Abstract:Objective To investigate the effect of dual control modes of mechanical ventilation in patients with ARDS.Methods ARDS 81 cases with mechanical ventilation used volume control modes、pressure control modes and dual control modes in turn randomly.Period of time later,monitored the change of the hemodynamic、ABG and the ventilatory parameters.Results The ventilation pressure of dual control modes was less than volume control modes and close to pressure control modes.RR、MV、oxidation index(OI) and hemodynamic have no obvious difference in three modes.Barotraumas didn't occur.Conclusion Dual control modes combines the characteristics of volume control mode and pressure control mode,improve the harmony between the patients and the ventilator.But be compared with traditional ventilation,dual control modes are in defect of predominance.None but treating with lung protective ventilatory strategies(LPV) can achieve the good ventilation effect.

Abstract:Mechanical ventilation is a important therapeutic approach for critically ill patients.The primary goals of mechanical ventilation are support of the ventilatory and oxygenation functions of the lung and reduction in work of breathing,while assuring or increasing lung volume.Various types of breaths and modes are available to facilitate synchrony between the patient and ventilator.The complex interactions of inspiratory pressure,I:E ratio,PEEP,and FiO2 must be appreciated to evaluate the beneficial and harmful effects in each patient.The primary determinants of oxygenation are FiO2 and mean airway pressure,whereas minute alveolar ventilation primarily affects CO2 exchange.Ideally,the inspiratory plateau pressure should be less than 30 cmH2O for prevention of ventilator associated lung injury.
Changes of Haemodynamics, Pulmonary Mechanics and Blood Gas in Different Ventilatory Modes During One-Lung Ventilation

Abstract:Objective: To investigate the changes of haemodynamics, pulmonary mechanics and blood gas in volume controlled and pressure controlled ventilatory patterns during one-lung ventilation (OLV). Methods: 20 patients with patent ductus arteriosus (PDA) underwent left thoracotomy PDA ligation with right OLV. The patients were divided into three groups: two-lung ventilation with volume controlled (TLV-VCV), one-lung ventilation with volume controlled (OLV-VCV), and one-lung ventilation with pressure controlled (OLV-PCV). After two-lung ventilation with VCV, one-lung ventilation was started by VCV and the ventilation mode was then switched to PCV. All measurements were made 25 min after initiation of the ventilation mode. The respiratory mechanics index was measured by side stream spirometry (SSS), including peak airway pressure (Ppeak), plateau pressure (Pplat), airway resistance (Raw), lung compliance (Cdyn) and inspiratory and expiratory minute ventilation (Mvi, Mve). Cardiac output (CO), stroke volume (SV), systemic vascular resistance (SVR), left ventricular ejectiontime (LVETi), and aortic blood flow acceleration (ACC) were also measured, by using the transesophageal Doppler (TED) monitor. Arterial blood gases was determined in every ventilation mode. Results: Ppeak、Pplat and Raw were significantly higher during OLV-VCV than that during TLV-VCV (P0.01), whereas Cdyn was significantly lower (P0.01). CO and SV decreased and SVR increased during OLV-VCV (P0.05). Arterial oxygen pressure (PaO 2) significantly decreased during OLV-VCV (P0.01). While PaO 2 was significantly increased during OLV-PCV (P0.05). Conclusion: Haemodynamics, respiratory mechanics and arterial oxygen tension declines result in hypoxemia during one-lung ventilation. Pressure controlled ventilation appeared to be a better alternative.
A Survey of the Practice of Non-Invasive Positive Pressure Ventilation in Emergency Department in China

