Abstract:The reform for medical insurance payment was the important method for standardizing medical service provider behavior,controlling costs and improving efficiency,which was also the important measurement for further improve and implement the reform of drug and health system.Since the new medical reform in 2009,the central and local government had paid more attention on the role of medical insurance payment reform.Local medical departments of different places implemented reform pilots.Through summarizing the foreign and domestic researches around capitation,bundle payment and global payment,the experiences and effectiveness of these pilots were evaluated so as to provide corresponding policy implications.

Abstract:Objective To investigate the outpatient non-payment situation of hospital medical insurance costs in order to provide solutions to effectively reduce medical insurance non-payment.Methods The outpatient non-payment situation of medical insurance costs in the recent 13 months in our hospital had been analyzed.Results The amount of outpatient nonpayment of medical insurance in the 13 months was 110 176.31 yuan for 3 072 cases,of which 97.53% were caused by clinicians' violation of medical insurance,medical affairs and drug administration regulations.The amount and person-time of non-payment obviously reduced after strengthening regulations.Conclusion There are various causes of non-payment of medical insurance and therefore medical insurance management department needs to apply modern information to strengthen the links with clinicians and the insured in order to play a better role in regulation and services.

Abstract:This article elaborated the connotation of public hospital medical insurance payment system and the importance of reform,summarizing and analyzing the practical exploration of promoting medical insurance payment system reform in the localities.Then it moved on to introduce new progress of medical insurance payment system reform abroad and the emerging mode of medical insurance payment, such as pay for performance,payment by results,and bundled payment.In the end,the authors put forward policy suggestions to improve medical insurance payment system at public hospitals in China, namely to build a modern healthcare payment system in line with the needs of medical service system.Such a system should be guided by comprehensive performance,restrained by cost budgeting, based on a diversified payment mode,and supported by information technology.In addition,it should have scientific payment standard and modern governance mechanism,and keep interactive development with commercial health insurance.

Abstract:Akin to the ongoing scenario in China,developed countries in the Occident have witnessed sharp increases of the medical expenses borne by their citizens.This scenario was not fundamentally turned around until management-based medical organizations emerged and evolved positively.In fact,management-based medical organizations have played an active role in curtailing medical expenses by means of creating economic incentive factors,and encouraging medical organs to take stricter control of their utilization of medical resources,as well as overseeing the provision of medical service directly.In today's China,it is advised to either embark on the management-based medicare system and strike up medicare cooperation into depth,or usher in the advance payment system,thus to prevent medical expenses from rising up dramatically in the country,and also ensure the general public to have access to medical service.

Abstract:To demonstrate the effect of "Global budget and Service unit" on health reform in China,qualitative analysis of the payment and quantitative survey of intention were used.The paper discovered that with the mixed payment,6 main expected effects were to bring about,such as the feeling of " Seeing a doctor is expensive" being eliminated and benefiting the public truly,the incomplete reimbursement mechanism of hospitals being improved,eliminating the problems of production,distribution,purchasing,using to maintain good order in pharmaceutical market, keeping the growth of medical expenditure in line with the social economic development,providing a balanced environment between financing and expenditures for insurance fund,and the Government' s easy regulating and operating. In a word,under the mixed payment," common people are benefited,health care providers are encouraged and regulators can conduct easily",which Premier Wen put forward clearly could be realized.Of course,there were some new and unavoidable problems.Since these problems were to be resolved by corresponding measures,they were so insignificant.

Abstract:Objectives To analysis the situation of the modes of the medical insurance payment of mental health institutions,and to provide the empirical basis for the formulation of relevant policies.Methods Investigating nine mental health institutions of three provinces in China by typical sampling method.Collecting relevant data of medical insurance of the hospitals and conducting in-depth interviews with the hospital managers,staves of department of medical insurance and doctors and nurses.Results The modes of payment of the major medical insurance included fee for service,global budget,the per-diem payment,capitation and etc.Different payment modes had its own advantages and disadvantages.Conclusions Medical insurance payment should be transformed from a single way to the compound mode,and the global budget standards based on the features of mental health institutions should be established scientifically and reasonably.
