Comparison of Effects of Three Kinds of Radiation Therapy Methods in the Treatment of Esophageal Carcinoma

Abstract:Objective To compare the effects of conventional fractionation radiation therapy,three-dimensional conformal radiation therapy(3DCRT) and intensity modulated radiation therapy(IMRT) in the treatment of esophageal carcinoma. Methods Among the 117 patients with esophageal carcinoma,38 patients were treated by conventional fractionation radiation therapy,32 patients were treated by 3DCRT,and 47 patients were treated by IMRT.The effects and toxicities of the three groups were compared retrospectively. Results The effective rates in the conventional fractionation radiation therapy group,the 3DCRT group and the IMRT group were 92.1%,96.9%,91.5%,respectively(P=0.617).The 1-year survival rates of the three groups were 77.9%,87.5%,86.7%,respectively(P=0.193);the 2-year survival rates of the three groups were 38.6%,55.1%,57.7%,respectively(P=0.211).The pain relief rate in the 3DCRT group + the IMRT group was 90.9%,and 84.6% in the conventional fractionation radiation therapy group(P=0.642).The incidence of acute radiation esophagitis in the 3DCRT group + the IMRT group was higher than that in the conventional fractionation radiation therapy group(P=0.012).The incidences of acute radiation pneumonia,radiation bronchitis and leukopenia in the 3DCRT group + the IMRT group were lower than those in the conventional fractionation radiation therapy group(P<0.001,P=0.028,P=0.010). Conclusion IMRT and 3DCRT groups can improve the clinical therapeutic effects in the treatment of esophageal carcinoma,and can lighten radiation injury.
