Non-invasive Positive Pressure Ventilation Reduces the Rate of Re-Intubation in Pediatric Patients after Cardiac Surgery

Abstract:Objective To evaluate the role of using non-invasive ventilation with bi-level positive airway pressure (BiPAP) in order to reduce the need of re-intubation in pediatric patients with respiratory failure after cardiac surgery. Method From January 2007 to December 2007, 25 patients aged from three months to 11 years with median 2.3 years operated on for cardiac surgery with respiratory insufficiency after extubation and re-intubation indicated were enrolled in this study. They were put on non-invasive nasal (mask) BiPAP ventilation before re-intubation. The arterial blood gas, A-aDO2 and PaO2/FiO2 were measured. In addition, clinical data including heart rate, respiratory rate, and the product of heart rate and systolic pressure were recorded before and after BiPAP. The software SPSSD 13.0 was used to process by ANOVA test for statistical analysis. Meanwhile, the outcome of these patients was analyzed. Results Twenty-five patients with 30 episodes of respiratory insufficiency were treated with BiPAP ventilation with median duration of 1.96 days ranged from 0.03 to 12 days. Of these respiratory failure episodes, 25 ones (83.3%) could be controlled by BiPAP and the needs of re-intubation were avoided. Five episodes of respiratory failure in 4 patients could not be quelled and the endo-tracheal tubes were inserted in these patients. All patients were saved with a median of mechanical ventilation duration of 3.4 days and ICU stay of 10.6 days. No major complications were observed. The heart rate, respiratory rate and the rate-pressure product were decreased significantly one hour after BiPAP (P < 0.05 all). Meanwhile, patients showed rapid improvement of oxygenation. The pH, SpO>2 and PaO2/FiO2 were increased significantly and A-aDO2 was decreased significantly (P < 0.05 all). The PaCO2, was decreased significantly four hours after BiPAP (P < 0.05). Conclusions Non-invasive nasal mask BiPAP can be used safely and effectively in children after cardiac surgery to improve oxygenation/ventilation, decreasing the work of breathing. It may be particularly useful in patients with high risk of re-intubation.

Abstract:There has been a dramatic increase in the past few decades in the number of children receiving noninvasive positive airway pressure (PAP) therapy at home. Although PAP therapy was first prescribed for children with obstructive sleep apnea, the indications have rapidly widened to include treatment for central hypoventilation syndromes, neuromuscular and chest wall disorders as well as primary respiratory diseases. Given the rapidly expanding use of PAP therapy in children, pediatric pulmonologists need to be familiar with the indications, technical and safety considerations as well as potential complications and challenges that may arise when caring for children using PAP therapy. This review article covers the definition of PAP therapy, modes, interfaces, devices, indications, contraindications, suggested settings, complications as well as the factors influencing the adherence.

Abstract:The effect of the opioid heptapeptide Met5-enkephalin-Arg6-Phe7 was measured on the contractions of the longitudinal muscle of the guinea-pig ileum and mouse vas deferens; on release of endogenous and newly synthesized acetylcholine from the longitudinal muscle strip preparation of the guinea-pig ileum and of 3H-noradrenaline from mouse vas deferens. Met5-enkephelin-Arg6-Phe7 depressed the contractions of the longitudinal muscle strip of guinea-pig ileum elicited by low frequency stimulation (IC50 366.5 +/- 71.5 nM). The Ke value of naltrexone against the heptapeptide was found to be 0.50 +/- 0.08 nM. Bestatin prolonged and strongly potentiated the action of the opioid peptide in this tissue while captopril proved to be only slightly effective. Met5-enkephalin-Arg6-Phe7 also inhibited neuroeffector transmission in the mouse vas deferens with an IC50 value of 9.44 +/- 2.24 nM. The Ke value of naltrexone was 6.33 +/- 1.22 nM against the heptapeptide. Met5-enkephalin-Arg6-Phe7 (10(-5) M) failed to affect the low frequency stimulated release of endogenous acetylcholine from the guinea-pig ileum preparation. Addition of either bestatin or captopril with the heptapeptide resulted in a reduction of acetylcholine output. The opioid peptide by itself decreased the release of newly synthesized acetylcholine outflow from this organ. Met5-enkephalin-Arg6-Phe7 also reduced the 3H-noradrenaline outflow from mouse vas deferens and this effect was antagonized by naltrexone. From our data we conclude that Met5-enkephalin-Arg6-Phe7 affects the neural transmission of peripheral autonomically innervated organs by depressing the release of neurotransmitter. In vitro it has enkephalin-like character with a preference for the so called delta receptor interaction.

