Abstract:Backgrounds: Although worldwide evidence tends to prove that diabetes adversely affects periodontal health, there are insufficient clues indicating whether periodontitis may aggravate metabolic control and systemic inflammation. This study, as a preliminary part of an ongoing research project, aims to clarify the relationship of periodontal parameters with metabolic levels and systemic inflammatory markers in patients with diabetes.Methods: A total of 140 qualified, adult patients with type 2 diabetes and periodontitis were recruited into this study. Periodontal examinations, including a full-mouth assessment of probing depths (PDs), bleeding on probing, gingival recession, and clinical attachment level, were determined. Blood analyses were carried out for glycated hemoglobin (HbA1c), fasting glucose, high-sensitivity C-reactive protein (hsCRP), tumor necrosis factor-alpha (TNF-alpha), and lipid profiles. Subjects were divided into three groups according to tertiles of the mean PD and compared.Results: Upon an analysis of covariance, subjects with an increased mean PD had significantly higher levels of HbA1c and hsCRP (P <0.05). No significant difference was found among different groups in the levels of serum TNF-alpha, fasting glucose, and lipid profiles (P >0.05). After controlling for age, gender, body mass index, duration of diabetes mellitus, smoking, regular physical exercise, and alcohol consumption, positive correlations were found between mean PD and HbA1c (r = 0.2272; P= 0.009) and between mean PD and hsCRP (r = 0.2336; P= 0.007). After adjustment for possible confounders, the mean PD emerged as a significant predictor variable for elevated levels of HbA1c and hsCRP (P<0.05).Conclusion: Chronic periodontitis was associated with glycemic metabolic and serum hsCRP levels in patients with type 2 diabetes. Serum N-3 Polyunsaturated Fatty Acid and High-sensitivity C Reactive Protein in Patients with Newly Diagnosed Type 2 Diabetes Mellitus

Abstract:Objective To investigate the serum n-3 polyunsaturated fatty acid and its relationship with high-sensitivity C reactive protein in patients with newly diagnosed type 2 diabetes mellitus.Methods Serum phospholipid fatty acid profile was analyzed with capillary gas chromatography.Immunolurbidimelric assay was used to measure the serum high-sensitivity C reactive protein (hs-CRP).Results The levels of n-3 Polyunsaturated fatty acid(n-3PUFA) was lower in newly diagnosed type 2 diabetes mellitus group(T2DM)than those in NC group(6.08%±1.66% vs 9.54%±51.54%,P0.01).The serum hs-CRP cocentration was significantly higher in T2DM group than that in NC group(7.26±1.34mg/L vs 0.83±0.42mg/L,P0.01).Hs-CRP cocentration was negatively correlated with n-3PUFA(P0.05).Conclusion Serum n-3 PUFA is lower in newly diagnosed T2DM patients than that in healthy people.Serum hs-CRP cocentration was negatively correlated with n-3PUFA.

Abstract:Objective: Recent evidence suggests that poor glycemic control is significantly associated with the development of macrovascular complications of diabetes. Studies have indicated that C-reactive protein (CRP) is an important risk factor for cardiovascular disease. The purpose of this study was to determine the relation between CRP and HbA(1c) in a large national sample of individuals with diabetes. Research design and methods: A nationally representative sample of noninstitutionalized U.S. adults aged 17 years and over with nongestational diabetes was derived from the National Health and Nutrition Examination Survey III (1988-1994) (n = 1,018). Respondents with diabetes were stratified by HbA(1c) level. The main outcome measure was elevated (>0.30 mg/dl) CRP. Results: In unadjusted analyses, respondents with diabetes who had elevated HbA(1c) levels (> or =9.0%) had a significantly higher percent of elevated CRP than people with low (<7%) HbA(1c) levels (P < 0.001). In adjusted regression analysis, after controlling for age, race, sex, smoking, length of time with diabetes, insulin, and BMI, HbA(1c) was significantly associated with an increased likelihood of elevated CRP for HbA(1c) >9.0% (OR 2.15, 95% CI 1.07-4.32) and for HbA(1c) >11.0% (4.40, 1.87-10.38). Higher HbA(1c) also predicted elevated CRP in the regression model when HbA(1c) was analyzed as a continuous variable (1.20, 1.07-1.34). Conclusions: In this study, the likelihood of elevated CRP concentrations increased with increasing HbA(1c) levels. These findings suggest an association between glycemic control and systemic inflammation in people with established diabetes.

