Mohnarin Report 2006-2007:Bacterial Distribution and Resistance of Central Nervous System Infections

Abstract:OBJECTIVE To determine the distribution and resistance of bacteria isolated from CSF in meningitis patients.METHODS Disc diffusion test,MIC test or E-test were used to detect the antimicrobial sensitivity of bacterial isolates from CSF samples.WHONET 5.4 was applied for analysis of the bacterial sensitive data from 84 tertiary Mohnarin member hospitals at different area in China from June 1,2006 to May 31,2007.RESULTS A total of 515 organisms from CSF were collected from 26 hospitals in the survey period,which included 328(63.6%) Gram-positive strains,184(35.8%) Gram-negative strains and 3 fungal strains(0.6%).Coagulase-negative Staphylococcus(CNS),Staphylococcus aureus and Enterococcus were the most common Gram-positive bacteria,while Acinetobacter,Escherichia coli and Pseudomonas aeruginosa were the most common Gram-negative bacteria.55.6% And 75.7% of S.aureus and CNS were meticillin-resistant,respectively.No Staphylococcus strain was resistant to vancomycin.All the strains of Streptococcus pneumoniae were sensitive to penicillin,ampicillin,ceftriaxone and vancomycin.No Enterococcus were resistant to vancomycin and teicoplanin.ESBLs positive rates of E.coli and Klebsiella pneumoniae were 29.7% and 14.3%.CONCLUSIONS CNS are the predominant organisms from CSF of central nervous system infection patients in China;the proportion of Staphylococcus is more common in surgical patients.S.pneumoniae and Neisseria meningitidis are detected rarely in China.Gram-positive bacteria are more common in pediatric patients than in adults.