Abstract:Objective To investigate the application of non-invasive positive pressure ventilation in emergency department in China.Methods A questionnaire survey was carried out in the emergency department in China,then the results were analyzed.Results Out of the 317 hospitals,non-invasive ventilators were available in the emergency department in 150 clinics (47.3％).Among them,144 clinics were the tertiary hospitals,and 6 clinics were the secondary hospitals.The leading five causes for NPPV were as follows:acute exacerbation of chronic obstructive pulmonary diseases (AECOPD) (29.7％),acute cardiogenic pulmonary edema (23.2％),ventilator weaning (11.2％),acute respiratory distress syndrome (ARDS) (9.0％),and extubation failure (8.7％).The bi-level positive airway pressure (BiPAP) S/T was the most commonly used ventilation mode (51.8％),followed by the continuous positive airway pressure (CPAP)(30.3％) and BiPAP (S)(12.5％).The oronasal mask (73.7％) and the nasal mask (24.2％) were the most favorable interfaces for NPPV.The reasons for NPPV underusing mainly included insufficient training (36.8％),lacking of related knowledge (22.9％),medical teaching staff shortage (21.1％) and poor compliance of patients (11.0％).The main causes of the failure of NPPV were as follows:poor tolerance (31.5％),irrational parameters setting (25.6％),unfitness of interface (17.8％),airway secretions (14.7％),and disorders of consciousness (10.4％).Conclusions In China,the types of patients most commonly treated with NPPV in the emergency department are those with AECOPD and acute cardiogenic pulmonary edema.Barriers to popularize the use of NPPV in the emergency department include availability of equipment,physician with familiarity in operating the NPPV,and human resources required for NPPV.

Abstract:Mechanical ventilation is essential in the treatment of acute respiratory distress syndrome(ARDS).Mechanical ventilation mode and parameter set are extremely important for improving the prognosis of patients.Research data have shown that low tidal volume ventilation(＜ 6 ml/kg predicted body weight,and plateau pressure is less than 30 cm H_2O)can reduce mortality and is standard treatment for ARDS.The individualized positive end expiratory pressure ventilation is also a promising study direction.Other methods such as prone position ventilation,alveolar recruitment can not be used as routine first-line treatment,but only be used.as rescue treatment under severe hypoxemia.Additionally,this article also reviews some characteristics of mechanical ventilation on the treatment of ARDS induced by H1N1 influenza in 2009.

Comparison of Clinical Outcomes in Critical Patients Undergoing Different Mechanical Ventilation Modes: a Systematic Review and Network Meta-Analysis

Abstract:Purpose To evaluate the effects of different mechanical ventilation modes on critical patients. Methods PubMed, Embase, Web of science, and Cochrane Library databases were searched from their inception to November 15, 2022 for randomized controlled trials on the application of different mechanical ventilation modes in critical patients. Two researchers independently screened the literature, extracted data, and assessed the risk of bias in the included studies. R4.2.1 was used for this network meta-analysis. Results Twenty-eight RCTs involving 3,189 patients were included. The interventions in these RCTs included NAVA (neurally adjusted ventilatory assist), PAV (proportional assist ventilation), ASV (adaptive support ventilation), Smartcare/PS (Smartcare/pressure support), PSV (pressure support ventilation), PSV_ATC (pressure support ventilation_automatic tube compensation), and SIMV (synchronized intermittent mandatory ventilation). The network meta-analysis showed that, compared with the PSV group, there was no significant difference in duration of mechanical ventilation, duration of ICU stay, and hospital stay between NAVA, SIMV, AVS, PAV, Smartcare/PS, and PSV_ATC groups. Compared with PSV, PAV improved the success rate of withdrawal of ventilator [OR = 3.07, 95%CI (1.21, 8.52)]. Compared with PSV and PAV, NAVA reduced mortality in the ICU [OR = 0.63, 95%CI (0.43, 0.93); OR = 0.45, 95%CI (0.21, 0.97)]. Conclusion NAVA can reduce mortality in ICU, and PAV may increase the risk of withdrawal of the ventilator. There was no significant difference between PSV and other mechanical ventilation modes (NAVA, SIMV, AVS, PAV, Smartcare/PS, and PSV_ATC) in the duration of mechanical ventilation, duration of ICU stay, or hospital stay. Due to the limitations, more high-quality studies are needed to verify these findings.