Abstract:There is interest in using blood C-reactive protein (CRP) to predict adverse prognosis outcomes patients with type 2 diabetes. This meta-analysis aimed to investigate the association between elevated baseline CRP level and unfavorable outcomes in type 2 diabetes patients. PubMed and Embase databases were systematically searched for studies on the association of elevated baseline CRP level with cardiovascular mortality and all-cause mortality from their inception to July 2018. Pooled risk ratio (RR) with 95% confidence intervals (CI) was calculated for the highest versus the lowest CRP level. Six prospective cohort studies and two post hoc analyses of randomized controlled trials involving 22,322 type 2 diabetes patients were included. Meta-analysis indicated that type 2 diabetes patients with the highest CRP level had a greater risk of all-cause mortality (RR 2.03; 95% CI 1.49–2.75) and cardiovascular mortality (RR 1.76; 95% CI 1.46–2.13). Subgroups analysis indicated that the increased cardiovascular and all-cause mortality risk was consistently found in different study design, follow-up duration or patients with or without cardiovascular risk/established cardiovascular disease subgroups. This meta-analysis indicates that elevated baseline serum CRP level is independently associated with future cardiovascular and all-cause mortality in type 2 diabetes patients.
Relationship of Serum C Reactive Protein Level with Serum Leptin Concentration in Type 2 Diabetic Patients

Abstract:Objective To explore the association of serum high-sensitivity C reactive protein (hs-CRP) level with leptin in patients with type 2 diabetes mellitns (T2DM). Methods 35 cases of T2DM patients were involved in the study and 16 normal subjects were taken as controls. Fasting serum leptin and CRP were measured .The relationship between them were also analyzed. Results Fasting serum hs-CRP concentration in T2DM group was higher than that in healthy control group, but serum ieptin levels is the same (P > 0.05). Partial correlation analysis demonstrated that fasting hs-CRP level was positively correlated with leptin (r =0.512 ,P =0. 018). Conclusion Fasting serum hs-CRP level is significantly raised in T2DM ,which is associated with leptin. There is chronic inflammatory reaction in T2DM patients,and leptin is possibly involved in inflammatory pathogenesis of T2DM.

Abstract:ObjectiveHigh-sensitivity C-reactive protein (hs-CRP) is an inflammatory marker. This study aimed to identify the correlation between hs-CRP levels and diabetic kidney disease (DKD) in patients with type 2 diabetes mellitus (T2DM). Materials/MethodsThis cross-sectional and observational study included 927 patients with T2DM. We collected the data of patients based on their medical data, including sociodemographic characteristics, concomitant diseases, laboratory results, and medical therapy. Multivariate logistic regression analysis was conducted to assess the relationship between hs-CRP levels and DKD. A restricted cubic spline (RCS) was used to assess the correlation of hs-CRP levels on a continuous scale with the DKD. ResultsIn total, 927 patients were recruited in our study. The median age of the recruited patients was 55 years, and there were 346 female patients and 581 male patients. The hs-CRP levels were evidently higher in patients with DKD than those without DKD. After adjusting for age, sex, diastolic blood pressure, systolic blood pressure, body mass index, neck circumference, waist circumference, hypertension, duration of diabetes, common carotid artery plaque, fasting plasma glucose, glycated hemoglobin, hemoglobin, erythrocyte, leukocyte, gamma-glutamyl transferase, albumin, urea nitrogen, uric acid and triglyceride, a significant increase in the odds ratios (ORs) for DKD in the fourth hs-CRP quartile compared with the first quartile was observed (P value for trend= 0.003), and the ORs (95% confidence intervals) in the fourth quartile of hs-CRP were 1.968 (1.244-3.114) for DKD compared to the first quartile.. Moreover, the RCS curves presented a positive association between hs-CRP and DKD in total subjects, male subjects and female subjects, respectively. ConclusionsThe results of our study indicated that hs-CRP levels were significantly and positively correlated with the presence of DKD, which may provide predictive and diagnostic values in clinical practice.

