Finite Element Analysis of Tibia after Revision of TKA Through Impaction Bone Grafting

Abstract:Objective To investigate the biomechanical variation in full-length tibia stress pattern and periprosthetic stress distribution after implantation with long or short stem in revision of TKA through impaction bone grafting.Methods Three-dimensional finite element models of tibia,long stem and short stem prosthesis were developed.The stress distributions on the tibia and the implants were measured.The stress changes of tibia and periprosthetic area were respectively quantitatively analyzed.Results Compared with preoperative tibia,there was no change of the full-length tibia stress pattern after implantation of both implants.Both prosthesis induced decrease of stress in the periprosthetic bone area,especially the long stem.The peak stress shielding of long and short stem were 94.4% and 75% respectively.The stress value of implant-cancellous bone was much higher than that of cortical bone.Conclusion Implantation of both implants may cause significant stress shielding in the proximal tibia.This may be related with periprosthetic bone loss and aseptic loosening of stems.This can also be used to explain the mechanism of postoperative tibia fracture and pain.
