Abstract:The moxibustion has a dual effect of tonification and purgation in TCM theories, which are based on two aspects: the actions of the meridian system and the roles of moxa and fire. Modern research works of the moxibustion mechanism mainly relate to the thermal effects, radiation effects, and pharmacological actions of moxa and its combustion products. Experimental results showed that moxibustion thermal stimulation affects both shallow and deep tissues of the skin, and the warm-heat effects of moxibustion have a close relation to the warm receptors or/and the polymodal receptor. The burning moxa radiation spectrum ranges from 0.8 to 5.6 μ m; peak is nearby 1.5 μ m, lying within the near infrared portion. There is an amazing consistency in the infrared spectrums of three types of indirect moxibustion and the unified spectrum of acupoints; all have their peaks of radiation near 10 μ m. Lots of ingredients had been identified from mugwort leaves and moxa smoke, which have a variety of biological activities; they were considered to participate in the comprehensive effects of moxibustion. Although lots of research works have been carried out and made some progress, there is still a great distance from fully understanding the mechanism of moxibustion.

Situation and Evaluation of Current Study on Heat-Sensitive Moxibustion Based on Literature Metrology

Abstract:Objective:To analyze and evaluate the current study on heat-sensitive moxibustion at home and abroad.The aim is to provide the basis for further research on heat-sensitive moxibustion.Methods:Literature was searched comprehensively through CNKI,VIP,WF and PubMed during 2006.1.1.-2016.8.15.Then the measurement results were analyzed by the bibliographic method.Results:A total of 755 papers in China were published in 184 medical journals and 22 SCI papers published in 10 journals.The number of literature in recent 6 years showed a significant upward trend.The first authors were more from three-A level hospitals,distributed in 27 provinces and cities.The type of literature was mainly clinical research.Seventy-eight papers were supported by national key basic research and development program (973 Program);22 papers SCI;65 papers supported by National Natural Science Foundation of China,15 papers supported by National Science and technology support program or the national science and technology research program;490 papers supported by provincial or departmental research projects.Heat-sensitive moxibustion can treat 185 kinds of diseases,distributed in 11 systems disorders.Conclusion:Heat-sensitive moxibustion has a good clinical application and popularization,but its clinical research standards and its mechanism of action need to be strengthened.

Abstract:A moxibustion device with the functions of auricular fumigation moxibustion and heat-sensitive moxibustion is designed. The smoke of the ignited moxa stick is used for the fumigation moxibustion at the external auditory canal, while the heat generated works on Dazhui (GV 14) for heat-sensitive moxibustion. The device consists of five parts, i.e. combustion chamber, smoke pipe, smoke processing chamber, power module and connector. It solves the limitations such as unpleasant experience in treatment, unfavorable temperature control, easy scalding and excessive manual dependence induced by usual fumigation moxibustion and during heat-sensitive moxibustion. This moxibustion device may improve the safety and convenience when delivering the treatment with fumigation moxibustion and heat-sensitive moxibustion, as well as the work efficiency of medical staff.
Moxibustion in Early Chinese Medicine and Its Relation to the Origin of Meridians: A Study on the Unearthed Literatures

Abstract:Moxibustion is an integral part of Traditional Chinese Medicine (TCM). It achieved higher level of recognition and had more general application in ancient times than in contemporary life. As the vital historical sources, the records of unearthed literatures offered precious insights to Chinese social life pattern and medical practice in Qin and Han dynasties (221 BC–220 AD). There was no surprise that the bamboo and silk documents excavated from Mawangdui (马王堆 ) tomb, Hantanpo ( 旱滩坡 ) tomb, and other relics had a large amount of texts relevant to moxibustion. This research sorted moxibustion recordings from seven unearthed literatures and discovered that moxibustion had been developed into different modalities and utilized to treat many diseases at that time. In addition, the indications, contraindications of moxibustion, and the method of postmoxibustion care were also discussed. On this basis, some hints were provided to support the hypothesis that the practice of moxibustion led to the discovery of meridians. All our preliminary results in the research have drawn attention for this old therapy and given a new source for its application in clinic and scientific research.

