Abstract:This paper analyzed the development of new rural cooperative medical scheme in Shandong and Ningxia Hui autonomous Region.The results shows that low financing,unreasonable payment systems,poor of management and rapidly growth of medical expenditure were found in this study.Some comments such as increasing financial level,improving payment systems reasonable and timely,training management staffs and alleviating the rapid growth of medical expenditure were introduced to policy makers.

Abstract:To thoroughly understand the New Rural Cooperative Medical System(NRCMS)in Rural China,this article systematically reviewed the NRCMS policies from 2003 to 2012 at the national level,and then analyzed these policies in terms of participation level,financing policy,reimbursement policy,fund management and expense control strategy.This article proposed that the focus of NRCMS polices had changed from participation rate and financing level,to reimbursement plan and fund management,and now to expense control strategy.Evolution analysis could help to understand and develop the NRCMS policies.【

Abstract:By analyzing the database of new type rural cooperative medical system(RCMS) and a household study conducted in 7 counties from 4 provinces,the paper introduces the achievements of RCMS in China.At present,RCMS is facing new challenge,including to exploit a sustainable financing mechanism;to pay attention on the potential financial risk;to adjust and improve the design of RCMS and inform a scientific and standardized compensation scheme;in some areas,the commercial insurance companies have been involved in the financial management of RCMS,it should be further standardization.

Abstract:Expatiated seven knobs in scheme frame of New Rural Cooperative Medical System (NRCMS) and primary patterns from the point of view of institutional economics. In virtue of ROCCIPI technique method, systematically dissertated implemental effect and existing problems of NRCMS in bygone six years from such three dimensionalities as government support, rural residents and medical service providers. Based on the above study, put forward some key strategies in order to insure sustainable development of NRCMS in China, including clarifying financing responsibilities and division mechanism of all level governments, upgrading management techniques and faculty of grass-roots government, cutting down adverse selection by compulsive or semi-compulsive participation, advancing security level by increasing personal financing standard of rural residents, giving equal attention to beneficial extension and beneficial degree of security patterns by compensating out-patient medical services, strengthening prompting and restrict of selective medical service providers to reduce moral hazards, and boosting up rural residents’ trust by cultivating social capital to come into being spontaneously mutual cooperation.
The Impact of Expanded Health System Reform on Governmental Contributions and Individual Copayments in the New Chinese Rural Cooperative Medical System

Abstract:In 2002, the Chinese central government created a new rural cooperative medical system (NCMS), ensuring that both central and local governments partner with rural residents to reduce their copayments, thus making healthcare more affordable. Yet, significant gaps in health status and healthcare utilization persisted between urban and rural communities. Therefore, in 2009, healthcare reform was expanded, with (i) increased government financing and (ii) sharply reduced individual copayments for outpatient and inpatient care. Analyzing data from China's Ministry of Health, the Rural Cooperative Information Network, and Statistical Yearbooks, our findings suggest that healthcare reform has reached its preliminary objectives-government financing has grown significantly in most rural provinces, especially those in poorer western and central China, and copayments in most rural provinces have been reduced. Significant intraprovincial inequality of support remains. The central government contributes more money for poor provinces than for rich ones; however, NCMS schemes operate at the county level, which vary significantly in their level of economic development and per capital gross domestic products (GDP) within a province. Data reveal that the compensation ratios for both outpatient and inpatient care are not adjusted to compensate for a rural county's level of economic development or per capita GDP. Consequently, a greater financial burden for healthcare persists among persons in the poorest rural regions. A recommendation for next step in healthcare reform is to pool resources at prefectural/municipal level and also adjust central government contributions according to the GDP level at prefectural/municipal level.

Abstract:This paper analyzed the medical saving account,a part of new rural medical cooperative scheme funding.It had found that this part of funding play an important role on enrollment,encouraging the enrollees to use outpatient services and strengthening new rural cooperative medical scheme.Following with their further develop- ment,the disadvantages such as low financing level,higher management cost,and absence in longitudinal accumula- tion emerged.Finally,some suggestions were proposed to the policymakers.

Abstract:This paper analyzed on the benefits that the participants gained to illustrate the development of the new rural cooperative medical scheme of Qianjiang County. The results showed that the scheme did well in the coverage and significance of the benefits and also promoted the utilization of the health services. The reimbursement did help the residents,especially the poor,reduce burden of diseases. The scheme is suggested to attract more participants,and meanwhile,its ways of controlling medical fees,its out-patient reimbursement mode characteristic of low level and wide coverage and its in-patient reimbursement mode more beneficial to the poor can be used as reference.

Abstract:Based on survey data from six counties/districts in the Eastern China, this study examines factors associated with the participation, satisfaction, and sustainability of the new cooperative medical scheme (NCMS) by conducting a two-level multivariate analysis. The results show that gender, socioeconomic status, informed knowledge about the policy, demand, subjectively expected premium contribution, and co-payment rate are significantly associated with participation. Satisfaction with overall performance of the NCMS is significantly determined by age, knowledge about the policy, costs of medical services, satisfaction with reimbursement, and subjectively expected co-payment rate. Sustainability measured by subjective continuity of use is significantly determined by knowledge about the policy of the NCMS and satisfaction with the overall performance of the NCMS. Challenges and policy implications are also discussed.

