A Prospective Clinical Study of Treatment of Enteral Nutrition on Superior Mesentery Artery Syndrome

Abstract:Objective: To explore the treatment effect of enteral nutrition(EN) on superior mesentery artery syndrome(SMAS).Methods: According to the course and severity of SMAS,fifteen patients treated with EN via nasojejunal tube or the tube of percutaneous endoscopic gatrostomy and jejunostomy(PEG/J)were in the prospective clinical study.Clinical symptoms,volume of gastrointestinal fluid,nutritional index,and radiological examination were observed.Results: The clinical symptoms of all patients were relieved dramatically in one week after catheter drainage.The clinical symptoms of those patients treated with EN by nasojejunal tube disappeared in three to four weeks,while the symptoms of those treated with PEG/J tube disappeared in four to eight weeks.All patients resumed normal diet within five to ten weeks.Two weeks after being treated with EN,body weight,body mass index(BMI),total body fat %(TBF%) of all the subjects increased significantly,and drainage volume decreased significantly.The correlation analysis showed that drainage volume in the proximal obstruction had obvious negative correlation with body weight,BMI,and TBF%.Compared with the aortomesenteric angle before treatment(11.7±2.7)°,the angle increased significantly before removal of the nutritional catheter(23.9±5.3)°.At follow-up,symptoms recurred in two patients,but they were cured after the etiological factors were removed.Conclusion: EN can improve nutritional status,increase body weight and fat content,enlarge aortomesenteric angle,relieve duodenal obstruction and clinical symptomsin SMAS.
