Rescue ICSI after Fertilization Failure in in Vitro Fertilization

Abstract:To analyze the benefit of day 1 rescue intracytoplasmic sperm injection (RICSI) of oocytes that failed to fertilize after conventional insemination. Retrospective analysis. ICSI Rescue (IR) cycles performed from Jan 2004 to July 2006 with total fertilization failure or low fertilization were reviewed. Of the 3342 cycles of standard IVF performed, 152 cycles (4.5%) involved total or near-total (<25% fertilization) fertilization failure. RICSI was performed 18–22 hrs post retrieval. 99 patients (99 cycles) with RICSI cycles who had embryos transferred only after fertilization by rescue ICSI and under 38 years old were analyzed . Among them, 20 patients underwent cryopreservation embryo transfer after RICSI (21 cycles), 45 patients had COH-ICSI cycles later. A 2PN rate of 55.2% was achieved in RICSI cycles, less than that in the 200 standard ICSI cycles (76.5%) performed during the same time period (P=0.000) and that in the 41 subsequent COH-ICSI cycles (76.2%) (P=0.000). 99 RICSI patients resulted in only RICSI embryo transfers, the clinical pregnancy rate after transfer of fresh embryos or cryopreserved embryo obtained after rescue ICSI was both significantly lower than that in subsequent COH-ICSI cysles among these patients (P=0.000). These transfers resulted in 7 live delivery (8 babies) and no birth defects were found. ICSI Rescue as a treatment for total fertilization failure or low fertilization could result in a viable pregnancy either from fresh or cryopreserved embryos obtainted though the pregnancy rate was quite low. No defects were reported. Pregnancy rate in the subsequent ICSI cycles is 36.6% if appropriate fertilization is achieved.