Abstract:The penetrating moxibustion technique is proposed based on experience of the ancients and clinical practice for many years. From the aspects of definition, action characteristics and technique at different parts, the advantage and application experience of penetrating moxibustion have been discussed. The adequate dose of moxibustion is necessary in penetrating moxibustion; in addition, moxibustion sensation should be penetrating; therefore, with a temperature of 43℃ for more than 20 min, sweating, flushing, speckle appearing after penetrating moxibustion. Due to individual differences of age, gender and constitution factors, the effects of penetrating moxibustion are different, in clinical practice the body reaction and patient's feelings should be deliberately focused on other than does or sore and blister of moxibustion. The technique is common used in the abdomen, waist and knee joint, etc.

Abstract:Objective: Moxibustion therapy achieves satisfactory therapeutic effects largely depending on the heat stimulation of burning moxa. Understanding the thermal characteristics of heating process is an effective way to reveal the underlying mechanisms of moxibustion therapy. Methods: This paper performs experimental study on temperature distributions of burning moxa sticks and fresh in vitro porcine abdominal tissue using an infrared camera and thermocouples. Meanwhile, a moxibustion model incorporating moxa stick burning model and tissue heat transfer model was established with consideration of radiation propagation and water evaporation. Results: The burning features of moxa sticks were acquired and the radiation energy generated by the burning moxa stick was absorbed and scattered in biological tissue, resulting in a large temperature gradient in the skin layer. And the water evaporation led to a mass loss and reduced skin surface temperature. The numerical model was verified by experimental results and the effects of moxibustion treatment distance and duration can be quantified based on model calculation. Conclusion: The detailed heat transfer process of moxibustion was obtained experimentally and numerically. During moxibustion, the radiation attenuation and water evaporation have a significant influence on the energy transport in biological tissue which cannot be ignored. The treatment distance of 3 cm is the recommended value to achieve the treatment efficacy without thermal damage and pain. Significance: This research would reveal the underlying mechanisms of moxibustion therapy. Besides, the developed models are expected to establish a guideline for moxibustion clinical treatment.

Abstract:There is no systematic and whole-process system for moxibustion standard development at home and abroad, which restricts the industry innovation and technological progress to a certain extent. The paper reviews the study status and finds that the technical standard is dominant in moxibustion standard development currently, represented by conventional moxibustion, heat-sensitive moxibustion, moxibustion on the Governor Vessel, moxibustion with seed-size moxa cone and herb-isolated moxibustion, etc. There are many gaps in the standards development of moxibustion material and device, equipment building, moxa smoke purification, and management and job. On the basis of explaining the standard framework of moxibustion, it is suggested that the moxibustion standardization should be deepened in the aspects of hierarchical technical operation, material selection, research and development of new devices, personnel training and equipment management.

Abstract:An automatic ash-removal heat-sensitive moxibustion device was developed, which could keep relatively constant temperature of heat-sensitive moxibustion, and realize the automatic ignition and automatic ash removal of moxa sticks during heat-sensitive moxibustion. The automatic ash-removal heat-sensitive moxibustion device comprises a bracket and a moxibustion box fixed on the top of the bracket; the bracket is composed of a base and a movable telescopic arm. This device can solve the problems of temperature instability, moxa ash blocking heat transfer and moxa ash falling during heat-sensitive moxibustion, avoiding the scalding caused by moxa ash falling, and reduce the workload of medical staff.