Abstract:Home noninvasive ventilation (NIV), including continuous (CPAP) and bilevel (BPAP) positive airway pressure, is increasingly used in children worldwide. In this narrative review, we present a comprehensive summary of the equipment available for home NIV in pediatrics, excluding neonates. NIV may be challenging in young children, as the majority of the equipment has been developed for adults. Regarding the interfaces, only a few masks have been specifically developed for young children in recent years, while older children may benefit from a large variety of interfaces. Even though much progress has been made, skin injuries are still present, and need to be managed rapidly. Several studies addressed the management of the side effects, but recent studies are lacking regarding orofacial anomalies. No recent study reported the available interfaces for young children and the strategies for an optimal mask fit. Regarding the devices, an adapted NIV device to pediatrics that allows an adequate patient's breathing detection should guarantee optimal ventilatory efficiency and monitoring of NIV. A close follow‐up and regular monitoring should be mandatory to rule out the potential issues, optimize NIV therapy and ascertain the efficacy of NIV. However, studies are lacking to guide the choice of devices in young children and the optimal management of home NIV in pediatrics. We summarized the characteristics of the different interfaces available for young children and the limitations of NIV devices. We finally addressed potential areas for future research on long‐term home NIV in children.

Abstract:Pediatric home invasive mechanical ventilation patients are a small but resource‐intensive cohort, requiring close monitoring and multidisciplinary care. Patients are often dependent on their ventilator for life support, with any significant complications such as equipment failure, tracheostomy blockage, or accidental decannulation becoming potentially life‐threatening if not identified quickly. This review discusses the indications and variations in practice worldwide, in terms of models of care, including home care provision, choice of equipment, and monitoring. With advances in technology, optimal monitoring strategies for home, continue to be debated: In‐built ventilator alarms are often inadequately sensitive for pediatric patients, necessitating additional external monitoring devices to minimize risk. Pulse oximetry has been the preferred monitoring modality at home, though in some special circumstances such as congenital central hypoventilation syndrome, home carbon dioxide monitoring may be important to consider. Children should be under regular follow‐up at specialist respiratory centers where clinical evaluation, nocturnal oximetry, and capnography monitoring and/or poly(somno)graphy and analysis of ventilator download data can be performed regularly to monitor progress. Recent exciting advances in technology, particularly in telemonitoring, which have potential to hugely benefit this complex group of patients are also discussed.

