Abstract:The american's health care system is the most complicated system around the world.It is criticized for its high cost,low return,lack of impartiality and etc.Hence the US government and insurance organizations carried on series of microinnovations and macro-reforms,such as managed care and DRG,aiming at extending coverage and reducing expenditure.We can learn from some its experience which will benefit our Chinese Health Care System reform progress.

Abstract:Medical care system is highly privatized both in US and India, which makes them a good example on China's way of proper privatization of medical care system in the future. Through clarification comparison, this research discovered three differences between US and India privatization of medical care system: (1)responsibility of government: the goal of India government is universal health coverage at a very basic level while US government provides disadvantaged groups with relatively good health-care services. (2)structure of privatized medical care system: profitable, non-profitable private and public medical care institutions are equally important while the profitable private medical care institutions are the most important and non-profit private institutions are much less important. (3)desire of cooperation between public and private sector: India government shows strong intention in cooperating with private sector while US government shows much less.

Abstract:It is now about 15 years since the introduction of the market into health care in China. This produced fundamental changes in the way that health care is financed and resulted in the disappearance of universal free basic health care. Responsibility for provision of health services has been devolved to the provincial and county governments, and healthcare providers have been given considerable financial independence. A fee for service system has been introduced, and several different payment mechanisms are now in operation. The new financing and pricing structures are responsible for greater inequity of access to services and more inefficient use of resources. These problems are widely acknowledged, and a range of solutions is being developed and tested. Since the introduction of the reforms the measurable health status of the population has not declined, probably as a result of overall improved socioeconomic conditions and a continued emphasis on prevention.

Abstract:Akin to the ongoing scenario in China,developed countries in the Occident have witnessed sharp increases of the medical expenses borne by their citizens.This scenario was not fundamentally turned around until management-based medical organizations emerged and evolved positively.In fact,management-based medical organizations have played an active role in curtailing medical expenses by means of creating economic incentive factors,and encouraging medical organs to take stricter control of their utilization of medical resources,as well as overseeing the provision of medical service directly.In today's China,it is advised to either embark on the management-based medicare system and strike up medicare cooperation into depth,or usher in the advance payment system,thus to prevent medical expenses from rising up dramatically in the country,and also ensure the general public to have access to medical service.

Abstract:The article introduced health systems of UK and USA,which were positioned at two extremes of the health system spectrum,the former run by an omnipotent government and the latter led by market. Recently,they have both passed revolutionary healthcare reform acts. The ideology of UK health reform is to devolve powers,become professional and realize de-administration,while that of USA is to strengthen government regulation on the basis of privatization acting as the main stream and enlarge the insurance coverage so as to ensure the equity of healthcare. These two different reforms lead them to a multi-cooperative health system led by government. The enlightenment to China was also discussed.

Abstract:At present, China’s 5.6% GDP for health care has not reached to the point that is beyond the bearing ability of the national economy. However, why the overly expensive health care expenditures have been complained by the public throughout the country? The paper thought that lacking of risk pooling of high medical expenditure, which resulted from the incomplete health insurance, brought forth the overly expensive health care. How to deal with the relationship between the market and the government, increase the government’s capital collecting during financing system reform in order to ensure stability and fairness, widen the cover of health insurance, enhance the risk pooling, and advance the medical expenditure control method should be a key during health insurance reform.

Abstract:Health needs a supporting safeguard mechanism. The U.S. insurance industry began to pay attention to the health risk since the 20th century,and raised the concept of health management in 1978. Up till now,its health care system and framework have been working well. Though American commercial insurance takes the responsibility of social security in other countries,the U.S. government is still playing an important role in health care sector in America, especially in legislating,total planning,maintaining social justice,providing tax preference,financing,and cooperating with non-government organizations and government procurement. Abstract:Many countries are importing managed care and price competition from the US to improve the performance of their health care systems. However, relatively little is known about how power is organized and exercised in the US health care system to control costs, improve quality and achieve other objectives. To close this knowledge gap, we applied social exchange theory to examine the power relations between purchasers, managed care organizations, providers and patients in the US health care system at three interrelated levels: (1) exchanges between purchasers and managed care organizations (MCOs); (2) exchanges between MCOs and physicians; and (3) exchanges between physicians and patients. The theory and evidence indicated that imbalanced exchange, or dependence, at all levels prompts behavior to move the exchange toward power balance. Collective action is a common strategy at all levels for reducing dependence and therefore, increasing power in exchange relations. The theoretical and research implications of exchange theory for the comparative study of health care systems are discussed.

Abstract:Objective: To review managed care's current cost management trends and the consumerism movement; to elucidate the pros and cons of key issues; and to describe the philosophy of focusing on the patient, also called patient-centric care, while improving the patient's care through value-based purchasing and plan design. Summary: Managed care is sometimes practiced using a silo approach with little concern for the consumer. In this model, medical and pharmaceutical issues are addressed in silos, and value is narrowly defined. Increasingly, cost and responsibility is shared with or shifted to the patients. Patients may be unable or unwilling to assume these costs or responsibilities. Several studies have demonstrated that they may react with noncompliance. Managed care's definition of value must expand and integrate across silos to consider the needs and interests of the patient's overall care, in particular, addressing key cost drivers in terms of diseases that cause recurring costs. Using predictive modeling can result in cost savings. A case study (Pitney Bowes) is included in this article.

Abstract:As the mainstream public health insurance program,Medicaid program is aimed at reimbursing the medical expenses of families with limited income and resources.From the implementation in 1965,the Medicaid has kept growing in a comparatively rapid speed in terms of its coverage rate and the amount of funds,which relieved the medical burden of people with low and middle income effectively.With many years' development,Medicaid program has formed relatively mature mechanism of fund-raising and reimbursement,which may shed some light upon the NCMS in China.

Abstract:China's current strategy to improve how health services are paid for is headed in the right direction, but much more remains to be done. The problems to be resolved, reflecting the setbacks of recent decades, are substantial: high levels of out-of-pocket payments and cost escalation, stalled progress in providing adequate health insurance for all, widespread inefficiencies in health facilities, uneven quality, extensive inequality, and perverse incentives for hospitals and doctors. China's leadership is taking bold steps to accelerate improvement, including increasing government spending on health and committing to reaching 100% insurance coverage by 2010. China's efforts are part of a worldwide transformation in the financing of health care that will dominate global health in the 21st century. The prospects that China will complete this transformation successfully in the next two decades are good, although success is not guaranteed. The real test, as other countries have experienced, will come when tougher reforms have to be introduced.

Abstract:Since the Basic Medical Insurance is to cover all citizens and farmers,China's Basic Medical Insurance Agencies should become positive and powerful purchasers of medical services.However up till now they are still "negative payers of medical services" with poor functions far from the real-sense purchasers,which have failed to prevent the medical service agencies from abusing their professional advantages thus have wakened the goal of medical socialization.Therefore,it is necessary to learn from the "Managed Medicare" experience from the United States in order to define China's Basic Medical Insurance Agencies as purchasers of medical services and to carry out the reform for public welfare.The functions of the Basic Medical Insurance Agencies should be converted to such aspects as selecting qualified hospitals,participating in setting up standard process for medical treatment and development of Diagnosis-Related Group(DRG),analyzing and evaluating the feedback information from patients,controlling improper medical cost and supervising the quality of medical service.
