Abstract:ObjectiveHigh-sensitivity C-reactive protein (hs-CRP) is an inflammatory marker. This study aimed to identify the correlation between hs-CRP levels and diabetic kidney disease (DKD) in patients with type 2 diabetes mellitus (T2DM). Materials/MethodsThis cross-sectional and observational study included 927 patients with T2DM. We collected the data of patients based on their medical data, including sociodemographic characteristics, concomitant diseases, laboratory results, and medical therapy. Multivariate logistic regression analysis was conducted to assess the relationship between hs-CRP levels and DKD. A restricted cubic spline (RCS) was used to assess the correlation of hs-CRP levels on a continuous scale with the DKD. ResultsIn total, 927 patients were recruited in our study. The median age of the recruited patients was 55 years, and there were 346 female patients and 581 male patients. The hs-CRP levels were evidently higher in patients with DKD than those without DKD. After adjusting for age, sex, diastolic blood pressure, systolic blood pressure, body mass index, neck circumference, waist circumference, hypertension, duration of diabetes, common carotid artery plaque, fasting plasma glucose, glycated hemoglobin, hemoglobin, erythrocyte, leukocyte, gamma-glutamyl transferase, albumin, urea nitrogen, uric acid and triglyceride, a significant increase in the odds ratios (ORs) for DKD in the fourth hs-CRP quartile compared with the first quartile was observed (P value for trend= 0.003), and the ORs (95% confidence intervals) in the fourth quartile of hs-CRP were 1.968 (1.244-3.114) for DKD compared to the first quartile.. Moreover, the RCS curves presented a positive association between hs-CRP and DKD in total subjects, male subjects and female subjects, respectively. ConclusionsThe results of our study indicated that hs-CRP levels were significantly and positively correlated with the presence of DKD, which may provide predictive and diagnostic values in clinical practice.

Abstract:Objective To study the clinical significance of measurement of serum cystatin C(Cysc),lipoprotein α(LPα) and high sensitivity C-reactive protein(hs-CRP) in type 2 diabetic mellitus(T2DM)patients with early renal impairment.Methods One hundred and twenty six patients were divided into three groups: 40 cases in simple diabetic group(DM),44 cases in early diabetic nephropathy group(EDN) and 42 cases in clinical diabetic nephropathy group(DN),and 70 healthy persons were chosen as normal control group.Serum levels of Cysc,LP(α) and hs-CRP were detected by BECKMAN UniCel Dxc600 automatic biochemistry analyzer.Results Serum levels of Cysc,LP(α)and hs-CRP in T2DM patients were higher than those of normal control group(P0.01),they were also significantly higher in DN group than in DM group and EDN group(P0.01).The serum levels of Cysc,LP(α)and hs-CRP were positively correlated with urinary mALB in T2DM patients.Conclusion Serum levels of Cysc,LP(α)and hs-CRP are sensitive indicators for detection of early renal impairment in patients with type 2 diabetes mellitus.

Abstract:目的探讨超敏C反应蛋白(high sensitivity C-reactive protein,hsCRP)与代谢综合征(metabolic svndrome,Ms)的相关性.方法选择2005年7月至2006年6月在浙江大学医学院附属邵逸夫医院住院体检的2453例;所有研究对象住院当天测量身高、体重、血压,于次日早晨空腹采血,用氧化物酶法检测血糖、血脂全套,采用免疫比浊法检测hsCRP,代谢综合征的诊断标准根据2004年中华医学会糖尿病学分会关于代谢综合征的建议.数据采用SPSS 13.0软件包分析,计量资料用均数±标准差((x)±s)表示,统计显著性用双侧检验,显著性水准定为α=0.05.组间差异分析应用t检验,多组间差异分析用单因素方差分析;hsCRP与代谢综合征各组分的相关性分析用斯比尔曼氏(Spearman)检验,并采用逐步回归(Stepwise)分析建立线性模型.对hsCRP发生代谢综合征的危险性用logistic分析.结果 2453例研究人群中,有代谢综合征者408例(占16.6%),代谢综合征者hsCRP水平(1.73±0.54 mg/L)与无代谢综合征者(1.46±0.58)mg/L比较,差异有统计学意义,P＜0.05;且随着代谢综合征组分的增加,hsCRP水平逐步升高(P＜0.05).相关分析显示hsCRP浓度和年龄、体重、收缩压、舒张压、空腹血糖、餐后2小时血糖、甘油三酯呈正相关,与HDL-C呈负相关.用逐步回归筛选变量,结果显示影响hsCRP水平的主要因素为体重指数、年龄、甘油三酯、高密度脂蛋白、舒张压,P＜0.05.其回归方程如下:y=0.172+0.237a+0.123b-0.111c+0.052d+0.045e.在调整年龄、性别及吸烟等影响因素后,hsCRP与代谢综合征的发生显著相关,(OR=1.126,P＜0.001).结论代谢综合征者hsCRP水平高于无代谢综合征者;且随着代谢综合征组分的增加,hsCRP水平逐步升高.hsCRP水平的升高是发生代谢综合征的危险因素。

