Abstract:Objective To investigate the therapeutic effect of vitrectomy-combined phacoemulsification and intraocular lens implantation on cataract-combined proliferative diabetic retinopathy(PDR).Methods Thirty patients(40 eyes) with cataract-combined PDR who had received vitrectomy-combined phacoemulsification and intraocular lens implantation from January,2008 to December,2011 in our hospital were selected for retrospective analysis.Postoperative follow-ups varied from 6 months to 24 months,and the average was 18.5 months.A series of factors including change of visual acuity,corneal endothelial cells count and central corneal thickness were tested at the final stage of follow-ups.Intra-operative and post-operative complications were also monitored.Results Preoperative visual acuity: From light perception(LP) to finger-counting(22 eyes);0.02-0.1(12 eyes);> 0.1-0.2(6 eyes).Postoperative visual acuity: From LP to finger-counting(8 eyes);0.02-0.1(6 eyes);> 0.1-0.2(8 eyes);> 0.2-0.3(12 eyes);> 0.3(6 eyes).Postoperative visual acuity was significantly improved than pre operative visual acuity(P < 0.05).No significant difference was found between pre-and post-operative corneal endothelial cells counts,which were(2965.15 ± 328.45) μm-2and(2697.22 ± 289.65) μm-2,respectively(P >0.05).No significant difference was found between pre and post operative central corneal thickness,which were(523.25 ± 33.15) μm and(524.78 ± 15.63) μm,respectively(P > 0.05).Intraoperative complications: operational areas bleeding(6 eyes),no serious complications such as posterior capsule breakage and iatrogenic retinal tear.Post-operative complications: transient elevated intraocular pressure(8 eyes);corneal edema of varying degrees(6 eyes);fibrinous exudation in anterior chamber(4 eyes);posterior capsule opacification(4 eyes);posterior synechia(4 eyes);temporary vitreous hemorrhage(2 eyes).Conclusion Vitrectomy-combined phacoemulsification and intraocular lens implantation is a safe and effective treatment for cataract-combined PDR with more favorable postoperative visual acuity and less intra and postoperative complications.
