Remifentanil combined with sevoflurane provides consistent and sustained controlled hypotension during endoscopic sinus surgery

Abstract:Objective To determine whether remifentanil combined with sevoflurane could induce controlled hypotension during endoscopoic sinus surgery. Methods Forty adults undergoing endoscopic sinus surgery and anesthetized with fentanil 4ug/kg, followed by remifentanil 0.2 to 0.5ug·kg-1·min-1 or nitrogiycerin 5 ug·kg-1·min-1 and fentanil (n=20,in each group). anesthesia maintained with sevoflurane at end-tidal concentration 2%. Results Controlled hypotension was achieved at the target mean arterial pressure (MAP) of 60mmHg(P<0.01) within (121±21)s and (62±9)s for remifentanil and nitroglycerin respectively, controlled hypotension was sustained in both groups throughout surgery and the surgical fields rating was good. Conclusion Remifentanil combined with sevoflurane in adults enable controlled hypotension, provided good surgical conditions for endoscopic sinus surgery with no need for additional use of a specific hypotensive agent.
