Comparsion study between the multiple tracts minimally invasively percutaneous nephrolithotomy combined with holmium laser in a single session and single tract percutaneous nephrolithotomy for the staghorn calculi

Abstract:[Objective] To investigate the clinical efficacy and safety of the multiple tracts minimally invasively pertutaneous nephrolithotomy combined with ureteroscope holmium laser (multi-MPCNL) in a single session compared with the procedures of requiring a single tract (single-MPCNL) combined with ESWL(extracorporeal shock wave lithotripsy) for the renal staghorn calculi. [Methods] Clinical data of 48 cases of staghorn calculi was analysed from September 2005 to September 2007, including 27 cases treated by multi-MPCNL and a cohort of 21 cases treated by single-MPCNL. The operation time, total operative blood loss, blood transfusion rate, postoperative hospital stay, complications rate, total treatment cost and calculus clearance at 3 month of postoperative follow-up between two groups were evaluated. [Results] Compared with single-MPCNL, multi-MPCNL was associated with significantly shorter operation time [(123±26) min vs (167±31) min, t=5.35, P=0.000], shorter postoperative hospital stay [(4.62±1.42) d vs (5.90±1.81) d, t =2.74, P =0.009] and less total treatment cost [(79.11±9.87) RMB vs (98.81±10.34) ×102 RMB, t =6.71, P =0.000], respectively. However, there was no significant difference in the drop of hemoglobin, blood transfusion rate and complications rate between two groups, respectively. After a follow-up period of 3 months, multi-MPCNL was also associated with higher stone clearance rate [88.89% (24/27) vs 76.19% (16/21), χ2=0.61, P =0.435] than that of single-MPCNL. [Conclusion] Multi-MPCNL in a single session which had advantages of higher removal speed of stone, shorter hospital stay and earlier return to work, less complications and higher stone clearance rate, less cost which was easily accepted by patients should be the first choice therapy for massive renal staghorn calculi.
