Abstract:Objective To investigate the effect of dual control modes of mechanical ventilation in patients with ARDS.Methods ARDS 81 cases with mechanical ventilation used volume control modes、pressure control modes and dual control modes in turn randomly.Period of time later,monitored the change of the hemodynamic、ABG and the ventilatory parameters.Results The ventilation pressure of dual control modes was less than volume control modes and close to pressure control modes.RR、MV、oxidation index(OI) and hemodynamic have no obvious difference in three modes.Barotraumas didn't occur.Conclusion Dual control modes combines the characteristics of volume control mode and pressure control mode,improve the harmony between the patients and the ventilator.But be compared with traditional ventilation,dual control modes are in defect of predominance.None but treating with lung protective ventilatory strategies(LPV) can achieve the good ventilation effect.

Abstract:Objective To evaluate the clinical application of mechanical ventilation therapy in the severe acute left ventricular failure.Methods 60 patients with severe acute left ventricular failure were randomly divided into two groups.The first group were treated with BiPAP and the second group were treated with ventilator through tracheal intubation.Before and after ventilatory support,the data of two groups including heart rate,respiratory rate,blood gas analysis were compared.Results After 2 hours most patients of the first group were improved with BiPAP.But still hypooxgenemia could not be corrected in 12 patients,which lead to endotracheal intubation and mechanical ventilation.9 patients died and the mortality was 30.0%.After 2 hours trearment the data in the second group were improved apparently.5 patients died and the mortality was 16.7%.Conclusion Mechanical ventilation is a kind of effective method in the treatment of severe acute left ventricular failure.Selecting the different model of ventilation according to the different condition is important.

Abstract:Objective: To investigate the changes of haemodynamics, pulmonary mechanics and blood gas in volume controlled and pressure controlled ventilatory patterns during one-lung ventilation (OLV). Methods: 20 patients with patent ductus arteriosus (PDA) underwent left thoracotomy PDA ligation with right OLV. The patients were divided into three groups: two-lung ventilation with volume controlled (TLV-VCV), one-lung ventilation with volume controlled (OLV-VCV), and one-lung ventilation with pressure controlled (OLV-PCV). After two-lung ventilation with VCV, one-lung ventilation was started by VCV and the ventilation mode was then switched to PCV. All measurements were made 25 min after initiation of the ventilation mode. The respiratory mechanics index was measured by side stream spirometry (SSS), including peak airway pressure (Ppeak), plateau pressure (Pplat), airway resistance (Raw), lung compliance (Cdyn) and inspiratory and expiratory minute ventilation (Mvi, Mve). Cardiac output (CO), stroke volume (SV), systemic vascular resistance (SVR), left ventricular ejectiontime (LVETi), and aortic blood flow acceleration (ACC) were also measured, by using the transesophageal Doppler (TED) monitor. Arterial blood gases was determined in every ventilation mode. Results: Ppeak、Pplat and Raw were significantly higher during OLV-VCV than that during TLV-VCV (P0.01), whereas Cdyn was significantly lower (P0.01). CO and SV decreased and SVR increased during OLV-VCV (P0.05). Arterial oxygen pressure (PaO 2) significantly decreased during OLV-VCV (P0.01). While PaO 2 was significantly increased during OLV-PCV (P0.05). Conclusion: Haemodynamics, respiratory mechanics and arterial oxygen tension declines result in hypoxemia during one-lung ventilation. Pressure controlled ventilation appeared to be a better alternative.
A Survey of the Practice of Non-Invasive Positive Pressure Ventilation in Emergency Department in China

Abstract:Objective To investigate the application of non-invasive positive pressure ventilation in emergency department in China.Methods A questionnaire survey was carried out in the emergency department in China,then the results were analyzed.Results Out of the 317 hospitals,non-invasive ventilators were available in the emergency department in 150 clinics (47.3％).Among them,144 clinics were the tertiary hospitals,and 6 clinics were the secondary hospitals.The leading five causes for NPPV were as follows:acute exacerbation of chronic obstructive pulmonary diseases (AECOPD) (29.7％),acute cardiogenic pulmonary edema (23.2％),ventilator weaning (11.2％),acute respiratory distress syndrome (ARDS) (9.0％),and extubation failure (8.7％).The bi-level positive airway pressure (BiPAP) S/T was the most commonly used ventilation mode (51.8％),followed by the continuous positive airway pressure (CPAP)(30.3％) and BiPAP (S)(12.5％).The oronasal mask (73.7％) and the nasal mask (24.2％) were the most favorable interfaces for NPPV.The reasons for NPPV underusing mainly included insufficient training (36.8％),lacking of related knowledge (22.9％),medical teaching staff shortage (21.1％) and poor compliance of patients (11.0％).The main causes of the failure of NPPV were as follows:poor tolerance (31.5％),irrational parameters setting (25.6％),unfitness of interface (17.8％),airway secretions (14.7％),and disorders of consciousness (10.4％).Conclusions In China,the types of patients most commonly treated with NPPV in the emergency department are those with AECOPD and acute cardiogenic pulmonary edema.Barriers to popularize the use of NPPV in the emergency department include availability of equipment,physician with familiarity in operating the NPPV,and human resources required for NPPV.