Abstract:Objective To evaluate the efficacy of proportional assist ventilation( PAV) in weaning the patients from ventilator after cardiac surgery as compared with pressure support ventilation (PSV) and continuous positive airway pressure (CPAP) in terms of respiratory and homodynamic parameters and gas exchange. Methods Forty-six ASAⅠorⅡpatients aged 18-72 yrs, weighing 40-65 kg were studied immediately after cardiac surgery including valve replacement and CABG. As soon as the patients regained consciousness and resumed spontaneous breathing, they were ventilated by PAV80, PSV and CPAP consecutively in randomized order. Each mode lasted for 30 minutes. VT, RR, peak inspiratory pressure (PIP), average airway pressure (Paw), PEEP, work of breathing (WOB), HR, MAP, CVP, and blood gases were monitored. Results During CPAP VT, PIP, Paw, PaO2 and SaO2 were significantly decreased while RR, rapid-shallow-breathing index (RSBI), Ti/Ttot, pressure-time product (PTP), patient's work of breathing (WOBp), pH and PaCO2 were significantly increased ( P 0.01) as compared with PAV80 and PSV modes. PAV80 resulted in a decrease in PIP and Paw and an increase in PTP as compared with PSV. There was no difference in arterial blood gases (PaO2, PaCO2) and MAP, HR and CVP between PAV80 and PSV modes. Conclusion PAV is a mode of choice in weaning the patients from ventilator after cardiac surgery.

Abstract:Mechanical ventilation (MV) is an effective management strategy for neonates with critical congenital heart disease or congenital tracheal stenosis (CTS). However, there is no standard for patient-specific mode selection. This study numerically investigated the aerodynamic effects of tracheal model with severe stenosis when by different levels of ventilator assist during Pressure Support Ventilation (PSV) and Neurally Adjusted Ventilatory Assist (NAVA). Based on medical images, a three-dimensional (3D) tracheal model with insertion of a cuffed endotracheal tube was reconstructed. The technology of Computational Fluid Dynamics (CFD) was applied to simulate the airflow in the trachea. The aerodynamic parameters, including pressure drop (PD), streamlines and rate of energy loss (ELR), were compared to assess the MV effects. The results indicated that high assist level, accompanied by high airflow velocity, should be the main cause of aerodynamic disorders in the airway during MV. Lower PD, ELR and relatively steady velocity of NAVA was observed. Compare with PSV, it was inferred that preserved auto-regulation of respiration during NAVA may have potential advantages for flow rate regulation in patient with CTS. CFD analysis is a potential noninvasive tool for obtaining tracheal aerodynamics, which will be helpful for making decisions of appropriate MV mode.

Abstract:Mechanical ventilation is an important and effective method for the treatment of pulmonary diseases patients with spontaneous breathing. Spontaneous breathing refers to the physiological breathing activity caused by the respiratory muscle. These patients retain some ability to breathe spontaneously, but do not reach the level of normal breathing. Mathematical simulation and modeling of the mechanical ventilation system are crucial for research on mechanical ventilation. In this paper, a novel pneumatic model of a mechanical ventilation system considering patients' spontaneous breathing is presented. Mathematical equations are accurately derived to explain the principles of the respiratory system and mechanical ventilation system. An experimental prototype is designed to confirm the correctness and validity of the pneumatic model. The goodness of fit shows that the mathematical simulation curve fits well with the experimental curve, thus confirming the accuracy of the pneumatic model. For patients with a certain degree of spontaneous breathing, the mechanical ventilation mode is set to the pressure support ventilation (PSV) mode, and variations in the flow, pressure and tidal volume curves are observed by changing specific respiratory mechanics parameters such as the compliance (C), the effective area of the throttle in the pneumatic model (A), and the muscle pressure difference (Delta P-mus). From the results, it can be concluded that the resistance of the mechanical ventilation system can be equivalent to A. The dynamic characteristics (mainly flow characteristics, tidal volume characteristics and pressure characteristics) of the mechanical ventilation system are directly influenced by variations in C, A and Delta P-mus. This study is an important reference for setting ventilation levels and ventilator control parameters. The results of this research are valuable for the diagnosis and treatment of respiratory diseases.
Volume assured pressure support mode use for non-invasive ventilation in pediatrics

