Abstract:To demonstrate the effect of "Global budget and Service unit" on health reform in China,qualitative analysis of the payment and quantitative survey of intention were used.The paper discovered that with the mixed payment,6 main expected effects were to bring about,such as the feeling of " Seeing a doctor is expensive" being eliminated and benefiting the public truly,the incomplete reimbursement mechanism of hospitals being improved,eliminating the problems of production,distribution,purchasing,using to maintain good order in pharmaceutical market, keeping the growth of medical expenditure in line with the social economic development,providing a balanced environment between financing and expenditures for insurance fund,and the Government' s easy regulating and operating. In a word,under the mixed payment," common people are benefited,health care providers are encouraged and regulators can conduct easily",which Premier Wen put forward clearly could be realized.Of course,there were some new and unavoidable problems.Since these problems were to be resolved by corresponding measures,they were so insignificant.

Abstract:Based on a description of the evolution,main reform practices and effects of public hospital payment system reforms,this paper focused on analyzing the existing problems found in the reforms.Such problems namely include lack of scientific and reasonable design of payment standards,lack of systemic and coordinated system reforms,and failure to fully mobilize the enthusiasm of medical workers to participate in the reforms,as well as other underlying causes.Based on such suggestions were put forward to promote public hospital payment system reforms,as negotiation of the payment standard between purchasers and providers,"combined approaches" in innovation in payment methods,advancing systemic payment system reform,and establishing a transmission mechanism of incentive to the medical personnel.

Abstract:Medical insurance payment model is transforming from project-based purchases to service bundle-based strategic purchases. The new form of bundled purchases should found on a scientifically-led design process of such bundles. The core to bundled purchase would be the payment standard, and the key to its success would be process control. Establishment of such a foundation, a core, and a key, would promote the current price standards, and lead service providers to a standardized medical service standard, so as to ensure a precise rewarding system of payment and service. The big data diagnosis-intervention packet(DIP)is able to fulfill mentioned ambitions by integrating insurance payment and supervision into one management. DIP is a full-process payment mode that encompasses pre-service estimation, in-service process control, post-service grading, and resource allocation. It is an innovative practice in line with China′s national conditions for the modern governance of medical security and medical services.

Abstract:Objective To analyze the main practices of capitation payment system reform in the case areas and put forward enlightenments and suggestions in this regard. Methods The implementation practices of the case areas were summarized, and descriptive statistical analysis was carried out on the implementation effects. Results By analyzing the effectiveness of the case areas′reform, it was found that the case areas are curbing the excessive growth of medical expenses (for example, outpatient fees per visit of Dingyuan county-level hospitals decreased from 245.11 yuan in 2015 to 218.40 yuan in 2017), increasing the actual compensation ratio of residents ( for example, the actual compensation ratio of Funan increased from 59.80% in 2015 to 63.28% in 2017), forming a medical treatment pattern within the county (for example, out-of-county compensation ratio in Dingyuan decreased from 37.38% in 2015 to 31.13% in 2017), achieving double-way referrals (for example, the number of referrals to superior hospitals of Jimo increased from 98 in 2015 to 328 in 2017), improving the subsidence of quality services, and controlling the risks of medical insurance funds. Conclusions At present, the reform of the case areas has been implemented steadily and achieved results. It is recommended to further improve such aspects as reform coordination, insurance standard setting, incentive mechanism establishment, and leadership to ensure the reform progress.

Abstract:To prove that the global budget under the "fee for service" is the efficient approach to realize the breakthrough of health reform,based on 30 years' database,the paper takes the policy of "Global budget,Structure adjustment" as an example.After analysing the case from portrait to landscape orientation,the paper finds that the achievement of the policy is prominent:the increasing rate of medical expenditure in Shanghai declined from 32.8%to 12.5%during policy implementation period,which was below the rate of GDP.216.2 billion yuan were economized in 10 years,so the medical burden was lighter than the whole nation and other cities such as Beijing.The result clues on:the global budget is crucial,but risk of high medical expenditure caused by fee for service and the irrational price criterion block the reform.In a word,global budget will be efficient combined by changing the post -payment to prepayment and stably increasing mechanism.

