Abstract:ObjectiveTo evaluate the diagnostic accuracy of MR imaging in the identification of labral and articular cartilage lesions in patients with acetabular dysplasia.Design and patientsPre-operative MR imaging was performed on 27 hips in 25 consecutive patients (16 males, 9 females, age range 19–52 years, mean age 31.2 years) with radiographic evidence of acetabular dysplasia (centre-edge angle of Wiberg <20 degrees). The average duration of symptoms was 16.2 months. Two musculoskeletal radiologists assessed MR images in consensus for the presence of abnormality involving the acetabular labrum and adjacent acetabular articular cartilage. A high resolution, non-arthrographic technique was used to assess the labrum and labral chondral transitional zone. Surgical correlation was obtained in all cases by a single surgeon experienced in hip arthroscopy and ten patients with normal hip MRI were included to provide a control group.ResultsThe acetabular labra in the dysplastic hips demonstrated abnormal signal intensity, and had an elongated appearance when compared with the control group (mean length 10.9 mm vs 6.4 mm). Morphological appearances in the labra included surface irregularity, fissures and cleft formation. MR imaging correctly identified the severity of chondral abnormality in 24 of 27 hips (89%) when compared with arthroscopic findings.ConclusionsMR imaging demonstrates an elongated labrum, focal intra-substance signal change and irregularity and fissuring of the margins in patients with acetabular dysplasia. Abnormality is also identified at the labral chondral transitional zone, where fissuring, focal clefts, chondral deficiency and subchondral cyst formation may be apparent. A high-resolution, non-arthrographic technique can provide an accurate preoperative assessment and evaluate the presence of premature osteoarthritis.

Abstract:Objective To compare the performance of 3 T MR arthrography and conventional MRI for diagnosing acetabular labrum tear.Methods Conventional MRI and MR arthrography of the hips of 53 patients were retrospectively analyzed by two radiologists independently.The MR findings were compared to hip arthroscopy.Results Of 53 patients,acetabular labral tear was arthroscopically confirmed in 31.The sensitivity,specificity,accuracy,positive predictive value (PPV) and negative predictive value (NPV) of MR arthrography for assessing the acetabular labral tear were 90.32％,77.27％,84.91％,84.85％,and 85.00％ for one radiologist,93.55％,77.27％,86.79％,85.29％,and 89.47％ for the second radiologist with good inter-observer consistency (k=0.797).The sensitivity,specificity,accuracy,PPV and NPV of conventional MRI for evaluating the acetabular labral tear were 54.84％,72.73％,62.26％,73.91％,and 53.33％ for the first radiologist,61.29％,77.27％,67.92％,79.17％,and 58.62％ for the second radiologist with good inter-observer consistency (k=0.733).The sensitivity,accuracy and NPV of MR arthrography for diagnosing acetabular labral tear were significantly higher than those of conventional MRI (P＜0.05).Conclusion 3 T MR arthrography of the hip is reliable and superior to conventional MRI for diagnosing acetabular labral tear.

Abstract:Objective To compare the diagnostic value of conventional MR imaging with MR arthrography for acetabular labral tears at 3.0 T MR imaging Methods Forty-four patients suspected of acetabular labral pathology were prospectively scanned with conventional MR imaging and MR arthrography of the affected hip joints. All MR data were retrospectively studied. Each acetabular labrum was divided into three regions of anterior, superior, and posterior, and the acetabular tear and its classification were recorded. Five patients underwent subsequent hip arthroscopy. Wilcoxon signed ranks test was used to compare the difference between MR imaging and MR arthrography, and Kappa value was adopted for the evaluation of agreement of these two techniques. Results One hundred and sixteen regions of acetabular labrum were evaluated to be concordant at MR imaging and MR arthrography. Only 16 regions had different results at MR imaging and MR arthrography, in which 9 were diagnosed to have tears at MR arthrography but non-tear at MR imaging, and 6 had tears at MR imaging but non-tear at MR arthrography, and the other one had tears at both exams but of different classifications. In 5 patients confirmed by hip arthroscopy, both the evaluations of MR imaging and MR arthrography were accordant with that of arthroscopy. There were no statistical difference between MR imaging and MR arthrography ( Z = 0. 347, P ＞ 0. 05 ) for diagnosing the acetabular labral tears, and good agreements (Kappa = 0. 781 ,P ＜ 0. 01 ) were found between these two methods. Conclusion Conventional MR imaging of hip joints at 3.0 T shows similar diagnostic value with MR arthrography in evaluating the acetabular labral tears.

