Validity of Long-delayed Recall Test on the Diagnosis of Alzheimer Disease

Abstract:Objective To compare validity on early diagnosis of Alzheimer's disease(AD) between immediate recall and long-delayed recall.To offer a new method in neuropsychological assessment in clinic practice.Methods 183 AD patients,including 118 mild,and 65 moderate in degree,1417 controls,including 1283 normal individuals,and 134 mixed controls suffered from other diseases without dementia were recruited.WHO-UCLA California verbal learning test(WHO-UCLA CVLT),Complex figure(CF),and Logic memory(LM) were conducted.Each test included immediate recall,long-delayed recall and long-delayed recognition.Score of immediate recall was sum of immediate recall of the 3 scales.Score of long-delayed recall was sum of long-delayed recall of the 3 scales.Score of long-delayed recognition was sum of long-delayed recognition of the 3 scales.Total score of recall was the sum of immediate recall,long-delayed recall and long-delayed recognition of the 3 scales.Results Adjusted educational level,age,sex,rural/urban status by multiple analysis covariance,there significant difference in total score of recall between normal controls(76.8±0.4),mixed controls without dementia(59.8±1.5),mild AD patients(31.5±1.5),moderate AD patients(19.3±2.0),P values between every two different groups were 0.0001.The cut-off points for diagnosis of AD were taken from 5~(th) percentage of normal controls of(different) kinds of recall score in different educational level respectively.The sensitivity was 81.87%,84.70%,87.13%,and the specificity was 94.13%,92.61%,and 93.53% for immediate recall,long-delayed recall,sum or recall respectively.Conclusions Compared with assessing immediate recall alone,this study showed assessing long-delayed recall,followed by long-delayed recognition besides immediate recall increased sensitivity in early diagnosis of AD.