1236 A Challenging Transition Between Volume Assured Modes in a Patient with Congenital Lung Abnormalities

Abstract:Abstract Introduction The advent of volume-assured modes for non-invasive ventilation (NIV) has improved treatment of more complicated sleep-disordered breathing (SDB), although can present challenges in choosing ventilatory modes in patients with abnormal pulmonary anatomy. Report of case(s) Our Patient is a 44 year-old female with hypoplastic right lung, pulmonary hypertension and chronic hypercapnic/hypoxic respiratory failure on nocturnal NIV for complex sleep disordered breathing. She failed CPAP, BIPAP and BIPAP S/T. Average volume assured pressure support (AVAPS) NIV was initiated with stabilization in awake carbon dioxide levels, improved sleep quality and daytime energy . She was transitioned to an iVAPS (intelligent volume-assured pressure support) ventilator when eligible for new NIV. Initial settings were empiric, targeting Alveolar Ventilation 3.3 l/min to approximate similar minute ventilation set on previous AVAPS (Assumed dead space ventilation 1.05 l/min or 70 cc x 15rr). Despite relatively similar recorded pressures and respiratory parameters between the two volume-targeted ventilators, she experienced air hunger, palpitations and increased morning headaches after change from AVAPS to iVAPS. Alveolar ventilation target was increased to 4.5 l/min, yet symptoms persisted. She subsequently returned to AVAPS, with symptom resolution. Conclusion This case highlights the uncertainty in transitioning volume assured ventilator modes that use different algorithms to achieve ventilatory targets. We hypothesized incorrect dead space estimates in the setting of her abnormal pulmonary anatomy may lead to inequivalent respiratory targets between modes, but this does not appear to be the case based on available data. The sensation of dyspnea is complex, thus it is possible subtle changes in rise time, breath delivery, cycling and triggering may have contributed to air hunger. Furthermore, many assumptions are made when transitioning between intelligent ventilator modes and these assumptions are based on “normal” respiratory dynamics. Abstract:It is a developing trend that satellites are used to interconnect heterogeneous network in the field of satellite communication. Besides transmitting application data, inter-satellite links (ISLs) will be used to transmit satellite management data, such as telemetry and telecommand data. The ground stations are managing entity. In this paper, the typical constellation composed of twenty-four medium Earth orbit (MEO) satellites are considered as a researching object. We present a routing algorithm satisfied ground stations distribution constraint. The characteristics of routing algorithms are combine satellite network with ground station managing network to make integrated planning. From the simulation results, it shows that our algorithm takes great advantage of data transmitting bandwidth of ISLs and save ground station managing resources. It is an important foundation for further studies of management methods on a large scale satellite networks.
Noninvasive Ventilation Via Bilevel Positive Airway Pressure Support in Pediatric Patients after Cardiac Surgery

