in the column system; however, only pyrite formation was confirmed byX-ray diffraction. The polysulfide pathway is most likely the primary mechanism of FeS transformation in the system, with S0 as an intermediate species formed through H2S oxidation.
Preparation and Biocompatibility Study of Contrast-Enhanced Hernia Mesh Material

Abstract:Background: Meshes play a crucial role in hernia repair. However, the displacement of mesh inevitably leads to various associated complications. This process is difficult to be traced by conventional imaging means. The purpose of this study is to create a contrast-enhanced material with high-density property that can be detected by computed tomography (CT). Methods: The contrast-enhanced monofilament was manufactured from barium sulfate nanoparticles and medical polypropylene (PP/Ba). To characterize the composite, stress tensile tests and scanning electron microscopy (SEM) was performed. Toxicity and biocompatibility of PP/Ba materials was verified by in vitro cellular assays. Meanwhile, the inflammatory response was tested by protein adsorption assay. In addition, an animal model was established to demonstrate the long-term radiographic effect of the composite material in vivo. Subsequent pathological tests confirmed its in vivo compatibility. Results: The SEM revealed that the main component of the monofilament is carbon. In vitro cell experiments demonstrated that novel material does not affect cell activity and proliferation. Protein adsorption assays indicated that the contrast-enhanced material does not cause additional inflammatory responses. In addition, in vivo experiments illustrated that PP/Ba mesh can be detected by CT and has good in vivo compatibility. Conclusion: These results highlight the excellent biocompatibility of the contrast-enhanced material, which is suitable for human abdominal wall tissue engineering.

Abstract:Aim: Hernia repair strengthens the abdominal wall with a textile mesh. Recurrence and pain indicate weak bonds between mesh and tissue. It remains a question which biomechanical factors strengthen the mesh-tissue interface, and whether surgeons can enhance the bond between mesh and tissue. Material and Methods: This study assessed the strength of the mesh-tissue interface by dynamic loads. A self-built bench test delivered dynamic impacts. The test simulated coughing. Porcine and bovine tissue were used for the bench test. Tissue quality, mesh adhesiveness, and fixation intensity influenced the retention power. The influences were condensed in a formula to assess the durability of the repair. The formula was applied to clinical work. The relative strength of reconstruction was related to the individual human abdominal wall. From computerized tomography at rest and during Valsalva's Maneuver, the tissue quality of the individual patient was determined before surgery. Results: The results showed that biomechanical parameters observed in porcine, bovine, and human tissue were in the same range. Tissues failed in distinct patterns. Sutures slackened or burst at vulnerable points. Both the load duration and the peak load increased destruction. Stress concentrations elevated failure rates. Regional areas of force contortions increased stress concentrations. Hernia repair improved strain levels. Measures for improvement included the closure of the defect, use of higher dynamic intermittent strain (DIS) class meshes, increased mesh overlap, and additional fixation. Surgeons chose the safety margin of the reconstruction as desired. Conclusion: The tissue quality has now been introduced into the concept of a critical and a gained resistance toward pressure-related impacts. A durable hernia repair could be designed from available coefficients. Using biomechanical principles, surgeons could minimize pain levels. Mesh-related complications such as hernia recurrence can potentially be avoided in incisional hernia repair.
Observation on mechanical properties and collagen content of porcine intestinal sub-mucosa biological mesh after implantation into rat abdominal wall

Abstract:Objective To compare local mechanical properties and the changes of the contents of type Ⅰ collagen fibers of the degradable porcine intestinal submucosa acellular matrix (SIS) biological mesh and non-absorbable artificial synthetic polypropylene mesh after implantation into the abdominal wall of rats.Methods A total of 60 adult Sprague-Dawley (SD) rats were randomly divided into three groups,20 rats in each group,and 2 cm×2 cm artificial defects were formed in abdominal wall of these rats.The polypropylene synthetic mesh (group A) and SIS biological mesh (group B) were implanted into abdominal wall of the rats,and the outcomes were compared with mesh-free implantation group (group C).At the third,ninth,fifteenth,and twenty-first months after operation,the maximal tensile resistance of the implanted tissue and the contractility of the muscles around the rectus abdominis muscle fibers were compared,and the content of collagen fibers in the implanted site was detected by immunohistochemistry.Results There were 10 rats of death after operation,including 4 rats in group A,3 rats in group B,and 3 rats in group C,and there was no statistically significant difference among these three groups (P＞ 0.05).There were 1 rat of abdominal wall bulge in group A,6 rats in group B anl no bulge in group C,and there was statistically significant difference among the three groups (P＜0.05).There were statistically significant differences between group B and group A,group C,respectively (all P＜0.05),and there was no statistically significant difference between group A and group C (P＞0.05).With the extension of postoperative time,the local tensions of defect repair in group A and group C were the strongest,which were statistically significantly better than that in group B (P＜0.05).In group B,the early tension was acceptable,then it decreased with the degradation of SIS and gradually increased with the proliferation of collagen fibers,the tension gradually increased.The contractility of the muscles around the rectus abdominis muscle fibers increased in each group,but there was no statistically significant difference between the two groups.The proliferation of local collagen fibers in group A and group C was more significant than that in group B (P＜0.05).There was no mesh exposure or erosion in the two groups.Conclusion The local tension of SIS mesh used to repair abdominal wall defect in SD rats is lower than that of polypropylene mesh.The utilization of SIS mesh in the repair of human pelvic organ prolapse requires more research evidence.