Abstract:Objective To study the changes of retinol binding protein-4(RBP-4) concentrations in patients with type 2 diabetes mellitus(T2DM) and its relationship with glucose and lipid metabolism.Methods Fifty-two patients with T2DM(DM group) and thirty health volunteers(control group) were detected fasting plasma glucose(FPG),total cholesterol(TC),triacylglycerol(TG),high density lipoprotein-cholesterol(HDL-C),low density lipoprotein-cholesterol(LDL-C),free fat acid(FFA),glycosylated hemoglobin(HbA1c),fasting insulin(FINS) and RBP-4,and were calculated the body mass index(BMI),the waist-hip-ratio(WHR) and HOMA-insulin resistance index(HOMA-IR).Results BMI,WHR,FPG,TC,TG,LDL-C,FFA,FINS,HbA1c,RBP-4 and HOMR-IR were significantly higher in DM group than those in control group(P0.05),while HDL-C was lower in DM group than that in control group(P0.05).RBP-4 was positively correlated with FPG,BMI,TG,TC,FFA,HOMA-IR(P0.05).HOMA-IR and FFA were the strongest influence factors of RBP-4 concentration.Conclusion The increased concentration of RBP-4 is correlated with the glucose and lipid metabolism,which may lead to insulin resistance and contribute to the onset and the development of T2DM.

Abstract:Background: This study aimed to investigate the relationship between retinol-binding protein 4 (RBP4) and gestational diabetes mellitus (GDM). Methods: Seventy-six women with and without GDM were recruited. Their blood samples were collected to detect RBP4, fasting plasma glucose (FPG), fasting insulin (Fins), triglyceride (TG), total cholesterol (TC), low-density lipoprotein, high-density lipoprotein (HDL) and glycosylated hemoglobin (HbA1c) levels. Results: RBP4 (21.42 ± 3.846 vs. 39.08 ± 8.293 μg/ml), FPG, Fins, homeostasis model assessment of insulin resistance (HOMA-IR), HbA1c, and TG levels were higher, while HDL levels were lower in women with GDM (p < 0.01). In healthy controls, RBP4 concentrations were positively correlated with HOMA-IR and TG and inversely correlated with FPG and HDL (p < 0.05). Serum concentrations of RBP4 in women with GDM were inversely correlated with TC and positively correlated with maternal weight gain during pregnancy (p < 0.05). The ROC curve was drawn with a correct rate of 93.4%. Conclusions: Concentrations of serum RBP4 were significantly higher in women with GDM, suggesting that elevated RPB4 level may play a role in the pathogenesis of GDM. Meanwhile, RBP4 might be a good predictor of GDM. RBP4 is correlated with TG and HDL, indicating that RBP4 plays a role in alterations of lipid metabolism in pregnant women.

