Abstract:There is certain failure and relapse rate after the reconstruction of pelvic floor.How to recognize and cope with these problems is in the interest of the pelvic reconstruction surgeon.This article introduces the definition of failure and relapse after the surgery dealing with pelvic organ prolapse and stress urinary incontinence,the reason and high risk factors for failure and relapse,and how to prevent and deal with relapse.The relapse rate after pelvic reconstruction is closely associated with the procedure chosen,the skill of the surgeon and the option of the mesh material.There are various methods to deal with relapse,including surgeries like vaginal sacrum fixation and sacrospinous ligament fixation and as well as the use of pessary ring.If mesh expulsion occurs,the exposed mesh needed to be trimmed,and in some severe case,regeneration material like SIS graft is used to repair the defect of the vaginal mucosa.The failure or relapse following initial stress incontinence traditional surgery or sling surgery can both be treated with tension-free mid urethral slings.