Abstract:Background To evaluate the association between high-sensitivity C-reactive protein (hsCRP) and erythrocyte sedimentation rate (ESR), and diabetic kidney disease (DKD) in patients with type 2 diabetes mellitus (T2DM). Methods A cross-sectional study was conducted in 1210 patients with T2DM, among whom 265 had DKD. The severity of DKD was assessed by estimated-glomerular filtration rate (eGFR) and urinary albumin creatinine ratio (ACR). The relationship between ESR, hsCRP and DKD was analyzed by multivariate logistic analysis. The relationship between ESR and eGFR, ESR or ACR was analyzed by multivariate linear regression. Results ESR (23.0 [12.0 ~ 41.5] mm/h versus 12.0 [7.0 ~ 22.0] mm/h, P < 0.001) and hsCRP (3.60 [2.20 ~ 7.65] versus 2.90 [1.80 ~ 5.60] mg/L mg/L, P < 0.01) values were significantly higher in patients with DKD than those without. Patients with higher ESR or hsCRP had lower eGFR and higher ACR. After adjusted for gender, age, hemoglobin, plasma proteins, HbA 1c , lipid profiles, and the usage of renin-angiotensin system inhibitors, ESR but not hsCRP was independently associated with the rate and severity of DKD in patients with T2DM. Conclusion ESR was independently associated with the rate and severity of DKD in patients with T2DM.

Abstract:ObjectiveTo study the relationship of serum homocysteine(Hcy),lipoproteinα(Lpα),cystatinC(CysC),high sensitivityC-reactive protein(hsCRP) with diabetic nephropathy(DN).Methods 192 patients with DM were determined 24h urinary albumin excretion rate(UAER),serum HCY、Lpα、CysC、hsCRP levels,according to urinary albumin excretion rate were divided into normal albuminuria group(NA) and microalbuminuria group(MA),clinical proteinuria group(CP),statistics of each group of HCY、Lpα,CysC,hsCRP,levels;120 healthy individuals were normal controls.Results Levels of HCY,Lpα,CysC,hsCRP in DM groups were higher than that of normal controls.HCY,Lpα,CysC,hsCRP levels in every group of DM was significantly difference(P < 0.05).HCY,Lp(α),CysC,hsCRP was significantly correlated with UAER respectively(r = 0.645,0.578,0.681,0.693,P < 0.05).Conclusion Serum HCY,Lp(α),CysC,hsCRP levels in DM increased with the increase of UAER may be related to the progression of DN,and they were valid indicators of diagnosis of early renal damage as UAER,Combined detection of them had important clinical significance in diagnosising and evaluating type 2 diabetic nephropathy.
Serum N-3 Polyunsaturated Fatty Acid and High-sensitivity C Reactive Protein in Patients with Newly Diagnosed Type 2 Diabetes Mellitus

