Abstract:Stroke is a leading cause of death and disability world-widely. The incidence rate of stroke has been increasing due to the aging population and lifestyle changes. At present, the only drug approved by the US Food and Drug Administration (FDA) for the treatment of ischemic stroke is tissue plasminogen activator (t-PA), but its clinical application is greatly limited because of its narrow time window and bleeding risk. Natural products have a long history of being used in traditional medicine with good safety, making them an important resource for the development of new drugs. Indeed, some natural products can target a variety of pathophysiological processes related to stroke, including oxidative stress, inflammation and neuronal apoptosis. Therefore, the development of high-efficiency, low-toxicity, safe and cheap active substances from natural products is of great significance for improving the treatment alternatives of patients with stroke. This article reviews the neuroprotective effects of 33 natural compounds by searching recent related literature. Among them, puerarin, pinocembrin, quercetin, epigallocatechin-3-gallate (EGCG), and resveratrol have great potential in the clinical treatment of ischemic stroke. This review will provide a powerful reference for screening natural compounds with potential clinical application value in ischemic stroke or synthesizing new neuroprotective agents with natural compounds as lead compounds.

Abstract:Stroke occurs when a cerebral blood vessel is blocked or ruptured, and it is the major cause of death and adult disability worldwide. Various pharmacological agents have been developed for the treatment of stroke either through interrupting the molecular pathways leading to neuronal death or enhancing neuronal survival and regeneration. Except for rtPA, few of these agents have succeeded in clinical trials. Recently, with the understanding of the pathophysiological process of stroke, there is a resurrection of research on developing neuroprotective agents for stroke treatment, and novel molecular targets for neuroprotection and neurorestoration have been discovered to predict or offer clinical benefits. Here we review the latest major progress of pharmacological studies in stroke, especially in ischemic stroke; summarize emerging potential therapeutic mechanisms; and highlight recent clinical trials. The aim of this review is to provide a panorama of pharmacological interventions for stroke and bridge basic and translational research to guide the clinical management of stroke therapy. Abstract:The treatments of ischemic stroke include ultra-early thrombolysis and neuroprotective drugs.Thrombolytic therapy is restricted to the time-window,while neuroprotective drugs,despite its unlimited time constraints,are also stuck.The conventional applications of neuroprotective agents in the treatment of cerebral ischemic injuries have turned out far from satisfactory.The remarkable biases between positive results in animal stoke models and negative outcomes in clinical trials lead to general doubts about neuroprotective therapies.In the 2008 Round Table Conference on Stroke,thorough reflection and analyses were performed on these billion-costed,practically non-effective neuroprotective drugs so far.However,a few drugs have been proved neuroprotective efficiency in randomized trials despite the bleak actuality.The novel antioxidant edaravone,a free radical scavenger,is among them.As the exclusively proven free radical scavenger,edaravone has yielded favorable results in phase III clinical trials.Cerebrolysin is another drug that gains interest,which has exhibited neuroprotective and neurotrophic characteristics in various animal models of brain ischemia.Several small scale clinical control experiments have confirmed the effectiveness and safety of cerebrolysin,and the phase III clinical trials are ongoing.The directions of neuroprotection studies would probably be the time-window of neuron protective treatment and the alteration in therapeutic modes in future.

Abstract:The pathophysiological mechanisms of ischemic cerebral injury involved in the release of excitatory amino acids, depolarization about the infarction, inflammatory reaction and neuron apoptosis. The development and trial of neuroprotective drugs have almost been launched in all links of ischemic cascade reaction. Many neuroprotective drugs were proved to be effective for cerebral ischemia in animal models, but they were not proved on clinical trials. The results and progresses of the preclinical phase and clinical application of various kinds of neuroprotective drugs on the cerebral ischemia were reviewed in this paper and the prospective uses of these drugs were also introduced.

Abstract:Stroke is a central nervous system disease that seriously affects people's quality of life and has the second highest rate of morbidity and mortality in the world. At present, clinical treatment strategies for acute ischemic stroke are mainly thrombolytic and thrombectomy therapy. However, these strategies are not able to protect patients from ischemic injuries. Therefore, treatment should further focus on methods that improve the survival rate of neurons around the ischemic area and neurological function. This article reviews current research and developments regarding small molecular compounds, including natural products and newly synthesized compounds that have neuroprotective effects and potential molecular targets for the recovery of post-ischemic stroke neural function that have been identified through lab investigations conducted during the last five years. Although pre-clinical studies of these compounds may not be immediately successful, current findings suggest that these therapeutic targets and neuroprotective agents are promising.