Abstract:There has been increasing interest in utilizing volume assured pressure support (VAPS) modes of ventilation for children, which historically had only been favored in adult populations. In addition to patients with obesity hypoventilation syndrome, newer pediatric populations for which it has recently been prescribed include congenital central hypoventilation syndrome and children with neuromuscular disease such as Duchenne muscular dystrophy and spinal muscular atrophy. Given its expanding use in pediatrics, greater familiarity with VAPS is essential for pediatric pulmonologists and sleep physicians. This review article will highlight methods of initiation for this mode, specific ventilator settings, discussion of suitable pediatric patient populations, ventilator titrations via formal polysomnograms and detailed ventilator data downloads specific interpretation. Finally, common challenges to be aware of and how to troubleshoot relevant machine alarms will be reviewed.

Analysis of the risk factors and pathogenic bacteria of ventilator-associated pneumonia

Abstract:OBJECTIVE To explore the risk factors of ventilator‐associated pneumonia and the distribution of the related pathogens ,so as to provide scientific evidence for the prevention and control of ventilator associated pneu‐monia. METHODS A retrospective analysis was conducted to 172 patients who were treated with mechanical venti‐lation in ICU from Feb. 2012 to Feb. 2014. The clinical features ,risk factors and the distribution of pathogens related to ventilator‐associated pneumonia were studied. All data were analyzed by SPSS 11. 0. RESULTS In 172 patients who were treated with mechanical ventilation ,78 were infected with ventilator‐associated pneumonia ,the incidence rate was 45. 35%. The sputum specimens of them were examined and 105 strains of pathogenic bacteria were found. Of them ,62 strains were gram‐negative bacilli ,accounting for 59. 05%. 27 strains were gram‐posi‐tive bacilli ,accounting for 25. 71%. 16 strains were fungi ,accounting for 15. 24%. The risk factors of ventilator‐associated pneumonia included age ,underlying diseases ,mechanical ventilation time ,and the existence of con‐sciousness disorder , tracheotomy , invasive operation , antibiotic application and duration of hospitalization. CONCLUSION There are many risk factors for ICU patients with ventilator‐associated pneumonia so we call for a strict ventilation index ,which can provide the corresponding nursing measures considering these risk factors. Strict implementation of aseptic management ,precaution of reflux and aspiration ,and rational use of antimicrobial drugs ,can effectively reduce the incidence of ventilator‐associated pneumonia.

Abstract:Patient-triggered modes of ventilation are currently the standard of practice in the care of term and preterm infants. Maintaining spontaneous breathing during mechanical ventilation promotes earlier weaning and possibly reduces ventilator-induced diaphragmatic dysfunction. A further development of assisted ventilation provides support in proportion to the respiratory effort and enables the patient to have full control of their ventilatory cycle. In this paper we will review the literature on two of these modes of ventilation: neurally adjusted ventilatory assist (NAVA) and proportional assist ventilation (PAV), propose future studies and suggest clinical applications of these modes.

Abstract:Background: Respiratory distress syndrome remains the most common admission diagnosis in the neonatal intensive care unit. Healthcare providers have a clear appreciation for the potential harm to pulmonary structures that have been associated with mechanical ventilation (MV) in the preterm infant. Although life sustaining, the goal is to optimally ventilate while limiting trauma to the neonatal lung in order to preserve long-term cardiopulmonary and neurodevelopmental outcomes. Purpose: To describe, compare, and contrast 2 primary methods of neonatal MV, pressure-limited ventilation (PLV) and volume-targeted ventilation (VTV), highlighting key considerations during therapy. Methods: A comprehensive search of the literature was completed using the following databases: CINAHL, Cochrane, Google Scholar, and PubMed. Research articles that were published in English over the last 10 years were reviewed for key information to describe and support the topic. Expert content review was conducted prior to publication by respiratory care providers, neonatal nurse practitioners, staff nurses, and neonatologist. Findings: Technology is rapidly evolving, with the newest mechanical ventilators providing the clinician with real-time data not previously available. Advanced microprocessors and feedback mechanisms can better support various ventilatory strategies including PLV and VTV. Renewed interest in volume ventilation has led many clinicians to ask about current evidence to support ventilatory modalities with regard to timing, settings, and short- and long-term effects. Implications for Practice: The clinician understands that neonatal pulmonary status is frequently changing based on gestational age, current age, and physiologic influences. Evidence supporting recommendations for the described MV modalities of PLV and VTV is provided for both preterm and term neonates. Implications for Research: Comparison between MV strategies, specifically PLV and VTV, including short- and long-term neurodevelopmental outcomes, is needed. Recommendations regarding physiologic tidal volume for the extremely preterm infant are lacking.