Abstract:Objective To investigate the serum n-3 polyunsaturated fatty acid and its relationship with high-sensitivity C reactive protein in patients with newly diagnosed type 2 diabetes mellitus.Methods Serum phospholipid fatty acid profile was analyzed with capillary gas chromatography.Immunolurbidimelric assay was used to measure the serum high-sensitivity C reactive protein (hs-CRP).Results The levels of n-3 Polyunsaturated fatty acid(n-3PUFA) was lower in newly diagnosed type 2 diabetes mellitus group(T2DM)than those in NC group(6.08%±1.66% vs 9.54%±51.54%,P0.01).The serum hs-CRP cocentration was significantly higher in T2DM group than that in NC group(7.26±1.34mg/L vs 0.83±0.42mg/L,P0.01).Hs-CRP cocentration was negatively correlated with n-3PUFA(P0.05).Conclusion Serum n-3 PUFA is lower in newly diagnosed T2DM patients than that in healthy people.Serum hs-CRP cocentration was negatively correlated with n-3PUFA.
Value of associated detection of serum CysC,Hcy and hs-CRP in diagnosis of early diabetic nephropathy

Abstract:Objective To explore the value of associated detection of serum cystatin C(CysC),homocysteine(Hcy) and high sensitive C-reactive protein(hs-CRP)in diagnosis of early diabetic nephropathy.Methods 86 patients with type 2 diabetes mellitus(T2DM) were divided into DM group (40 cases) without diabetic nephropathy and DN group (46 cases) with diabetic nephropathy and other healthy physical subjects (40 cases) were as control group.Serum CysC,Hcy and hs-CRP were examined by automatic biochemical analyzer,and urinary albumin(mALB) was detected for comparison.Results In type 2 diabetes group,CysC,Hcy,hs-CRP and mALB were significantly higher than those in the normal control group(P0.01) and the positive rate of combined detection for CysC,Hcy and hs-CRP was 91.9%,significantly higher than that of single test.Conclusion The levels of CysC,Hcy and hs-CRP are increased with mALB increase in the DN patients,indicating which may be related to the progress of DN and could be increased with the kidney damage being increased gradually in diabetic nephropathy,being as the new risk factors.They might be a predictor of early diabetic renal injury and renal damage.CysC,Hcy and hs-CRP are the sensitive indicators of early kidney damage,and their joint detection will be more sensitive.

Abstract:Objective To investigate the association of hsCRP with the endothelial dependent vasodilatation function in T2DMpatients.Methods Compared the level of hsCRP and endothelial dependent vasodilatation in T2DM patients with or without subclinical atherosclerosis(SAS).Results It was showed significant difference of percentage of diameter change of reactive hyperemia in three groups,percentage of diameterchange of reactive hyperemia in diabetics was higher than that of control group(P<0·05).There were significantly negative correlation between hsCRP and percentage of diameter change of reactive hyperemia in T2DM(P< 0·05).Conclusion Our findings suggest a negative correlation between hsCRP and endothelial dependent vasolilatation function in T2DM.
Urine Haptoglobin/Creatinine Ratio Correlates with Tubular Injury Biomarkers and Severity of Albuminuria in Type 2 Diabetes Patients

Abstract:Abstract This aim of this study was to investigate the correlation between urine haptoglobin/creatinine ratio(UHCR) and tubular injury biomarkers or severity of albuminuria in type 2 diabetes. Recruited T2DM patients (n=120) were divided into three groups based on urine albumin/creatinine ratio (UACR): normal albuminuria (Normo group, UACR<30 mg/g, n=40), microalbuminuria (Micro group, 30 mg/g ≤UACR<300 mg/g, n=38), and macroalbuminuria (Macro group, UACR ≥300 mg/g, n=42), with matched healthy individuals (NC group, n=30) as controls. ELISA assay was used to detect the levels of urine haptoglobin, NGAL and KIM-1. Automated biochemical analyzer was applied to detect the concentrations of urine creatinine and retinol binding protein (RBP). To eliminate concentration errors, urine NGAL, KIM-1, and RBP were normalized by creatinine and expressed as UHCR, UNCR, UKCR, and URCR, respectively. We found UHCR was significantly increased in T2DM patients as compared with NC group. Among the three diabetic groups, the Macro group had the highest level of UHCR, whereas the Normo group had the lowest level. T2DM patients with higher UHCR levels also had higher levels of UNCR, UKCR, and URCR. Spearman’s correlation analysis indicated that UHCR was positively correlated with UACR, UNCR, UKCR, or URCR levels, and negatively correlated with eGFR. ROC curve analysis showed that UHCR was a sensitive and specific indicator for early diagnosis of DN. Stepwise multiple regression analysis showed that UHCR and UNCR were independent variables for UACR. Our research demonstrated that UHCR correlates with tubular injury biomarkers, including UNCR, UKCR, and URCR, and is independently associated with the severity of albuminuria in T2DM.

