Analysis of surgical timing and therapeutic effect of vitrectomy for proliferative diabetic retinopathy combined with vitreous hemorrhage

Abstract:Objective To investigate the surgical timing and therapeutic effect of vitrectomy for proliferative diabetic retinopathy(PDR)combined with vitreous hemorrhage.Methods Retrospective analysis was taken in patients with PDR at Ⅳ stage to Ⅴstage who were taking vitrectomy in our hospital from March 2004 to March 2007.According to the time of vitreous hemorrhage,patients were divided into be and less than 3 months group(36 cases,36 eyes)and more than 3 months group(48 cases,48 eyes).All PDR patients were followed up for 22 months in average postoperatively.Best corrected visual acuity and complications at eyes were recorded.Results Postoperative best corrected visual acuity of patients in both two groups were obviously improved comparing with preoperative visual acuity.Compared with preoperative visual acuity,postoperative visual acuity improved in 33 cases(91.7%),unchanged in 1 case(2.8%)and decreased in 2 cases(5.6%),and there were 4 cases(11.1%)with vitreous hemorrhage,2 cases(5.6%)with neovascular glaucoma and 1 case with branch retinal vein occlusion in be and less than 3 months group postoperatively.Compared with preoperative visual acuity,postoperative visual acuity improved in 39 cases(81.3%),unchanged in 5 cases(10.4%)and decreased in 4 cases(8.3%),and there were 3 cases(6.3%)with vitreous hemorrhage and 2 cases(4.2%)with neovascular glaucoma in more than 3 months group.No difference was found in improving rates of postoperative visual acuity between the two groups(P>0.05).Conclusion Vitrectomy has obvious significance for the prognosis of visual acuity for patients with PDR at Ⅳ stage to Ⅴstage.
