Mohnarin Report in 2009:Bacterial Distribution and Resistance in Bloodstream Infections

Abstract:Objective To determine the bacterial distribution and resistance of blood stream infections in China.Methods Disc diffusion test,MIC test and E-test were used to detect the antimicrobial sensitivity of bacterial isolates from blood specimens.WHONET 5.5 software was applied for analysis the bacterial sensitive data from 114 Mohnarin member hospitals at different area in China from January to December 2009.Results A total of 10699 strains were collected in the survey period,of which gram positive cocci and gram negative organisms accounted for 56.5%(6044 strains)and 43.5%(4655 strains),respectively.The isolated rates of coagulase negative Staphylococcus(4066 strains,38.0%) took the first place,E.coli(1776 strains,16.6%),K.pneumoniae(789 strains,7.4%),S.aureus(775 strains,7.2%) and Enterococcus species(764 strains,7.1%) were also the most important pathogens which caused blood infection.Methicillin-resistant strains in S.aureus(MRSA) and coagulasenegative Staphylococcus(MRCNS)accounted for an average of 45.4% and 80.2%,respectively.None of Staphylococcus showed resistance to vancomycin and linezolid.The resistant rates of E.faecium and E faecalis to vancomycin were 4.3% and 1.4%,while to teicoplanin were 5.1% and 1.2%,respectively.None of Enterococcus showed resistance to linezolid.The ESBLs positive rates of E.coli and K.pneumoniae were 70.2% and 53.3%,respectively.The resistant rates of A.baumannii to imipenem were 60.2%.Conclusion Staphylococcus is still the predominant pathogen in bloodstream infections.Separating rate of coagulasenegative Staphylococcus is increasing.The ESBLs positive rates of E.coli and Klebsiella spp are increasing.Imipenem resistant rate of A.baumannii is rising significantly.
