Clinical Significance of Retroperitoneal Laparoscopic Pyeloplasty for Ureteropelvic Junction Obstruction

Abstract:Objective To evaluate the choice of indications and clinical effects of retroperitoneoscopic pyeloplasty for ureteropelvic junction(UPJ) obstruction,and to review its technical details.Methods The clinical data of 13 patients with UPJ obstruction who underwent retroperitoneoscopic dismembered Anderson-Hynes type pyeloplasty were retrospectively analyzed.There were 7 men and 6 women.Their age ranged from 11 to 47 years.Of them 1 case experienced holmium laser endopyelotomy failure,1 had high ureteral insertion,3 had constriction segment 2 cm or complete atresia of UPJ,4 had simple severe hydronephrosis,2 had compression from crossing vessels,and 2 were complicated with pelvic stones.Results Retroperitoneoscopic pyeloplasty was performed successfully in all 13 cases.The operative time ranged from 120 to 200 min(mean,175 min).The blood loss ranged from 60 to 110 ml(mean,75 ml).The mean postoperative hospital stay was 7.5 d(range,5-8 d).There was no perioperative complication.During the follow-up of 3-19 months,no anastomotic stoma stricture of UPJ developed;and hydronephrosis resolved and renal function improved.Conclusions Laparoscopic pyeloplasty offers a feasible,effective and less invasive alternative to open surgery with reduced morbidity,shorter hospital stay,and faster convalescence.Laparoscopic pyeloplasty should become the operation of first choice in cases of hydronephrosis secondary to crossing vessels,when there is great pyelic dilation,and for the treatment of failure of previous endopyelotomy and high ureteral insertion.