Abstract:Objective To analyse the X-ray and Magnetic resonance imaging (MRI) features of acetabular labral tears, so that to improve the recognition of the disease. Methods Imaging features of 19 patients with acetabular labral tears confirmed by arthroscopy were analyzed retrospectively (All cases were examined by X-ray, 14 patients underwent MRI examinations). Results ① 17 of the 19 patients had at least one osseous abnormality consistent with femoroacetabular impingement (FAI) under arthroscopy, among them 7 cases were cam impingement, 3 cases were pincer impingement and 7 cases were mixed impingement. ② X-ray manifestations: 12 of the 19 patients had FAI, among them 6 cases were cam impingement, 2 cases were pincer impingement and 4 cases were mixed impingement. 5 cases showed various degrees of degenerations of hip joint. 2 patients had no identifiable structural abnormalities. ③ MRI showed various degrees of injury of labrum in 6 patients, joint effusion in 5 cases, bone marrow edema of femoral head in 3 cases. Conclusion FAI has much to do with acetabular labral tears. X-ray examination is important for FAI diagnosis. Given its high sensitivity and accuracy, MRI is an effective preoperative tool for defining the location and extent of labral tears. Combined with X-ray imaging and clinical manifestation, MRI can provide a reliable basis for clinical diagnosis and treatment.

Abstract:Objective To evaluate the value of high-resolution hip MRI for diagnosing acetabular labrum tears. Methods 3.0 T hip MRI of 122 patients was independently reviewed by two radiologists and compared with the results of arthroscopy. Results Acetabular labrum tear was arthroscopically diagnosed in 87 of the 122 patients. The sensitivity and specificity of MRI were 60.92%and 74.29% (radiologist A), 64.37% and 74.29% (radiologist B), respectively, with good consistency between the two observers (Kappa value=0.645). Conclusion High-resolution hip MRI has moderate sensitivity and high positive predictive value for diagnosing acetabular labrum tears. Paralabral cysts are the specific MRI manifestations.

Abstract:Magnetic resonance imaging (MRI) of the hip has been valuable in the diagnosis of occult osseous abnormalities and of periarticular soft tissue disorders. MRI has been less useful in the evaluation of acetabular labral tears and other intra-articular abnormalities. Image optimization is more technically challenging in the hip than in smaller joints because the overlying soft tissues are thicker, resulting in decreased spatial and contrast resolutions that may not be adequate to distinguish the acetabular labrum from subchondral bone, articular cartilage, and joint capsule. MR arthrography (MRA) extends the capabilities of conventional MRI because contrast solution separates intra-articular structures and outlines abnormalities. In hips with suspected acetabular labral injury, arthrographic MR images demonstrate the location and length of tears and the presence of associated capsular defects. Arthrographic MR images may also enable the accurate diagnosis of cartilage lesions and intra-articular loose bodies. Abstract:Objective To investigate the 3.0 T MRI appearance of acetabular labral tears. Methods MR 3.0 T images of 20 surgically proved patients with acetabular labral tears were retrospectively reviewed for analyzing the characteristic MRI findings of acetabular labral tears. Results MRI 3.0 T can show acetabular labral tears, the sensitivity, specificity, PPV, NPV and accuracy of MRI for diagnosing acetabular labral tears were 90% ( 27 / 30 ), 96.7% ( 29 / 30 ), 96.4% ( 27 / 28 ), 90.6% ( 29 / 32 ) and 93.3% ( 56 / 60 ). A majority of acetabular labral tears were axial direction and located in the root of 9 - 11 o'clock positions. Conclusions MRI 3.0 T was a reliable imaging method in the diagnosis of acetabular labral tears, acetabular labral tears were majorly located in anterior portions of the labrum.