Abstract:Aim: To observe the levels of C-reactive protein (CRP) and interleukin-6 (IL-6) in the newly onset type 2 diabetics and the effects of glucose control on CRP and IL-6. Methods: (1) Forty-eight newly onset type 2 diabetics (22 males and 26 females) in the Department of Endocrinology, Jiangsu Provincial People's Hospital from February to June 2003 were enrolled with the average age of (50±7) years old and body mass index (BMI) of (25.64 ±2.92) kg/m2. Knowing and agreeing with the therapeutic regimen well, all the patients whose diagnosed meeting the criterion of American Diabetes Association (ADA) took the oral hypoglycemia drug complex metformin 0.25-0.5 g, 3 times a day for 3 months to control their blood glucoses. Serum levels of CRP and IL-6 were assayed with chemiluminescence immunometric assay and enzyme linked immunosorbent assay (ELISA), respectively before and after the glucose control. (2) Thirty healthy subjects (13 males and 17 females) on the clinic for health examination of Jiangsu Province Official Hospital worked as controls with the average age of (51 ±7) years old and BMI of (25.06±3.10) kg/m2, fasting blood glucose (FBG) and P2hBG both were in normal range. They all knew the result of identification. Good comparabilities of age, sex, BMI, blood pressure and blood lipid were available in two groups. (3) As both CRP and IL-6 were in positive skewness distribution, the data of CRP and IL-6 were given as median and quartile, analyzed by logarithm. (4) The Student's t test analysis was used in the comparison of groups. Results: All the 48 blood samples of diabetes and 30 controls were obtained and involved in the result analysis. (1) Serum CRP levels in T2DM were (9.89±5.71) mg/L, (6.22±4.19) mg/L respectively, both significantly higher than those in normal control group (2.50±1.40) mg/L (P < 0.01). (2) Serum IL-6 levels in T2DM were (13.33±4.08) ng/L, (7.93±3.26) ng/L, respectively, both remarkably higher than those of normal controls (3.63 ±2.12) ng/L (P < 0.01). (3) Both levels of CRP and IL-6 were markedly decreased in T2DM after treatment with complex metformin following the improvement of hyperglycemia (P < 0.01). Conclusion: (1) Inflammatory factors such as CRP and IL-6 shows abnormality in the newly onset type 2 diabetics. (2) Good glucose control can both significantly decrease levels of CRP and IL-6.

Abstract:Objective: To investigate the levels of resisitin and C reactive protein, and their relationship with diabetic in patients. Methods: The levels of resistin and C reactive protein were determined in 62 diabetic patients and 42 control subjects. Results: The levels of C reactive protein and resistin were (5.62±0.80))mg/L,(37.43±11.52)μg/L,respectively, compared with control subjects(C reactive protein: (3.62±1.1) mg/L, Resisitin: (18.24±5.84) ng/ml),P0.01, and there is a positive relationship between C reactive protein and resistin (r=0.86,P0.01). Conclusion: It was found that the levels of resistin in diabetes mellitus were higher than that in control subjects, C reactive protein also has similar appearance, and there was a positive relationship between them.

Abstract:This study aimed to examine the relationship between hemoglobin A1c (HbA1c), one of the indicators of diabetes, and high sensitivity C-reactive protein (hs-CRP), one of the indicators of inflammation. Raw data from the Korean National Health and Nutrition Examination Survey (KNHANES, 2015-2017) was analyzed. Among the patients diagnosed with diabetes, 1,479 adults were selected as subjects for our study, and their HbA1c levels, hs-CRP levels, sex, age, body mass index (BMI), waist circumference, level of triglycerides and high-density lipoprotein (HDL)-cholesterol, hypertension, receipt of diagnosis, monthly average income, education, and drinking and smoking habits were recorded. Multiple regression analysis of hs-CRP was performed by dividing hs-CRP into quartiles using HbA1c as the dependent variable. In Model 1, sex, age, and BMI were adjusted, and in Model 2, sex, age, BMI, waist circumference, level of triglycerides and HDL-cholesterol, hypertension, and receipt of diagnosis were adjusted. In Model 3, Model 2 parameters along with monthly average household income, education level, and drinking and smoking habits were adjusted. HbA1c levels increased as the hs-CRP quartile increased, that is, 2nd Quartile=0.307, p=0.003; 3rd Quartile=0.431, p=0.001; and 4th Quartile=0.550, p=0.001. Of the various factors related to diabetes, this study examined the relationship between inflammation and diabetes.