Abstract:Mechanical ventilation is essential in the treatment of acute respiratory distress syndrome(ARDS).Mechanical ventilation mode and parameter set are extremely important for improving the prognosis of patients.Research data have shown that low tidal volume ventilation(＜ 6 ml/kg predicted body weight,and plateau pressure is less than 30 cm H_2O)can reduce mortality and is standard treatment for ARDS.The individualized positive end expiratory pressure ventilation is also a promising study direction.Other methods such as prone position ventilation,alveolar recruitment can not be used as routine first-line treatment,but only be used.as rescue treatment under severe hypoxemia.Additionally,this article also reviews some characteristics of mechanical ventilation on the treatment of ARDS induced by H1N1 influenza in 2009.

Abstract:BackgroundAlthough protective mechanical ventilation (MV) has been used in a variety of applications, lung injury may occur in both patients with and without acute respiratory distress syndrome (ARDS). The purpose of this study is to use machine learning to identify clinical phenotypes for critically ill patients with MV in intensive care units (ICUs).MethodsA retrospective cohort study was conducted with 5013 patients who had undergone MV and treatment in the Department of Critical Care Medicine, Peking Union Medical College Hospital. Statistical and machine learning methods were used. All the data used in this study, including demographics, vital signs, circulation parameters and mechanical ventilator parameters, etc., were automatically extracted from the electronic health record (EHR) system. An external database, Medical Information Mart for Intensive Care III (MIMIC III), was used for validation.ResultsPhenotypes were derived from a total of 4009 patients who underwent MV using a latent profile analysis of 22 variables. The associations between the phenotypes and disease severity and clinical outcomes were assessed. Another 1004 patients in the database were enrolled for validation. Of the five derived phenotypes, phenotype I was the most common subgroup (n=2174; 54.2%) and was mostly composed of the postoperative population. Phenotype II (n=480; 12.0%) led to the most severe conditions. Phenotype III (n=241; 6.01%) was associated with high positive end-expiratory pressure (PEEP) and low mean airway pressure. Phenotype IV (n=368; 9.18%) was associated with high driving pressure, and younger patients comprised a large proportion of the phenotype V group (n=746; 18.6%). In addition, we found that the mortality rate of Phenotype IV was significantly higher than that of the other phenotypes. In this subgroup, the number of patients in the sequential organ failure assessment (SOFA) score segment (9,22] was 198, the number of deaths was 88, and the mortality rate was higher than 44%. However, the cumulative 28-day mortality of Phenotypes IV and II, which were 101 of 368 (27.4%) and 87 of 480 (18.1%) unique patients, respectively, was significantly higher than those of the other phenotypes. There were consistent phenotype distributions and differences in biomarker patterns by phenotype in the validation cohort, and external verification with MIMIC III further generated supportive results.ConclusionsFive clinical phenotypes were correlated with different disease severities and clinical outcomes, which suggested that these phenotypes may help in understanding heterogeneity in MV treatment effects.
Practice Pattern Of Aerosol Therapy Among Patients Undergoing Mechanical Ventilation In Mainland China: A Web-Based Survey Involving 447 Hospitals