Abstract:ObjectivesMagnetic resonance imaging (MRI) and magnetic resonance arthrography (MRA) have been advocated for the diagnosis of acetabular labral tears. The purpose of this study was to determine the sensitivity and specificity of MRI and MRA in diagnosing acetabular labral tears using meta-analysis.MethodsPertinent published and unpublished literature databases were reviewed. A two-by-two table was constructed to calculate the sensitivity and specificity of MRI or MRA investigations against surgical outcomes. Pooled sensitivity and specificity and Receiver Operating Characteristic curve (ROC) evaluations were performed. Methodological quality of each study was assessed using the QUADAS (Quality Assessment of Diagnostic Accuracy Studies) tool.ResultsNineteen papers assessing 881 hips were reviewed. Conventional MRI was assessed in 13 studies and MRA was assessed in 16 studies. Whilst both MRI (0.5–3T) and MRA (0.5–3T) presented with a moderate sensitivity and specificity (sensitivity 66%, 87%; specificity 79%, 64%), diagnostic accuracy of MRA appeared to be superior to MRI in detecting acetabular labral tears on ROC curve interpretation. The literature poorly described population characteristics, assessor blinding, with limited sample sizes.ConclusionsMRI and MRA may be useful adjuncts in the diagnosis of acetabular labral tears in adults. MRA appears to be superior to conventional MRI.

Abstract:Magnetic resonance imaging (MRI) of the hip joint is performed for the diagnostics of many clinical pathologies. For clarification of pathologies of the hip joint conventional MRI and MR arthrography are used. The use of MRI either with or without the intravenous administration of contrast material is performed to diagnose bone marrow lesions, necrosis of the femoral head, inflammatory and tumorous lesions affecting the hip joint and its components. After the intra-articular administration of a diluted solution of contrast material, which results in distension of the joint, MR arthrography is performed for the diagnosis of intra-articular pathologies, such as lesions of the labrum acetabulare, chondral lesions and free intra-articular bodies. In recent times, MR arthrography is carried out with continuous traction of the limb to be examined because this leads to a distension of the joint and its components and in this way particularly lesions of the labrum with involvement of cartilage can be visualized even better. This article gives an overview of MRI and the accuracy of detecting lesions of the labrum acetabulare and the influence on the therapeutic approach.
Diagnosis of acetabular labral tears: Comparison of three-dimensional intermediate-weighted fast spin-echo MR arthrography with two-dimensional MR arthrography at 3.0 T

Abstract:Background Magnetic resonance (MR) arthrography is the ideal imaging modality for the acetabular labrum. Three-dimensional (3D) fast spin-echo (FSE) sequences have similar diagnostic performance as two-dimensional (2D) conventional MR imaging for ligaments, menisci, or bone marrow edema in the knee. Purpose To compare the diagnostic accuracy and inter-observer reliability of 3D intermediate-weighted FSE sequence and 2D FSE sequences for the diagnosis of acetabular labral tears. Material and Methods Institutional review board approval was obtained and informed consent was waived for 45 patients (47 hips) who underwent 3D and 2D MR arthrography and subsequent arthroscopic surgery. The 3D sequences were performed using volumetric intermediate-weighted fast spin-echo imaging with fat suppression (voxel size, 0.6 × 0.6 × 1.2 mm; imaging time, 6 min 38 s). Labral tear was retrospectively and independently evaluated by two radiologists in four areas of the labrum (anterosuperior, posterosuperior, anteroinferior, and posteroinferior) on 3D and 2D FSE sequences. Statistical differences between the sensitivity and specificity of the methods were analyzed with the McNemar test, using arthroscopic findings as the reference standard. Inter-observer agreement was calculated using kappa statistics. Results Arthroscopic findings confirmed labral tears at 40 anterosuperior, 23 posterosuperior, 0 anteroinferior, and 2 posteroinferior quadrants. Sensitivity and specificity were 74% and 89% for 2D FSE sequences, and 78% and 92% for 3D FSE sequences, respectively. Sensitivities and specificities for the methods were not different statistically (P > 0.05). Inter-observer agreement for labral tear was substantial for 2D FSE sequences (k = 0.774) and almost perfect for 3D FSE sequences (k = 0.842). Conclusion 3D intermediate-weighted FSE MR arthrography is excellent for diagnosing acetabular labral tears. Sensitivity, specificity, and inter-observer reliability were similar to conventional 2D MR arthrography. For evaluation of the labrum, 3D FSE MR arthrography is more time-efficient than 2D FSE MR arthrography.

