Radical nephrectomy for renal cell carcinoma: Experience on 166 patients

Abstract:Objective This study was to report our experience on RN in patients with renal cell carcinoma.Methods Clinical data,including the duration of operation,the amount of bleeding during operation,the duration of hospitalization,complications of operation and survival rates of 166 patients with histologically confirmed RCC after RN were reviewed.Results No patient died during surgery.The duration of operation ranged from 60 minutes to 360 minutes,with a median of 150 minutes.The amount of bleeding ranged from 50ml to 10000ml,with a median of 200ml.16 patients received transfusion with a mean of 11.5 units of RBC(ranged from 1 unit to 75 units).In 24 patients(14.5%)the amount of bleeding was over 500ml,including 6 patients underwent thrombectomy for tumor thrombus in the inferior vena cava,with bleeding ranged from 100 ml to 6000 ml(median:1100 ml).The duration of hospitalization ranged from 6 days to 46 days,with a median of 15 days.The intra-operative complications including spleen injuries in 2 patients,inferior vena cava injury and duodenum injury in 1 patient,and vessel injuries in 2 patients,were treated successfully intra-operatively without consequlence sequelae.The postoperative complications including heart failure in one patient and incomplete ileus in 2 patients were cured conservatively.Mean follow-up was 23.6 months(range 6 month to 78.6 months).The 1-year,3-year and 5-year overall survival rate of patients was 93.6%,84.1% and 74.8%,respectively.The 1-year overall survival rate of patients with stage Ⅰ,stage Ⅱ,stage Ⅲ and stage Ⅳ disease was 100%,100%,88.2% and 61.5%,respectively.The 3-year overall survival rate were 97.0%,95.5%,75.6% and 52.7%,respectively.The 5-year overall survival rates of patients with stage Ⅰ,stage Ⅱ,stage Ⅳdisease were 88.8%,86.8% and 0%.Conclusion RN is effective for treatment of RCC with accepted complications.The outcome after RN for patients with stageⅠandⅡ disease is optimic.