Abstract:Background and objectiveAerosol therapies are widely used for mechanically ventilated patients. However, the practice pattern of aerosol therapy in mainland China remains unknown. This study aimed to determine the current practice of aerosol therapy in mainland China.MethodsA web-based survey was conducted by the China Union of Respiratory Care (CURC) from August 2018 to January 2019. The survey was disseminated via Email or WeChat to members of CURC. A questionnaire comprising 16 questions related to hospital information and 12 questions related to the practice of aerosol therapy. Latent class analysis was employed to identify the distinct classes of aerosol therapy practice.Main resultsA total of 693 valid questionnaires were returned by respiratory care practitioners from 447 hospitals. Most of the practitioners used aerosol therapy for both invasive mechanical ventilation (90.8%) and non-invasive mechanical ventilation (91.3%). Practitioners from tertiary care centers were more likely to use aerosol therapy compared with those from non-tertiary care centers (91.9% vs. 85.4%, respectively; p = 0.035). The most commonly used drugs for aerosol therapy were bronchodilators (64.8%) followed by mucolytic agents (44.2%), topical corticosteroids (43.4%) and antibiotics (16.5%). The ultrasonic nebulizer (48.3%) was the most commonly used followed by the jet nebulizer (39.2%), the metered dose inhaler (15.4%) and the vibrating mesh nebulizer (14.6%). Six latent classes were identified via latent class analysis. Class 1 was characterized by the aggressive use of aerosol therapy without a standard protocol, while class 3 was characterized by the absence of aerosol therapy.ConclusionsSubstantial heterogeneity among institutions with regard to the use of aerosol therapy was noted. The implementation of aerosol therapy during mechanical ventilation was inconsistent in light of recent practice guidelines. Additional efforts by the CURC to improve the implementation of aerosol therapy in mainland China are warranted.

Clinical Comparative Study of Mechanical Ventilation on Pulmonary-Interior and Exterior Respiratory Failure

Abstract:Objective The purpose was to explore the lung protective ventilatory strategies about pulmonary-interior and exterior respiratory failure.Methods We retrospectively studied the clinical data of 25 respiratory failure cases, including 12 interior cases and 13 exterior cases.Moreover,we also analyzed the effect of difference in respiratory mechanics on the pattern fabrication,parameters layout and regulation as well as the lung protection method.Results On the condition that there was no difference in APACHE-Ⅱ grade,mean age,and Vt between groups( P 0 05),whereas the results of Crs was significantly lower than those in exterior group (P0 01).The mortality rates were accounted for 41 7% and 15 4%,respectively, which had markedy statistical difference in interior and exterior group(P0 01),and they all died of MOF (APACHE-Ⅱ grade).Conclusions We should carry out different lung protection method in mechanical ventilation of pulmonary-interior and exterior respiratory failure.The former firstly selects pre-setting pressure mechanical ventilation and the latter firstly selects pre-setting volume mechanical ventilation.In conclusion,there is a principle that setting middle or low Vt,appropriate frequency and adding PSV,CPAP/PEEP according to individual ventilation demand,which is helpful to reducing VILI,morbidity and mortality. Abstract:Objective:To compare the effect of face mask mechanical ventilation(FMMV)and endotracheal intubation mechani- cal ventilation(ETMV)for the treatment of chronic obstructive pulmonary diseases(COPD)with respiratory failure(RF). Methods:COPD patients with RF in EICU were compared about FMMV and ETMV from 2003 to 2007.Results:FMMV im- proved consciousness and arterial blood gas levels in 4 hours;in the FMMV group,the ventilation times.length of ICU stay and hospital stay were shorter than those with ETMV;the mortality of inpatients,the rate of need for intubation,and ventilator-as- sociated pneumonia were lower in FMMV group.Kelly score,3 patients in FMMV group,the rate of need for intubation was lower than those 4-6.Conclusion:FMMV could be successfully applied to most COPD patients with RF,but it must he per- formed carefully in patients with severely altered level of consciousness.
Experience of prone position ventilation in clinical nursing application

Abstract:: To understand the effect of supine ventilation in clinical nursing. Application method: 15 cases of respiratory failure and ARDS patients in ICU of our department were treated with recumbent ventilation and nursing measures in recent two years. Effect evaluation: after treatment, 12 patients showed significant improvement of symptoms, and 9 patients were successfully transferred to the department for further treatment. Conclusion: supine ventilation can improve the patient's hypoxia symptoms, increase functional residual air volume, improve ventilation blood flow ratio, reduce mediastinal and cardiac pressure on the lungs, and improve the compliance of the thoracic cavity.