Abstract:Many ongoing studies of neuroprotective strategy of stroke include albumin,magnesium and cryotherapy therapy.There are also a number of promising neuroprotectants in preclinical development including ginsenoside Rd,NA-1(Tat-NR2B9c),an inhibitor of postsynaptic density-95 protein etc.Thirty years' study of neuroprotective strategy of stroke experiences the development process from the peak to the trough then to new climax.However,to achieve better development,future research should focus on:conduct a strict design;the object of study should converse from dental caries animal to primate;the evaluation method should converse from behavioral assessment to radiographic evaluation;research paradigm should extend to vascular neural network.

Abstract:As the first most common cause of death in China,stroke has become a public health problem that seriously affects national economy and people′s livelihood. Unfortunately,only 3% to 5% of stroke patients receive tissue plasminogen activator(tPA)treatment,the only pharmacological therapy ap?proved for ischemic stroke,and no drug is available for hemorrhagic stroke. Therefore,there is an ur?gent need to develop new drugs for stroke therapy. Despite the awareness that neuroprotective agents could be a common strategy for the treatment of both ischemic and hemorrhagic stroke,numerous neu?roprotective agents have showed failure in clinical trials. Combined with the current therapeutic strategies and drug development of stroke,this paper elaborated the stroke injury mechanisms and corresponding clinical drug research targeting excitotoxicity,oxidative and nitrosative stress,and inflammation. From a new perspective,this paper has proposed a novel therapeutic strategy targeting inherent defense mechanisms against stroke,with nicotinamide phosphoribosyltransferase(Nampt)- nicotinamide ade?nine dinucleotide defense system as an example to present our experimental evidence that Nampt can serve as an anti-stroke target and nicotinamide mononucleotide as an anti-stroke agent under development. It is hoped that the bottleneck of stroke therapy can be overcome with unremitting efforts so as to reduce the financial burden and mental stress,and bring benefits to people around the world.

Abstract:Stroke has become the second leading cause of death in the world, and the most common type is the ischemic stroke. Due to its rapid onset and complex conditions, ischemic stroke is a major neurological disorder that causes disability. Ischemic stroke mainly results from atherosclerosis, and the pathogenesis of ischemic stroke mainly includes energy metabolism disorders in the brain, the toxicity of excitatory amino acids, oxidative/nitrification stress, inflammatory response, apoptosis, and autophagy. With the characteristics of multi-component and multi-target, traditional Chinese medicine could be used to treat ischemic stroke at different stages. This article summarized the latest research progress on the pathogenesis of ischemic stroke and commonly used traditional Chinese medicine for treatment of ischemic stroke in order to provide references for the further research and clinical treatment of ischemic stroke.

Abstract:Stroke is the second most common cause of death in the world especially in the ageing population.1 Thrombolysis and intravascular embolectomy are the main treatment and populated rapidly.2 3 However, the beneficiary is still limited because the mechanistic studies are behind. The basic research on stroke is improved because of new devices, novel techniques and the development of other disciplines.4 The update of a broad range of topics related to stroke is a widely appreciated feature. In order to improve the understanding of cellular and molecular mechanisms of stroke, the 2nd Summit of Chinese Basic Science Research on Stroke 2018 has been held in Beijing, China. Total of 60 basic stroke researchers from China and abroad attended this meeting. The meeting focused on six fields: (1) neuroprotection in acute ischaemic stroke; (2) neurorepair and regeneration; (3) neuroinflammation and neuroimmunology; (4) cognitive disorder in stroke; (5) drug/small molecule therapy for stroke; and (6) advances in optical imaging. Although 95% of published preclinical studies on ‘neuroprotective agents’ (1990–2018) reported positive outcomes in animal models of ischaemic stroke, none translated to successful phase III trials.5 Studies are therefore focusing other approaches. Abstract:Objective After analyzing the research about stroke treatment, the text focus on the mechanism of ischemic stroke injury and provide a reference for the future development of therapeutic drugs and clinical treatment. Methods The literature on the mechanism of stroke injury was searched through the database of Baidu academic, Pubmed, CNKI at home and abroad, and the mechanism of cerebral ischemia-reperfusion injury was screened and analyzed. Results Cerebral ischemia-reperfusion injury is accompanied with the treatment of ischemic stroke.The mechanisms involved in stroke include energy metabolism disorders, acidosis, oxidative / nitrotic stress, Ca2+overload, mitochondrial dysfunction, excitatory amino acids excessivly released, inflammation, blood-brain barrier damage. Conclusion The mechanism of stroke injury is extremely complicated and the research has been deep into the molecular level. Both signaling pathway and targeted therapy are hot topics in recent years.