Relationships Between C-reactive Protein, White Blood Cell Count, and Insulin Resistance in a Chinese Population

Abstract:This study is to clarify whether C-reactive protein (CRP) and white blood cell (WBC) count influence insulin homeostasis to the same degree. Serum CRP and peripheral WBC were measured in 739 subjects with normal glucose regulation, 512 with impaired glucose regulation, and 502 newly diagnosed diabetic patients. Levels of insulin resistance (IR) were assessed using the index of homeostasis model (HOMA-IR). Serum CRP and WBC were significantly correlated with HOMA-IR and risk factors of IR. Relative risks of IR for each 1-SD increase of Ln (CRP) and Ln (WBC) were 1.28 (1.10–1.47) and 1.15 (1.01–1.31), respectively after adjustment for age, sex, obesity measurements, and other traditional risk factors. Additional adjustment for WBC slightly attenuated the association between CRP and IR [1.25 (1.08–1.45); P = 0.003] whereas adjustment for CRP substantially attenuated the association of WBC with IR toward null (P = 0.134). Moreover, individuals with both high levels of CRP and WBC were at higher risks of IR than those with high CRP or WBC alone. Both CRP and WBC were significantly associated with risks of IR. CRP might be a more effective biomarker in terms of the association with IR.

Abstract:Purpose: To O assess the association of serum C-reactive protein (CRP), a sensitive systemic marker of inflammation, with metabolic syndrome in obese subjects. Methods: We studied 153 obese subjects and 47 normal control subjects in Shanghai area. Obese subjects are divided into two groups according to the degree of their glucose tolerance. Their serum CRP level and anthropometrical parameters, plasm glucose, lipid, fast insulin(FIN), proinsulin(PI) were measured. We tested whether serum CRP level are associated with metabolic syndrome in obesity. Results: (1) The obese subjects had higher serum CRP level than normal controls(P<0.001), there was no significant different between the obese subjects and the obese subjects with impairment of glucose metabolism (P = 0. 413); (2) There was significant positive correlation between serum CRP and W, WHR, SBP, TG, PI, P/I Ratio, HOMA-IR; (3) After adjusted AGE and BMI, there was positive correlation between CRP and SBP, WHR, PI yet, the correlation between serum CRP and other parameters disappeared. Conclusions: (1) There was significant increase of serum CRP level in obesity. Increase of serum CRP was closely correlation with metabolic syndrome; (2) β-cell impairment was possiblely association to increase of serum CRP; (3) BMI, SBP, WHR and PI were main factors relation to increase of serum CRP.

Abstract:We conducted this single-center, retrospective, cohort study to examine whether insulin resistance (IR) and high-sensitivity C-reactive protein (hsCRP) have a relationship with metabolic abnormalities in patients with type 2 diabetes mellitus (T2DM). In a total of 3758 patients (n = 3758) with T2DM, we analyzed medical records and thereby evaluated their baseline characteristics such as age, sex, duration of T2DM, systolic blood pressure (SBP), diastolic blood pressure (DBP), waist circumference, body mass index (BMI), visceral fat thickness (VFT), fasting plasma insulin levels, C-peptide levels, glycated hemoglobin (HbA1c), fasting plasma glucose (FPG), postprandial plasma glucose (PPG), homeostatic model assessment of insulin resistance (HOMA-IR), homeostatic model assessment of β-cell function (HOMA-β), aspartate aminotransferase (AST), alanine aminotransferase (ALT), total cholesterol (TC), triglyceride (TG), high-density lipoprotein (HDL), low-density lipoprotein (LDL), albuminuria, intima-media thickness (IMT) and hsCRP. The patients were stratified according to the tertile of the K index of the insulin tolerance test (KITT) or hsCRP. Thus, they were divided into the lowest (≥2.37), middle (1.54–2.36) and highest tertile (0–1.53) of KITT and the lowest (0.00–0.49), middle (0.50–1.21) and highest tertile (≥1.22) of hsCRP. Moreover, associations of KITT and hsCRP with metabolic abnormalities, such as steatotic liver disease (SLD), metabolic syndrome (MetS), albuminuria, diabetic retinopathy and carotid atherosclerosis, were also analyzed. There was a significant positive correlation between the prevalence of SLD, MetS, albuminuria and diabetic retinopathy and KITT (p < 0.001). Moreover, there was a significant positive association between the prevalence of SLD, MetS and albuminuria and hsCRP (p < 0.001). In conclusion, our results indicate that clinicians should consider the relationships of IR and hsCRP with metabolic abnormalities in the management of patients with T2DM. However, further large-scale, prospective, multi-center studies are warranted to confirm our results.