Abstract:Medical service pricing system is the basic system of health economics, which has a profound impact on physicians′ medical behavior and the running mode of hospital. Rui′an City, Wenzhou City, Zhejiang Province, freed up the cost space of drug and consumables based on the reasonable diagnosis and treatment behavior of doctors and selection of drug and consumables in the hospital procurement link. 68% of the free space was used to improve the price of medical services. 248 medical service items with obvious cost inversion were selected, such as nursing, traditional Chinese medicine, diagnosis, etc., and the price adjustment range was determined according to the agreed free space limit. Meanwhile, the credit mechanism of Alipay was applied to synchronously implement " vacating space and adjusting structure" , so as to solve the game problem between medical treatment and medical insurance, broaden the path of the " translation compensation method" of medical service price reform, and produce policy superposition and linkage effect with the reform of medical insurance payment mode.

Abstract:Objective:To analyze the current situation of the differential payment policies of health insurance,provide reference to further improve diagnosis of classification and corresponding economic leverage stimulus policy.Methods:Using the official literature review and comprehensive evaluation method,we summarized the related policy of differential payment of health insurance in 32 provinces of our country (including the four municipalities,five autonomous regions).Taking Zhejiang Province as an exampie,we focused on the analysis of the concrete policies of the 11 cities in the province and made a comparative analysis of the differences between the cities.Results:Some provinces and cities had differentiated payment policy of health insurance before the hierarchical diagnosis and treatment policy but the implementation effect was not good.In terms of comprehensive scoring,Zhejiang,Shanghai,Jiangsu,Qinghai had the higher scores (5 points),Chongqing,Hainan,Hunan,Guangxi,Inner Mongolia had the lower scores (2 points),and the policies were not found in Guizhou and Tibet.Although 11 cities in Zhejiang Province had perfected differentiation health insurance payments,there were big difference among them.Conclusion:The implementation depth of differentiation health insurance payments policies was different and the health "difference" payment standard setting was relatively chaotic.In top level design,we should pay attention to the evaluation of the difference effect,and establish the incentive mechanism of the large hospital under the difference of health insurance,promote and establish the policy for the differential payment of chronic diseases.

Abstract:OBJECTIVE: To provide information about the reforming basic insurance drug list in China, especially in Shanghai for reference.METHODS: To introduce development of drug classified management in China and compare different experiences in the management of OTC drugs' reimbursement in Sweden, Germany, Netherlands and Fenland .RESULTS CONCLUSION: The inclusion criteria of OTC drugs' reimbursement must be identified.OTC drugs within the insurance drug list should be equated with Rx drugs and those out of the insurance drug list should be fixed with favourable prices.

Abstract:Objective To investigate the outpatient non-payment situation of hospital medical insurance costs in order to provide solutions to effectively reduce medical insurance non-payment.Methods The outpatient non-payment situation of medical insurance costs in the recent 13 months in our hospital had been analyzed.Results The amount of outpatient nonpayment of medical insurance in the 13 months was 110 176.31 yuan for 3 072 cases,of which 97.53% were caused by clinicians' violation of medical insurance,medical affairs and drug administration regulations.The amount and person-time of non-payment obviously reduced after strengthening regulations.Conclusion There are various causes of non-payment of medical insurance and therefore medical insurance management department needs to apply modern information to strengthen the links with clinicians and the insured in order to play a better role in regulation and services.

Abstract:The paper presented the payment system theory of the primary health care service in China,the current status of the health service system,and analyzed main challenges for the time being.Based on such studies,the authors made the following policy proposals.The first is adjustment of the fiscal payment method of the government to such institutions in line with the classification guidance principle; the second is to shift the payment method of primary public health services to the post payment of fee-for-service; the third is reform of the performance-based salary system to link payment to medical workers directly with the amount and quality of their services; the fourth is a set of result-oriented performance appraisal indicators,with rising proportion of performance pay; the fifth is to integrate the outpatient clinic fund covered by medical insurance and the primary public health service fund,into a capitation payment.
Effects and problems in the global budget management of health insurance in Shanghai

Abstract:Since 2002,the global budget management of health insurance in Shanghai has gone through three stages: exploration,development,and maturity.The main practices include global budget,prepaid by month,postponement of payment by categories,settling accounts by season,circulating information by season,half-yearly evaluation,year-end liquidation,assessment and cost overruns sharing,and the implementation of"giving four items publicity"and "three rounds of negotiation."The reforms have controlled medical expenses,encouraged hospital selfmanagement,created a good environment for hospital reform,and strengthened the monitoring mechanism.However,there are still some problems with reform,including lack of standardization in the internal management of medical institutions,contradictions between cost control and health service,and lack of effective supporting measures.We recommend a three-pronged approach to perfecting global budget management: optimizing indicators,strengthening supervision,and developing payment reform.
