Abstract:Objective:This study is aimed at reporting 6 cases of temporal bone carcinoid and analyzing clinical and prognostic features of temporal bone carcinoid.Method:Clinical data from 6 cases of temporal bone carcinoid treated from July 2008 to July 2015 at the Chinese PLA General Hospital were reviewed.Based on temporal bone lesions row subtotal temporal bone resection or temporal bone extensive excision.Result:Six patients imaging data shows a different range of temporal bone lesions.Pathological both temporal bone carcinoid.The final diagnosis is unilateral primary temporal bone carcinoid.All of them received surgical partial resection of the temporal bone in our hospital,and there were no serious complications.Follow-up of 5 cases had no recurrence,and 1 case died spread to the liver.Conclusion:The diagnosis of temporal bone carcinoid relies mainly on pathology and immunohistochemistry.Temporal bone carcinoid could be treated by surgical.

Abstract:Background The aim of the present systematic review was to summarize evidence on odontogenic carcinosarcoma, analyzing clinical, epidemiological, imaging, histopathological, immunohistochemical, therapeutic, and prognostic features of this tumor. Materials and Methods This systematic review followed the Preferred Reporting Items for Systematic Reviews and Meta‐Analyses (PRISMA) guidelines. Searches were performed in the Ovid MEDLINE (Wolters Kluwer), PubMed (National Library of Medicine), Web of Science (Thomson Reuters), Scopus (Elsevier), and LILACS (Latin American and Caribbean Center on Health Sciences Information) databases, without publication date or language restrictions. Case reports or case series of OCS reporting clinical, radiological, and histopathological data that confirmed the diagnosis were selected. The Joanna Briggs Institute—University of Adelaide tool was used for critical appraisal of the included articles. Results Odontogenic carcinosarcoma is a rare, aggressive tumor associated with high mortality; however, the metastasis rate is low. The tumor has a male predilection. The mean patient age is 40 years, but there is no predilection for age. The left posterior mandible is the most affected site, but no specific radiographic features have been reported. Conclusion Given its rarity, dentists, oral‐maxillofacial surgeons, and physicians need to be aware of odontogenic carcinosarcoma in order to increase the diagnostic potential, preventing delays in diagnosis and treatment and thus contributing to lower morbidity of the tumor.

Abstract:PURPOSE:To analyze the clinical features, histopathological features, treatment and prognosis of carcinosarcoma in oromaxillofacial head and neck region.METHODS:The clinical and histopathological data of 10 cases with carcinosarama were collected from database of Shanghai Ninth People's Hospital and further analyzed.RESULTS:A total of 10 cases (8 males and 2 females) of carcinosarcoma were collected and analyzed. The age of patients ranged from 20 to 81 years (mean: 44, median:45.5). The tumor size was related to the primary site. The main clinical manifestations indicated rapid increase of foreign body sensation or mass, even feeling of pain. Pathologic examination confirmed the presence of carcinoma and sarcomatoid stroma. Carcinosarcoma had low incidence and was easily misdiagnosed. Diagnosis depended on pathologic examination especially immunohistochemistry. The prognosis was poor, and 5 year survival rate was 50%.CONCLUSIONS:Diagnosis to carcinosarcoma depends on pathological examination after operation. Complete resection + free flap transplantation + postoperative radiotherapy and chemotherapy are conventional treatment.

Abstract:Background and Purpose: Odontoma is the most common odontogenic tumor. It includes 2 types, the compound and complex odontomas. There has not been a series study of the clinical and histologic features of odontomas from Taiwan. This study evaluated the clinicopathologic features of odontoma in Taiwanese. Methods: Cases of odontoma treated from 1998 to 2002 identified from medical records were included. The microscopic features, radiographic features, and clinical history of the patients were reviewed and analyzed. Results: A total of 81 odontomas in 81 patients (36 males and 45 females) were included. There were 62 compound and 19 complex odontomas. The mean age of the patients was 18 years with the majority of odontomas occurring in the first (32%) and second decade (38%) of life. Odontomas had a marked predilection for the maxilla (70%) and for the anterior region of the jaw (83%), particularly for the anterior maxilla (62%), Sixty four (79%) of the 81 odontomas were associated with 80 impacted teeth, including 71 permanent teeth, 2 deciduous teeth, and 7 supernumerary teeth. Of the 71 impacted permanent teeth, the maxillary central incisor (27%) was-most commonly affected, followed by the maxillary canine (26%) and mandibular canine (24%). Histologic examination revealed enamel matrix in 90%, dentin in 100% cementum in 88%, pulp tissue in 96%, fibrous capsule in 93%, ghost cells in 83%, reduced enamel epithelium in 86%, and nests of odontogenic epithelium in 58% of odontomas. Dentigerous cyst was associated with 9% of odontomas. Conclusions: In this series, odontomas occurred most often in the first and second decade of life. Although complex odontomas are usually found in the posterior jaw, in this Taiwanese series they were most commonly found in the anterior maxilla. Odontoma is frequently associated-with an impacted tooth and occasionally with a dentigerous cyst. No recurrence of odontomas was found-after surgical excision with follow-up to 15 years.

