The significance of Tomita scoring system in spinal metastases

Abstract:Objective:To study the value of Tomita scoring system on spinal metastases.Method:A retrospective review of 447 patients with spinal metastases was performed,291 males and 156 females,with mean age of 56.1 years.The primary tumors were lung cancer with 92 cases,prostate cancer with 69 cases,breast cancer with 62 cases,gastrointestinal cancer with 34 cases,kidney cancer with 27 cases,liver cancer with 19 cases,thyroid cancer with 15 cases,esophagus cancer with 7 cases,lymphoma with 5 cases,ovary cancer with 4 cases,uterus cancer with 3 cases,bladder cancer with 2 cases,and pancreas cancer with 1 case.Besides,the primary tumors of 107 cases were unknown.The cancer had metastasized to vital internal organs including lungs,liver and/or brain in 215 cases.Surgery had been done in 183 cases.According to the clinical,pathological and radiographic data,every case was scored with Tomita scoring system.The relation between the survival period and Tomita score was analyzed.Result:The period of follow-up ranged from 3 to 92 months with the average of 23.5 months.The median survival of 145 patients with high grade primary tumor of lungs,liver and gastrointestinal tract was 4.4 months.The median survival of 146 patients with low grade primary tumor of thyroid,breast and prostate was 12 months.The median survival of 107 patients with unknown primary tumor was 6.8 months.The median survival of 65 patients with Tomita score of 2 to 3 points was 34 months.The median survival of 49 patients with Tomita score of 4 to 5 points was 13 months.The median survival of 128 patients with Tomita score of 6 to 7 points was 8 months.The median survival of 205 patients with Tomita score of 8 to 10 points was 4 months.The difference of the survival among them was significant(P<0.0001).Conclusion:Tomita score will be a key prognostic factor in making a surgical strategy for treatment of patents with spinal metastases.In selected patients with solitary spinal metastasis of slow-growth primary tumor,surgery can provide a longer life span.
