The mid-term outcome of transforaminal lumbar interbody fusion(TLIF)for the treatment of two-level lumbar degenerative disease

Abstract:Objective To explore the mid-term outcomes of transforaminal lumbar interbody fusion﹙TLIF﹚ for the treatment of two-level lumbar degenerative disease.Methods From Sep 2005 to Mar 2009,25 two-level cases of lumbar degenerative disease were treated with TLIF.There were 17 males and 8 females,with a mean age of 56.5 years ﹙range,42 to 68 years﹚.The mean course of the disease was 3.4 years ﹙range,1.0 to 12 years﹚.There were 11 cases in L3to L5and 14 cases in L4to S1.Preoperatively,the score of VAS was 6.6±1.1,the score of ODI was 40.4±3.2 and the score of modified Prolo was 8.5±2.1.Results The mean operation time was 206.2±51.5 min ﹙range,180 to 240 min﹚,the mean intra-operative blood loss was 503.2±30.5ml ﹙range,420 to 600ml﹚,and the mean hospitalization time was 21.3±2.2d ﹙range,14 to 30d﹚.All cases were followed up from 2.5 to 5.5 years ﹙mean 4.8 years﹚.Postoperatively,the scores of lumbar function ﹙VAS,ODI and mProlo﹚ significantly improved compared with preoperative scores ﹙ <0.05﹚.The clinical results by modified Prolo score were excellent in 15 cases,good in 8,fair in 1 and poor in 1 with choiceness rate 92%.43 levels got interbody bony fusion from total 50 levels of 25 cases confirmed by three dimensional reconstructive CT 12 months after operation,the fusion rate was 86%.There were no complications of instrument loosening and breaking,pseudoarticulation formation and interbody fusion cage subsidence.3 cases of superficial infection were healed with drainage.2 cases suffered from rod loosening and slip.Conclusions This study shows that two-level TLIF is an effective method with good clinical effect and high fusion rate.However,long-term study is required.
