Abstract:Objective To compare the clinical effects of three different surgical treatments including dynamic hip screw (DHS),proximal femoral nail antirotation (PFNA) and hemiarthroplasty on the treatment of femoral intertrochanteric fractures of the elderly.Methods From Jun.2011 to Sep.2014,145 cases of elderly patients with intertrochanteric fracture were selected according to the selection criteria in the orthopedic department of the Shaanxi Provincial People's Hospital,including 66 males and 79 females,with a mean age of (80.5 ±-3.2) years (from 75 to 92 years).The patients with intertrochanteric fracture were operated by DHS,PFNA,or hemiarthroplasty.SPSS 19 statistical software was used for statistical analysis.The operative time,intraoperative blood loss,ambulation time,weight-bearing time after the operation were analyzed using one factor analysis of variance and multiple comparisons.Harris hip scores were analyzed using analysis of variance for repeated measurement data.Results The operation time of the PFNA group (68.0 ± 13.0) min was the shortest,that of the DHS group (110.0 15.0) min was the longest;the operation time of the eementless prosthesis group (70.0 ±-5.0) min was shorter than the cement prosthesis group (85.0 ± 10.0) min;the difference was significant among the groups (F =87.022,P ＜ 0.05).The intraoperative blood loss was significantly different (F =376.353,P ＜ 0.05) between the DHS group (390.0-± 10.0) ml and the PFNA group (145.0 ± 15.0) ml.In terms of weight bearing time after operation,the difference was significant (F =376.353,P ＜ 0.05).The hemiarthroplasty group were significantly earlier than the PFNA and DHS groups,while the cement prosthesis group was earlier than the cementless prosthesis group.As for postoperative complications,the PFNA group were superior than other groups.The postoperative complications of the DHS group,the PFNA group,the cement prosthesis group and the eementless prosthesis group were 21.4％ (6/28),13.3％ (4/ 30),17.1％ (6/35),17.3％ (9/52) respectively.Harris scores of the hemiarthroplasty group in the same time point were higher than those of the PFNA groups;Harris scores of the PFNA group were higher than those of the DHS group;the differences were statisticaiiy significant (all P ＜ 0.05).The differences between the cementless prosthesis group and cement prosthesis group in the same time point were not statistically significant (P ＞ 0.05).In terms of Harris hip score,the difference was not statistically significant among the groups at 12 months after operation (P ＞ 0.05).Conclusions For the elderly patients with unstable femoral intertrochanteric fractures,DHS should be chosen carefully.For the patients with good physical condition,better bone quality,a preferred intramedullary fixation can be chosen (PFNA);for the patients with severe osteoporosis who are not suitable for internal fixation or have internal fixation failure,or the life span of the artificial joint is short after an expected replacement,the long-stem hemiarthroplasty is a safe and effective surgical approach.