Abstract:In patients with ALI/ARDS,mechanical ventilation with lung protective ventilation strategies results in decreased mortality and enhances the number of days without ventilator use.While guidelines now strongly advise using lung protective ventilation in ALI/ARDS patients,there are presently no widely agreed-upon guidelines for the use of ventilation with lung protective ventilation strategies in patients who with non-ALI/ARDS conditions.Literatures concerning animal and clinical studies on lung protective mechanical ventilation in patients with non-ALI/ARDS conditions were searched out,and aspects including post ventilation oxygenation indices,cytokine levels,and lung histology were reviewed.The inconsistent results from different trials,however,do not definitely support the use of lung protective mechanical ventilation.Nevertheless,more prospective studies are needed to establish the optimal ventilator management strategies for patients with non-ALI/ARDS conditions.
Comparison on the effect of mechanical ventilation with different suctioning in respiratory failure patients

Abstract:Objective To investigate the effect on the parameters of respiratory and circulatory systems in respiratory failure patients receiving mechanical ventilation by different suctioning. Methods 40 respiratory failure patients receiving mechanical ventilation were randomly divided into control group and experimental group (20 patients per group). Individuals in experimental group were treated with closed endotracheal suctioning and the others were treated with open endotracheal suctioning. The parameters of respiratory and circulatory systems were observed before and after suctioning. All the subjects used own control. Results The parameters of respiratory and circulatory systems did not change significantly in experimental group. The heart rate,SpO2,Ppeak,Pplat have changed significantly and the SBP,RR and PaO2 also reached the statistical significance before and after suctioning in control group. However,Vt and PaCO2 did not change significantly(P ＞ 0.05).Conclusions Respiratory failure patients treated with closed endotracheal suctioning was safer and more effective than traditional open-suctioning.

Abstract:【Objective】To study the juncture of invasive ventilation in treatment of severe respiratory failure of chronic obstructive pulmonary disease.【Methods】24 patients with invasive ventilation in RICU were divided into 2 groups:mild case group(n =13) and severe case group(n =11).Compared the duration of MV(h),days in ICU(d),days in hospital(d),mortality in hospital(%) and the incidence of ventilator-associated pneumonia(%).【Result】 Significant difference between the mild case group and severe case group except the incidence of ventilator-associ-ated pneumonia.【Conclusion】Incision of trachea in earlier time maybe better for the patient of respiratory failure of chronic obstructive pulmonary disease if there has no conspicuous effect in short term and no taboo.

Abstract:Objective To study the clinical application of mechanical ventilation in cardiac diseases. Methods Thirty cases with various cardiac diseases caused heart failure or fatal arrhythmia were treated with mechanical ventilation. Before and after ventilation support,the heart rate,blood pressure, SaO2 and other blood analysis were recorded. Results After ventilation support treatment, the heart rate and breathe rate reduced significantly (P 0. 05) , the SaO2 ,PO2 markedly increased(P 0. 05). Conclusion Mechanical ventilation can not be only applied to respiratory failure, but also applied to heart failure.
Comparison of Clinical Outcomes in Critical Patients Undergoing Different Mechanical Ventilation Modes: a Systematic Review and Network Meta-Analysis

Abstract:Purpose To evaluate the effects of different mechanical ventilation modes on critical patients. Methods PubMed, Embase, Web of science, and Cochrane Library databases were searched from their inception to November 15, 2022 for randomized controlled trials on the application of different mechanical ventilation modes in critical patients. Two researchers independently screened the literature, extracted data, and assessed the risk of bias in the included studies. R4.2.1 was used for this network meta-analysis. Results Twenty-eight RCTs involving 3,189 patients were included. The interventions in these RCTs included NAVA (neurally adjusted ventilatory assist), PAV (proportional assist ventilation), ASV (adaptive support ventilation), Smartcare/PS (Smartcare/pressure support), PSV (pressure support ventilation), PSV_ATC (pressure support ventilation_automatic tube compensation), and SIMV (synchronized intermittent mandatory ventilation). The network meta-analysis showed that, compared with the PSV group, there was no significant difference in duration of mechanical ventilation, duration of ICU stay, and hospital stay between NAVA, SIMV, AVS, PAV, Smartcare/PS, and PSV_ATC groups. Compared with PSV, PAV improved the success rate of withdrawal of ventilator [OR = 3.07, 95%CI (1.21, 8.52)]. Compared with PSV and PAV, NAVA reduced mortality in the ICU [OR = 0.63, 95%CI (0.43, 0.93); OR = 0.45, 95%CI (0.21, 0.97)]. Conclusion NAVA can reduce mortality in ICU, and PAV may increase the risk of withdrawal of the ventilator. There was no significant difference between PSV and other mechanical ventilation modes (NAVA, SIMV, AVS, PAV, Smartcare/PS, and PSV_ATC) in the duration of mechanical ventilation, duration of ICU stay, or hospital stay. Due to the limitations, more high-quality studies are needed to verify these findings.