Objective: To design an oestradiol-releasing electrospun poly-l-lactic acid (PLA) mesh and evaluate its ability to stimulate new extracellular matrix and new blood vessel formation. Design, setting, and participants: Human adipose derived mesenchymal cells (ADMSC) were isolated from fat. PLA meshes with micro- to nano-sized fibres containing 1%, 5%, and 10% oestradiol were constructed and used for in vitro and in vivo experiments. Intervention: The angiogenic potential of the fibrous meshes was evaluated using an in vivo chorioallantoic membrane and an in vitro chick aortic arch assays. Outcome measurements and statistical analysis: Oestradiol release was measured fluorometrically. The effect of fibrous meshes on proliferation and extracellular matrix (ECM) production of ADMSC was assessed using immunohistology. Mechanical properties were tested using a tensiometer. Results and limitations: The ultrastructure of the mesh was not affected by the inclusion of oestradiol and mechanical properties were only slightly modified. Oestradiol was released from PLA meshes over a 5-mo period. ADMSCs cultured on oestradiol-releasing PLA meshes produced more ECM involving collagen I, collagen III, and elastin. Oestradiol-releasing meshes doubled new blood vessel formation in the chorioallantoic membrane assay (p=0.001) and outgrowth of pro-angiogenic cells in the aortic arch assay (p=0.001). Further studies in longer-term animal models are required to confirm these results. Conclusions: Oestradiol-releasing PLA meshes increase ECM production and stimulate angiogenesis. As such, they are promising candidate materials to be used in pelvic floor repair and to improve the initial healing phase of a repair material following implantation. Patient summary: In this study, we designed a tissue engineered material to be used to support weakened pelvic floor tissues in women to avoid the complications associated with current surgical mesh. Our results showed that this material can stimulate new blood vessel formation in simple chick assays and tissue production in vitro. Both properties should help with the integration of this material into patients' tissues and merit further study in physiologically relevant animal models.
Functional and morphological evaluation of different polypropylene-mesh modifications for abdominal wall repair

Abstract:Modern surgical hernia repair depends increasingly on synthetic meshes for the reconstruction of the abdominal wall. Despite the undisputed advantages of the polypropylene (PP) meshes currently available (Marlex, Prolene), reports of complications after implantation are increasing. Although, serious complications such as perforation and fistula formation are rare, minor and local complaints such as seromas, misfeelings and a decreased abdominal wall mobility are observed in about one-half of the patients. In regard to the exaggerated strength of the currently available mesh modifications a reduction of the material should improve the integration of the meshes into the artificial abdominal wall. In the present study, the commercially available basic mesh Prolene has been compared to two newly constructed PP-mesh modifications with reduced amounts of PP. The modifications have gradually been adopted to the physiological requirements of abdominal wall stability and mobility by reducing the amount of PP to 64% (E-BLUE) and 24% (variant A) of the Prolene mesh (developed by ETHICON, Norderstedt, Germany). All PP-mesh variants have been implanted in a rat model and studied by 3D-photogrammetry, tensiometry, light- and electron microscopy, as well as morphometry over implantation intervals of 3, 7, 14, 21 and 90 days. The data show that current constructions of PP-meshes are oversized and definitely restrict abdominal wall mobility in the present model. Sufficient stability of the artificial abdominal wall is even guaranteed by PP-mesh modifications with a reduction of PP-quantity to about 25% of the Prolene mesh. The degree of fibrosis directly correlated with abdominal wall restriction, whereas the formation of connective tissue in the interface PP-fibre/host-issue depends on the amount and activity of the inflammatory reaction. The quantity and quality of inflammation, again, directly relies to the amount of PP and to the surface area in contact with the recipient tissues. Altogether, the present study suggests that a modification of the PP-meshes could be helpful to prevent major and minor complications of surgical PP-meshes.