Abstract:Background: Noninvasive bilevel positive airway pressure (BiPAP) support ventilation applied by nasal mask in children with impending respiratory failure enhances oxygenation/ventilation, decreases the work of breathing, and may obviate the need for an artificial airway. This study was to provide a prospective evaluation of clinical experience in this special population, suggesting a wider role for this less intrusive ventilatory support modality. Methods: Twenty-five patients (3 months to 11 years, mean 2.3 years) who underwent corrective cardiac surgery and developed respiratory insufficiency after extubation were enrolled in the study. All patients required airway support or oxygenation/ventilatory support and were firstly treated with noninvasive BiPAP ventilation before re-intubation. The changes of clinical symptoms and arterial blood gas were measured. Results: The 25 patients with 30 episodes of respiratory insufficiency requiring airway support or oxygenation/ventilatory support were treated with BiPAP ventilation with a mean duration of 1.96 days (range, 0.03 to 12 days). No major complications were observed. Twenty-five episodes (83.3%) benefited from BiPAP and avoided re-intubation. One hour after institution of BiPAP, the patients showed an acute improvement of oxygenation. pH increased from 7.37±0.02 to 7.41±0.01, SaO2 increased from 93.8±1.0% to 97.7±0.4%, PaO2/FiO2 increased from 189.9±25.0 to 253.6±21.2 mmHg, and A-aDO2 decreased from 241.8±18.7 to 182.1±16.5 mmHg (all P<0.05). Four hours after BiPAP, PaCO2 significantly decreased from 44.0±2.1 to 38.9±0.8 mmHg (P<0.05). Meanwhile, heart rate decreased from 157±4 to 139± 4 beats/minute, respiratory rate decreased from 46±2 to 37±2 breaths/minute, rate-pressure product decreased from 17 230±685 to 14 046±423 mmHg · beats/minute (all P<0.05). Five episodes in 4 patients were unable to stabilize progression of respiratory failure and an artificial airway was subsequently placed. All patients survived with a mean mechanical ventilation duration of 3.4 days and an ICU stay of 10.6 days. Conclusions: Noninvasive nasal mask BiPAP can be safely and effectively used in children after cardiac surgery to improve oxygenation/ventilation, decrease the work of breathing. Abstract:Background: Respiratory distress syndrome remains the most common admission diagnosis in the neonatal intensive care unit. Healthcare providers have a clear appreciation for the potential harm to pulmonary structures that have been associated with mechanical ventilation (MV) in the preterm infant. Although life sustaining, the goal is to optimally ventilate while limiting trauma to the neonatal lung in order to preserve long-term cardiopulmonary and neurodevelopmental outcomes. Purpose: To describe, compare, and contrast 2 primary methods of neonatal MV, pressure-limited ventilation (PLV) and volume-targeted ventilation (VTV), highlighting key considerations during therapy. Methods: A comprehensive search of the literature was completed using the following databases: CINAHL, Cochrane, Google Scholar, and PubMed. Research articles that were published in English over the last 10 years were reviewed for key information to describe and support the topic. Expert content review was conducted prior to publication by respiratory care providers, neonatal nurse practitioners, staff nurses, and neonatologist. Findings: Technology is rapidly evolving, with the newest mechanical ventilators providing the clinician with real-time data not previously available. Advanced microprocessors and feedback mechanisms can better support various ventilatory strategies including PLV and VTV. Renewed interest in volume ventilation has led many clinicians to ask about current evidence to support ventilatory modalities with regard to timing, settings, and short- and long-term effects. Implications for Practice: The clinician understands that neonatal pulmonary status is frequently changing based on gestational age, current age, and physiologic influences. Evidence supporting recommendations for the described MV modalities of PLV and VTV is provided for both preterm and term neonates. Implications for Research: Comparison between MV strategies, specifically PLV and VTV, including short- and long-term neurodevelopmental outcomes, is needed. Recommendations regarding physiologic tidal volume for the extremely preterm infant are lacking.

Abstract:Patient-triggered modes of ventilation are currently the standard of practice in the care of term and preterm infants. Maintaining spontaneous breathing during mechanical ventilation promotes earlier weaning and possibly reduces ventilator-induced diaphragmatic dysfunction. A further development of assisted ventilation provides support in proportion to the respiratory effort and enables the patient to have full control of their ventilatory cycle. In this paper we will review the literature on two of these modes of ventilation: neurally adjusted ventilatory assist (NAVA) and proportional assist ventilation (PAV), propose future studies and suggest clinical applications of these modes.

Abstract:Noninvasive ventilation (NIV), i.e. without tracheal intubation, has been reintroduced for the treatment of respiratory failure to reduce the complications of mechanical ventilation. Nowadays, NIV with positive pressure is the preferred method, applied through a mask held in place by a harness. Several masks can be used (nasal, bucconasal facial) and a variety of means can be used to keep them in place. Many respirators can be selected, ranging from those traditionally used in the intensive care unit(ICU) to specific NV respirators and conventional ICU respirators with specific software for NIV. Many respiratory modalities can be used according to the respirator (biphasic positive airway pressure [BIPAP], proportional assist ventilation, pressure support, synchronized intermittent mandatory ventilation [SIMV], etc.). NIV is mainly indicated in exacerbations of chronic respiratory failure: neuromuscular diseases, pretransplantation cystic fibrosis, and obstructive sleep apnea syndrome. It is also indicated in acute respiratory failure: pneumonia, status asthmaticus, and acute lung edema. The main contraindications are a weakened airway protection reflex(absent cough reflex) and hemodynamic instabiity. The advantages of NIV derive mainly from avoiding the complications associated with invasive ventilation. NIV also presents some disadvantages, especially the greater workload involved to ensure good patient adaptation to the respirator. The most common sequelae of NIV are skin lesions due to pressure on the nasal bridge.
Adaptive Pressure Control Ventilation: Set It and Don't Forget It?