Abstract:Abstract Objective. The objective of this study was to evaluate the effects of continuous leg traction on contrast-enhanced MR imaging of the hip joint and to determine whether MR imaging under these conditions is useful for demonstrating acetabular labral tears. Design and patients. Nineteen hips underwent MR imaging with a T1-weighted spin-echo sequence, followed by MR imaging under continuous leg traction after intravenous injection of gadolinium-DTPA. Joint fluid enhancement and labral contour detection were evaluated. Eleven hips had labral tears shown by conventional arthrography, arthroscopy and macroscopic surgical findings. Assessment of labral tears by MR imaging was correlated with the diagnosis based on these standard techniques. Results and conclusions. Joint fluid enhancement was obtained in all hips at 30 min after injection. Superior and inferior labral surfaces were completely delineated in 1 hip on the unenhanced MR images, and in 7 and 13 hips, respectively, on the enhanced images under traction. The enhanced images under traction depicted 9 of the 11 labral tears. Comparison between the unenhanced image and the enhanced image under traction avoided mistaking undercutting of the labrum for a tear in 4 hips. Contrast-enhanced MR imaging under traction was valuable for detecting labral tears non-invasively and without radiation. Follow-up examinations using this method in patients with acetabular dysplasia can help to clarify the natural course of labral disorders and enable better treatment planning.

[Application of Ultrasound for Evaluation of Anterosuperior Acetabular Labral Tear].

Abstract:OBJECTIVE:To explore the value of ultrasound in the diagnosis of anterosuperior acetabular labral tear.METHODS:A total of 102 patients [(42 males and 60 females, age from 13 to 60 years, average age was (35.14±9.16) years] with suspected anterosuperior acetabular labral tear were included in this study, including 44 left hip joints and 58 right hip joints. All the patients received hip joint ultrasound and magnetic resonance imaging (MRI) evaluation before arthroscopy surgery. Using arthroscopy as golden standard, the sensitivity, specificity, positive predictive value, negative predictive value and accuracy between ultrasound and MRI were calculated and compared.RESULTS:There were 91 antero-superior acetabular labral tears of 102 patients which were confirmed during arthroscopy surgery. Sixty-nine patients were diagnosed correctly by ultrasound, including 60 anterosuperior acetabular labral tears and 9 with no acetabular labral tears, whereas 2 were false-positive and 31 were found to be false-negative. The sensitivity, specificity, positive predictive value, negative predictive value and accuracy by ultrasound were respectively 65.93%, 81.82%, 96.77%, 22.50% and 67.65%. In contrast, seventy-seven patients were diagnosed correctly by MRI, including 70 anterosuperior acetabular labral tears and 7 with no acetabular labral tears, whereas 4 were false-positive and 21 were found to be false-negative. For MRI, the sensitivity, specificity, positive predictive value, negative predictive value and accuracy were respectively 76.92%, 63.64%, 94.59%, 25.00% and 75.49%. The results of ultrasound and MRI were in accordance in 68 of the 102 patients. There were 51 anterosuperior acetabular labral tears of the 68 patients who were diagnosed by both ultrasound and MRI, whereas there were 17 with no acetabular labral tears of the 68 patients who were diagnosed by both ultrasound and MRI. The results of ultrasound and MRI were inconsistent in 34 of the 102 patients. In 11 of the 34 patients, in which case ultrasound diagnosed anterosuperior acetabular labral tear, MRI found no acetabular labral tear. Whereas, in 23 of the 34 patients, in which case MRI diagnosed anterosuperior acetabular labral tear, ultrasound found no acetabular labral tear. As compared with MRI findings, ultrasound had a lower accuracy for anterosuperior acetabular labral tear than MRI, there was statistical difference on the accuracy for anterosuperior acetabular labral tear (P<0.01).CONCLUSION:Although ultrasound had a slightly lower sensitivity for anterosuperior acetabular labral tear, it had a higher specificity than MRI. Dynamic evaluation of antero-superior acetabular labral tear is an advantage of ultrasound. Ultrasound could be used as a feasible method to evaluate anterosuperior acetabular labral tear.