Abstract:Objective We assayed the variation of high-sensitivity C-reactive protein(hs-CRP)in hypertensives and normotensives to explore the relationship between hs-CRP and essential hypertension.Methods Blood pressure,hs-CRP,total cholesterol,high-density lipoprotein cholesterol,total triglycerides,fasting blood glucose,uric acid,and blood urea nitrogen were measured in 218 hypertensives and 206 age,sex,area-matched normotensives.Results The levels of hs-CRP were significantly higher in hypertensives than that in normotensives [(2.0±3.0)vs(0.1±0.2)mg/L,P<0.01].Log-transformed hs-CRP was positively correlated with age(r=0.156),body mass index(r=0.551),blood urea nitrogen(r=0.158),uric acid(r=0.385),fasting blood glucose(r=0.320),total triglycerides(r=0.317,all P<0.01),whereas negatively with high-density lipoprotein cholesterol(r=-0.317,P<0.01).Receiver operator characteristic(ROC)curve analysis revealed that a cutoff value of 0.285 mg/L for hs-CRP conferred the highest sum of sensitivity(90.7%)and specificity(89.3%)in predicting the essential hypertension risk.Logistic regression analysis showed that hs-CRP remains as an independent risk factor for essential hypertension after adjusting for age,body mass index,total triglycerides,fasting blood glucose,uric acid,blood urea nitrogen and high-density lipoprotein cholesterol.Conclusions Serum hs-CRP levels were increased in patients with essential hypertension,suggesting that hs-CRP associated with essential hypertension.Serum hs-CRP level is an independent risk factor for essential hypertension.
Relationship of High-Sensitivity C-Reactive Protein Concentrations and Systolic Heart Failure

Abstract:Excessive activated proinflammatory cytokines may promote extracellular matrix alterations which induce adverse left ventricular remodeling in systolic heart failure (SHF). We sought to identify whether high-sensitivity C-reactive protein (hsCRP) levels were independently associated with SHF.In our retrospective case-control study, 2236 subjects were included, and 260 patients had SHF. Blood sample collection, clinical laboratory tests, electrocardiogram and echocardiography examinations were performed. The questionnaires were completed by professional interviews.In 2236 subjects, the prevalence rate of SHF were 1.7, 1.8, 8.4 and 32.6% between hsCRP concentrations (<1 mg/L, ≥1 to <3 mg/L, ≥3 to <10 mg/L and ≥10 mg/L, respectively) (p=0.000). hsCRP concentrations (<1 mg/L, ≥1 to <3 mg/L, ≥3 to <10 mg/L and ≥10 mg/L) were associated in a linear trend with N-terminal pro-brain natriuretic peptide (NT-proBNP, p=0.000) and left ventricular ejection fraction (LVEF, p=0.000). hsCRP was also significantly related to NT-proBNP, LVEF and SHF (r=0.232, p=0.000; r=-0.358, p=0.000 and r=0.413, p=0.000, respectively). In logistic regression model, after adjusting for heart failure risk factors, compared with the low concentration of hsCRP (<1 mg/L), the high concentration of hsCRP (≥10 mg/L) was significantly independently associated with SHF (odds ratio = 10.78 [1.303 to 89.10], p=0.027).Low to high concentration of hsCRP showed a linear trend association with SHF. A high concentration of hsCRP was independently associated with SHF.
Hsc-Rp AS a Predictor of Subclinical Atherosclerosis in Newly Diagnosed Type 2 Diabetes Mellitus