Abstract:Millions of critically ill patients require mechanical ventilation each year, and for each of those patients, clinicians must choose a mode of ventilation.1 Ventilator modes are preselected patterns of controlled and dependent variables that determine how positive-pressure breaths are delivered.2,3 Many factors potentially influence how modes are selected: institutional and hospital unit culture, ventilator brand and mode availability, patient characteristics, and clinician preference all affect mode choice. Unfortunately, few data exist to inform these decisions. Most patients in routine clinical care receive a volume-targeted mode using either volume control–continuous mandatory ventilation (VC-CMV) or adaptive pressure control–continuous mandatory ventilation (APC-CMV).4,5 In VC-CMV, the ventilator delivers a set tidal volume, fixed inspiratory flow, and variable inspiratory pressure. In APC-CMV, the ventilator sets an algorithmically determined inspiratory pressure predicted to deliver a target tidal volume, and the tidal volume is not directly controlled. The ventilator adjusts the inspiratory pressure from breath to breath until the target tidal volume is achieved at the lowest inspiratory pressure. Transpulmonary Pressure-Guided Mechanical Ventilation in Severe Acute Respiratory Distress Syndrome in PICU: Single-Center Retrospective Study in North India, 2018-2021

Abstract:Objectives: In this study, we have reviewed the association between esophageal pressure-guided positive end-expiratory pressure (PEEP) setting and oxygenation and lung mechanics with a conventional mechanical ventilation (MV) strategy in patient with moderate to severe pediatric acute respiratory distress syndrome (PARDS). Design: Retrospective cohort, 2018-2021. Setting: Tertiary PICU. Patients: Moderate to severe PARDS patients who required MV with PEEP of greater than or equal to 8 cm H2O. Interventions: Esophageal pressure (i.e., transpulmonary pressure [PTP]) guided MV vs. not. Measurements and main results: We identified 26 PARDS cases who were divided into those who had been managed with PTP-guided MV (PTP group) and those managed with conventional ventilation strategy (non-PTP). Oxygenation and lung mechanics were compared between groups at baseline (0 hr) and 24, 48, and 72 hours of MV. There were 13 patients in each group in the first 24 hours. At 48 and 72 hours, there were 11 in PTP group and 12 in non-PTP group. On comparing these groups, first, use of PTP monitoring was associated with higher median (interquartile range) mean airway pressure at 24 hours (18 hr [18-20 hr] vs. 15 hr [13-18 hr]; p = 0.01) and 48 hours (19 hr [17-19 hr] vs. 15 hr [13-17 hr]; p = 0.01). Second, use of PTP was associated with higher PEEP at 24, 48, and 72 hours (all p < 0.05). Third, use of PTP was associated with lower Fio2 and greater Pao2 to Fio2 ratio at 72 hours. Last, there were 18 of 26 survivors, and we failed to identify an association between use of PTP monitoring and survival. Conclusions: In this cohort of moderate to severe PARDS cases undergoing MV with PEEP greater than or equal to 8 cm H2O, we have identified some favorable associations of oxygenation status when PTP-guided MV was used vs. not. Larger studies are required.
Noninvasive Respiratory Support for Pediatric Critical Asthma

Abstract:Pediatric asthma is a significant cause of emergency department visits and hospital admissions. While most patients respond well to standard pharmacologic treatments, those with more severe disease frequently require noninvasive respiratory support (NRS) and adjunct therapies, or admission to an intensive care unit-a condition termed critical asthma. NRS modalities include high-flow nasal cannula (HFNC), continuous positive airway pressure (CPAP), and noninvasive ventilation (NIV) to deliver standard air-oxygen mixtures or helium-oxygen (heliox). Each NRS modality offers distinct physiological benefits, primarily aimed at reducing work of breathing, enhancing gas exchange, and optimizing aerosol delivery. Despite the growing use of NRS, robust evidence supporting its efficacy in pediatric critical asthma is limited, with few published clinical trials and a heavy reliance on observational studies to inform clinical practice. This narrative review explores the current evidence, physiological rationale, practical considerations, and future research directions for the use of NRS in pediatric critical asthma. The goal is to provide clinicians with a comprehensive overview of the benefits and limitations of NRS modalities to better inform therapeutic decisions and improve patient outcomes.