Abstract:Introduction and hypothesis To investigate the tissue reactions of a novel porcine-derived urinary bladder matrix/small intestinal submucosa (UBM/SIS) biological mesh and SIS mesh implanted in a rabbit vaginal defect model. Methods Thirty-two rabbits were implanted with UBM/SIS mesh (Group A) and SIS mesh (Group B), respectively. Rabbits were sacrificed at 7, 14, 60, and 180 days after implantation. The tensile strength, elongation at break, and elastic modulus of the tissue were measured using biomechanical methods. The inflammatory response, cell infiltration, vascularization, and collagen fibers were observed. Results Compared with Group B, the tensile strength and elongation at break of group A was higher at 14, 60, and 180 days. The elastic modulus of group A was lower at 180 days. Inflammatory response of group A was milder at 14, 60, and 180 days. There was more cell infiltration in group A at 7 and 14 days. Vascularization was higher in group A at 7 days and 14 days. The order of collagen in group A was better at 14, 60, and 180 days. The proportion of thick red fibers in both groups showed an increasing trend. At 14 days, group A had more thick red fibers. Conclusions The novel UBM/SIS composite mesh had a milder inflammatory response; earlier induction of cell infiltration, angiogenesis, and collagen regeneration. Collagen fibers had a better order. It has a higher tensile strength and greater elongation at break, and can be used as a potential material for the treatment of pelvic organ prolapse.

Abstract:BACKGROUND: Postsurgical adhesions severely affect the patients' quality of life causing various complications like bowel obstruction or chronic pain. Especially the implantation of alloplastic prostheses in IPOM position for hernia repair carries a high risk of adhesion formation due to the close contact between mesh and viscera. The extent of adhesions mainly depends on the type and textile characteristics of the implanted mesh. The aim of this study was to examine the degree of adhesion formation of a newly developed, elastic thermoplastic polyurethane (TPU) mesh in comparison to polypropylene (PP) meshes in IPOM position in a rabbit model.METHODS: Sixteen female chinchilla rabbits were laparoscopically operated. Two different meshes were placed to the left and the right lower abdominal wall in IPOM position in each rabbit. After 7 or 21 days, midline laparotomy was performed, the degree of adhesion formation was examined by the Diamond score and mesh elongation was measured under a force of 3N. Finally, the abdominal walls were explanted for immunohistochemical and histopathological investigations.RESULTS: TPU meshes showed significantly lower Diamond scores than PP meshes. After explantation, mesh elongation of the TPU mesh was significantly larger than expansion of PP under a force of 3N. Thus, the TPU mesh preserved its elastic properties after 7 and 21 days. The amount of CD68 positive, Ki67 positive and apoptotic cells within the granuloma around the fibers did not show significant differences between the study groups.CONCLUSIONS: The newly developed TPU mesh seems to reduce peritoneal adhesion formation in IPOM position in a rabbit model compared to PP meshes after 7 and 21 days. Immunohistochemistry did not reveal differences in biocompatibility of the two meshes used.

Abstract:Abstract A fabric hydrogel composite hernia mesh with biomechanical and mesh–tissue interface dual compliance is designed for scarless abdominal wall reconstruction. The mesh is composed of a polyester knitted fabric and chitosan–polyacrylamide hydrogel complex. The mechanical properties of the composite mesh are adjusted to resemble the human abdominal wall by alkali treatment of chitosan to achieve good biomechanical compliance. An adhesion group is introduced into the composite mesh that forms covalent bonds with tissue, eliminating the need for suturing, reducing stress concentration at the fixation site, achieving mesh‐tissue interface compliance, and improving the simplicity of the operation. Covalent bonds and hydrogen bonds make the mesh have strong adhesion (70.1 ± 3.2 kPa) and repeatable (four times) robust adhesiveness. In vivo experiments using a rabbit abdominal wall defect model demonstrate quick adhesiveness and excellent functional reconstruction. Biomechanical and mesh–tissue interface dual compliance allow the tissue to regenerate an intact abdominal wall structure. The adhesive fabric hydrogel composite hernia mesh offers significant clinical values for repairing abdominal wall defects and provides design ideas for repairing other load‐bearing soft tissues.
