Retroperitoneal Laparoscopic Anderson-Hynes Dismembered Pyeloplasty(Report of 25 Cases)

Abstract:Objective:To assess the clinical value of retroperitoneal laparoscopic dismembered Anderson-Hynes pyeloplasty.Methods:The clinical data of 25 patients with ureteropelvic junction obstruction (UPJO) who underwent retroperitoneoscopic dismembered Anderson-Hynes pyeloplasty were retrospectively analyzed.Mean age of 16 men and 9 women with UPJO was 32 years (range 18 to 43).The lesion was found on the left in 14 cases,on the right side in 11 cases.All the patients were diagnosed preoperatively by intravenous pyelography (IVP).retrograde pyelography (RP) or magnetic resonance urography (MRU).Results:25 cases were all successfully treated with laparoscopic managemen.The mean operating time was 135 min (120-160 min).The mean introperative bleeding was 75 ml (50-100 ml),but no blood transfusion.The mean postoperative hospital stay was 7.3 days(5- 8 d).One case had postoperative urine leakage.During the follow-up of 3 to 18 months,25 cases did IVP examination, and no anastomotic stoma stricture of UPJ developed and hydronephrosis improved.Demonstrated by B ultrasonic examination.Hydronephrosis was resolved in 20 cases,mild in 3,and moderate in 2.Conclusions:Retroperitoneal laparoscopic Anderson-Hynes dismembered pyeloplasty is safe and efficient and has advantage of minimal invasion.It will substitute the open surgery and become the first choice for treating UPJO.