Abstract:During the treatment of pediatric critical cases,the mechanical ventilation support is one of the most important factors.Neonatologists are trying to find a better mode of ventilation support.Neurally adjusted ventilatory assist (NAVA) is a new mode of mechanical ventilation controlled by diaphragmatic electrical signals.Because the mechanism of ventilation support is different from the conventional ventilators,NAVA provides a different effect of respiratory support varying with the needs of the newborn.Pediatric studies had been review to provide more information of NAVA to pediatrics.

Abstract:Invasive mechanical ventilation (MV) is one of the most practiced interventions in the intensive care unit (ICU) and is unmistakably lifesaving for children with acute respiratory failure (ARF). However, if delivered inappropriately (i.e. ignoring the respiratory system mechanics and not targeted to the need of the individual patient at a specific time point in the disease trajectory), the side effects will outweigh the benefits. Decades of experimental and clinical investigations have resulted in a better understanding of three important detrimental effects of MV. These are ventilation-induced lung injury (VILI), patient self-inflicted lung injury (P-SILI), and ventilation-induced diaphragmatic injury (VIDD). VILI, P-SILI, and VIDD have in common that they occur when there is either too much or too little ventilatory assistance.Conclusion: The purpose of this review is to give the paediatrician an overview of the challenges to prevent these detrimental effects and titrate MV to the individual patient needs.

Comparison of volume-controlled ventilation, pressure-controlled ventilation and pressure-controlled ventilation-volume guaranteed in infants and young children in the prone position: A prospective randomized study

Abstract:Study objective: To explore if the pressure-controlled ventilation (PCV) and pressure-controlled ventilation-volume guaranteed (PCV-VG) modes are superior to volume-controlled ventilation (VCV) in optimizing intraoperative respiratory mechanics in infants and young children in the prone position. Design: A single-center prospective randomized study. Setting: Children's Hospital, Zhejiang University School of Medicine. Patients: Pediatric patients aged 1 month to 3 years undergoing elective spinal cord detethering surgery. Interventions: Patients were randomly allocated to the VCV group, PCV group and PCV-VG group. The target tidal volume (VT) was 8 mL/kg and the respiratory rate (RR) was adjusted to maintain a constant end tidal CO2. Measurements: The primary outcome was intraoperative peak airway pressure (Ppeak). Secondary outcomes included other respiratory and ventilation variables, gas exchange values, serum lung injury biomarkers concentration, hemodynamic parameters and postoperative respiratory complications. Main results: A total of 120 patients were included in the final analysis (40 in each group). The VCV group showed higher Ppeak at T2 (10 min after prone positioning) and T3 (30 min after prone positioning) than the PCV and PCV-VG groups (T2: P = 0.015 and P = 0.002, respectively; T3: P = 0.007 and P = 0.009, respectively). The prone-related decrease in dynamic compliance was prevented by PCV and PCV-VG ventilation modalities at T2 and T3 than by VCV (T2: P = 0.008 and P = 0.015, respectively; T3: P = 0.015 and P = 0.014, respectively). Additionally, there were no significant differences in other secondary outcomes among the three groups. Conclusion: In infants and young children undergoing spinal cord detethering surgery in the prone position, PCV-VG may be a better ventilation mode due to its ability to mitigate the increase in Ppeak and decrease in Cdyn while maintaining consistent VT.