Abstract:BACKGROUND Sclerosing odontogenic carcinoma is a rare primary intraosseous neoplasm that was featured recently as a single entity in the World Health Organization classification of Head and Neck Tumors 2017, with only 14 cases published to date. The biological characteristics of sclerosing odontogenic carcinoma remain indistinct because of its rarity; however, it appears to be locally aggressive, with no regional or distant metastasis reported to date.CASE SUMMARY We reported a case of sclerosing odontogenic carcinoma of the maxilla in a 62-year-old woman, who presented with an indolent right palatal swelling, which progressively increased in size over 7 years. Right subtotal maxillectomy with surgical margins of approximately 1.5 cm was performed. The patient remained disease free for 4 years following the ablation surgery. Diagnostic workups, treatment, and therapeutic outcomes were discussed.CONCLUSION More cases are needed to further characterize this entity, understand its biological behavior, and justify the treatment protocols. Resection with wide margins of approximately 1.0 to 1.5 cm is proposed, while neck dissection, post-operative radiotherapy, or chemotherapy are deemed unnecessary.

Abstract:Odontogenic carcinoma with dentinoid (OCD) is a rare and controversial entity, which has not yet been included in the current World Health Organization classification of odontogenic lesions. Owing to the small number of reported cases, the clinicopathological characteristics, biological behavior, prognosis, and appropriate treatment strategies for OCD remain to be defined. Herein, we present an additional case of OCD with a focus on the differential diagnosis and review of the pertinent literature, in order to enable better recognition by oral clinicians and pathologists and further characterization of this entity.

Abstract:Context.-Odontogenic tumor composed predominantly of clear cells is a rare neoplasm of the jaws that was initially designated as clear cell odontogenic tumor. Subsequent reports in the literature, however, all have indicated that this tumor exhibits an aggressive behavior characterized by infiltrative local growth, recurrence, or metastases.Objective.-To ascertain the clinicomorphologic features and biologic behavior of this tumor group, we present 5 additional cases using the term clear cell odontogenic carcinoma.Design.-Histologic and immunohistochemical examinations were performed on clear cell odontogenic carcinomas from 5 patients. Clinical and follow-up data were recorded, and the literature was reviewed.Results.-The patients were 1 man and 4 women with an average age of 42.4 years at diagnosis. Three tumors occurred in the mandible and the other 2 in the maxilla. Four of the 5 cases occurred in the anterior or premolar areas of the jaws, appearing as poorly marginated radio-lucencies. All cases consisted of islands and sheets of large clear cells and small basaloid cells with scanty eosinophilic cytoplasm, which were separated by thin, mature, fibrous septae. The tumors were unencapsulated and showed aggressive infiltration of the surrounding muscle and perineural tissues. Most of the clear cells contained cytoplasmic glycogen. Immunocytochemically, the tumor cells were positive for pan-keratin, cytokeratin 19, and epithelial membrane antigen, but were negative for vimentin, S100 protein, desmin, smooth muscle actin, human melanoma antigen (HMB-45), and alpha (1)-antichymotrypsin. Follow-up data showed 4 of 5 patients to have multiple local recurrences, and I of these 4 had submandibular lymph node metastasis and a fatal outcome due to uncontrollable tumor growth.Conclusion.-These results suggest that odontogenic clear cell neoplasms are at least low-grade malignancies and should be classified as carcinomas.

Abstract:Objective To make a summary of clinical and pathological features of benign cementoblastoma.Methods We reported 12 cases of benign cementoblastoma,and analyzed their clinical and pathological data,including patient ages at diagnosis,sex,occurrence,clinical presentation,radiographic and pathological features and their treatment.Results Patient ages at diagnosis ranged from 13 to 58 years,with a mean age of 33.8 years.The tumors affected 6 males and 6 females.The main site of tumors involvement was mandibular(66.7%),with retromolar area most commonly involved(91.7%).Pain(25%) and swelling(75%) were the most common symptoms.Radiographically,most cemetoblastomas exhibited a central opacity surrounded by a radiolucent halo.Microscopically,cementoblastomas were formed by proliferating cementum-like tissue attached to tooth root and exhibiting prominent reversal lines.A regime of tumor resection and affected tooth extraction can get good therapeutic result.Conclusion Benign cementoblastoma occurs more commonly in youth and middle aged patients,with premolar-molar region of jaw frequently involved;usually it presents with pain or expansion of jaw;X ray and histopathological features combined can distinguish it from osteoblastoma,cemento-ossifying fibroma and atypical osteosarcoma.