Abstract:Using a panel data from on-the-spot survey in China's rural area,this paper aims to understand the progress of the implementation of New Cooperative Medical System(NCMS) and the challenges it faces at present.The results show that while the NCMS program performed very well in coverage,enrollment,and fund raising,farmers still knows little about the reimbursement policy and the program nature.The paper also finds that the programme could not meet its goal of providing financial risk protection against catastrophic illnesses.In addition,it also indicates that the behaviour of medical service providers may weaken the effect of NCMS because of inappropriate incentives.The policy recommendation is proposed basing on the findings.

Abstract:Based on field study in two counties in Heilongjiang province,this paper analyzed two issues about the implementation of new Rural Cooperative Medical System(RCMS). First,RCMS has adversely affected the local medical service system. The health agencies in charge of RCMS may take advantage of it to protect their subsidiary medical institutions,and the two-way referral provisions in the current difficult to implement in rural areas. Second,the medical savings accounts referenced by many countries is not as propagandized to reimbursement minor ailments medical expenses,but to reduce the actual admission cost and keep cash flow of RCMS. These issues need to be resolved in future for strengthening RCMS.

Abstract:Objective:To test the scientificalness and operability of the ideas and methods of developing reimbursement plans.Methods:This article took data of hospitalization reimbursement from sampling areas in 2011 as an example and used the new rural cooperative medical decision-making platform as a tool to develop the plans.Results:This study took welfare-risk plans as examples.(1)Totally 41 639 600 yuan should be funded to eliminate medical impoverishment risks at 80% ratio of reimbursement;(2)The study set 600 yuan as deductible that corresponds to the average risk of the participants.It cut the medical costs into four segments(600~,3 000~,10 000~,22 000~)and set ratio of reimbursement respectively at terms of the probability of hospitalization and the distribution of financial risk of the hospitalized group;(3)Based on the above reimbursement plans,the goal of balance of medical insurance fund with 605 300 yuan of capital funds was achieved.Conclusion:The ideas and methods of the development of reimbursement plans are scientific and operative.

Abstract:In recent years, the central government in China has been leading the re-establishment of its rural health insurance system, but local government institutions have considerable flexibility in the specific design and management of schemes. Maintaining a reasonable balance of funds is critical to ensure that the schemes are sustainable and effective in offering financial protection to members. This paper explores the financial management of the NCMS in China through a case study of the balance of funds and the factors influencing this, in six counties in two Chinese provinces. The main data source is NCMS management data from each county from 2003 to 2005, supplemented by: a household questionnaire survey, qualitative interviews and focus group discussions with all local stakeholders and policy document analysis. The study found that five out of six counties held a large fund surplus, whilst enrolees obtained only partial financial protection. However, in one county greater risk pooling for enrolees was accompanied by relatively high utilisation levels, resulting in a fund deficit. The opportunities to sustainably increase the financial protection offered to NCMS enrolees are limited by the financial pressures on local government, specific political incentives and low technical capacities at the county level and below. Our analysis suggests that in the short term, efforts should be made to improve the management of the current NCMS design, which should be supported through capacity building for NCMS offices. However, further medium-term initiatives may be required including changes to the design of the schemes. Copyright (C) 2009 John Wiley & Sons, Ltd.

Abstract:Through a literature review of about 2 010 papers from 2003 to 2012,the difficulties during the implementation of New Rural Cooperative Medical System(NRCMS)were summarized as follows :(1)The financing level was low and lacked a long-term steady growth mechanism of financing,and the county-based risk pool was too small.(2)Only a minority of rural residents have been benefited from NRCMS due to the unreasonable reimbursement policies.(3)Rural residents knew little about the operation of the funding and the management cost was high.(4)The medical expenses of NRCMS grew rapidly and there were no effective measures to control cost.(5)The organization management,resource and system guarantee of NRCMS were inadequate.The problems mentioned above all pointed to the urgent need to solve the technical difficulties in implementing NRCMS.Therefore,two issues should be addressed : one is the methodology of developing,evaluating and amending the schemes,the other is the strategies on medical expenses control.

Abstract:This article mainly discussed three issues:the development of New Rural Cooperative Medical Scheme(NRCMS)under the background of China's rapid industrialization,the choice of NRCMS's management system as well as the evaluation of NRCMS's social effects.Through theoretical and empirical analysis,this paper suggested that NRCMS should adapt to and contribute to the development and changes of society and economy in the future.The author also holds that the Ministry of Health has advantages in management of NRCMS.Experimental results indicate that NRCMS has made significantly positive effect on health and education.

Abstract:Objective To analyze the major influencing factors of satisfaction degree in new cooperative medical scheme(NCMS).Methods Logistic regression analysis and other statistical methods were taken for analysis of social economy conditions of farmers in Beijing rural area in 2007.Results More that 90% of the farmers had a desire to join NCMS.The farmers' satisfaction degree to NCMS was correlated with the satisfaction of the government propaganda,family income,knowing degree of reimbursement,transparency of the policy and way to pay the fee(P<0.05).Conclusion The influencing factors of farmers' satisfaction to NCMS are their satisfaction degree to government propaganda,family income,knowing degree of reimbursement,transparency of the policy and way to pay the fee.The propaganda on NCMS should be enhanced and the guaranteeing plane of completion and optimization should be open,thus to meet demands of medical service in different levels.