Abstract:Introduction Recent studies suggested that the energy delivered by the mechanical ventilator to the lungs termed the mechanical power can induce and increase the risks of ventilator induced lung injury. The components of the mechanical power include the variables delivered by the ventilator: tidal volume, respiratory rate, inspiratory flow, airway pressure. Adaptive Ventilator Mode-2 (AVM-2) is a pressure-controlled mode with an optimal targeting scheme based on the inspiratory power equation that adjusts the respiratory rate and tidal volume to achieve a target minute ventilation. This mode conceptually should reduce the mechanical power delivered to the patients and thus reduce the incidence of ventilator induced lung injury. Methodology A bench study using a lung simulator (TTL, Michigan Instruments, Michigan, USA) was conducted. We constructed a passive single compartment normal respiratory mechanics model with compliance of 50 ml/cmH2O, and resistance of 10 cmH2O/L/s, with IBW 70 kg. We compared three different ventilator modes: Adaptive Ventilation Mode-2 (AVM-2), Pressure Regulated Volume Control (PRVC), and Volume Controlled Ventilation (VCV) in four different scenarios: 2 levels of minute ventilation 7 and 10.5 Lit/min (Experiment 1 and 2 respectively), each with 2 different PEEP levels 5 and 10 cmH2O (Experiment A and B respectively) termed Experiments 1A, 1B, 2A, and 2B respectively. The AVM-2 mode automatically selects the optimal tidal volume, and respiratory rate per the dialed percent minute ventilation with an I:E ratio of 1:1. In the PRVC, VCV we selected target tidal volume 6ml/kg/IBW (420 ml), and respiratory rate adjusted to match the minute ventilation for the AVM-2 mode. I:E ratio was kept 1:2 to avoid intrinsic PEEP. The study was conducted using a bellavistaTM 1000 e Ventilator (Vyaire Medical, Illinois, USA). The mechanical power delivered by the ventilator for each mode was computed and compared between the three modes in each experiment. Statistical analysis was done using Kruskal-Wallis test to analyze the difference between the three modes, post HOC Tukey test was used to analyze the difference between each mode with the confidence intervals, P < 0.05 was considered statistically significant. Results There were statistically significant differences between all the three modes regarding the ventilator delivered mechanical power. The AVM-2 mode delivered significantly less mechanical power than VCV which in turn was less than PRVC. Experiment 1A: AVM-2 8.76 土 0.05, VCV 9.78 土 0.04, PRVC 10.82 土 0.08, P < 0.001 Experiment 1B: AVM-2 11.27 ± 0.09 VCV 12.81 ± 0.05, PRVC 13.88 ± 0.06, P < 0.001. Experiment 2A: AVM-2 14.76 ± 0.05, VCV 15.79 ± 0.05, PRVC 18.29 ± 0.07, P < 0.001, Experiment 2B: AVM-2 18.76 ± 0.04, VCV 20.56 ± 0.04, PRVC 21.17 土 0.03, P < 0.001. Discussion AVM2 mode delivered less mechanical power compared to two conventional modes using low tidal volume in a normal lung model. This might reduce the incidence of ventilator induced lung injury. Results need to be validated in more clinical studies.