Contribution of High Sensible Crp (Hscrp) in the Assessment of the Risk of Onset of Type 2 Diabetes and its Cardiovascular Complications

Abstract:Introduction: diabetes is a metabolic disease characterized by chronic hyperglycemia. The latter plays an essential role in inflammation by stimulating the production of pro-inflammatory cytokines or by increasing their secretion by an oxidative mechanism. These cytokines control the hepatic synthesis of an inflammatory protein called C-Reactive Protein (CRP). Thus, CRP could serve as a biomarker of risk of onset, monitoring and prognosis for type 2 diabetes. To answer this problem, we conducted this study, the purpose of which was to determine the predictive role of hsCRP in the risk of type 2 diabetes mellitus and its cardiovascular complications. Methodology: this is a prospective and cross-sectional case-control study involving 200 participants including 100 control women and 100 women with type 2 diabetes (mean age was respectively 49.89 years ± 8.26 & 51.92 years ± 7.18 ; p=0.066). The interviews were conducted on the basis of a questionnaire. Physical examination collected biometrics data and cardiovascular constants. The biochemical parameters were analyzed by an automated Abbott device. Results: we found, in control women, a positive correlation between hsCRP and BMI (rho=0.40 p<0.0001), waist-to-hip ratio (rho=0.24 p<0.0001), systolic blood pressure (rho=0.30 p=0.003), diastolic blood pressure (rho=0.28 p=0.006), total body fat (rho=0.48 p<0.0001), visceral fat level (rho=0.47 p<0.0001), glycated hemoglobin (rho=0.29 p=0.003), fasting insulin (rho=0.22 p=0.026), C-peptide level (rho=0.35 p=0.0003), to HOMA-IR (rho=0.21 p=0.034), total cholesterol (rho=0.24 p=0.016), HDL cholesterol (rho=0.24 p=0.019), apolipoprotein B level (rho=0.25 p=0.013). At the same time, hsCRP was negatively correlated with adiponectin level (rho=-0.21 p=0.04) and the nitric oxide level (rho=-0.26 p=0.01). In contrast, in women with type 2 diabetes, hsCRP was positively associated with BMI (rho=0.38 p=0.007), WHR (rho=0.43 p=0.002), total body fat (rho=0.25 p=0.014), heart rate (rho=0.34 p=0.001) and glycated hemoglobin (rho=0.21 p=0.036). Conclusion: hsCRP has a prognostic value in the evaluation of cardiovascular risk. It seems to play an important role in the pathogenesis of type 2 diabetes mellitus and its cardiovascular complications. It could thus be considered as a biomarker for the screening, monitoring and prognosis of type 2 diabetes mellitus.