Abstract:Background Knee osteoarthritis is a major cause of pain and functional limitation. Complementary and alternative medical approaches have been employed to relieve symptoms and to avoid the side effects of conventional medication. Moxibustion has been widely used to treat patients with knee osteoarthritis. Our past researches suggested heat-sensitive moxibustion might be superior to the conventional moxibustion. Our objective is to investigate the effectiveness of heat-sensitive moxibustion compared with conventional moxibustion or conventional drug treatment. Methods This study consists of a multi-centre (four centers in China), randomised, controlled trial with three parallel arms (A: heat-sensitive moxibustion; B: conventional moxibustion; C: conventional drug group). The moxibustion locations are different from A and B. Group A selects heat-sensitization acupoint from the region consisting of Yin Lingquan(SP9), Yang Lingquan(GB34), Liang Qiu(ST34), and Xue Hai (SP10). Meanwhile, fixed acupoints are used in group B, that is Xi Yan (EX-LE5) and He Ding (EX-LE2). The conventional drug group treats with intra-articular Sodium Hyaluronate injection. The outcome measures above will be assessed before the treatment, the 30 days of the last moxibustion session and 6 months after the last moxibustion session. Discussion This trial will utilize high quality trial methodologies in accordance with CONSORT guidelines. It will provide evidence for the effectiveness of moxibustion as a treatment for moderate and severe knee osteoarthritis. Moreover, the result will clarify the rules of heat-sensitive moxibustion location to improve the therapeutic effect with suspended moxibustion, and propose a new concept and a new theory of moxibustion to guide clinical practices. Trial Registration The trial is registered at Controlled Clinical Trials: ChiCTR-TRC-00000600.

Abstract:Moxibustion penetration is a therapy in which the moxibustion sensation promoted by moxa is felt deeply inside the tissues. When moxibustion is practiced to certain extent, the sensation conducts from the moxibustion site to the deep tissues and far ends, or muscular pulsation appears at the moxibustion site, or the patient feels comfortable, painful, heavy and itching or the patient has reaction as skin redness and even perspiration throughout the body after the moxibustion. The best effect of moxibustion is expected when local site becomes slightly and evenly red or there is sweating. The observation has shown that the moxibustion dosage is the key to enhancing the therapeutic effect.

Abstract:"Unblocking fu organs" is one of the essential principles of Ma's warm moxibustion technique, characterized as "dredging" and "harmonizing" for either deficiency or excess condition. Under the guidance of this therapeutic thought, the acupoints for moxibustion are mainly selected from the middle and lower parts of the body. Regarding the therapeutic approach, the acupoint prescription for moxibustion should be formed in line with warming and promoting circulation of fu organs; the moxibustion degree should be specially considered, in which, the mild moxibustion is recommended to induce promoting action; and the systematic moxibustion technique should be the root for dredging fu organs and regulating zang organs. Ma's mild moxibustion technique stresses on removing the obstruction of fu organs and emphasizes promoting the qi activity of sanjiao (triple energizer) and regulating the balance of five zang organs.

Abstract:Moxibustion has a history of thousands of years. Umbilical moxibustion achieves prevention and treatment of diseases through moxibustion at the golden section of the human body-the umbilicus. It has enough clinical practice experience. Traditional treatment methods have problems such as irritating smoke, uncontrollable temperature, experience-based, and difficult to quantify. Based on the principle of traditional umbilical moxibustion, a structural design of an electronic smokeless umbilical moxibustion instrument is proposed, which belongs to indirect moxibustion and has the advantages of smokeless, electric heating, portability, temperature monitoring and controllability. The internal heat transfer process of the moxibustion instrument is mainly heat convection and heat radiation. Based on ANSYS software, the internal temperature field of the moxibustion instrument is analyzed, and the steady-state temperature distribution and the temperature change characteristic curve of different positions with time are obtained to verify the rationality of the structural design. At the same time, lay a research foundation for the measurable and quantifiable effects of moxibustion on umbilical therapy.