Abstract:Objective To explore the relationship between high-sensitive C-reactive protein (hsC-RP) and the incidence of subclinical atherosclerosis (AS) in newly diagnosed type 2 diabetes. Methods A prospective study was designed to investigate the changes of hsC-RP level after multifactorial intervention of hyperglycemia, hypertension, dyslipidemia and aspirin anticoagulation in 161 newly diagnosed type 2 diabetics. Results Baseline hsC-RP level was significantly higher in patients with AS than that in patients without AS [1.34 (0.09～6.34) mg/L vs 0.56(0.04～7.84) mg/L, P<0.05]. The hsC-RP level was significantly lower after one-year intervention than at baseline in patients without AS, but not in patients with AS. The incidence of AS was higher in group 4 than in the other three groups (30.0% vs 10.0%, 11.1% and 9.8% respectively, P=0.029). Conclusions The higher hsC-RP level may predict the risk of subclinical AS in newly diagnosed type 2 diabetics with multifactorial intervention.
Urinary RBP As an Independent Predictor of Renal Outcome in Diabetic Nephropathy.

Abstract:Background. Renal tubular impairment is prevalent in diabetic nephropathy (DN) and the histological severity predicted renal outcome. Biomarkers of tubular injury also increased in the urine of DN patients. The retrospective study aimed to assess the prognostic value of clinically widely applied urinary tubular injury markers, retinol-binding protein (RBP), beta 2-microglobulin (beta 2-MG) and N-acetyl-beta-D-glucosaminidase (NAG) in DN. Method. A total of 305 patients with biopsy-proven DN were enrolled. The baseline urine total protein and components including albumin, IgG, RBP, beta 2-MG and NAG were retrieved from medical records. The primary outcome was end stage renal disease (ESRD). Cox proportional hazard analysis and restricted cubic splines were performed to evaluate the association of parameters with ESRD. Nomograms were constructed and concordance index (C-index) was used to measure the prediction ability. Result. The levels of urinary RBP, beta 2-MG and NAG were positively correlated with the severity of interstitial fibrosis and tubular atrophy (IFTA). Positive correlations were also observed among beta 2-MG, NAG and mesangial expansion. Urinary RBP was not correlated with any glomerular lesions. Urinary RBP, beta 2-MG and NAG were risk factors for ESRD in hazard analysis with adjustment for age, gender and body mass index (BMI). The hazard ratios increased with the increment of baseline levels. In the multivariate Cox model including serum creatinine (SCr), total urinary protein, urinary albumin, urinary IgG and the tubular injury biomarkers, urinary RBP (with every g/mol.Cr increase: HR 1.06, 95% CI 1.03-1.10, p =0.001) remained as an independent risk factor for ESRD in DN patients. Patients were divided by the medium value of urinary RBP into the low RBP and high RBP groups. Survival analysis showed that significantly more patients in the high RBP progressed to ESRD compared to those in the low RBP group (p =0.02) when urinary total protein was less than 3.5 g/g. The C-index of the nomogram incorporating age, gender, BMI, SCr and total urine protein was 0.757. The value increased to 0.777 after adding urinary RBP to the model. Conclusions. Urinary RBP excretion was only correlated with the severity of IFTA and independently predicted ESRD in DN patients.
Relationship Between Serum High Sensitive C-reactive Protein and Vascular Dysfunction in Hemodialysis Patients

Abstract:Objective:To explore the relationship between high sensitivity C-reactive protein (hs-CRP) and vascular access failure in hemodialysis patients.Methods:From July 2017 to June 2018, 105 patients with regular hemodialysis were selected from the blood purification center of Beijing Haidian hospital.According to the presence or absence of VAF, the patients were divided into two groups: VAF+ group and VAF - group.The difference of hs CRP level between the two groups was compared.Results:During the 12-month follow-up, 17 patients had more than one event of vascular failure.The total protein level of VAF+ group was significantly lower than that of VAF-Group((65.61±4.01) g/L and( 68.98±5.66) g/L, P=0.021). The male proportion of VAF+ group was lower than that of VAF-group (29.41% vs 53.41%, P=0.07), but the difference was not statistically significant.There was no significant difference in the annual change level of hs CRP between VAF + group and VAF Group((0.33±5.49) mg/L and(0.57±4.45) mg/L, P=0.840). The level of total protein was the independent protective factor of VAF( OR=0.889, 95% CI 0.797-0.991, P=0.034). The level of hs CRP was not significantly correlated with vascular access events( OR=1.018, 95% CI 0.926-1.119, P=0.713). Conclusion:There is no correlation between hs CRP and VAF.The exact mechanism of inflammation in intimal hyperplasia needs further study.
Investigation of the Serum Interleukin-1 Receptor Antagonist,interleuleukin-1Β and High-Sensitivity C-reactive Protein Levels in Type 2 Diabetic Patients with Nephropathy