Abstract:Stroke is a leading cause of death and disability worldwide, and approximately 87% of cases are attributed to ischemia. The main factors that cause ischemic stroke include excitotoxicity, energy metabolism disorder, Ca+ overload, oxidative damage, apoptosis, autophagy, and inflammation. However, no effective drug is currently available for the comprehensive treatment of ischemic stroke in clinical applications; thus, there is an urgent need to find and develop comprehensive and effective drugs to treat postischemic stroke. Traditional Chinese medicine (TCM) has unique advantages in treating ischemic stroke, with overall regulatory effects at multiple levels and on multiple targets. Many researchers have studied the effective components of TCMs and have achieved undeniable results. This paper reviews studies on the anticerebral ischemia effects of TCM monomers such as tetramethylpyrazine (TMP), dl-3-n-butylphthalide (NBP), ginsenoside Rg1 (Rg1), tanshinone IIA (TSA), gastrodin (Gas), and baicalin (BA) as well as effective extracts such as Ginkgo biloba extract (EGB). Research on the anticerebral ischemia effects of TCMs has focused mostly on their antioxidative stress, antiapoptotic, anti-inflammatory, proangiogenic, and proneurogenic effects. However, the research on the use of TCM to treat ischemic stroke remains incompletely characterized. Thus, we summarized and considered this topic from the perspective of pharmacokinetics, pharmacological effects, and mechanistic research, and we have provided a reference basis for future research and development on anticerebral ischemia TCM drugs.

Abstract:Stroke is the most common cerebrovascular disease worldwide,which seriously affects life quality of survivals and results in huge economic burden of families and society.In terms of clinical treatment for ischemic stroke,apart from thrombolytic therapy with recombinant tissue-type plasminogen activator (rt-PA),the occurrence and successful application of endovascular thrombectomy in patients of ischemic stroke is a major breakthrough.Meanwhile,many novel clinical drugs for ischemic stroke therapy have entered into clinical trials.Most of basic and clinical researches have showed promising results in ischemic stroke therapy.This review mainly summarizes the progress of research during the period of Twelfth Five-Year Plan for National Economic and Social Development on treatment of ischemic stroke,including omics technologies,gene therapy,microRNA (miRNA) interference and stem cell therapy.Stem cell therapy has shown great potential since many clinical trials have been completed or are ongoing.The development and mutual transformation of basic and clinical research will provide valuable and comprehensive information for the precise treatment of ischemic stroke.

Abstract:Ischemic or hemorrhagic stroke usually leads to neurological deficits. Thus, improvement of neurological behavior after stroke is an important task. And neurological behavior is an important indicator of the efficacy of most pharmacological therapies in postischemic/hemorrhagic prognosis. Potential strategies include promoting neuronal regeneration, attenuating neuronal death, mitigating neuroinflammation, and decreasing peripheral immune cell infiltration. The purpose of this special issue “Therapeutic Strategies for Poststroke Neurological Behavior: From Molecular to Cellular” is to provide an overview of the recent studies for poststroke therapies. The special issue focused on poststroke therapies both in basic and clinical researches, how they improve neurobehavioral function in both animal models and patients, and the underlying mechanisms of pharmacological interventions. This special issue published 12 selected papers regarding the above specific topics, and the details are summarized below. Q. Huang et al. did a survey on the relationship of body mass index (BMI) and migraine for Chinese adult people. J. Tian et al. used magnetic resonance imaging (MRI) to evaluate Notch signaling-mediated angiogenesis in ischemic rat. L. Wu et al. found that Biochanin A could reduce inflammatory injury and neuronal apoptosis following subarachnoid hemorrhage (SAH). R. Lan et al. proved that Xiao-Xu-Ming decoction could reduce mitophagy activation and improve mitochondrial function in cerebral ischemia and reperfusion injury. J. Qu et al. found that MST1 suppression could reduce early brain injury after subarachnoid hemorrhage in mice. G. Wang et al. proved that PPAR promoted hematoma clearance in a rat model of intracerebral hemorrhage. Y. Xu et al. reviewed the pathogenesis of necroptosisdependent signaling pathway in cerebral ischemic disease. H. Yang et al. demonstrated that histone deacetylase inhibitor could alleviate neurological dysfunction after experimental intracerebral hemorrhage in mice. D. Ke et al. proved that hypertriglyceridemia was associatedwith reduced leukoaraiosis severity in patients with small vessel stroke. L. Wen et al. performed a single-center study for the long-term efficacy and safety of carotid artery stenting among the elderly patients in China. B. Zhu et al. proved that inhibition of GRASP65 phosphorylation protected against cerebral ischemia-reperfusion injury. And moreover, L. Chen et al. studied the effect of carotid artery morphological variations on cognitive function. We guest editors expect that this special issue will provide readers available data of recent advances in poststroke therapies and may stimulate interest for further research in this area.
