Wide-angle viewing system in operation of severe proliferative diabetic retinopathy

Abstract:Objective To evaluate the value and applicability of a wide-angle viewing system (WAVS) during vitrectomy for eyes with extensive fibrovascular proliferation secondary to proliferative diabetic retinopathy (PDR). Methods In a retrospective, noncomparative, interventional study, 21 consecutive cases (28 eyes) of severe PDR with extensive fibrovascular proliferation were subjected to vitrectomy using a WAVS between Jan 2008 and December 2009 in Department of Ophthalmology of Shanghai Xinghua Hospital. The mean age of the patients was (57.1±10.7) years (range, 37-82 years). The patients were followed up for a mean of (12.5±3.9) months (range, 5-24 months). The patients underwent vitrectomy using WAVS with four-port bimanual technique, peeling of fibrovascular membrane and/or internal limiting membrane, intraocular laser coagulation, air-fluid exchange, tamponading of air and expansive gases or silicon oil were performed according to the condition of diseases. Results The procedures were finished successfully in all patients. Postoperatively, the retina was fully attached in all eyes (100%), best corrected visual acuity (BCVA) was obtained in 22 eyes (78.6%), unchanged visual acuity was found in 5 eyes (17.9%), and decreased visual acuity was found in 1 eye (3.6%). Their LogMAR was greatly improved compared with that before operation (1.10±0.43 vs. 2.20±0.76, t=6.168, P<0.01). Conclusion Application of WAVS is satisfactory in vitrectomy after severe PDR, and it provides a clear, wild-angle, and up-right surgical field, with the advantage of easy membrane peeling, electrocautery, operation performing, and panretinal photocoagulation. It is safe and practicable for complicated surgical manipulations of PDR.