Abstract:Objective To provide reference for future clinical diagnosis and treatment of carcinoma of the floor of mouth by analyzing its clinical characteristics,treatment and prognosis.Methods 34 patients with carcinoma of the floor of mouth were investigated retrospectively.All patients were treated in our hospital between 1992 and 2002.Results A total of 34 cases(28 males,6 females,averaged 58 years,the ratio of male to female was 4.7∶1) were included in this study.The tumors were located in anterior(22 cases,64.7%)and posterior(12 cases,35.3%) floor of the mouth,respectively.The rate of lymph node metastasis was 41.2%.All patients were treated with surgery,chemo-therapy and surgery,radio-therapy and surgery and chemo-therapy and radio-therapy and surgery,respectively,and the 5-year-survival rates were 45.5%,60.0%,50.0% and 62.5%.Conclusion The carcinoma of the floor of mouth often happened in elderly people,especially in males.The lymph node metastases happened regularly.The combination therapy is better in the treatment of carcinoma of the floor of mouth.

Radiological findings of SOC have been characterized in detail, and an imaging classification scheme has been proposed to further discuss the diversity of radiographic features. Due to the rarity of the disease, a comprehensive understanding of SOC is needed, and close collaboration between clinicians, radiologists, and pathologists is crucial to avoid misdiagnosis.

Abstract:BackgroundAmeloblastic carcinoma (AC) is an odontogenic malignant tumor which is closely related to benign ameloblastoma. Because of its rarity, diagnosis and treatment are difficult. In this study, we summarized and analyzed the clinical and biological characteristics of AC.ResultsFifteen patients with AC and a median age of 53 years were identified. Among of them, five patients who were tested carried a BRAF-V600E mutation. Two patients presented with cervical lymph nodes and lung metastases. Primary AC was more invasive, and the bone destruction ability of the primary type was more radical than that of the secondary type.ConclusionsThis study revealed that the BRAF-V600E mutation was related to the aggressive behavior of AC, and early radical resection is crucial. Moreover, targeted therapy may be a new direction in the future.

Abstract:The oral cavity stands as one of the pivotal interfaces facilitating the intricate interaction between the human body and the external environment. The impact of diverse oral microorganisms on the emergence and progression of various systemic cancers, typified by oral cancer, has garnered increasing attention. The potential pathogenicity of oral bacteria, notably the anaerobic Porphyromonas gingivalis and Fusobacterium nucleatum , has been extensively studied and exhibits obvious correlation with different carcinoma types. Furthermore, oral fungi and viruses are closely linked to oropharyngeal carcinoma. Multiple potential mechanisms of oral microbiota-induced carcinogenesis have been investigated, including heightened inflammatory responses, suppression of the host immune system, influence on the tumor microenvironment, anti-apoptotic activity, and promotion of malignant transformation. The disturbance of microbial equilibrium and the migration of oral microbiota play a pivotal role in facilitating oncogenic functions. This review aims to comprehensively outline the pathogenic mechanisms by which oral microbiota participate in carcinogenesis. Additionally, this review delves into their potential applications in cancer prevention, screening, and treatment. It proves to be a valuable resource for researchers investigating the intricate connection between oral microbiota and systemic cancers.

Abstract:BACKGROUND AND OBJECTIVE:Treatment of pulmonary carcinoid is a comprehensive modality focusing on surgery. Thus, accurate and timely preoperative and intraoperative pathological diagnoses are very crucial. The aim of this study is to retrospectively analyze the pathological characteristics of pulmonary carcinoid, its natural history, and patterns of disease progression to obtain evidence for clinical decision making.METHODS:Clinical and pathological data of 32 patients with pulmonary carcinoid were retrieved and retrospectively analyzed. The pathological characteristics and their relationship with clinical diagnosis and treatment effect were systematically studied.RESULTS:Among the 32 patients, 18 had typical carcinoid and 14 had atypical carcinoid. The male-to-female ratio was 2.2:1, and the average age was (44±15) years. Almost half of the patients were discovered by accident. The average maximum diameter was (3.1±1.3) cm. About 27 of all 32 cases (84.4%) were stage I, 2 were stage IIa, 2 were stage IIIa, and 1 was stage IV. The follow-up time was 5.2-9.7 years. The 5-year progression free survival of 15 typical carcinoid patients and 12 atypical carcinoid patients were 100% and 92.9%, respectively. The characteristic neuroendocrine morphology and variable expression levels of cytokeratin, chromogranin A, synaptophysin, CD56, and index of Ki-67 were important factors that differentiated pulmonary carcinoid from other tumors.CONCLUSIONS:Pathological diagnosis of pulmonary carcinoid must combine morphology with immunohistochemistry. Early-stage patients can also achieve good effect after surgery.

Abstract:Microorganisms in the oral cavity are abundant in the human body. At present, more than 700 species of oral microorganisms have been identified. Recently, a lot of literature has indicated that the oral microbiota plays an important role in the occurrence, development, and prognosis of oral squamous cell carcinoma (OSCC) through various mechanisms. And researchers are now trying to utilize oral microbiota in cancer diagnosis and treatment. However, few articles systematically summarize the effects of oral microbes in the diagnosis, treatment, and disease outcomes of oral cancer. Herein, we made a summary of the microbial changes at cancerous sites and placed more emphasis on the mechanisms by which the oral microbiome promotes cancerization. Moreover, we aimed to find out the clinical value of the oral microbiome in OSCC.