Abstract:Background: Acetabular labral tears are highly associated with hip dysplasia. Magnetic resonance arthrography (MR arthrography) is the expensive and time-consuming contemporary gold-standard method in the radiological assessment of acetabular labral tears. Purpose: To assess the diagnostic ability of noninvasive ultrasound (US) examination compared to MR arthrography in diagnosing acetabular labral tears in dysplastic hip joints. Material and Methods: The study compared US examination and MR arthrography diagnosis of labral tears in 20 consecutively referred dysplastic hip joints. Results: The ability to diagnose acetabular labral tears upon US examination was calculated: sensitivity 44%, specificity 75%, positive predictive value 88%, and negative predictive value 25%. Conclusion: The ability of US examination in diagnosing acetabular labral tears is not yet good enough. The technique is still to be developed, and more experience, especially with the interpretation of US examinations, is needed.

Abstract:Objective To determine the diagnostic value of shoulder MR arthrography in revealing the injuries of anterior glenoid labrum,and to compare MR arthrography with arm in neutral position with MR arthrography with arm in abduction and external rotation(ABER)position.Methods MR arthrography of the shoulder,including additional oblique axial sequences with the patient in the ABER position,was performed in 44 patients.The injuries of anterior glenoid labrum of these patients were retrospectively evaluated.The result was compared with that of arthroscopy.Results For displaying the abnormalities of anterior glenoid labrum,the sensitivity,specificity,and accuracy of axial MR images with arm in neutral position were 79.3%(23/29),100%(15/15),and 86.4%(38/44),respectively.While the results of ABER position oblique axial images were 93.1%(27/29),100%(15/15),and 95.5%(42/44), respectively.The difference of sensitivity between axial and ABER-position scans was statistically significant (P<0.05).Conclusion MR arthrography with shoulder in the ABER position is more sensitive than conventional axial scanning in diagnosing the injuries of anterior glenoid labrum.

Abstract:Objective To observe the diagnostic value of shoulder MR arthrographic fat-suppressed three-dimensional(3D) isotropic volumetric interpolated breath-hold examination(VIBE) sequence for labral tears.Methods MR arthrographic fat-suppressed 3D isotropic VIBE and conventional T1W turbo spin-echo sequences were performed in 79 patients.The results of arthroscopy were regarded as gold standards,the sensitivity,specificity and accuracy of fat-suppressed 3D VIBE and conventional TSE images for detection of labral tears were calculated and compared by 2 radiologists,respectively.K values were calculated to quantify the level of interobserver agreement.Results Twenty-seven of the 79 patients were classified as SLAP lesions,44 as anterior labral tears,and 10 as posterior labral tears.There was no statistical difference between fat-suppressed 3D isotropic VIBE and conventional TSE regarding the sensitivity(81.48%—88.89%,85.19%—92.59%) and specificity(82.69%—84.62%,84.62%—86.54%) of SLAP lesions,sensitivity(88.64%—90.91%,90.91%—95.45%) and specificity(94.29%—97.14%,97.14%) of anterior labral tears,nor sensitivity(80.00%—90.00%,80%) and specificity(92.75%—95.65%,94.20%—95.65%) of posterior labral tears.At interobserver comparison,the consistency was very good.Conclusion Fat-suppressed 3D isotropic VIBE MR arthrography is a reliable method to evaluate labral tears.