Abstract:[Objective] To investigate the serum interleukin-1 receptor antagonist (IL-Ra), interleuleukin-1β (IL-1β) and high-sensitivity C-reactive protein (hsCRP) levels in type 2 diabetic patients with nephropathy (DN), and to provide the basis for understanding of the pathogenesis process of DN. [Methods] This study included 87 patients with type 2 diabetes mellitus and 30 healthy subjects, and diabetic patients were further divided into three subgroups according to their urinary album in excretion rate (UAER): 33 patients have normal UAER (T2DM group), 28 patients have micro-albuminura (Pre-DN group) and 26 patients have clinical proteinura (clinical DN group). Serum IL-Ra and IL-1β levels were also measured by enzyme- linked immunosorbent assay (ELISA). [Results] ①Four groups were matched in age, sex and BMI (P 0.05). ②Among four groups, the levels of serum IL-Ra, IL-1βand hsCRP were highest in clinical DN group [(387.4±55.3) pg/mL, (61.3±12.1) pg/mL and (8.26±1.87) mg/L, respectively, P 0.05~0.001]. The levels of serum IL-Ra, IL-1βand hsCRP in T2DM group, Pre-DN group and healthy subjects group were (201.7±67.8) pg/mL, (15.8±6.0) pg/mL and (3.18±1.21) mg/L; (219.3±57.2) pg/mL, (29.1±8.1) pg/mL and (5.71±1.57) mg/L; (167.1±53.4) pg/mL, (12.4±2.6) pg/mL and (1.10±0.19) mg/L, respectively. and it is significantly higher in the levels of serum IL-1βand hsCRP in the Pre-DN group than those in T2DM group and healthy subjects group. ③IL-1 Ra/IL-1β ratio was gradually decreased with disease progression. ④Correlative analyses revealed a positive correlation between serum IL-1Ra, IL-1β or hsCRP levels and UAER (r =0.576, 0.453 and 0.782, respectively,P 0.01~0.001), and a negative correlation between IL-1 Ra/IL-1β ratio and UAER in type 2 diabetic patients with nephropathy(r =-0.376, P 0.01). [Conclusion] The diabetic nephropathy patients have higher serum IL-Ra,IL-1βand hsCRP levels, which correlated with the degree of diabetic nephropathy.
Association of high-sensitivity C-reactive protein and diabetic nephropathy in patients with type 2 diabetes: a Mendelian randomization study

Abstract:Introduction Observational studies support the relationship between C-reactive protein (CRP) level and diabetic nephropathy (DN) in patients with diabetes. The research question regarding whether the relationship between serum high-sensitivity C-reactive protein (hsCRP) level and DN is causal lacks experimental evidence. Therefore, this study aimed to evaluate the causality between hsCRP and DN based on Mendelian randomization (MR) analysis. Research design and methods A total of 2332 participants with type 2 diabetes from the Taiwan Biobank database was analyzed. Genetic risk scores (GRSs), which comprise four validated CRP loci as two instrumental variables, were calculated as unweighted and weighted scores to evaluate the causal relationship of hsCRP with DN risk. The two-stage regression model was used to estimate OR and 95% CI. Results The analyses of the observational study showed that the hsCRP level was significantly associated with DN after multivariate adjustment (adjusted OR 1.15; 95% CI 1.01 to 1.32). Unweighted/weighted GRSs for log-transformed hsCRP satisfied MR assumptions 1 and 3, respectively; that is, a significant association with hsCRP was observed but that with DN was absent (adjusted OR 1.00, 95% CI 0.92 to 1.09; 1.00, 0.72 to 1.39, respectively). The MR analyses demonstrated that a 1-unit increase in the log-transformed genetically predicted hsCRP by unweighted and weighted GRSs was associated with DN, demonstrating ORs of 1.80 (95% CI 1.51 to 2.14) and 1.67 (95% CI 1.40 to 1.98), respectively. Conclusions The current study provided experimental evidence that hsCRP level was causally related to DN. These findings suggest that the elevated hsCRP may be a causal risk factor for DN in patients with type 2 diabetes.