Abstract:A multifunctional moxibustion treatment machine is designed and developed to assist the heat-sensitive moxibustion therapy. Through the motion control of the stepping motor by programmable logic controller (PLC), the automatic control is obtained for the acupoint detection of heat-sensitive moxibustion therapy and the manual operation of moxibustion. The skin temperature is monitored in real-time, using infrared non-contact temperature measurement technology. Based on the deviation of the temperature set value and the monitoring one, the distance between the moxibustion device and the exerted region is adjusted automatically by PLC so that the temperature is controlled practically. The multifunctional moxibustion treatment machine based on the heat-sensitive moxibustion therapy is capable of the operation control of mild moxibustion, circling moxibustion, sparrow-pecking moxibustion and along-meridian moxibustion techniques, as well as real-time monitoring of skin temperature. The temperature change curve of this machine is coincident with that obtained by the manual operation of heat-sensitive moxibustion. This multifunctional moxibustion treatment machine assists the delivery of heat-sensitive moxibustion therapy and it is satisfactory in temperature control and precise in operation.

Abstract:In the past 20 years, the acupuncture-moxibustion discipline has made a great progress in clinical research, method construction, standard formulation, guideline promotion, basic theory and key scientific issue research. Internationally, the development of acupuncture and moxibustion has gradually begun to pay more attention to the basic issues of the discipline itself from focusing on clinical evidence. The National Institute of Health of USA pays close attention to the construction of acupoint knowledge base and database and to the transformation of peripheral nerve stimulation techniques, which brings forth opportunities and challenges for the development of acupuncture-moxibustion discipline. In the present paper, we analyze the shortcomings of the current development of acupuncture and moxibustion, put forward some strategies for high-quality development in the future, and sort out the basic scientific issues to form an academic consensus. We should employ modern scientific language to express the scientific connotations of the basic theory of acupuncture and moxibustion, and build an open and self-consistent modern theoretical system. In addition, we also should attract more multidisciplinary talents to harmoniously and assiduously work together, insist on continuous innovation to open up a new situation in the transformation of basic scientific research achievements, and establish a new theoretical system of "somato-medicine" represented by acupuncture and moxibustion. In this way, we will guide the acupuncture-moxibustion discipline to make an original contribution to the modern life science and future medicine.

Abstract:Based on the literature review and theories of biophysical theory, the pathway and mechanism of the heat of sandwiched moxibustion stimulating acupoints are analyzed. It is concluded that the heat of sandwiched moxibustion is transferred to the skin of acupoints by heat conduction and thermal radiation. The heat conduct from the skin to deep tissues and other parts was carried out through the biological heat conduction systems. The investigation combined with theories of biological heat conduction was benefit to the research on clinical mechanisms of indirect moxibustion. In addition to heat effect, the infrared resonance of indirect moxibustion and acupoints may be the base of clinical effects of indirect moxibustion.
[On the Considerations About Heating Materials and Temperature of Moxibustion in Clinical Practice]

Abstract:Moxibustion, one of the traditional Chinese medicine therapy, plays an important role in the treatment of a variety of clinical conditions, particularly the chronic and deficiency problems. In the present paper, the author reviewed the history of development of moxibustion therapy, and the related materials and heating temperature in ancient China, and also reviewed the history of heating therapy (similar to moxibustion) appearing in ancient Egypt, Greek, Libya, India, Europe, etc. The author thinks that the efficacy of moxibustion intervention mainly depends on the heating temperature and the heated area of the skin (not the heating materials) according to his own research group's experimental outcomes. Some problems of moxibustion should be noted, for example, the ignited moxa-smoke induced environmental contamination which may lead to chronic injury particularly to the health of the related practitioners. The author forecasts that some succedaneous materials of mugwort as physical heating energy (mechanical, electrical, microelectronic, solar energy), chemical, biological and medical heating reagents (acupoint receptor-activator), etc., high technique medical devices are probably developed in the future, minimizing some defects of moxa and expanding clinical application of moxibustion therapy.

Abstract:The paper reviews the inheritance, innovation and development of heat-sensitive moxibustion; and explores the path for the clinical development of moxibustion of traditional Chinese medicine moxibustion (TCM). Practice has shown that the laws of clinical research on TCM moxibustion refer to phenomenon discovery, exploration of rules, technological innovation, verification of curative effects, theory sublimation, returning to clinical practice, discipline construction, and experimental research. It is deeply realized that TCM research should be based on clinical practice, originated from classics, focused on theoretical innovation and in serve of clinical practice.
