Controlled study on different hepatic blood flood occlusion in hepatectomy of hepatocellular carcinoma

Abstract:Objective To evaluate the optimal method for hepatic blood occlusion in hepatectomy of hepatocellular carcinoma (HCC).Methods A retrospective analysis was performed on 83 patients with hepatic carcinoma treated with hepatectomy between April 2002 to August 2005.Resection surgery was done in patients,24 of whom were subjected to hepatectomy with optional half-hepatic blood occlusion which precluded the ipsilateral hepatic blood and kept the contralateral hepatic blood,32 cases to full-hepatic blood,and 27 cases to ischemic preconditioning.The effects of blood occlusion on the liver function and blood loss were evaluated.Results There was no operatic mortality.The liver function was recovered to the normal level before operation.Two weeks after operation,95.8%(23/24) of patients with optional half-hepatic blood occlusion,74.1%(20/27) of patients with ischemic preconditioning and 56.3%(18/32) of patients with full-hepatic blood occlusion had the recovery of liver function.There was significant difference between them statistically (P<0.05 or P<0.01).Blood loss was(188±31.70)ml,(196±45.54)ml and(389±74.10)ml during operation respectively.Conclusion The method of optional half-hepatic blood occlusion for hepatectomy of HCC isone of the effective hepatic blood occlusion methods reducing perioperative blood loss and quickly recovering liver functions.