A Study on the Relationship between Serum hs-CRP Level and the Risk of Cardiovascular Complications in Patients with Chronic Renal Disease

Abstract:Objective: To explore the relationship between serum high sensitivity C reactive protein (hs-CRP) and risk of cardiovascular complications in patients with chronic kidney disease (CKD). Methods: 82 patients with CKD and 21 healthy subjects were selected as the subjects. According to the glomerular filtration rate, the patients with CKD were divided into group CKDl to 2, group CKD3 to group 4, group CKD5, and the healthy control group. The serum C reactive protein, pro-BNP, urea nitrogen, uric acid, creatinine, prealbumn, albumin, homocysteine, triglyceride, cholesterol, high density lipoprotein, low density lipoprotein, calcium, phosphorus, and hemoglobin levels were detected and compared between different groups. Patients with myocardial ischemia and ventricular hypertrophy and cardiac valve calcification were also evaluated. Results: With the decrease of e GFR, the level of hs-CRP showed an upward trend, and there were significant differences in the serum hs-CRP levels between different groups (P＜0.01). There were significantly positive correlation between hs-CRP and blood urea nitrogen, creatinine, phosphorus, uric acid, triglyceride, homocysteine, pro-BNP (P＜0.05);there were significantly negative correlation between hs-CRP and serum albumin, hemoglobin, calcium, high density lipoprotein (P＜0.05); the correlation between hs-CRP and prealbumin, cholesterol low density lipoprotein showed no statistical significance (P＞0.05).Hs-CRP was used as dependent variable, and other related indicators were used as independent variables for multiple stepwise regression analysis, the results showed that uric acid, Hb and Hcy entered multiple stepwise regression equation. Myocardial ischemia and valvular calcification were used as dependent variables. Hs-CRP was used as independent variable to do logistic regression analysis, the results showed that serum hs-CRP level was a risk factor for myocardial ischemia and valve calcification (OR＞1). Conclusions: The increase of serum hs-CRP level in CKD patients is closely related to the decrease of renal function, and it is a risk factor for myocardial ischemia and cardiac valve calcification.

Abstract:BACKGROUND:Evidences show that subclinical chronic inflammation is involved in the pathogenesis of diabetic nephropathy. The aim of this study was to examine the relationship between serum C-reactive protein (CRP), serum uric acid, and albuminuria in Chinese type 2 diabetic patients.METHODS:A total of 1162 type 2 diabetic patients were recruited. All participants had relevant clinical and laboratory measurements. CRP was measured using a particle enhanced immunoturbidimetric assay.RESULTS:In the multiple linear regression model, natural log-transformed CRP (lnCRP) and uric acid were independent predictors of natural log-transformed urinary albumin to creatinine ratio (lnACR) (β = 0.18, 95% CI 0.10-0.27, P < 0.001 and β = 0.18, 95% CI 0.09-0.27, P < 0.001). The interaction of lnCRP with uric acid was also associated with lnACR (β = 0.04, 95% CI 0.02-0.06, P < 0.001). In the full-adjusted logistic regression model, the OR for albuminuria of the patients in the third tertile levels of CRP and uric acid was 3.94 compared with patients in the first tertile levels of CRP and uric acid (95% CI 1.73-8.94, P < 0.001).CONCLUSIONS:Elevated serum CRP and increased serum uric acid level were associated with albuminuria in Chinese type 2 diabetic patients. Moreover, CRP and uric acid had an interactive effect on albuminuria.
