Abstract:PURPOSE: Cervical lymph node metastases reduce survival rate of patients with oral squamous cell carcinoma by up to 50%. Since Crile's landmark publication in 1906, radical neck dissection (RND) has remained the gold standard for excision of regional lymph nodes metastases of head and neck malignancies. Since 1950's, the focus on RND associated morbidity had urged clinicians to seek for modified procedures that could provide oncologic cure with less or acceptable morbidity. Definitely, any modification of RND must demonstrate equivalent oncologic efficacy. The purpose of the present study was to evaluate the oncologic efficacy, surgical safety and postoperative morbidity of a rational procedure of modified radical neck dissection. METHODS: By retrospective analysis, data of 85 cases of classic RND was collected as the control. The experimental group include 97 cases who underwent a modified procedure named as rational radical neck dissection (RRND), in which the external jugular vein (EJV), deep branches of the cervical nerve and greater auricular nerve were preserved. The internal jugular vein (IJV) was ligated and the spino-accessory nerve was excised in all cases. The evaluated parameters included post-operative general condition, regional edema, neck recurrence, shoulder disfunction and auricle sensibility. The pathological evaluation on cervical lymph node metastases were also recorded and correlated with the clinical stage and preoperative adenopathy estimation (cN). The obtained data were analyzed with Chi-Square test. RESULTS: Compared with RND group, patients with RRND had minor edema on the face and head. Furthermore, in cases of bilateral RND, preserving of the EJV could lessen bleeding during operation and post-operative effusion, and reduce the possibility of prophylactic tracheotomy that was necessary in bilateral RND. 19.66% of cN0 and 14.29% of T1 cases were diagnosed as lymph nodes metastasis pathologically. The 5-year survival rates without cervical recurrence after RND and RRND were 92.54% and 94.60%, respectively. There was no significant difference between the 2 groups (P>0.05). The shoulder malfunction of patients in 1 year after RRND was less severe than those of RND (P<0.05). Numbness of the auricular skin 6 months after RRND was less severe than that of RND (P<0.05). CONCLUSION: It was suggested that RRND had less postoperative reactions and better shoulder function compared with RND, without impairment on the oncologic safety of the neck dissection. The inconsistency between cN and pN hinted the necessity of elective neck dissection in management of cN0 patients in given instance. It was indicated that RRND could be used as an elective method for cN0 patients and radical modality in cases of definite cervical metastasis with acceptable oncologic efficacy and postoperative morbidity.