Abstract:A normally functioning hip joint is imperative for athletes who use their lower extremities with running, jumping, or kicking activities. Sports-related injuries of the hip and groin are far less frequent than injuries to the more distal aspect of the extremity, accounting for less than 10% of lower extremity injuries. Despite the lower incidence, hip and groin injuries can lead to significant clinical and diagnostic challenges related to the complex anatomy and biomechanical considerations of this region. Loads up to 8 times normal body weight have been documented in the joint in common daily activities, such as jogging, with significantly greater force expected during competitive athletics. Additionally, treatment for hip and groin injuries can obviate the participation of medical and surgical specialties, with a multidisciplinary approach frequently required. Delay in diagnosis and triage of these injuries may cause loss of time from competition and, potentially, early onset of degenerative changes. Magnetic resonance imaging (MRI) of the hip has proven to be the gold standard for the diagnosis of sports-related hip and groin injuries in the setting of negative radiographs. With its exquisite soft tissue contrast, multiplanar capabilities, and lack of ionizing radiation, MRI is unmatched in the noninvasive diagnosis of intra-articular and extra-articular pathology, as well as intraosseous processes. This review focuses on MRI of common athletic injuries of the hip and groin, including acetabular labral tears, femoral acetabular impingement syndrome, muscle injuries around the hip and groin (including athletic pubalgia), and athletic osseous injuries.
Reliability of Conventional Hip MRI in Detecting Labral Tear and Labrocartilagenous Lesions in cases of Femoroacetabular Impingement, A Comparative Study with Hip Arthroscopy

Abstract:Background: Imaging studies play a crucial role in diagnosing femoroacetabular impingement (FAI), including plain radiography and Magnetic Resonance Imaging (MRI). FAI is a combined pathology of bony abnormality, labral and labrocartilagenous erosions. Surgical treatment for such cases has become more established and preoperative imaging is the roadmap that includes the assessment of labrum and articular cartilage. Methods: During a period of 2 years, thirty-seven patients with a clinical diagnosis of FAI were retrospectively enrolled in this study, including 17 men and 20 women, aged 27-62 years. There were 22 right hips and 15 left hips. MRI was done for all patients to identify bony details, labral and chondral abnormalities and to exclude coexisting disorders. The imaging findings were compared with the arthroscopic data. Results: 15 patients had Pincer FAI, 11 patients had CAM, and 11 patients had combined Cam/Pincer FAI. Labral tear was detected in 100 % of patients, 97 % had an anterosuperior labral tear. 82 % of patients had partial thickness cartilage lesions and 8 % had full thickness cartilage lesions. MRI had a sensitivity of 100 % compared to hip arthroscopy in detecting labral tear, 60 % in detecting cartilage erosion. Conclusion: Conventional hip MRI detects bony changes in FAI, type of impingement and associated labral tear and cartilage erosions in comparison to the hip arthroscopy.
Anterior Shoulder Dislocation: Evaluation with MR and MR Arthrography

Abstract:Objective To summary the MR findings of anterior shoulder dislocation. Methods The routine MR images and MR arthrography images of 26 patients with anterior shoulder dislocation or recurrent dislocation were retrospectively analyzed. The emphasis was focused on the bone structure of the shoulder, the anteroinferior labrum, and the anterior capsular insertion type.Results The Hill sachs lesion was found in 77%(20/26)patients and abnormality of the anteroinferior labrum (Bankart lesion) was seen in 88%(23/26). Type II and type III capsular insertions were only found in 19%(5/26)of all patients, and the presence of osseous bankart lesion was only 8%(2/26). In revealing the abnormalities of the labrum and capsular, MR arthrography was superior to routing MR. Conclusion The main MR appearances of anterior shoulder dislocation were abnormalities of the anterior labrums and Hill sachs lesions. MR arthrography is more helpful to diagnose the abnormalities of anteroinferior labrum than routing MR examination.