Abstract:OBJECTIVE: The rate of cardiovascular disease is increasing in developed countries progressively with estimates predicting 22 million by 2030. Based on these cardiovascular events lies atherosclerosis, a condition intricately linked to chronic inflammatory processes. Among fundamental clinical biomarkers, C-reactive protein (CRP) stands out as a backbone of inflammatory activity. Notably, the excessive production of CRP, often linked with obesity, plays a pivotal role in the dysregulation of triglyceride apo B-100 fractional catabolism, thus emerging as a significant cardiovascular risk factor. Apart from atherosclerotic processes, the interplay between high CRP levels and impaired fasting glucose (IFG) is also gaining recognition as a messenger of disrupted glucose metabolism, potentially ushering in the onset of a prediabetic state.METHODS: Our retrospective analysis scrutinized the biochemical data - namely low-density lipoprotein cholesterol (LDL-C), triglycerides, fasting blood sugar, and CRP levels-of 3500 patients from an internal medicine outpatient clinic seen from August 2006 to May 2007. Our objective was to dissect the correlations among these parameters. Exclusion criteria were omitting individuals with acute or chronic inflammation, known inflammatory diseases, diagnosed diabetes, coronary artery disease, lipid metabolism disorders, those on lipid-lowering agents, and anyone outside the age bracket of 18-65 years. This study was conducted in strict adherence to the ethical principles outlined in the Declaration of Helsinki.RESULTS: As a result of our study, the ratio of CRP levels above 0.8 was significantly higher in patients with IFG according to the World Health Organization criteria (6.1-6.9 mmol/L or 109-124 mg/dL) than in individuals with normal fasting glucose (70-108 mg/dL). (19.7%, 17.2%, respectively) (p<0.001). In addition, the ratio of CRP levels above 0.8 was also higher in patients with triglyceride levels between 151 and 199 mg/dL) and over 500 mg/dL. (23.2%, 24.1%, respectively) (p<0.012). However, the relationship between CRP levels and LDL-C total cholesterol was not statistically significant (p>0.05).CONCLUSION: This retrospective study suggests the imperative for a proactive approach in the clinical evaluation of patients exhibiting elevated CRP, especially in the context of preemptive management of prediabetes. In light of these findings, we think that elevated CRP may be a warning sign for prediabetic status and may be useful in early diagnosis.
Plasma Hscrp and Glucose Tolerance in Essential Hypertensive Patients

Abstract:Objective To test the hypothesis that the plasma high sensitivity C-reactive protein (hsCRP) level in essential hypertensive patients were related with various degree of glucose tolerance. Methods One hundred forty six essential hypertensive patients were divided into NGT, IGT and T2DM groups according their OGTT results. Blood pressure and laboratory studies, including hsCRP level were determined. Results (1)Age, gender, body weight, BMI, SBP, DBP, LDL-C and TG index between three group were not significant different among groups; (2)The hsCRP level gradually increased from NGT to T2DM groups (P0.01). Plasma hsCRP level was positively correlated with glucose tolerance degree in essential hypertension patients(r=0.72, P0.05). Conclusion hsCRP was related with the abnormal glucose tolerance in essential hypertensive patients. Essential hypertension patients with high level of hsCRP should be underwent OGTT test to find out the glucose tolerance status and appropriate treatment should be administered.

Abstract:BACKGROUND:Evidences show that subclinical chronic inflammation is involved in the pathogenesis of diabetic nephropathy. The aim of this study was to examine the relationship between serum C-reactive protein (CRP), serum uric acid, and albuminuria in Chinese type 2 diabetic patients.METHODS:A total of 1162 type 2 diabetic patients were recruited. All participants had relevant clinical and laboratory measurements. CRP was measured using a particle enhanced immunoturbidimetric assay.RESULTS:In the multiple linear regression model, natural log-transformed CRP (lnCRP) and uric acid were independent predictors of natural log-transformed urinary albumin to creatinine ratio (lnACR) (β = 0.18, 95% CI 0.10-0.27, P < 0.001 and β = 0.18, 95% CI 0.09-0.27, P < 0.001). The interaction of lnCRP with uric acid was also associated with lnACR (β = 0.04, 95% CI 0.02-0.06, P < 0.001). In the full-adjusted logistic regression model, the OR for albuminuria of the patients in the third tertile levels of CRP and uric acid was 3.94 compared with patients in the first tertile levels of CRP and uric acid (95% CI 1.73-8.94, P < 0.001).CONCLUSIONS:Elevated serum CRP and increased serum uric acid level were associated with albuminuria in Chinese type 2 diabetic patients. Moreover, CRP and uric acid had an interactive effect on albuminuria.
